OBJECTIVE: To evaluate the best reconstruction window for noninvasive coronary angiography when using a 16-detector row computed tomography (CT) scanner with a gantry rotation time of 370 milliseconds. METHODS: In a pilot study, 189 coronary artery segments of 21 patients with a mean heart rate of 65 beats per minute (bpm, maximum: 45-94 bpm) were investigated using a 16-detector row CT scanner. Raw data were reconstructed in 10% increments from 40% to 70% of the RR interval. Two experienced observers independently evaluated the image quality of the coronary arteries in a segmental fashion. A 5-point ranking scale was applied, with 1 being very poor (no evaluation possible); 2, poor; 3, moderate; 4, good; and 5, very good. RESULTS: In the mean of all patients, the best reconstruction window was found to be at 60% of the RR interval. In patients with higher heart rates, the best reconstruction window was found to be at an earlier stage of the R wave-to-R wave interval. CONCLUSIONS: Initial results show that good diagnostic image quality could be achieved for all evaluated segments of the coronary tree with image reconstructions at 60% of the R wave-to-R wave interval in patients with heart rates of 70 bpm or less. Using a 16-detector row CT scanner with a gantry rotation time of 370 milliseconds, the need for adapting the reconstruction window to each segment for the best image quality was overcome in those cases. In patients with heart rates faster than 70 bpm, reconstructions at an earlier stage within the cardiac cycle were necessary.  相似文献

OBJECTIVES: Evaluation of coronary arteries at higher heart rates and in the presence of coronary stents remains problematic. The utilization of dual source computed tomography (DSCT) might improve the visualization of the coronary arteries under these conditions by imaging at a temporal resolution of 83 milliseconds, independent of heart rate. MATERIALS AND METHODS: Vessel phantoms (diameter 2-4 mm) were attached to a robotic device to simulate cardiac motion and scanned with a DSCT system. The phantoms had either inserts leading to 50% stenosis or carried stents. Images were evaluated for motion artifacts and measurements of the normal, stenotic, and in-stent lumen at different heart rates (50-120 bpm) were performed. Quantile regression analysis was performed to investigate heart rate dependence of the measurement errors. RESULTS: Visualization of the stenoses and stents was possible without motion artifacts at heart rates of up to 120 bpm. Image quality was similar for the static (0 bpm) and the dynamic (50-120 bpm) scans. Errors for diameter measurements of the vessel lumen and the stenotic lumen were low (3-mm vessel: 1-2%), but considerable for in-stent diameter measurements (3-mm stent: 27-32%). A window/level setting of 1500/300 Hounsfield units was more favorable for stent evaluation. No heart rate dependence was found. CONCLUSIONS: Depiction of coronary stents with DSCT is possible across a large range of simulated heart rates without motion artifacts and with image quality superior to that of previous generations of CT scanners.  相似文献

The image quality and optimal reconstruction interval for coronary arteries in heart transplant recipients undergoing non-invasive dual-source computed tomography (DSCT) coronary angiography was evaluated. Twenty consecutive heart transplant recipients who underwent DSCT coronary angiography were included (19 male, one female; mean age 63.1 +/- 10.7 years). Data sets were reconstructed in 5% steps from 30% to 80% of the R-R interval. Two blinded independent observers assessed the image quality of each coronary segments using a five-point scale (from 0 = not evaluative to 4 = excellent quality). A total of 289 coronary segments in 20 heart transplant recipients were evaluated. Mean heart rate during the scan was 89.1 +/- 10.4 bpm. At the best reconstruction interval, diagnostic image quality (score >/=2) was obtained in 93.4% of the coronary segments (270/289) with a mean image quality score of 3.04 +/- 0.63. Systolic reconstruction intervals provided better image quality scores than diastolic reconstruction intervals (overall mean quality scores obtained with the systolic and diastolic reconstructions 3.03 +/- 1.06 and 2.73 +/- 1.11, respectively; P < 0.001). Different systolic reconstruction intervals (35%, 40%, 45% of RR interval) did not yield to significant differences in image quality scores for the coronary segments (P = 0.74). Reconstructions obtained at the systolic phase of the cardiac cycle allowed excellent diagnostic image quality coronary angiograms in heart transplant recipients undergoing DSCT coronary angiography.  相似文献

Multi-detector computed tomography (CT) scanners, by virtue of their high temporal and spatial resolution, permit imaging of the coronary arteries. However, motion artifacts, especially in patients with higher heart rates, can impair image quality. We thus evaluated the performance of a new dual-source CT (DSCT) with a heart rate independent temporal resolution of 83 ms for the visualization of the coronary arteries in 14 consecutive patients. METHODS: Fourteen patients (mean age 61 years, mean heart rate 71 min(-1)) were studied by DSCT. The system combines two arrays of an X-ray tube plus detector (64 slices) mounted on a single gantry at an angle of 90 degrees With a rotation speed of 330 ms, a temporal resolution of 83 ms (one-quarter rotation) can be achieved independent of heart rate. For data acquisition, intraveous contrast agent was injected at a rate of 5 ml/s. Images were reconstructed with 0.75 slice thickness and 0.5 mm increment. The data sets were evaluated concerning visibility of the coronary arteries and occurrence of motion artifact. RESULTS: Visualization of the coronary arteries was successful in all patients. Most frequently, image reconstruction at 70% of the cardiac cycle provided for optimal image quality (50% of patients). Of a total of 226 coronary artery segments, 222 (98%) were visualized free of motion artifact. In summary, DSCT constitutes a promising new concept for cardiac CT. High and heart rate independent temporal resolution permits imaging of the coronary arteries without motion artifacts in a substantially increased number of patients as compared to earlier scanner generations. Larger and appropriately designed studies will need to determine the method's accuracy for detection of coronary artery stenoses.  相似文献

目的 探讨双源CT冠状动脉成像的图像质量和重组时相与心率的关系.方法 对95例临床可疑冠心病患者进行双源CT增强扫描,检查前均不使用β受体阻滞剂控制心率.按扫描时心率分为3组:低心率组(≤70 次/min)26例,中心率组(71~90 次/min)37例,高心率组(t≥91 次/min)32例.利用回顾性心电门控重建出10%~100% R-R时相的图像,分析不同R-R时相冠状动脉的图像质量情况及其与心率的关系.结果 低、中、高心率3组图像质量评分分别为(1.08±0.27)、(1.32±0.58)、(1.47±0.61)分,低心率和中心率组图像质量评分差异无统计学意义(P>0.05),低心率组图像质量评分显著优于高心率组(P<0.05),中心率组和高心率组图像质量评分差异无统计学意义(P>0.05).只有1.4%(19/1386)的冠状动脉节段不可评价.74例患者(77.9%)可在单一重组时相获得冠状动脉各段最佳图像质量.低心率组23例(88.5%)最佳重组时相在舒张中晚期;高心率组27例(84.4%)的最佳重组时相前移至舒张早期和收缩末期.结论 双源CT能够在相当宽的心率范围内提供优良的图像质量;多数患者可在单一时相获得各支冠状动脉最佳质量图像,随着心率的增快最佳重组时相从舒张中晚期前移至舒张早期和收缩末期.  相似文献

PURPOSE: To evaluate reconstruction intervals and image quality in dual-source computed tomography (DSCT) coronary angiography for optimal placement of the ECG-pulsing window. MATERIALS AND METHODS: DSCT coronary angiography was performed in 60 patients. Thirteen datasets were reconstructed in 5% increments from 20-80%. Two readers independently assessed image quality of each segment in each percentage-interval, using scores ranging from 1 (no motion artifacts) to 4 (nonevaluable). RESULTS: Mean heart rate (HR) was 69.0 +/- 18.9 beats per minute (bpm) (range, 35-117 bpm). Diagnostic image quality (scores 1-3) was found in 97.8% of all segments (763 of 780). The 70% RR-interval provided best image quality in all patients and all HRs. The narrowest reconstruction window providing diagnostic image quality was 60-70% for HR <60 bpm, 60-80% for 60-70 bpm, 55-80% for 70-80 bpm, and 30-80% for HR >80 bpm. CONCLUSIONS: DSCT coronary angiography provides best image quality for various HRs at 70%. The ECG-pulsing window can be adapted according to the HR while maintaining diagnostic image quality.  相似文献

Sub-second multidetector-row computed tomography systems provide great potential for the further improvement of CT coronary angiography (CTCA). However, because the temporal resolution (TR) of such CT systems is insufficient, blurring and artifacts produced by fast cardiac motion remain as unresolved issues. Previous TR investigations of CTCA were based on the retrospective electrocardiogram-gated multisegment reconstruction technique. However, the results obtained may not necessarily be correct because the TR of multisegment reconstruction may not be substantial due to the insufficient periodicity of cardiac motion. The optimal TR required for better CTCA images was evaluated with use of a dual-source CT system, which has various substantial TR modes (83, 125, and 165 ms). CTCA images of 147 patients with heart rates (HRs) ranging from 36 to 117 beats/minute (bpm) were evaluated visually on a 4-point scale. Our results revealed not only that the 165-ms TR is sufficient for low HRs (≤60 bpm), but also that the 83- and 125-ms TRs are unnecessary for such HRs. The image quality with the 125-ms TR mode was acceptable for the left anterior descending artery, left circumflex artery, and right coronary artery at low to intermediate HRs (≤70 bpm). At high HRs (>70 bpm), the 83-ms TR mode resulted in an excellent image quality for all cases except those with very rapid RCA motion. Adequate TRs for a wide range of heart rates (52–106 bpm) are thus clarified.  相似文献

The reconstruction intervals providing best image quality for non-invasive coronary angiography with 64-slice computed tomography (CT) were evaluated. Contrast-enhanced, retrospectively electrocardiography (ECG)-gated 64-slice CT coronary angiography was performed in 80 patients (47 male, 33 female; mean age 62.1±10.6 years). Thirteen data sets were reconstructed in 5% increments from 20 to 80% of the R-R interval. Depending on the average heart rate during scanning, patients were grouped as <65 bpm (n=49) and ≥65 bpm (n=31). Two blinded and independent readers assessed the image quality of each coronary segment with a diameter ≥1.5 mm using the following scores: 1, no motion artifacts; 2, minor artifacts; 3, moderate artifacts; 4, severe artifacts; and 5, not evaluative. The average heart rate was 63.3±13.1 bpm (range 38–102). Acceptable image quality (scores 1–3) was achieved in 99.1% of all coronary segments (1,162/1,172; mean image quality score 1.55±0.77) in the best reconstruction interval. Best image quality was found at 60% and 65% of the R-R interval for all patients and for each heart rate subgroup, whereas motion artifacts occurred significantly more often (P<0.01) at other reconstruction intervals. At heart rates <65 bpm, acceptable image quality was found in all coronary segments at 60%. At heart rates ≥65 bpm, the whole coronary artery tree could be visualized with acceptable image quality in 87% (27/31) of the patients at 60%, while ten segments in four patients were rated as non-diagnostic (scores 4–5) at any reconstruction interval. In conclusion, 64-slice CT coronary angiography provides best overall image quality in mid-diastole. At heart rates <65 bpm, diagnostic image quality of all coronary segments can be obtained at a single reconstruction interval of 60%.  相似文献

Purpose

To describe prospective ECG-triggered dual-source CT dual-step pulsing (pECGdual_step) for evaluation of coronary arteries and cardiac function.

Methods

Fifty-one consecutive patients pre- or post-cardiovascular surgery were examined with adaptive sequential tube current modulated (pECGdual-step) 128-slice dual-source CT without heart rate control (main padding window: 40% RR interval?>65 bpm/70% RR interval?<65 bpm). Image quality of coronary arteries was graded (4-point scale), and cardiac function was evaluated.

Results

Mean HR was 68 bpm. Thirty-seven patients were in stable sinus rhythm (SR); 14 had arrhythmia. Image quality of coronary arteries was diagnostic in 804/816 (98%) of segments. The number of non-diagnostic segments was higher in patients with arrhythmia as compared to those in SR (4% vs. 0.5%; p?=?0.01), and there were fewer segments with excellent image quality (79% vs. 94%; p?<?0.001) and more segments with impaired image quality (p?<?0.001 and p?=?0.002). Global and regional LV function could be evaluated in 41 (80%) and 47 (92%) patients, and valvular function in 48 (94%). In 11/14 of patients with arrhythmia, the second step switched to full mAs, increasing radiation exposure to 8.6 mAs (p?<?0.001). The average radiation dose was 3.8 mSv (range, 1.7–7.9) in patients in SR.

Conclusion

pECGdual-step128-slice DSCT is feasible for the evaluation of coronary arteries and cardiac function without heart rate control in patients in stable sinus rhythm at a low radiation dose.  相似文献

Purpose

To assess the influence of temporal resolution on image quality of computed tomographic (CT) coronary angiography by comparing 64-row Dual Source CT (DSCT) and Single Source CT (SSCT) at different heart rates.

Methods

An anthropomorphic moving heart phantom was scanned at rest, and at 50 beats per minute (bpm) up to 110 bpm, with intervals of 10 bpm. 3D volume rendered images and curved multi-planar reconstructions (MPRs) were acquired and image quality of the coronary arteries was rated on a 5-points scale (1 = poor image quality with many artefacts, 5 = excellent image quality) for each heart rate and each scanner by 3 observers. Paired sample t-test and Wilcoxon Signed Ranks test were used to assess clinically relevant differences between both modalities.

Results

The mean image quality scores at 70, 100 and 110 bpm were significantly higher for DSCT compared to SSCT. The overall mean image quality scores for DSCT (4.2 ± 0.6) and SSCT (3.0 ± 1.1) also differed significantly (p < 0.001).

Conclusion

These initial results show a clinically relevant overall higher image quality for DSCT compared to SSCT, especially at heart rates of 70, 100 and 110 bpm. With its comparatively high image quality and low radiation dose, DSCT appears to be the method of choice in CT coronary angiography at heart rates above 70 bpm.  相似文献

We explored quantitative parameters of image quality in consecutive patients undergoing 64-slice multi-detector computed tomography (MDCT) coronary angiography for clinical reasons. Forty-two patients (36 men, mean age 61 +/- 11 years, mean heart rate 63 +/- 10 bpm) underwent contrast-enhanced MDCT coronary angiography with a 64-slice scanner (Siemens Sensation 64, 64 mm x 0.6 mm collimation, 330 ms tube rotation, 850 mAs, 120 kV). Two independent observers measured the overall visualized vessel length and the length of the coronary arteries visualized without motion artifacts in curved multiplanar reformatted images. Contrast-to-noise ratio was measured in the proximal and distal segments of the coronary arteries. The mean length of visualized coronary arteries was: left main 12 +/- 6 mm, left anterior descending 149 +/- 25 mm, left circumflex 89 +/- 30 mm, and right coronary artery 161 +/- 38 mm. On average, 97 +/- 5% of the total visualized vessel length was depicted without motion artifacts (left main 100 +/- 0%, left anterior descending 97 +/- 6%, left circumflex 98 +/- 5%, and right coronary artery 95 +/- 6%). In 27 patients with a heart rate < or = 65 bpm, 98 +/- 4% of the overall visualized vessel length was imaged without motion artifacts, whereas 96+/-6% of the overall visualized vessel length was imaged without motion artifacts in 15 patients with a heart rate > 65 bpm (p < 0.001). The mean contrast-to-noise ratio in all measured coronary arteries was 14.6 +/- 4.7 (proximal coronary segments: range 15.1 +/- 4.4 to 16.1 +/- 5.0, distal coronary segments: range 11.4 +/- 4.2 to 15.9 +/- 4.9). In conclusion, 64-slice MDCT permits reliable visualization of the coronary arteries with minimal motion artifacts and high CNR in consecutive patients referred for non-invasive MDCT coronary angiography. Low heart rate is an important prerequisite for excellent image quality.  相似文献

OBJECTIVES: The aim of this study was to quantify image quality gains of a moving coronary plaque phantom using dual-source computed tomography (DSCT) providing 83 milliseconds temporal resolution in direct comparison to 64 slice single-source multidetector CT (MDCT) with a temporal resolution of 165 milliseconds. MATERIALS AND METHODS: Three cardiac vessel phantoms with fixed 50% stenosis and changing plaque configurations were mounted on a moving device simulating cardiac motion. Scans were performed at a simulated heart frequency of 60 to 120 bpm. Image quality assessment was performed in different anatomic orientations inside a thoracic phantom. RESULTS: A significant improvement of image quality using the DSCT could be found (P=0.0002). Relevant factors influencing image quality aside from frequency (P=0.0002) are plaque composition (P<0.0001), as well as orientation (P<0.0001). CONCLUSION: Scanning with 83 milliseconds temporal resolution improved image quality of coronary plaque at higher heart frequencies.  相似文献

目的:探讨双源CT(DSCT)对冠状动脉造影具有再次重新定义的价值。方法:对520例患者进行常规冠脉造影检查,分析双源CT冠状动脉造影的技术方法,步骤及特殊的射线剂量调控技术(适应ECG门控剂量调控技术)及其功能。结果:520例患者中心率小于100次/min的患者共有390例,占75%,其平均心率为75±5.6次/min(60—100次/min)。心率高于100的患者有130例,占25%,其平均心率为(110.6+10.8)次/min(101~130次/min)。冠状动脉重建的最佳时相均为心脏收缩期。结论:双源CT比传统CT少50%的放射剂量,在不需要控制心率的情况下完成心脏成像,提供高质量的冠状动脉和心脏图像,提高诊断冠状动脉病变的准确性。  相似文献

目的量化评估64层螺旋CT冠状动脉各分支不同重组时相图像质量,探讨冠状动脉CT成像最佳重组时相与心率关系。资料与方法102例患者均采用64层螺旋CT回顾性心电门控冠状动脉成像,男68例,女34例,平均年龄(58.1±9.7)岁,平均心率(66.4±11.5)次/min。心率<65次/min(n=43)为Ⅰ组,65~75次/min(n=34)为Ⅱ组,>75次/min(n=25)为Ⅲ组,每例患者的4支冠状动脉(左主干、左前降支、左回旋支、右冠状动脉)共分为12个节段用于图像质量分析。扫描原始数据以间隔5%在20%~80%时相分别回顾性重组冠状动脉图像,采用横断位、曲面重组、容积再现等方法对图像质量综合评分。结果Ⅰ组60%、65%和70%为最佳时相,Ⅱ组60%、65%时相为最佳时相,Ⅲ组右冠状动脉较优时相为35%、40%,左冠状动脉较优时相为60%、65%。结论心率和重组时相的选择是决定冠状动脉图像质量的重要因素。平均心率≤75次/min,冠状动脉各分支图像质量在心脏运动的舒张中期(60%、65%)最佳;>75次/min时,左右冠状动脉分别进行重组能明显提高冠状动脉的成像质量。  相似文献

In a newly developed dual-source computed tomography system (DSCT) the relation of heart rate and image quality and the possible advantages of the system’s superior temporal resolution in the evaluation of left ventricular parameters as compared to results of cardiac magnetic resonance imaging (MRI) were assessed. Coronary CT angiography was performed using a DSCT (Somatom Defintion, Siemens Medical Solutions, Forchheim, Germany) in 21 patients (mean age 62±8; 15 male, 6 female). Image quality of the coronary arteries, the heart valves, and the left ventricular myocardium was assessed using a three-point grading scale. Ten of these patients also underwent cardiac MRI for the assessment of left ventricular function, using a SSFP (steady-state free precession) sequence. Left ventricular ejection fractions (LV-EF), the end-systolic volumes (ESV), and the end-diastolic volumes (EDV) were measured employing MRI and DSCT datasets. The image quality ratings for the coronary arteries at the optimal reconstruction interval were diagnostic even in patients with high heart rates (1.42±0.49). Analysis of global LV function using DSCT quantified from CTA datasets showed a good correlation with results of cardiac MRI [EF: r=0.75 (p=0.01); ESV: r=0.72 (p=0.19); EDV: r=0.71 (p=0.02)]. The dual-source CT system offers robust image quality of the coronary arteries, independent of the heart rate, and provides combined diagnostic imaging of coronary arteries, the heart valves, the myocardium, and the global left ventricular function.  相似文献

目的:评价平均心率、心率波动和心率变异性对双源CT冠状动脉影像质量的影响。方法:100例患者进行了双源CT冠状动脉血管成像。原始数据经多期相重建优选出最佳收缩期和舒张期影像,以4分法评定系统进行影像质量评价。利用Pearson相关分析评价平均心率、心率波动和心率变异性对冠状动脉的影像质量的影响,对有相关性者行进一步分析。结果:100例患者,共纳入分析的血管为299支,296支血管可满足影像评价(99%)。平均心率、心率波动和心率变异性对总的冠状动脉影像质量没有影响,仅平均心率对舒张期冠状动脉影像质量有负影响(P〉0.05),尤其对右冠状动脉的影响更明显,70bpm以上时影像质量即下降,80bpm以上时影像质量甚至难以满足评价。结论:平均心率、心率波动和心率变异性对双源cr冠状动脉总的影像质量没有影响,但平均心率对舒张期冠状动脉的影像质量有负影响,对右冠状动脉的影响更大,当心率大于80bpm时,利用舒张期的数据难以获得RCA满意的影像质量,应该利用最佳收缩期的数据进行影像重组。  相似文献

The purpose of this study was to assess segment image quality at high heart rates using 16-slice computed tomography and differential reconstruction for major coronary vessels. According to the following protocol, 16-slice CT coronary angiography in 46 patients with a mean heart rate of 86.3+/-11.8 was reconstructed. At three transverse planes, preview series were obtained and motion artifacts evaluated in 5% increments from 0-95% within the cardiac cycle. Relying on image quality in the previews, reconstructions were performed at three z-positions for each patient. Segment image quality was assessed in terms of artifacts and visibility. The effects of heart rate and trigger delay on image quality were analyzed. Optimal image quality was achieved at 25 to 35% of the cardiac cycle for the left circumflex (CX) and right coronary artery (RCA) or 30 to 40% for the left main (LM) and left anterior descending artery (LAD). Sixteen-slice CT and differential reconstruction produced good image quality with a low percentage of motion-degraded proximal and middle segments (8.8%). Grades were 1.5 for the LM, 1.9 for the LAD, 2.0 for the CX and 2.3 for the RCA. At high heart rates, good image quality of the coronary arteries is achieved by 16-slice CT and a sophisticated reconstruction strategy at peak to late systole.  相似文献

The lower the heart rate the better image quality in multislice computed tomography (MSCT) coronary angiography. We prospectively assessed the influence of heart rate on per-patient diagnostic accuracy and image quality of MSCT coronary angiography and compared adaptive multisegment and standard halfscan reconstruction. A consecutive cohort of 126 patients scheduled to undergo conventional coronary angiography was examined with 16-slice CT. For all heart rate groups, per-patient diagnostic accuracy was significantly higher for multisegment than halfscan reconstruction with values of 95 vs. 79% (p < 0.05, <65 bpm, 38 patients), 85 vs. 66% (p < 0.05, 65–74 bpm, 47 patients), and 78% vs. 41% (p < 0.001, >74 bpm, 41 patients). Differences in diagnostic accuracy between adjacent heart rate groups were only significant for halfscan reconstruction for the comparison between the 65–74 and >74 bpm group (p < 0.05). The vessel lengths free of motion artifacts were significantly longer with multisegment reconstruction in all heart rate groups and for all coronary arteries (p < 0.005). For noninvasive MSCT coronary angiography, both per-patient diagnostic accuracy and image quality decline with increasing heart rate, and multisegment reconstruction at high heart rates yields similar results as standard halfscan reconstruction at low heart rates.  相似文献

The image quality ratings for the coronary arteries at the optimal reconstruction interval were diagnostic even in patients with high heart rates (1.42+/-0.49). Analysis of global LV function using DSCT quantified from CTA datasets showed a good correlation with results of cardiac MRI [EF: r=0.75 (p=0.01); ESV: r=0.72 (p=0.19); EDV: r=0.71 (p=0.02)]. The dual-source CT system offers robust image quality of the coronary arteries, independent of the heart rate, and provides combined diagnostic imaging of coronary arteries, the heart valves, the myocardium, and the global left ventricular function.  相似文献

OBJECTIVE: The objective of our study was to assess the effect of beta-blockers on image quality of ECG-gated 16-MDCT coronary angiography. MATERIALS AND METHODS: Coronary CT angiography was performed in two groups: group 1, 24 volunteers (mean age, 50 years; mean heart rate, 69 beats per minute [bpm]; range, 47-97 bpm); and group 2, 15 patients with current ischemic heart disease (mean age, 54 years; mean heart rate, 54 bpm; range, 48-69 bpm) who were premedicated with 20-40 mg of oral propranolol 1 hr before the study. CT scans were obtained on a 16-MDCT scanner with a 12 x 0.75 mm collimation and 420-msec rotation using nonionic contrast material (80 mL; injection rate, 4 ml/sec). Images were reconstructed at 30-80% of the cardiac cycle in increments of 5%. Image quality of the following eight coronary segments was assessed by two radiologists in consensus: left main coronary artery; proximal and middle segments of the left anterior descending (LAD) and left circumflex (LCX) coronary arteries; and the proximal, middle, and distal segments of the right coronary artery (RCA). Image quality was assessed, using a 5-point grading scale, as grades 1-5. Images assessed as grade 4 or 5 were considered to be of diagnostically acceptable quality. RESULTS: In group 1, grade 4 or 5 image quality was achieved for visualization of 92% of the left main coronary arteries; 96% of the proximal LAD coronary arteries; 88% of the middle LAD, proximal LCX, and middle LCX coronary arteries; 83% of the proximal RCAs; 58% of the middle RCAs; and 96% of the distal RCAs. In group 2, this level of image quality was achieved in 100% of the left main coronary arteries, proximal and middle LAD arteries, and proximal LCX arteries; 87% of the middle LCX arteries; and 93% of the proximal, middle, and distal RCAs. CONCLUSION: Reduction of heart rates with beta-blocker premedication improves the image quality of CT coronary angiography, especially in terms of the visualization of the right coronary artery.  相似文献
