Objective To understand the serum adiponectin levels in normal subjects and type 2 diabetes patients with macrovascular complications, to investigate the correlation between adipnectin and macrovascular complications in type 2 diabetes patients. Methods One hundred and two normal subjects, 116 type 2 diabetes patients and 123 type 2 diabetic patients with macrovascular complications were recruited in the current study. The serum adiponectin levels among three groups were compared, and the factors affecting the serum adiponectin were investigated. Results ①The serum adiponectin level was significantly lower in type 2 diabetic patients (8. 62 ± 2. 97) mg/L than that in normal subjects (10. 03 ± 4.41) mg/L, and was the lowest in type 2 diabetic patients with macrovascular complications(6. 17 ± 2. 55) mg/L(P < 0.05). ②Serum adiponetin level was negatively correlated with BMI,WHR,HOMA-IR,fasting insulin level, HbAlC and TG(r = -0.492, -0. 581, -0. 813, -0. 754, -0.619, -0.387, P<0.05). ③In a general multivariate regression, HOMA-IR fasting insulin and HbAlc.were negatively correlated with serum adipnectin level (r = - 0. 828, - 0. 769, - 0. 631, P < 0. 01). Conclusions The serum adiponectin level in type 2 diabetic patients is significantly decreased and even more in type 2 diabetic patients with macrovascular complications. These results suggest that lower serum adiponectin level is related to macrovascular complications in type 2 diabetic patients and maybe plays an important role in atherosclerosis in type 2 diabetic patients.  相似文献

Objective To study the relationship between serum level of homocysteic acid(Hcy)and type 2 diabetic mellitus(T2DM),and to explore the influencing factors serum for Hey level in T2DM patients.Methods Totally T2DM 122 patients(T2DM group),67 patients with chronic kidney diease(CKD group)and 50 healthy controls(CON group)were enrolled in the investigation.The variation of serum Hcy of all groups was observed,and the correlation of serum Hcy level with estimated glomerular filtration rate(eGFR),fasting serum glucose,HbA1C,Urea and creatinine was analyzed.Serum Hcy was measured by enzymatic cycling assay;HbA1C was measured by high-pressure liquid chromatography assay;and eGFR was calculated by using the Levey-modified Modification of Diet in Renal Disease formula.Results There was no significant difference in serum level of Hcy between T2DM group and CON group(P>0.05).The serum concentration of Hey was significantly higher in CKD group than that of T2DM group and CON group(P<0.01).There was only 8.20% exceeding the upper limit of 95% of CON group in T2DM group,while 64.18% exceeding the upper limit of 95% of CON group in CKD group.There was no significant difference of Hcy level between patients with eGFR≥90 mL·(min·1.73 m2)-1 in T2DM group and healthy controls(P>0.05),but the Hey level was significantly higher in patients with eGFR<90 mL·(min·1.73 m2)-1 than that of healthy controls(P<0.05).In T2DM group and CKD group,serum Hey level was negatively correlative to eGFR(P<0.01).positively correlative to Urea and creatinine(P<0.01);but serum Hcy level did not correlate with fasting serum glucose,HbA1C,total cholesterol and triglyceride(P>0.05).Conclusion Renal function is an independent risk factor for serum homocysteine level in type 2 diabetes rather than diabetes itself.  相似文献

Objective To investigate the relationship between the levels of serum leptin and oxidative stress in patients with hyperglycemia crisis. Methods A total of 96 patients with diabetic ketoaeidosis (DKA) and nonketotic hyperglycemia (NKH) were treated on a low-dose insulin protocol using intravenous infusion of insulin with the established rate of 0.1U·kg-1·h-1,with the patients on intravenous fluids and receiving nutrition by mouth and vein. The levels of serum leptin, 8-iso-prostaglandin F2α (8-iso-PGF2α, the activities of superoxide dismutase (SOD), total antioxidant capacity (TAC) and the contents of malondialdehyde (MDA) in 96 patients with hyperglycemia crisis on admission and after insulin therapy with resolution of hyperglycemia and ketoacidosis (72 hours) were measured. Another 35 healthy individuals served as normal control. Results The activities of SOD, TAC and the levels of leptin before treatment were lower in patients with hyperglycemia crisis than in normal controls, and the levels of MDA and 8-iso-PGF2a were more markedly elevated than those in normal controls (all P<0. 05). The activities of SOD, TAC and the levels of leptin in patients after treatment were significantly higher than those in patients before treatment, and the levels of MDA and 8-iso-PGF2a. were significantly lower than those in patients on admission (all P<0. 05). There was significant positive correlation between leptin and MDA in patients before treatment (r=0. 38, P<0. 05), and the level of leptin was negatively correlated with MDA and 8-iso-PGF2a in patients after treatment (r1 = - 0. 35, r2= - 0. 37, both P < 0. 05). In stepwise regression analysis, MDA and 8-iso-PGF2α showed a significant association with leptin. Conclusion The levels of leptin are significantly lowered in patients with hyperglycemia crisis.
