Clinical and immunological criteria for predicting the course of uveitis]

Clinical and immunological parameters are studied in 205 patients with uveites of different etiology in order to define the criteria permitting the prediction of uveitis course and relapses. The prognosis is unfavorable for autoimmune conditions, including peripheral, systemic, syndromal, tuberculous uveites and mixed infections. Clinical cure is unstable in diffuse inflammatory changes, persistent cellular infiltration of the vitreous body, formation of cyclic membranes in it, in cataract complications, and exudative hemorrhagic chorioretinitis. High titers of antibodies to tissue antigens and deficiency or hyperimmunoglobulinemia A, discoordination of T and B immunity, and sensitization to several antigens are risk factors for relapses. Clinical and immunological parameters should be analyzed for predicting the disease course and prescribing the basic and antirelapse therapy.

Intravenous fluconazole use in the treatment of fungic endogenous endophthalmitis: case report

The authors describe a case of endogenous Candida albicans endophthalmitis in one extremely low birth-weight newborn refractory to endovenous amphotericin B treatment that presented resolution with the use of endovenous fluconazole. Clinical aspects of endogenous Candida albicans endophthalmitis are also pointed out by a review of the literature.

Comprehensive immunologic evaluation of patients with uveal and retinal pathology

Clinical and immunologic examinations of 120 patients with central tapetoretinal abiotrophy, central serous choriopathy, central choroiditis, endogenic uveitis, thrombosis of the central retinal vein have revealed deviations in the T and B immunity systems and in autoimmune reactions to retinal S antigen, to uveopigmented antigens, and to human hepatic intracellular lipoprotein, as against the controls. The findings evidenced an imbalance of the T and B immunity parameters: reduced counts of T lymphocytes, theophylline-sensitive T suppressors, elevated immunoregulatory cell index, increased counts of B lymphocytes, circulating immune complexes, and blood serum immunoglobulins A and G. No essential shifts in leukocyte adhesion and antibody-dependent K cell cytotoxicity were found. The detected shifts are characteristic of the immune disorders common for various forms of uveoretinal abnormalities.     

Trimethoprim-polymyxin B ophthalmic solution in treatment of surface ocular bacterial infections

A safety and efficacy study comparing the clinical and bacteriologic effectiveness of trimethoprim-sulfacetamide-polymyxin B-neomycin-gramicidin in a group of patients with surface ocular bacterial infections was conducted. The results demonstrated TSP to be as effective as the other solution (both clinically and bacteriologically), with fewer adverse experiences. A second study was conducted comparing TSP with trimethoprim-polymyxin B (TP) and found TP to be superior to TSP in effecting bacteriologic cures. Clinical response was similar in both groups, and the low incidence of mild adverse experiences was approximately the same. It appears that the combination of trimethoprim with polymyxin B is safe and highly efficacious, both clinically and microbiologically, for the treatment of surface ocular bacterial infections.

Primary ocular natural killer/T-cell lymphomas: clinicopathologic features and diagnosis

PURPOSE: Primary ocular natural killer/T-cell lymphoma (NKTL) is rare. This study aims to characterize the clinical manifestations, diagnosis and treatment methods of primary ocular NKTL. METHODS: Clinical data of seven patients with ocular NKTL from 1982 to 2004 were collected. A morphologic, immunohistochemical and molecular analysis was undertaken. RESULTS: Clinical manifestations and computed tomography of the patients were typical. LCA CD56 CD3epsilon Ki-67 granzyme B (GrB) and T-cell intracellular antigen-1(TIA-1) were mostly positive. Clonal T-cell-receptor gene rearrangements of five patients showed negative findings, and the detection of Epstein-Barr virus showed positive. All patients died 5 months to 28 months after presentation. CONCLUSION: Ocular NKTL is a highly aggressive clinical course with poor prognosis. The diagnosis relies on clinicopathologic findings.

Neoplasia intraepitelial conjuntival. Interferón como terapia de rescate tras fracaso de la mitomicina C

Clinical caseThe case of a 60 year-old male with a conjunctival lesion diagnosed as conjunctival intraepithelial neoplasia (CIN), who was treated with mitomycin-C for 3 weeks with minimal improvement. The therapy was change to interferon 2B. Six month later, and after a complete remission of the lesion, the treatment was suspended, with no signs of relapse.DiscussionThe treatment of these lesions is currently made with chemotherapy and immunotherapy agents, such as mitomycin-C, 5-fluorouracil, and interferon alfa 2B. The latter, even although is the least used, gives excellent results with fewer secondary effects than mitomycin-C, resulting in an optimal therapy for the non-invasive treatment of CIN lesions.
