目的:分离培养大鼠胰腺星状细胞,为体外研究胰腺纤维化提供细胞模型。方法:采用酶消化结合Nycodenz密度梯度离心法分离胰腺星状细胞,通过倒置显微镜观察细胞形态,油红O染色和免疫细胞化学染色desmin,α-SMA进行细胞鉴定,并绘制传代后细胞生长曲线。结果:原代培养的大鼠胰腺星状细胞呈星形或梭形生长;原代胰腺星状细胞油红O染色胞浆均可见红色密集脂滴,直至传代后消失;原代胰腺星状细胞培养48 h后,desmin表达开始减弱,α-SMA表达开始增强;传代后desmin基本不表达,α-SMA阳性表达100%;传代后第3 d-5 d的胰腺星状细胞处于快速增殖阶段。结论:酶消化结合Nycodenz密度梯度离心法分离培养大鼠胰腺星状细胞纯度高,重复性好,可以满足体外研究的需要。  相似文献

目的:采用雨蛙素反复腹腔注射诱导小鼠慢性胰腺炎,评估胰腺发生纤维化的程度及病变特点。方法:健康雄性C57BL/6小鼠20只,随机分为两组,模型组反复腹腔注射雨蛙素,对照组腹腔注射与模型组等体积的生理盐水,于第7周第1d留取胰腺组织,光镜下进行胰腺腺泡细胞萎缩、炎性细胞浸润、间质纤维组织增生评分;另一部分行苦味酸-天狼星红染色,偏振光显微镜下观察胰腺间质胶原增生情况;第三部分行免疫组织化学染色,观察α-SMA表达情况。结果:模型组小鼠胰腺HE染色显示明显腺泡细胞萎缩、炎性细胞浸润及纤维化;苦味酸-天狼星红染色显示显著胰腺间质胶原增生,评分明显升高;免疫组织化学染色显示星状细胞活化标志物α-SMA表达明显增强。结论:雨蛙素反复多次腹腔注射可诱导小鼠产生明显的胰腺纤维化病变,纤维化发生机制可能与胰腺星状细胞活化分泌过多细胞外间质有关。  相似文献

Renal transplantation is the optimum treatment for end‐stage renal failure. B cells have been identified in chronic allograft damage (CAD) and associated with the development of tertiary lymphoid tissue within the human renal allograft. We performed renal transplantation in mice to model CAD and identified B cells forming tertiary lymphoid tissue with germinal centers. Intra‐allograft B220+ B cells comprised of IgMhigh CD23? B cells, IgMlo CD23+ B cells, and IgMlo CD23? B cells with elevated expression of CD86. Depletion of B cells with anti‐CD20 was associated with an improvement in CAD but only when administered after transplantation and not before. Isolated intra‐allograft B cells were cultured and shown to synthesize multiple cytokines, the most abundant of these were GRO‐α (CXCL1), RANTES (CCL5), IL‐6 and MCP‐1 (CCL2). Tubular loss was observed with T cell accumulation within the allograft and development of interstitial fibrosis, whilst type III collagen deposition was observed in areas of F4/80+ macrophages and PDGFR‐β+ and transgelin+ fibroblasts, all of which were reduced by B cell depletion. We have shown that intra‐allograft B cells are key mediators of CAD. B cells possibly contribute to CAD by intra‐allograft secretion of cytokines and chemokines.

Introduction

Pancreatic trauma occurs in approximately 4% of all patients sustaining abdominal injuries. The pancreas has an intimate relationship with the major upper abdominal vessels, and there is significant morbidity and mortality associated with severe pancreatic injury. Immediate resuscitation and investigations are essential to delineate the nature of the injury, and to plan further management. If main pancreatic duct injuries are identified, specialised input from a tertiary hepatopancreaticobiliary (HPB) team is advised.

Methods

A comprehensive online literature search was performed using PubMed. Relevant articles from international journals were selected. The search terms used were: ‘pancreatic trauma’, ‘pancreatic duct injury’, ‘radiology AND pancreas injury’, ‘diagnosis of pancreatic trauma’, and ‘management AND surgery’. Articles that were not published in English were excluded. All articles used were selected on relevance to this review and read by both authors.

Results

Pancreatic trauma is rare and associated with injury to other upper abdominal viscera. Patients present with non-specific abdominal findings and serum amylase is of little use in diagnosis. Computed tomography is effective in diagnosing pancreatic injury but not duct disruption, which is most easily seen on endoscopic retrograde cholangiopancreaticography or operative pancreatography. If pancreatic injury is suspected, inspection of the entire pancreas and duodenum is required to ensure full evaluation at laparotomy. The operative management of pancreatic injury depends on the grade of injury found at laparotomy. The most important prognostic factor is main duct disruption and, if found, reconstructive options should be determined by an experienced HPB surgeon.

Conclusions

The diagnosis of pancreatic trauma requires a high index of suspicion and detailed imaging studies. Grading pancreatic injury is important to guide operative management. The most important prognostic factor is pancreatic duct disruption and in these cases, experienced HPB surgeons should be involved. Complications following pancreatic trauma are common and the majority can be managed without further surgery.  相似文献

Introduction  The presentation and outcome of patients with acinar cell carcinoma (ACC) of the pancreas compared to the more common ductal cell adenocarcinoma (DCA) may be distinct. This study combines the experience with ACC from multiple academic institutions to better understand its natural history and outcomes. Methods  This study is a multi-institutional retrospective review of patients with ACC. Results  Between 1988 and 2008, 17 patients were identified with pathologically confirmed ACC. Median age at presentation was 59 years. Common presenting symptoms were abdominal pain (60%), back pain (50%), and weight loss (45%). Fifteen patients underwent 16 operations: pancreaticoduodenectomy (nine), distal pancreatectomy (four), and exploratory laparotomy (three). Mean tumor size was 5.3 cm. American Joint Commission on Cancer tumor stages were stage I (two), stage II (eight), stage III (four), and stage IV (three). Overall, 1- and 5-year survival rates were 88% and 50%, respectively. In resected cases (13), 1- and 5-year survival rates were 92% and 53%, respectively. Median survival in resected cases was 61 months. This is in contrast to 1,608 patients with ductal cell adenocarcinoma who underwent resection identified from recent literature reports where the average median survival was only 24 months. There was no discernable difference in the outcomes of patients with ACC between United States and Germany patients. Conclusion  Acinar cell carcinoma of the pancreas is rare and appears to have a presentation and outcome distinct from the more common pancreatic DCA. Our aim was to study the impact of subclinical inflammation on the development of interstitial fibrosis and tubular atrophy (IF/TA) on a 1‐year protocol biopsy in patients on rapid steroid withdrawal (RSW). A total of 256 patients were classified based on protocol biopsy findings at months 1 or 4. Group 1 is 172 patients with no inflammation, group 2 is 50 patients with subclinical inflammation (SCI), group 3 is 19 patients with subclinical acute rejection (SAR) and group 4 is 15 patients with clinical acute rejection (CAR). On the 1‐year biopsy, more patients in group 2 (SCI) (34%, p = 0.004) and group 3 (SAR) (53%, p = 0.0002), had an IF/TA score > 2 compared to group 1 (control) (15%). IF/TA was not increased in group 4 (CAR) (20%). The percent with IF/TA score > 2 and interstitial inflammation (Banff i score > 0) was higher in group 2 (16%, p = 0.004) and group 3 (37%, p < 0.0001) compared to group 1 (3%). In a multivariate analysis, patients in groups 2 or 3 had a higher risk of IF/TA score > 2 on the 1‐year biopsy (OR 6.62, 95% CI 2.68–16.3). We conclude that SCI and SAR increase the risk of developing IF/TA in patient on RSW.  相似文献

目的:探讨白细胞介素18(IL-18)对肾小管萎缩和间质纤维化的作用.方法:在体外实验中应用不同浓度的IL-18处理人类近端肾小管上皮细胞株(HK-2).应用Hoechst 33258染色法检测细胞凋亡;应用流式细胞技术和免疫组化技术检测细胞表达α-平滑肌肌动蛋白(α-SMA)表达和细胞形态学的变化.结果:低浓度短时间 IL-18作用对细胞凋亡无影响,而较高浓度IL-18作用可促使肾小管上皮细胞凋亡;IL-18具有显著促进肾小管上皮细胞表达α-SMA作用,并呈剂量和时间依赖关系,而且表达α-SMA的细胞具有肌成纤维细胞的形态学特征.结论:肾病状态下IL-18可能具有促进肾小管萎缩和肾间质纤维化的作用.  相似文献

Surgical resection of pancreatic cancer offers the only chance for prolonged survival. Pancretic resections are technically challenging, and are accompanied by a substantial risk for postoperative complications, the most significant complication being a pancreatic fistula. Risk factors for development of pancreatic leakage are now well known, and several prophylactic pharmacological measures, as well as technical interventions have been suggested in prevention of pancreatic fistula. With better postoperative care and improved radiological interventions, most frequently complications can be managed conservatively. This review also attempts to address some of the controversies related to optimal management of the pancreatic remnant after pancreaticoduodenectomy.  相似文献

Clinically relevant fistula after distal pancreatic resection occurs in 5–30% of patients, prolonging recovery and considerably increasing in-hospital stay and costs. We tested whether routine drainage of the pancreatic stump into a Roux-en-Y limb after distal pancreatic resection decreased the incidence of fistula. From October 2001, data of all patients undergoing pancreatic distal resection were entered in a prospective database. From June 2003 after resection, the main pancreatic duct and the pancreatic stump were oversewn, and in addition, anastomosed into a jejunal Roux-en-Y limb by a single-layer suture (n = 23). A drain was placed near the anastomosis, and all patients received octreotide for 5–7 days postoperatively. The volume of the drained fluid was registered daily, and concentration of amylase was measured and recorded every other day. Patient demographics, hospital stay, pancreatic fistula incidence (≥30 ml amylase-rich fluid/day on/after postoperative day 10), perioperative morbidity, and follow-up after discharge were compared with our initial series of patients (treated October 2001–May 2003) who underwent oversewing only (n = 20). Indications, patient demographics, blood loss, and tolerance of an oral diet were similar. There were four (20%) pancreatic fistulas in the “oversewn” group and none in the anastomosis group (p < 0.05). Nonsurgical morbidity, in-hospital stay, and follow-up were comparable in both groups. Introduction  Earlier detection of pancreatic cancer may help identify patients for whom surgical intervention could provide cure or prolong life. In this article, we report the occurrence of breast cancer, melanoma, squamous cell carcinoma of the alveolar ridge, colon cancer, a desmoid tumor of the abdominal wall, and pancreatic adenocarcinoma in a 65-year-old woman. She was identified as having the familial atypical multiple mole melanoma–pancreatic cancer syndrome (FAMMM–PC) with a germline p16 mutation at amino acid position 15. Discussion  Patients with this syndrome traditionally present with multiple nevi and melanoma, and a subset also present with other cancers, including pancreatic cancer; however, no FAMMM–PC patient has yet been described with this constellation of cancers, including squamous cell carcinoma of the alveolar ridge and a desmoid tumor. Recognition of the tumors this population of patients is susceptible to developing and their genetic associations can help guide the surgeon in screening, surveillance, and eventually prevention of many of these cancers.  相似文献

Ischemia reperfusion injury (IRI) is pivotal for renal fibrosis development via peritubular capillaries injury. Coagulation represents a key mechanism involved in this process. Melagatran® (M), a thrombin inhibitor, was evaluated in an autotransplanted kidney model, using Large White pigs. To mimic deceased after cardiac death donor conditions, kidneys underwent warm ischemia (WI) for 60 min before cold preservation for 24 h in University of Wisconsin solution. Treatment with M before WI and/or in the preservation solution drastically improved survival at 3 months, reduced renal dysfunction related to a critical reduction in interstitial fibrosis, measured by Sirius Red staining. Tissue analysis revealed reduced expression of transforming growth factor-β (TGF-β) and activation level of its effectors phospho-Smad3, Smad4 and connective tissue growth factor (CTGF) after M treatment. Fibrinolysis activation was also observed, evidenced by downregulation of PAI-1 protein and gene expression. In addition, M reduced S100A4 expression and vimentin staining, which are markers for epithelial mesenchymal transition, a major pathway to chronic kidney fibrosis. Finally, expression of oxidative stress markers Nox2 and iNOS was reduced. We conclude that inhibition of thrombin is an effective therapy against IRI that reduces chronic graft fibrosis, with a significantly positive effect on survival.  相似文献

Patients with end‐stage cystic fibrosis (CF) and severe CF‐related diabetes (CFRD) may benefit from combined lung‐pancreatic islet transplantation. In the present study, we report the long‐term follow‐up of four end‐stage CF patients treated with combined bilateral lung and pancreatic islet transplantation from the same donor. All patients were C‐peptide negative (<0.5 μg/L) and inadequately controlled despite intensive insulin treatment. One patient was transplanted with 4 019 ± 490 islet equivalent/kg injected into the transverse colic vein using a surgical approach. In the remaining three patients, islets were cultured for 3–6 days and transplanted by percutaneous transhepatic catheterization of the portal vein. In all patients, islet allograft recovery was recognized by elevation in the plasma level of C‐peptide (>0.5 μg/L). At 6 months after transplantation, one patient showed multiple episodes of acute lung transplant rejection and a progressive decline in pancreatic islet cell function. Three out of four patients experienced an improved control of glucose levels with a HbA1c of 5.2%, 7% and 6% respectively at 1.5, 2 and 15 years follow‐up. Compared with the pretransplant period, there was a 50% reduction in mean daily insulin needs. Pulmonary function remained satisfactory in all patients. In conclusion, our cases series shows that combined bilateral lung and pancreatic islet transplantation may be a viable therapeutic option for patients with end‐stage CF and CFRD.  相似文献

A 27-year-old man, an alcohol abuser, had alcoholic pancreatitis complicated by a 3.2 cm pancreatic tail speudocyst and intrasplenic pseudocysts presenting with left upper quadrant pain of one-month duration. Surgical resection or percutaneous drainage of the cystic lesions of the pancreas and the spleen was refused. Analgesic agents were given for relief of abdominal pain. Three months later, another episode of alcoholic pancreatitis occurred. A computed axial tomographic scans of the abdomen showed diffuse enlargement of the pancreas with a 3.0-cm pseudocyst in the pancreatic tail, but there was no evidence of previous intrasplenic pseudocysts. The patient was treated conservatively and was discharged on the 7th hospital day. There was no recurrence of abdominal pain after 3 months follow-up.  相似文献
