目的探讨血管腔内治疗脾动脉瘤的安全性和有效性。方法回顾性分析2010年1月至2014年12月本科收治的48例脾动脉瘤患者的资料,腔内治疗方法:弹簧圈动脉瘤(14例)或载瘤动脉栓塞术(19例),覆膜支架隔绝术(2例),支架辅助弹簧圈瘤体内填塞(5例)和多层裸支架隔绝术(8例)。术后1、3、6、12个月采用CT血管造影检查随访,记录并评价围手术期和随访期的临床结果指标。结果本组病例瘤体均治疗成功,支架植入患者的脾动脉均通畅。围手术期无手术相关死亡,8例患者弹簧圈栓塞后出现栓塞后综合征,均于3~5天后缓解。随访时间23.9(3~59)个月,采用弹簧圈栓塞瘤体或载瘤动脉33例:4例患者发现有部分脾脏梗死,但无明显临床症状,2例患者因瘤体内再灌注接受再次手术;采用覆膜支架植入或裸支架辅助弹簧圈栓塞7例:动脉瘤隔绝或栓塞良好,无内漏,支架通畅;采用多层裸支架隔绝术8例:术后12个月6例(75%)患者瘤腔达到完全血栓化,分支动脉通畅。其余病例未出现严重并发症。随访期患者均未观察到动脉瘤增大、破裂或复发。结论血管腔内治疗脾动脉瘤安全,疗效显著。  相似文献

目的探讨NeuroformⅡ支架植入联合MatrixⅡ弹簧圈栓塞治疗颅内宽颈动脉瘤的疗效、技术要点、安全性及并发症防治。方法诊断为颅内宽颈动脉瘤的病人11例,其中大脑前动脉瘤2例,后交通动脉瘤4例,眼动脉瘤1例,大脑中动脉瘤2例,椎基底动脉瘤2例。7例先行NeuroformⅡ支架瘤颈成形,将微导管通过支架网眼置入动脉瘤内,填塞弹簧圈;4例先置入微导管于动脉瘤内,再释放支架后栓塞,术后3~6个月随访。结果所有病例栓塞操作均顺利完成,无手术并发症;其中致密填塞8例,部分致密填塞3例,术后病人均恢复良好,4例短期随访无再出血及血栓栓塞症状发生。结论NeuroformⅡ支架联合MatrixⅡ弹簧圈治疗颅内宽颈动脉瘤安全、有效。  相似文献

目的观察Pipeline血流导向装置(PED)治疗颅内复杂动脉瘤的有效性及安全性。方法回顾性分析接受PED治疗的11例颅内复杂动脉瘤患者的临床资料。结果11例患者共12个动脉瘤,植入12枚PED,其中10例植入1枚PED、1例2枚,3个动脉瘤单纯植入PED、9个植入PED联合弹簧圈栓塞;术后立即造影示动脉瘤内对比剂滞留,PED贴壁良好,完全覆盖瘤颈,载瘤动脉通畅。术后1例发生局部急性脑梗死,经治疗后康复。术后随访6~9个月,中位随访时间7.5个月,患者症状均明显缓解或消失,未发现严重并发症,动脉瘤闭塞率75.00%(6/9)。根据O'Kelly-Marotta(OKM)分级:A级0个动脉瘤,B级1个,C级2个,D级9个。改良Rankin评分(mRS):0分10例,1分1例。结论PED治疗颅内复杂动脉瘤的效果及安全性均较好。  相似文献

目的探讨采用弹簧圈栓塞动脉瘤及载瘤动脉治疗出血性椎动脉夹层动脉瘤的疗效及并发症。方法收集经颅脑CT及全脑DSA确诊的椎动脉夹层动脉瘤破裂出血患者17例,采用弹簧圈栓塞动脉瘤及载瘤动脉,观察疗效及并发症。结果 17例动脉瘤及载瘤动脉均完全闭塞,无死亡病例。术后当天1例发生Wallenberg综合征,恢复良好;1例术后3个月出现脑积水,行侧脑室-腹腔分流术后遗留轻度残疾;随访2~36个月,无再出血病例。术后6个月12例接受DSA复查,动脉瘤完全不显影,载瘤动脉未发生再通。结论应用弹簧圈栓塞夹层动脉瘤及载瘤动脉治疗出血性椎动脉夹层动脉瘤疗效确切,具有一定应用价值。  相似文献

目的总结血管腔内介入治疗腹腔内脏动脉瘤(Visceral artery aneurysms,VAA)的方法及临床效果。方法选取2015年1月至2020年1月确诊为VAA病例,采用保守治疗、单纯栓塞、瘤腔栓塞联合载瘤动脉栓塞、覆膜支架置入、裸支架+弹簧圈瘤腔内栓塞术式对不同类型的VAA进行治疗,统计其术后疗效。结果全组病人顺利完成治疗,无死亡病例,1次栓塞成功率(92.16%,47/51),2次成功率100%。14例保守治疗,随访期间无动脉瘤增大;6例行单纯弹簧圈栓塞、36例行载瘤动脉远近端栓塞或瘤腔栓塞联合载瘤动脉栓塞术、覆膜支架植入术7例、裸支架+弹簧圈瘤腔内栓塞术2例,治疗效果满意。结论 EVT治疗VAA具有创伤小、麻醉风险低、术后快速康复的优势,但应根据具体病情选择合适的治疗方法。  相似文献

目的探讨支架辅助弹簧圈栓塞在治疗急诊治疗颅内破裂宽颈微小动脉瘤(最大径≤3mm)中的操作技巧及临床疗效。方法回顾急诊支架辅助弹簧圈栓塞治疗的7例颅内破裂宽颈微小动脉瘤患者资料,分析治疗方法、疗效、并发症、预后及6~12个月随访结果,评价支架技术的安全性、有效性及操作技巧。结果术后即刻造影显示,完全栓塞5例,次全栓塞1例,单纯植入支架1例;未发生破裂出血及血栓栓塞事件。术后3个月改良式格拉斯哥预后评分(GOS)结果显示,6例恢复正常,1例恢复良好。6~12个月随访未发现动脉瘤再破裂出血。结论支架辅助弹簧圈急诊栓塞治疗颅内破裂宽颈微小动脉瘤安全有效。  相似文献

2015年5~12月我科行主动脉腔内修复术(endovascular aortic repair,EVAR)治疗腹主动脉瘤(abdominal aortic aneurysm,AAA),同期应用Viabahn支架重建肠系膜下动脉(inferior mesenteric artery,IMA)2例。1例肾下型AAA合并双髂总动脉瘤样扩张,Riolan动脉弓形成,行EVAR治疗AAA,同期采用"烟囱"技术IMA内植入Viabahn支架成功,术后3个月随访复查CTA示支架位置良好,瘤体隔绝成功,Viabahn支架通畅。1例右侧髂总动脉巨大动脉瘤,行EVAR治疗髂总动脉瘤,髂内动脉弹簧圈栓塞,IMA内植入Viabahn支架成功,术后6个月随访复查CTA示支架位置良好,瘤体隔绝成功,Viabahn支架通畅。我们认为复杂AAA应用Viabahn支架合理重建IMA预防肠缺血,简化手术操作,近期疗效满意。  相似文献

目的 探讨Neuroform Atlas支架在颅内动脉分叉处宽颈动脉瘤栓塞中的价值。方法 回顾性分析2021年1~9月16例采用Neuroform Atlas支架辅助弹簧圈栓塞治疗颅内动脉分叉处宽颈动脉瘤的临床和影像学资料。动脉瘤位于大脑中动脉分叉处8例,大脑前动脉分叉处4例,大脑前、中动脉分叉处2例,大脑前动脉A2远端分叉处1例,大脑后动脉P2分叉处1例。破裂动脉瘤10例(术前Hunt-Hess分级Ⅰ级6例,Ⅱ级2例,Ⅲ级2例),未破裂动脉瘤6例。结果 均在单一Neuroform Atlas支架辅助下完成栓塞,技术成功率100%。术后即刻造影显示动脉瘤完全闭塞13例,瘤颈残留2例,瘤体残留1例。术中及围手术期未观察到介入操作相关并发症。出院前改良Rankin量表(mRS)0~1分11例,2分3例,3分2例。16例随访时间3~14个月,(7.8±3.2)月。mRS评分0~1分14例,2分1例,3分1例。9例术后3~6个月行DSA,动脉瘤完全闭塞8例,瘤颈残留1例,9例均未见载瘤动脉狭窄或支架内闭塞。结论 Neuroform Atlas支架辅助弹簧圈栓塞治疗颅内动脉分叉处宽颈动脉瘤安全,...  相似文献

目的总结颅内后循环动脉瘤的特点,探讨其血管内介入治疗的临床疗效。方法回顾性分析40例行血管内介入治疗的颅内后循环动脉瘤患者的临床和影像学资料、介入治疗过程,观察期效果及术后随访结果。结果 40例患者共发现42个后循环动脉瘤,均完成血管内介入治疗,其中8个行单纯弹簧圈栓塞,28个行支架辅助栓塞,1个行Onyx胶栓塞,5个动脉瘤及载瘤动脉同时闭塞。术后即刻DSA造影显示动脉瘤完全栓塞30个,近全栓塞6个,部分栓塞6个。术后6个月随访DSA造影显示动脉瘤完全栓塞36个,近全栓塞4个,部分栓塞1个。患者出院时行改良Rankin量表(mRS)评分,0分35例,1分3例,2分1例,1例死亡为6分;出院后3~6个月随访mRS评分0分38例,1分1例,无动脉瘤复发及新发神经功能障碍病例。结论颅内后循环动脉瘤具有特殊的临床与影像学表现,且复杂动脉瘤较为常见,对于颅内后循环动脉瘤,血管内介入治疗是一种安全有效的治疗方法。  相似文献

目的观察血管内介入治疗大脑后动脉夹层动脉瘤的效果。方法回顾性分析12例接受血管内介入治疗的大脑后动脉夹层动脉瘤患者,评价治疗效果。结果12例大脑后动脉夹层动脉瘤中,P1段2例,P1-P2段2例,P2段6例,P3段2例,均成功实施栓塞治疗;对其中2例单纯以弹簧圈栓塞动脉瘤,8例以弹簧圈闭塞载瘤动脉,2例植入支架。术后即刻行复查造影显示Raymond分级1级10例,2级2例。术后4例诉头痛,经对症治疗后2周内好转;未见动脉瘤破裂出血及脑梗死。随访期间影像学检查均未见动脉瘤复发,无新发神经功能障碍及颅内再次出血;末次随访时改良Rankin量表0分10例,1分1例,2分1例。结论血管内介入治疗大脑后动脉夹层动脉瘤效果较好,且安全性较高。  相似文献

OBJECT: Intracranial stent placement combined with coil embolization is an emerging procedure for the treatment of intracranial aneurysms. The authors report their results using intracranial stents for the treatment of intracranial aneurysms. METHODS: A prospectively maintained database was reviewed to identify all patients with intracranial aneurysms that were treated with intracranial stents. Ten lesions, including eight broad-based aneurysms and two dissecting aneurysms, were treated in 10 patients. Four lesions were located in the cavernous segment of the internal carotid artery, two at the vertebrobasilar junction, two at the basilar trunk, one at the basilar apex, and one in the intracranial vertebral artery. Attempts were made to place stents in 13 patients, but in three the stents could not be delivered. Altogether, intracranial stents were placed in 10 patients for 10 lesions. Results that were determined to be satisfactory angiographically were achieved in all 10 lesions. Two patients suffered permanent neurological deterioration related to stent placement. In two patients, the aneurysm recurred after stent-assisted coil embolization. In one case of recurrence a second attempt at coil embolization was successful, whereas in the second case of recurrence parent vessel occlusion was required and well tolerated. CONCLUSIONS: Intracranial stents can be a useful addition to coil embolization by providing mechanical, hemodynamic, and visual benefits in the treatment of complex, broad-based aneurysms.  相似文献

BACKGROUND: Recent advances in stent technology have allowed for negotiation of often tortuous posterior circulation intracranial vasculature. Stent-assisted coil embolization is a novel treatment for complex wide-necked aneurysms, as stents provide a buttress that allows for coil deposition while preventing coil herniation into the parent vessel lumen. We describe a case of stent-assisted coil embolization of a complex wide-necked vertebral confluence aneurysm. CASE DESCRIPTION: A 61-year-old woman presented with a Hunt-Hess III, Fisher Grade III subarachnoid hemorrhage secondary to a ruptured vertebral confluence aneurysm demonstrated on angiography. The patient underwent emergent angiography and attempted coiling of a vertebral confluence aneurysm. Because of the aneurysm's complex wide neck and the presence of subclavian steal syndrome, the coils repeatedly herniated into the left vertebral and basilar artery lumina. A flexible coronary stent was deployed across the aneurysm neck, preventing coil herniation and allowing for greater coil deposition. The patient tolerated the procedure and underwent repeat coiling 2 months postoperatively because of mild coil compaction. This resulted in 100% occlusion and the patient is neurologically normal except for a sixth nerve palsy which had been present after the hemorrhage. CONCLUSION: Recent advances in stent technology allow negotiation of the tortuous posterior circulation vasculature. Stent-assisted coil embolization of complex, wide-necked vertebral confluence aneurysms may be an alternative intervention for these surgically challenging lesions.  相似文献

OBJECT: The WingSpan stent is a new self-expandable neurovascular stent designed for endovascular treatment of intracranial atheromatous lesions. The authors report their experience with the use of this stent for the endovascular treatment of intracranial aneurysms. METHODS: Thirty-seven patients with 40 wide-necked intracranial aneurysms were treated using the WingSpan stent. Twenty-two aneurysms (55%) were small and 18 (45%) were large or giant. In all but 4 aneurysms, embolization was completed by packing the aneurysm sac with platinum coils. In 4 dissecting aneurysms that were fusiform or too small and wide necked to be catheterized, the stent was used alone. In these cases, the stent bridged the aneurysm neck to allow for flow redirection and the potential stent-induced endothelization effect. RESULTS: Follow-up angiograms obtained in 3 of 4 aneurysms, treated with only stent placement, demonstrated aneurysmal thrombosis and parent artery remodeling in 2 patients and moderate decrease in size in 1. Follow-up angiography obtained at 6 months to 1 year in 31 aneurysms after stent-supported coil embolization demonstrated complete occlusion in 23 aneurysms (74.2%) with a progressive thrombosis rate of 66.7% (10 of 15 aneurysms), and a recanalization rate of 16.1%. CONCLUSIONS: In treating wide-necked intracranial aneurysms, the WingSpan Stent System is very flexible, secure, and effective. Its delivery system is very easy and exact in that it exerts higher outward radial force, thus providing an excellent conformability and a strong scaffold to hold the coils in place. It may offer an effective treatment when used alone in some fusiform or very wide-necked, small dissecting aneurysms in which other surgical or endovascular treatment strategies are not deemed feasible.  相似文献

BACKGROUND: The treatment of very small cerebral aneurysms with maximal diameter less than 3 mm remains a challenge for endovascular and surgical treatment. Endovascular treatment of these lesions may be difficult and associated with high risk of complications because of their small size. Our purpose was to assess the feasibility and results of endovascular treatment of these lesions. METHODS: We conducted a retrospective review of our experience and results of endovascular treatments for a series of 11 consecutive patients with 11 very small aneurysms. Of 11 aneurysms, 10 were acutely ruptured, and 1 was unruptured with a previous subarachnoid hemorrhage from another aneurysm. Aneurysms were located at the internal carotid artery (n = 4), the anterior communicating artery (n = 6), and the vertebral artery (n = 1). Seven patients were treated with coil embolization, and remodeling technique was used in 1 case. Three cases underwent intravascular stent implantation. Coil packing was done after in 2 of 3 aneurysms, and stent implantation alone was used in the remaining aneurysm. RESULTS: Coil embolization and stent deployment were carried out without difficulty in all cases. Coil packing was not available after stent implantation in 1 case for unsuccessful navigation of microcatheter into the aneurysm sac. Immediate angiography demonstrated complete occlusion in 10 cases and nearly complete occlusion in 1 case with stent implantation alone. No stent thrombosis and aneurysmal rupture was encountered during treatment. With the exception of 1 patient (Hunt and Hess grade 4) who died of pneumonia 4 weeks after treatment, no clinical evidence of neurologic deterioration and hemorrhagic complication was seen during the follow-up period in the remaining 10 patients. Follow-up angiography for 3 to 12 months (mean, 5.3 months) was available in 6 (60%) of 10 surviving patients, and no aneurysm recanalization was found. CONCLUSIONS: Endovascular treatment may be a feasible and effective therapeutic alternative for very small aneurysms. The long-term efficacy and durability of endovascular treatment for these lesions remains to be determined in a large series.  相似文献

OBJECT: The aim of this study was to report on a novel technique in which metallic embolization coils were combined with the Onyx liquid embolic agent in the aneurysm sac to achieve a more durable result after endovascular treatment. This therapeutic procedure was performed in selected cases in which, based on the authors' experiences, either coil embolization or Onyx alone would likely have failed. The authors report long-term clinical and angiographic follow-up results in 20 consecutive intracranial aneurysms treated using this combination for defined indications. METHODS: Twenty aneurysms in 20 patients were treated with a combination of embolic coils and Onyx. Four aneurysms were giant; 13, large; and three, small. This new technique was used when standard Onyx or coil treatment with balloon assistance was determined to involve a higher possibility of recanalization, because either an adjunctive stent insertion could not be performed or the Onyx technique could not be used due to an unsuccessful seal test or intraaneurysm balloon prolapse. In one case, an adjunctive stent was placed before coil placement and Onyx deposition to control the material in the sac of the aneurysm, which had a fusiform neck. All aneurysms were completely occluded after using this technique. No clinical or technical adverse events occurred in any of the cases. Follow-up angiography was performed in all patients: 3-year studies in six patients, 2-year studies in five, and 1-year studies in nine. None of these studies demonstrated aneurysm regrowth or parent artery occlusion. CONCLUSIONS: The combination of the embolic coils and the Onyx liquid embolic agent provides very durable aneurysm occlusion for defined indications.  相似文献

OBJECT: The authors investigated the feasibility, safety, and short-term outcome of stent treatment for intracranial aneurysms, stenoses, and dissections. METHODS: One hundred twenty-three consecutive patients with intracranial saccular, dissecting, and fusiform aneurysms, atherosclerotic lesions, and dissections were selected for intracranial stent implantation with or without adjunctive coil placement. One hundred eleven patients (mean age 47 years, range 3-73 years) underwent stent treatment; 12 patients (9.8%) were not treated. These 111 patients were divided into four groups: in Group 1 there were 62 patients with saccular aneurysms; Group 2 included nine patients (10 lesions) with dissecting or fusiform aneurysms; in Group 3 there were 36 patients with symptomatic intracranial atheromatous stenoses of more than 50%; and Group 4 included four patients with symptomatic intracranial dissections. All patients underwent computerized tomography scanning and/or magnetic resonance imaging and cerebral digital subtraction angiography preoperatively. Of the 72 aneurysms in Groups 1 and 2, 59 (82%) were treated with combined endovascular stent implantation and endosaccular coil placement. In 67 aneurysms (93%) we achieved complete or nearly complete obliteration. All patients with arterial narrowing achieved residual stenoses of less than 30% postangioplasty. One patient required repeated angioplasty. The morbidity rate in the series was 10.9% and the mortality rate was 6.3%. CONCLUSIONS: These findings indicate that stent treatment is feasible and seems to be an effective modality for arterial reconstruction. This versatile tool allows the treatment of a wide variety of challenging intracranial lesions.  相似文献

Background

Despite accumulated experience and improved understanding of the tools, endovascular treatment of intracranial aneurysms continues to have risks linked to the technique itself, and induces procedure-related complications. The purpose of this study was to report our series of stent salvage using the Enterprise stent for procedure-related complication during coil embolization in patients with ruptured intracranial aneurysms.

Methods

Parent artery thrombosis, parent artery dissection, and coil protrusion were considered to be the procedure-related complications. There were 18 consecutive cases (3 unruptured and 15 ruptured aneurysms) with procedure-related complications rescued by the Enterprise stent from December 2008 to December 2011. Follow-up angiography was performed in 14 of the 15 patients with ruptured aneurysms between 6 and 30 months (mean 14.6 months) after the procedure.

Results

The procedure-related complications were parent artery dissection (n?=?1), parent artery thrombosis (n?=?4), and coil protrusion (n?=?10). There was no complication related to delivering or deploying of the Enterprise stent. Initial radiographic results showed 8 cases of complete occlusion and 7 cases of neck remnant. There was no change in the angiographic results during the follow-up periods.

Conclusions

Facing with procedure-related complications during coil embolization of ruptured intracranial aneurysms, the closed-cell designed Enterprise stent might be a useful option for the salvage technique by restoring blood flow and minimizing thromboembolic events.  相似文献

Background and Object Complex intracranial aneurysms present a treatment challenge for both open and endovascular modalities of treatment. This report seeks to illustrate a series of patients with aneurysms treated with telescoping stents as a method of flow diversion for small and fusiform intracranial aneurysms.Material and Methods A retrospective evaluation of six patients treated with a telescoping stent technique utilizing available stents (at that time before the pipeline era) for complex cerebral aneurysms between January 2009 and January 2010 was performed. Five patients had dissecting aneurysms and one patient had a small superior hypophyseal artery aneurysm. One of the patients was treated in the setting of a Hunt and Hess grade IV subarachnoid hemorrhage. Follow-up cerebral angiography was performed postprocedure at 6 months.Results At a mean follow-up period of 9 months, all the patients experienced complete or near-complete occlusion (>95%). No periprocedural complications were noted in this series. No episodes of hemorrhage or thromboembolic complications occurred.Conclusions Overlapping Neuroform and Enterprise stents may induce complete thrombosis of intracranial aneurysms and facilitate parent artery remodeling. The use of self-expanding stents is still an adequate treatment modality, especially if there is a need for vessel wall stabilization rather than flow diversion. The technique is also a sufficient alternative in small intracranial aneurysms not readily amenable to open surgical treatment or traditional endovascular coil embolization.  相似文献

We describe a case of the combined application of endovascular stent implantation and Guglielmi detachable coil packing for the treatment of a vertebro-basilar fusiform aneurysm and review the literature on stent placement to treat cerebral aneurysms. A 70-year-old female presented with an acute headache from subarachnoid hemorrhage. A fusiform aneurysm with a broad-based neck and dome, measuring 15 mm, involving the union of the vertebral arteries and the proximal basilar artery was demonstrated on cerebral angiography. The aneurysm was judged to be inoperable and treated conservatively. Twelve days later the patient was transferred to our hospital for endovascular therapy. An intravascular stent (MultiLink) was placed across the base of the aneurysm through the right vertebral artery. After this, coil placement in the aneurysm around the stent was performed via a microcatheter guided from the left vertebral artery. After that a microcatheter was guided from the right vertebral artery through the interstices of the stent into the aneurysm, and additional coils were placed. Final angiography showed subtotal occlusion of the aneurysm and excellent blood flow of the parent artery through the stent. There were no new neurological deficits. Neither rerupture nor ischemic event has occurred. The use of stents provides another treatment for managing the difficult entity of intracranial aneurysms.  相似文献
