急性发热性嗜中性皮病亦称斯威特(Sweeet)综合征。是一种原因不明的慢性易复发性红斑与结节性皮肤病,临床常见但泛发伴有水疱,表皮坏死者少见。我们发现1例报道如下。临床资料患者男,43岁,农民。因感冒发热,伴发扁桃体炎,在当地医院静脉滴注青霉素5天好转。3周后在颈部、面部及四肢相继出现淡红色至暗红色斑块和结节,自觉灼热疼痛,触痛明显以致病人有时不能忍受,下肢膝关节痛,上肢肘、腕关节痛。在当地医院按病毒感染性疱疹治疗(药物不详),效果不佳,于2003年12月来我站就诊,  相似文献

Neutrophilic dermatosis of the dorsal hands is a recently described disorder, which is similar to Sweet's syndrome. It is characterized by erythematous plaques, pustules and haemorrhagic bullae located solely on the dorsal surface of the hands. We describe a 57-year-old man with neutrophilic dermatosis of the dorsal hands that occurred following exposure to a chemical fertilizer. There are few cases reported in the literature regarding neutrophilic dermatosis and the aetiology remains unclear. For the present case, we propose that neutrophilic dermatosis of the dorsal hands might have been induced by the chemical fertilizer.  相似文献

Pustular dermatosis of the dorsal hands is a peculiar variant of Sweet's syndrome described by Strutton et al. Nevertheless, not all authors accept this entity. We present a retrospective study of eight cases of neutrophilic vasculitis of the hands. Three patients presented with lesions only on the dorsal hands, three with lesions on the palmar surface and two with lesions on the dorsal and palmar surfaces of the hands. The patients with lesions on the dorsal surface presented a pustular aspect, and the histopathological examination revealed a high degree of vascular damage with presence of vasculitis and, in one case, with fibrinoid necrosis. Therefore, the term pustular vasculitis of the dorsal hands introduced by Strutton et al. should probably be conserved. Our cases may have demonstrated a peculiar clinicopathological disease with its own entity.  相似文献

Neutrophilic dermatosis of the dorsal hands (NDDH) was originally described as pustular vasculitis (PV) of the hands. Recent debate focuses on categorizing this disorder in the family of neutrophilic dermatoses (ND), as opposed to a primary vasculitis. Neutrophilic dermatoses encompass a wide spectrum of diseases characterized by a dense infiltration mainly composed of neutrophils. Neutrophilic dermatosis of the dorsal hands is currently considered a localized variant of Sweet syndrome. Cocaine abuse has been related to a wide range of mucocutaneous manifestations, including neutrophilic dermatoses such as pyoderma gangrenosum. The authors of this study present a patient with neutrophilic dermatosis of the dorsal hands, in which cocaine abuse was identified as a probable trigger.  相似文献

Erosive pustular dermatosis of the leg is a distinct form of spongiform amicrobial pustulosis. The disorder typically affects the lower limbs of elderly patients presenting with chronic venous insufficiency and stasis dermatitis, and has a chronic course. Three elderly patients with chronic venous ulcers are described, who developed pustules and moist eroded lesions on the leg. The clinical and histological features were typical for erosive pustular dermatosis. The lesions rapidly responded to topical treatment with either tacrolimus or corticosteroids. Of note, this condition was associated with a diverticular disease in two patients, while in another patient an epidermoid carcinoma of the tongue was present. Erosive pustular dermatosis of the leg is an uncommon but distinct skin disorder typically associated with trophic changes of the lower limbs. Our observations raise the question of the relation of erosive pustular dermatosis of the leg with the group of neutrophilic dermatoses. Topical immunotherapy with tacrolimus may constitute a novel therapeutic option for this frequently recalcitrant condition.  相似文献

Neutrophilic dermatosis of the dorsal hands is a recently described disorder. It is still debatable if it constitutes a separate entity, a variant of Sweet syndrome or pyoderma gangrenosum or an overlap disorder of both. We report the clinical features, histopathological findings, and evolution of a 35-year-old patient with the diagnosis of neutrophilic dermatosis of the dorsal hands and distant lesions on the forehead.  相似文献
