Participation is often the comprehensive objective of treatment but also an indication of the extent to which the process of occupational therapy is client-centred. The purpose of this study was to explore levels of participation during occupational therapy among clients in the area of mental health from the occupational therapists' perspectives. Additionally the authors sought to identify factors that might hinder client participation. Postal questionnaires were sent out to 670 Swedish occupational therapists working with persons with mental illness and learning disabilities. The questionnaire required therapists to rate clients' levels of participation during occupational therapy. Findings indicated that the most common level of participation for the clients was interdependent, meaning that problems, goals, and plans were identified jointly and collaboratively with the occupational therapist. However, more than 20% of the clients were described as being dependent. Almost 90% of the occupational therapists rated client participation in therapy to be very important and nearly 70% claimed that client participation in general needed to be increased. Occupational therapists rated the primary barriers to participation as being clients' inability to participate and organizational and financial problems. Implications of these findings for education in client-centred practice approaches are discussed.  相似文献

In recent years, the occupational therapy profession has been under increased pressure to document the outcomes of interventions in order 10 improve the quality of client care and to provide evidence of the effectiveness and efficiency of services to recipients and sponsors. The literature to date has focused primarily on establishing therapy outcomes from a therapist perspective. However, attention must also be paid to the views of our consumers regarding the outcomes of therapy. The purpose of this study was to determine whether clients and therapists agree regarding client scores on three outcome measures. The study included 4 occupational therapists and 20 clients with stroke who rated clients' status in terms of personal activities of daily living, instrumental activities of daily living and quality of life, at admission of clients as well as discharge from rehabilitation. The results indicated that there was generally only moderate agreement between therapists' and clients' ratings at both admission and discharge. These findings suggest that therapists should continue to spend time exploring client status with their clients so that both parties have the best possible understanding of the clients' capabilities, and the outcome of therapy. The findings also suggest the need to administer client outcome assessments as well as to utilize client self-reports.  相似文献

Background/Aim:  Translating knowledge to practice, also called ‘knowledge translation’, is increasingly recognised as a driving force to strengthen and improve the healthcare system. How knowledge translation fits with occupational therapy practice deserves examination. Methods:  This paper will explore how an action process model, the Knowledge‐To‐Action Process, may advance knowledge translation in occupational therapy. Occupational therapists typically view knowledge in a broad sense, encompassing research, tacit knowledge, expert opinion and client evidence. The Knowledge‐To‐Action Process facilitates application of client, therapist and research knowledge to occupational therapy practice. Results:  Examination of knowledge translation through the lens of the Knowledge‐To‐Action Process creates awareness of the value of client, therapist and research knowledge. It also highlights opportunities as practitioners to implement knowledge translation. Conclusions:  Models able to flexibly reflect an occupational therapy perspective of knowledge have a potentially vital role in successful knowledge translation. Furthermore, these models allow therapists and other stakeholders to analyse complex situations and identify targeted knowledge translation strategies.  相似文献

Each year millions of the world's children witness, are victimized by or participate in traumatic events, placing them at risk for developing post-traumatic stress disorder. If this disorder is left untreated, a child's developmental trajectory can be compromised. Occupational therapists are in key positions to recognize the signs and symptoms of post-traumatic stress in children that interfere with functioning. Expressive therapies, long used in occupational therapy practice, are well suited to help children achieve an adaptive response to trauma. Two brief case studies are presented, describing the application of occupational therapy using expressive media with children who experienced post-traumatic stress disorders. The results of this preliminary investigation suggest that occupational therapists working in early intervention programmes can be helpful in identifying children in need and in providing treatment based on expressive therapies, stress management and cognitive-behavioural modalities. Further research is recommended to examine the efficacy of different occupational therapy frames of reference in treating children with post-traumatic stress disorder. Aim:  This paper documents the subjective experience of a Western trained occupational therapist as she establishes a service in a community that is developing its health-care services. The community is located in the Dominican Republic.
Method and Results:  Ethnographic interviewing was used to document the tasks and events that occurred during this 6 month project. Challenges arose related to the region's developing health, education and community services, training the local workers and the reaction of the recipients of occupational therapy service. The contrast in beliefs, values and cultural customs between the therapist and the local people contributed to the challenge.
Conclusion:  This study indicates that exporting Western occupational therapy services without any changes causes significant conflict for the professional and the clients. Furthermore, this study demonstrates the importance of understanding cultural differences between the therapist and client, as well as the need for occupational therapy services in communities that seek to improve the health and abilities of the local people requiring rehabilitation services.  相似文献

Home health occupational therapy plays an important and growing role in patients' recovery. Home health treatment is unique in that the therapist is a visitor in the patient's home environment. A phenomenological approach was used to examine perceptions of five individuals who were interviewed twice to collect their impressions of home health occupational therapy. Participants' goals for therapy as solicited by home health occupational therapists and participants perceived goals were not always pursued collaboratively. Therapeutic and compensatory occupations were incorporated less often than purposeful exercise. Participants valued those occupations engaged in, despite limited physical recovery. Participants' perceptions of occupational therapists were generally positive. Participants were unprepared for discharge, which was perceived as significant and they as helpless to prevent it despite desire for continued services. Few occupations reported in their treatment coincided with participants' difficulty distinguishing between occupational and physical therapies. Continued research focused on patients' perceptions of home health occupational therapy would be useful in further examining this phenomenon and would enlighten home health occupational therapy practice.  相似文献

SUMMARY

Establishing a partnering relationship between the occupational therapy assistant (OTA) and the occupational therapist (OT) is imperative in order to create an optimal working environment and deliver effective client care. A developmental model that provides students with opportunities to practice such a relationship has been used by an OTA and OT program in southwestern Ohio. This developmental Partnering Model gives students an opportunity to interact, work as teams, and collaborate during their educational experience. Each aspect of the model provides students with experiential learning opportunities. Educators will serve their OTA and OT students well in providing opportunities for them to develop and practice the skills they will need to create an optimal working environment and deliver effective client care as occupational therapy practitioners.  相似文献

OBJECTIVE: To describe the extent and nature of demonstrated professional partnerships between occupational therapists and Aboriginal health workers in rural and remote communities of North Queensland. The study identifies ways in which professional partnerships improve client services and enhance occupational therapy outcomes through exploring the aspects of communication, collaboration and bridging cultural boundaries. DESIGN: Data collected via in-depth, semistructured telephone interviews. SETTING: Aboriginal and mainstream health and human service organisations in rural and remote North Queensland. Rural and remote areas were identified using the Accessibility and Remoteness Index of Australia codes. PARTICIPANTS: Seven participants working in rural and remote areas of North Queensland, comprising four occupational therapists and three Aboriginal health workers. All participants were female. RESULTS: Participants identified five core themes when describing the extent and nature of professional partnerships between occupational therapists and Aboriginal health workers. Themes include: professional interaction; perception of professional roles; benefits to the client; professional interdependence; and significance of Aboriginal culture. According to participants, when partnerships between occupational therapists and Aboriginal health workers were formed, clients received a more culturally appropriate service, were more comfortable in the presence of the occupational therapist, obtained a greater understanding of occupational therapy assessment and intervention, and felt valued in the health care process. CONCLUSIONS: This study substantiates the necessity for the formation of professional partnerships between occupational therapists and Aboriginal health workers. The findings suggest that participation in professional partnerships has positive implications for occupational therapists working with Aboriginal clients and Aboriginal health workers in rural and remote regions of North Queensland.  相似文献

In a client-centred approach, clients and therapists work together to define the occupational performance problem, the focus of and need for intervention and the preferred outcomes. Application of specific theories or techniques to involve clients in goal-setting may influence the therapist to use a client participation approach. The Canadian Occupational Performance Measure (COPM) presents a structure for formulating the treatment goals identified by the client in co-operation with the therapist. The aim of this study was to test the responsiveness of the Swedish version of the COPM. After translation into Swedish, the COPM was introduced to 21 occupational therapists who performed data collection. A sample of 108 clients within geriatric, neurologic and orthopaedic rehabilitation identified 418 problems at initial scoring and reassessment. Inclusion criteria for patients were the need for rehabilitation interventions and the ability to communicate well enough in an interview. The results indicate that the Swedish version of the COPM is responsive to change, with 73% of the problems identified having a change in score of 2 points or more.  相似文献

Creative activities have historically been used in occupational therapy, and although their usage has declined in recent decades, they are still used in Swedish practice. The aim of this study was to better understand how occupational therapists use creative activities in practice. A web‐based survey was sent to 520 occupational therapists, of which 304 (58.5%) responded. The main reason identified for using creative activities was to strengthen the client's occupational performance, well‐being and self‐esteem. The expected outcomes of applying creative activities were to support the client in self‐expression and experiencing joy and desire. More than half of the occupational therapist respondents did not use creative activities to the extent they desired. Creative activities in occupational therapy are still used as a vital treatment to strengthen the clients' occupational performance abilities. The survey has looked at only a professional perspective on effectiveness of applying creative activities in occupational therapy. More research is needed to evaluate how occupational therapists internationally apply creative activities. Physical therapists' (PTs') perceptions of 26 physical therapy tasks regarding complexity, uniqueness to PTs, and delegation were studied. A questionnaire describing 26 PT tasks was sent to 200 PTs in Pennsylvania identified as working in acute care, rehabilitation, or private practice. The response rate was 45.5%. Eleven of the 26 tasks identified as most unique to PTs were also identified as highly complex. None of these 11 tasks was delegated frequently to a nurse or a PT aide, and only two were delegated to an occupational therapist. All 11 unique tasks were delegated to the physical therapist assistant (PTA), with some level of supervision provided by the majority of respondents. Results indicated there are activities within patient care that are perceived by PTs to be unique to their profession. Also, the role of the PTA may be expanding, warranting further research in this area.  相似文献

The aim of this study was to describe how people with developmental disabilities experienced the use of time aids one year after their introduction by an occupational therapist. Data were obtained through semi-structured interviews. The analysis was performed using a phenomenographic qualitative approach. The results showed that independence and autonomy should be considered as two separate phenomena. Increased independence did not always lead to increased autonomy or vice versa. Four different relationships between these two phenomena were found. They all illustrate the different priorities of the participants and different levels of independence and autonomy. Concerning the usability of time aids, the occupational therapist has to recognize the importance of having frequent communication with the client to understand the phenomena that may affect the use of the aids. The occupational therapist should always try to involve both the client and significant support persons while introducing time aids. Future research in time aids with clients with developmental disabilities could involve multiple interviews with participants combined with participant observations and interviews.  相似文献

The purpose of this qualitative study was to explore clients’ perspectives on the outcome of occupational therapy practice in rehabilitation and, furthermore, to look at certain components of practice and explore whether and how they may influence clients’ practice experience. Data were gathered through open, unstructured interviews with 20 adult clients after discharge from rehabilitation institutions in Iceland. Analysis of data was performed using both the constant comparative method and the coding procedures of the grounded theory approach. Results emerged around three interwoven categories: balance and enjoyment, enabling everyday life, and building a new future. Although mainly positive, people did also offer some negative points of view. Positive outcomes of occupational therapy are experienced at different levels and enhanced by organization of the therapy process, purpose and meaning of occupations, and the relationship between client and therapist. It is concluded that occupational therapists working in rehabilitation may have to pay more attention to their clients’ occupational issues and needs and involve them more in a goal-directed therapy process. Occupations used in therapy should be more balanced to meet the different needs of men and women and reflect the whole occupational spectrum of each individual.  相似文献

Abstract

Aims: The purpose of this pilot study was to assess the feasibility of a quantitative approach to study occupational therapist–client interactions. Role plays were videotaped in which 10 therapists met three client-actors. A questionnaire assessed the occupational therapists' and the patient-actors' opinion of the role-play experience. A modified version of the Roter Interaction Analysis System (RIAS), OT-RIAS, was used to analyse data. Major findings: Occupational therapists considered the role play experience to be “highly realistic” (mean score 5.4 on a 1–6 Likert scale). The actors' comfort role playing clients was also high (mean = 4.0; range 1–5). Inter-coder reliability for the OT-RIAS was highly significant (r = 0.919; p = 0.000). The modified version of this system fits well in an occupational therapy context. Principal conclusion: The quantitative approach used in this study to explore the therapist–client relationship appears to be effective. The role plays and RIAS may be useful in research exploring correlation between the OT communication style and outcome measures such as client satisfaction or comprehension, or in order to assess and offer feedback in teaching communication skills programmes.  相似文献

The therapeutic effects of music are being recognized increasingly in the field of rehabilitation medicine. More music therapists are being employed in physical medicine and rehabilitation centres, with the goal of using music therapy services to assist in the physical recovery and health maintenance of clients. In spite of the benefits of physical rehabilitation programs, client participation to their full potential is often not observed. Music encourages participation in exercises and activities. Music can ease the discomfort and difficulty associated with exercise and therapy activities, and help ensure consistent participation. One of the goals of the music therapist is to provide a means for the client to express him/herself in a musical activity. Music therapy and rehabilitation medicine are starting to find a common niche in working together with clients who have various neurological, orthopaedic, and paediatric conditions. Therapeutic application of music in rehabilitation contributes to the quality of life of individuals with disabilities. Combined goals could include improving strength, range of motion, balance, communication, and cognition. Continued efforts in clinical practice and research will build on the information already available to further define possible applications of music therapy in rehabilitation, and its outcome and benefits. Occupational therapists can use the therapeutic medium of music, and the services of the music therapy discipline, in assisting clients to maximize their functional independence in their daily occupational roles.  相似文献
