A repair technique for a fractured solder joint in a multiple-unit fixed partial denture has been presented. The advantages of this technique include (1) retention of part of an existing restoration; (2) provision of additional support to minimize future fracture; (3) removal of the implant crown for periodic evaluation and/or treatment; and (4) retention of a fixed partial denture where a removable prosthesis might otherwise be needed. Commonly observed complications associated with a conventional fixed partial denture (FPD) include loss of retention and tooth fracture. This report describes the occurrence of an unusual FPD abutment fracture and subsequent treatment. The distal abutment of an FPD developed severe periodontal disease with mobility. This article describes a procedure for fabricating an optimal provisional restoration before tooth preparation. The procedure involves the fabrication of an indirect-direct provisional fixed partial denture. A case of a collarless metal ceramic fixed partial denture with preparations made at a slightly supragingival level is reported. The absence of the metal collar in the buccal portion of the abutments provides an excellent esthetic appearance. A patient underwent immediate implant placement and immediate provisional restoration with nonocclusal loading in the right central incisor area. A provisional custom abutment and a cemented provisional restoration were fabricated. At the impression appointment, an implant level impression was made and the provisional abutment was scanned for fabrication of the definitive custom abutment. Most dental patients insist on the use of provisional prostheses throughout healing and osseointegration when replacing extracted teeth with implants in esthetically sensitive areas. Removable appliances of some kind are normally used for this purpose, but patients often consider them to be too cumbersome. This can lead to decreased case acceptance and compliance with the use of the provisional restoration, which can compromise the final result of treatment. Custom fixed solutions to this problem exist, but they tend to be more complicated, less practical, and more expensive than other options now available. The Monodont bridge, a new system of prefabricated components for the creation of provisional fixed partial dentures, can be more esthetic, more retentive, more functional, more cost-effective, and more universally applicable than any other available techniques. This can raise patient tolerance of provisional prostheses and thus increase case acceptance, while fostering a more predictable esthetic result with regard to soft tissue contours and emergence profile.  相似文献

This article describes the treatment of a patient for whom a fiber-composite laminate dowel and core and polyethylene fiber were used as a conservative alternative to provide a dowel-and-core and fixed partial denture foundation. The treatment plan included removing the existing restoration on the maxillary right first molar, placing a fiber-reinforced resin dowel and core for retention and reinforcement of the maxillary first premolar, and fabricating a definitive restoration using polyethylene fiber to reinforce the fixed partial denture.  相似文献

Color stability of provisional crown and fixed partial denture resins   总被引:1,自引:0,他引:1  
STATEMENT OF PROBLEM: Provisional resins are used extensively during fixed prosthodontic treatment and exhibit varying degrees of color change/discoloration over time. Data are needed to help predict color stability of provisional resins. PURPOSE: The purpose of this in vitro study was to measure the color change of 12 provisional prosthodontic materials after immersion in artificial saliva and artificial saliva-coffee solutions for 1, 2, and 4 weeks. MATERIAL AND METHODS: Twelve provisional materials were studied. Methacrylates consisted of Alike, Jet, Temporary Bridge Resin, Unifast, and Zeta CC. Bis-acryl resins included Instatemp, Integrity, Luxatemp, Protemp Garant, Provipont, Provitec, and Temphase. Ten specimens (25.4 mm x 25.4 mm x 2.4 mm) of each material were fabricated. Five specimens were stored at 37 degrees C in artificial saliva and 5 were stored in a solution of artificial saliva (400 cc) and coffee (800 cc). Baseline color was measured using a colorimeter. Further color measurements were made after 1, 2, and 4 weeks of immersion. CIE L *a *b * values were recorded and color differences (DeltaE) between baseline and each storage interval were calculated using a 2-way ANOVA with Bonferroni adjustment for multiple comparisons in conjunction with an overall alpha=.05. RESULTS: Results indicated the presence of strong interaction between material and storage solution regardless of the aspect of color considered (P<.0001 for DeltaE, DeltaL *, Deltaa *, and Deltab *). Coffee solution relative to saliva solution had the most significant impact on color change for Luxatemp, Protemp, and Temphase materials at 4 weeks, but the greatest overall color change for both coffee and saliva solutions was found for Provipont (DeltaE=9.40 coffee; 8.51 saliva) and the least overall color change for both solutions was found for Zeta CC (DeltaE=0.31 coffee; 0.23 saliva). CONCLUSIONS: Under the conditions of this study, 12 provisional crown and fixed partial denture resins demonstrated varying tendency to discolor over a range of time periods when immersed in artificial saliva and artificial saliva-coffee solutions.  相似文献
