Femoral and tibial tunnel widening following ACL reconstruction using hamstring autograft has been described. Greater tunnel widening has been reported with suspensory fixation systems. We hypothesized that greater tunnel widening will be observed in patients whose hamstring autograft was fixated using a cortical, suspensory system, compared to double cross-pin fixation on the femur. We performed clinical and radiographic evaluation on 46 patients at minimum 2 years after primary ACL reconstruction. We measured subjective and objective outcomes including KT-1000 and AP, lateral radiographs. A musculoskeletal radiologist, independent of the surgical team, measured tunnel width, while correcting for magnification, at the widest point and at 1 cm away from tibial and femoral tunnel apertures. Patients in the suspensory graft fixation group exhibited significantly greater absolute change and greater percent change in femoral tunnel diameter compared to patients with double cross-pin fixation (P ≤ 0.05). This difference was noted on both AP and lateral radiographs and at both measurement sites. There was no significant difference between groups for tibial tunnel widening, IKDC subjective scores or KT-1000 side to side differences. There was significantly more femoral tunnel widening associated with the use of the endobutton suspensory fixation system compared to the use of double cross-pins for fixation within the tunnel.  相似文献

The purpose of this study was to evaluate tibial tunnel widening prospectively after anterior cruciate ligament (ACL) reconstruction with hamstring tendon grafts using Rigidfix (DePuy Mitek, Raynham, MA) femoral fixation and Intrafix (DePuy Mitek) tibial fixation. Fifty-six consecutive patients who underwent ACL reconstruction with a minimum of 2 years’ postoperative evaluation were reviewed. On the anterior–posterior (AP) and lateral radiographs, the diameter of the tibial tunnel was measured at proximal, middle, and distal positions, and the shape of the tibial tunnels were classified. Tunnel widening was defined as widening of greater than 2 mm. Group I was defined as cases with no tunnel widening, and group II was defined as cases with tunnel widening. Postoperative laxity evaluations were performed using Lachman test, pivot-shift test, and instrumented laxity testing using the KT-1000 arthrometer. On the AP radiographs, the average diameter of the tibial tunnel increased 8.8% at 6 months and 8.5% at 12 months postoperatively compared to the immediate postoperative day. On the lateral radiographs, the average diameter of the tibial tunnel increased 7.2% at 6 months and 8.1% at 12 months year postoperatively compared to the immediate postoperative day. The tunnel shape evaluation revealed predominantly linear type in 53 patients (95%). Group I was 42 patients (75%), and group II was 14 (25%). The average KT-1000 measurement was 1.0 ± 1.8 mm in group I and 2.1 ± 2.8 mm in group II (n.s.). The Lachman and pivot-shift tests showed no significant differences between the two groups. In conclusion, hamstring ACL reconstruction using Rigidfix and Intrafix fixation showed less widening of the tibial tunnels than observed in previously published studies.  相似文献

We report a prospective series evaluating the incidence and degree of tunnel widening in a well-matched series of patients receiving a hamstring or patella tendon graft for anterior cruciate ligament (ACL) deficiency. We correlated tunnel widening with clinical factors, knee scores, KT-1000 and isokinetic muscle strength to determine the clinical significance of this finding. Seventy-three patients at least 12 months post-ACL reconstruction were evaluated. Thirty-eight patients had received a doubled semitendinous and gracilis graft and 35 a bone-patella tendon-bone graft. All patients underwent a similar endoscopic procedure and accelerated postoperative rehabilitation. Tunnel widening was determined using standardized anteroposterior (AP) and lateral X-rays adjusted for magnification. A limited series of MRIs was performed to validate these measurements. There was a significant difference in the degree of tunnel widening between the two groups. The mean increase in femoral tunnel area in the hamstring group was 100.4% compared with a decrease of 25% in the patella tendon group (P = < 0.0001). In the tibial tunnel the mean increase in the hamstring group was 73.9% compared with a decrease of 2.1% in the patella tendon group (P = < 0.0001). The MRIs validated the plain film measurements. Tunnel widening did not correlate with the clinical findings, knee scores, KT-1000 or isokinetic muscle strength. Tunnel widening is marked in the hamstring group. Tunnel widening does not correlate with instability or a poor clinical outcome in the short term. The long-term implications of this finding are still to be determined. The objective of this study was to quantify, in vivo, the reduction of knee laxity obtained by an extra-articular procedure, added to hamstring single-bundle (SB) anterior cruciate ligament (ACL) reconstruction in controlling coupled tibial translation during the Lachman and drawer tests. Twenty-eight patients were evaluated with a computer-assisted kinematic evaluation protocol; patients with associated ligament tears or meniscal damages were not included in the study. All patients underwent an hamstring ACL with an additional extra-articular procedure. During the intervention, tibia was tracked during the Lachman and drawer tests with ACL-deficient knee, after SB fixation and after extra-articular plasty fixation, performed with the remnant part of the hamstring tendons, from end of lateral condyle to Gerdy’s tubercle. Statistical analysis was performed to see whether there was a difference in knee laxity after the tests in the three steps. At 30°, the SB graft reduces AP translation of about 5 mm (P < 0.05) while the extra-articular procedure controls lateral tibial compartment, reducing translation by 1.6 mm (P < 0.05). At 90° the SB graft reduces AP translation more in the lateral compartment (P < 0.05), while the extra-articular procedure contributes in controlling tibial translation reducing laxity by 1 mm (P < 0.05) in both compartments. Result shows that, in vivo, the addition of an extra-articular procedure to single-bundle ACL reconstruction may be effective in controlling coupled tibial translation during the Lachman test and reduces AP laxity at 90° of flexion.  相似文献

The aim of this study was to look at the clinical outcome of polylactide carbonate (PLC) interference screws in knee ligament reconstruction surgery. We prospectively followed up 59 patients who underwent primary hamstring anterior cruciate ligament (ACL) reconstructions using PLC (Calaxo screw; Smith and Nephew, Andover, MA) screws to secure the graft in the tibial tunnel. The average age was 34 years (range 17–55 years, SD 8.93). Twenty-three (39%) patients presented with complications [synovitis in 15% (nine) patients, prominent tibial swelling in 34% (20) patients and both in seven patients]. In comparison, similar complications were not seen in two groups of 59 age and sex matched patients in whom PLLA (Bio RCI; Smith and Nephew, Andover, MA) screws or Titanium (RCI; Smith and Nephew, Andover, MA) screws were used by the same surgeon. The problems encountered in the PLC screw group did not adversely affect knee stability. Six patients underwent exploration of the tibial tunnel site. A sterile white cheesy substance was removed, leaving an empty tibial tunnel. The ACL graft was found to be well attached to tibial tunnel in all cases. The PLC screw diameter, surface area or tibial tunnel diameter did not have any correlation to the occurrence of complications. Two patients required multiple washouts, one of whom developed a deep infection. The degradation of PLC screws does not follow the gradual and controlled pattern demonstrated in the ovine model. The unpredictable screw degradation, and the reaction to it can lead to serious clinical consequences.  相似文献

Twenty-six patients with anteroposterior (AP) laxity of the knee, associated with torn anterior cruciate ligament (ACL), were prospectively randomized for arthroscopic lower femoral tunnel placed single- or double-bundle reconstruction using hamstring tendons. We evaluated AP and rotational stabilities under regular loads (a 100-N anterior load and a 1.5-N m external–internal load) before and after ACL reconstruction, comparing single- and double-bundle reconstruction with our original device for applying quantitative tibial rotation and the navigation system intraoperatively. No significant differences were found between the two groups in AP displacement and total range of tibial rotation at 30° and 60° of knee flexion. We found that a lower femoral tunnel placed single-bundle reconstruction reproduced AP and rotational stability as well as double-bundle reconstruction after reconstruction intraoperatively.  相似文献

Sixty-eight patients were clinically evaluated preoperatively, 3, 5, 7, 9 months, 1 and 2 years after ACL reconstruction, 34 with patellar tendon graft, 34 with hamstring graft. Outcome regarding graft choice and anterior knee laxity (P = 0.04) was in favour of patellar tendon graft. Hamstring graft led to a larger laxity, 2.4 mm compared with patellar tendon graft, 1.3 mm at 1 year and 2.5 mm and 1.5 mm, respectively, at 2 years (P = 0.05). There was a significant difference in rotational knee stability in favour of the patellar tendon graft at all test occasions but 9 months. A general effect regarding graft choice and muscle torque was found at 90°/s for quadriceps (P = 0.03) and hamstrings (P ≤ 0.0001) and at 230°/s for hamstrings (P ≤ 0.0001). No treatment effect regarding graft choice and one-leg hop test, postural sway or knee function was found. No group differences in anterior knee pain were found at any of the test occasions but 2 years in favour of hamstring graft compared to patellar tendon graft (P = 0.04). Patellar tendon graft resulted in higher activity level than hamstring graft at all test occasions but 1 year (P = 0.01). Patellar tendon ACL reconstruction led to more stable knees with less anterior knee laxity and less rotational instability than hamstring ACL reconstruction. Hamstring graft patients had not reached preoperative level in hamstring torque even 2 years after ACL reconstruction. Athletes with patellar tendon graft returned to sports earlier and at a higher level than those with hamstring graft.  相似文献

Patellar tendon graft has been the most frequently used material in anterior cruciate ligament (ACL) reconstruction, but the hamstring tendons have been increasingly used as well; however, which graft is to be preferred is not adequately supported by existing clinical studies. In this prospective randomized clinical trial, the study hypothesis was that the hamstring tendons are equally good graft material as the patellar tendon in ACL reconstruction. Ninety-nine patients with laxity due to a torn ACL underwent arthroscopically assisted reconstruction with graft randomization according to their birth year to either patellar tendon with metal interference screw fixation or double looped semitendinosus and gracilis tendons with fixation similar to the Endobutton technique using a titanium metal plate suspension proximally and screw-washer postdistally. Excluding preoperative Lysholm knee score, there were no significant differences between the two groups in the preoperative and operative data. A standard rehabilitation regimen was used for all the patients, including immediate postoperative mobilization without a knee brace, protected weight bearing for 2 weeks, and return to full activity at 6–12 months postoperatively. Forty patients in the patellar tendon group and 39 patients in the hamstring tendon group were available for clinical evaluation at median 5 years after surgery (ranges 3 years 11 months–6 years 7 months). The results revealed no statistically significant differences with respect to clinical and instrumented laxity testing, isokinetic muscle torque measurements, International Knee Documentation Committee ratings, Lysholm (knee score), Tegner (activity level) and Kujala patellofemoral knee scores. There was an enlargement of the drill tunnels, statistically more in the hamstring tendon group, but no increase from 2 to 5 years in either group. Narrowing of the joint spaces (IKDC measurement method) from 2 to 5 years postoperatively was seen in both the groups, however, without difference between the two groups.  相似文献

Bioabsorbable interference screws are commonly used to secure the graft during anterior cruciate ligament (ACL) reconstruction, in part because they result in less image degradation on subsequent magnetic resonance imaging (MRI). However, some bioabsorbable screws are associated with abnormalities on MRI examination not reported with metallic interference screws. We describe a finding on knee MRI examination after ACL reconstruction using a polylactide carbonate (PLC) bioabsorbable screw that we believe to be previously unreported with any other bioabsorbable screws. The finding raised suspicion of hemorrhage or infection, neither of which were present clinically. Analysis of tissue from the tibial tunnel suggested an explanation for the MRI finding: calcite crystals. An additional five patients with knee MRI examinations after ACL reconstruction using a PLC screw were reviewed and correlated with clinical findings with four having similar imaging abnormalities present. The PLC (Calaxo screw, Smith and Nephew, Andover, MA) screw used in these patients has been recalled in the United States and Europe by the manufacturer after a greater than expected incidence of adverse reactions, and legal action may be pending.  相似文献

BACKGROUND: During the past decade, bioabsorbable interference screws have become increasingly popular in endoscopic reconstructions of the anterior cruciate ligament. With these screws, there is no need for a second operation for removal and no complicating factor if later revision surgery is necessary. Several pullout studies have found similar results between metal and bioabsorbable interference screws; however, few studies have investigated the clinical outcome. HYPOTHESIS: There is no difference in the clinical outcome of bioabsorbable interference screws compared with metal screws. STUDY DESIGN: Randomized controlled clinical trial; Level of evidence, 1. METHODS: Forty-one patients were randomized for the use of either metal interference screws (20 patients) or bioabsorbable poly-l-lactic acid screws (21 patients). The patients were followed with clinical examinations at 6, 12, and 24 weeks and at 1 and 2 years postoperatively. RESULTS: Subjective knee function was better in the patients in the metal screw group; they had less pain at rest, a higher Tegner score, a higher Lysholm score, and better subjective knee function at 2 years compared to the bioabsorbable screw group. However, there was no difference in stability between the groups.Conclusion and CLINICAL RELEVANCE: Because of the inferior results in the bioabsorbable screw group in our study, and until larger studies show otherwise, we do not find the advantages of using bioabsorbable screws sufficient to warrant the routine use of poly-L-lactic acid screws in anterior cruciate ligament reconstructions.  相似文献

In this study, mid to long-term results of anterior cruciate ligament reconstruction with hamstring tendons and Transfix technique were evaluated. Anterior cruciate ligament (ACL) reconstruction with four-strand hamstring tendon was performed with Transfix technique on 271 (198 males, 73 females; mean age 25.7; 17–52) patients with anterior cruciate ligament ruptures. The patients were followed up with clinical examination, Lysholm and Tegner activity scales, IKDC scoring system, KT-1000 test and radiological examination. The mean follow-up period was 82 (48–100) months; 204 (75%) patients had no subjective complaints. According to the KT-1000 test, only 14 (5%) patients had more than 5 mm laxity postoperatively, whereas, 161 (59%) patients had more than 5 mm laxity preoperatively. In addition to this, only 19 (7%) patients had Lysholm scores less than 80 postoperatively, whereas 154 (57%) patients scored less than 80 preoperatively. When compared with Tegner activity scale, 189 (70%) patients scored <6 preoperatively and only 24 (8%) postoperatively; 78 (29%) patients scored D preoperatively and only 5 (2%) patients scored D postoperatively on the basis of the IKDC scoring system. Our functional results were found to be satisfactory in more than 90% of patients. Commonly seen problems in ACL reconstruction such as inaccurate graft placement and tunnel widening were found to be consistent with the values in relevant literature. However, we demonstrated that the functional results and the stability of the knee were not related with tunnel widening. This study concludes that the reconstruction of ACL with hamstring tendons and the Transfix technique is reasonably successful, safe and causes low morbidity. Furthermore, we believe that proper graft preparation, accurate tunnel placement, notch-plasty, fixation and rehabilitation program are all as important as the choice of graft and fixation material.  相似文献

PURPOSE: Anterior cruciate ligament (ACL) reconstructive surgery has seen significant progress since bioabsorbable interference screws have replaced synthetic metallic screws for ligament graft fixation within the femoral and tibial osseous tunnel. Our study compared the MR images of patients who underwent ACL reconstruction using bioabsorbable interference screws with those of patients who underwent ACL reconstruction using metallic screws to evaluate the MR aspects of degradation and osteointegration processes of bioabsorbable interference screws post-operatively. MATERIALS AND METHODS: Between September 1999 and December 2002 we performed MRI on 40 patients who underwent arthroscopic ACL reconstruction with patellar tendon auto-graft using PDLLA bioabsorbable screws in 35 cases and metallic screws in 5 cases. After surgery, all patients underwent an intensive rehabilitation programme along with clinical evaluation using the standard knee ligament evaluation form of the International Knee Documentation Committee (IKDC) and radiological examinations. MR studies were performed 1, 2 and 3 years post-operatively using a 0.5-Tesla MR scanner with T1- and T2*-weighted and STIR sequences. RESULTS: Thirty-three patients were clinically classified as group A IKDC (full return to sports activity) and 7 patients as group B IKDC. No foreign-body reaction was found in patients treated with bioabsorbable interference screws, both from a clinical point of view and from laboratory exams. Ferromagnetic artefacts were found in all patients treated with metallic interference screws. In patients with bioabsorbable interference screws, degradation of the screws and absence of significant artefacts allowed correct evaluation of the signal of the reconstructed ligament throughout its length, the osseous tunnels and the joint cavity. In 34 of 35 patients with bioabsorbable interference screws, the screw could not be detected due to complete degradation. In 8, 12 and 4 cases, a small cyst-like formation due to screw hydrolysis was present at the screw site within 1, 2 and 3 years from surgery respectively. Only in 4 cases was the process of osteointegration of the screw and bone plug complete 3 years after the operation, with consequent restoration of bone morphology. DISCUSSION AND CONCLUSIONS: The use of bioabsorbable interference screws is a valuable alternative to synthetic metallic fixation implants as the absence of artefacts allows correct post-operative MR follow-up. MRI is the only technique able to visualize all the portions of the transplant and to evaluate the healing process. Bioabsorbable interference screws usually degrade within one year. However, full osteointegration requires a long time and may not be complete 3 years after surgery. The presence of cyst-like formations at the screw site has to be regarded as a normal feature of the screw degradation process. Therefore, the use of bioabsorbable interference screws is recommended as it enables MR follow-up of the knee after ACL reconstruction.  相似文献

Tunnel enlargement is a frequent issue after ACL reconstruction and the “synovial bathing effect” is thought to be among the biological factors contributing to this phenomenon. Since the amount and the pressure of the synovial fluid inside the knee joint are higher and the length of its presence is longer in patients with post-operative septic arthritis after ACL reconstruction, we reviewed the tunnel enlargement in these cases in order to better evaluate this phenomenon in such a hostile environment. The null hypothesis of this study was that the highly represented phenomenon of the “synovial bathing effect” that occurs in an infected ACL reconstruction would not affect the amount of post-operative tunnel widening. A case-control study was done. At a mean follow up of 10 months (range 9–11 months) eight patients with septic arthritis following ACL reconstruction (group A) were radiologically reviewed using a CT scan and the diameters of femoral and tibial tunnels were measured. The results were compared with a control group (B) of uncomplicated ACL reconstruction cases operated by the same surgeon using the same technique. Although patients of Group A experienced a bigger amount of tunnel enlargement than patients of group B both on femoral (9.53 ± 1.07 vs. 9.35 ± 1.52 mm) and tibial side (10.07 ± 1.3 vs. 9.92 ± 0.74 mm), no clinical or statistically significant differences were detected between the groups (P > 0.05). No significant tunnel enlargement could be detected in patients of group A when compared with patients of group B. This seems to minimize the role of biological factors contributing to tunnel widening.  相似文献

Anatomical observation and biomechanical studies have shown that the anterior cruciate ligament (ACL) mainly consists of two distinct bundles, the anteromedial (AM) bundle and posterolateral (PL) bundle. Conventional single-bundle ACL reconstruction techniques have focused on the restoration of the AM bundle while giving limited attention to the PL bundle. The purpose of this prospective, randomized clinical study is to compare the outcomes of ACL reconstruction when using either double-bundle or single-bundle technique and bioabsorbable interference screw fixation, and similar rehabilitation with both techniques. Sixty-five patients were randomized into either double-bundle (n = 35) or single-bundle (n = 30) ACL reconstruction with hamstring tendons and bioabsorbable screw (Hexalon, Inion Company, Tampere, Finland) fixation in both groups. The evaluation methods were clinical examination, KT-1000 arthrometer measurements, radiographic evaluation, as well as International Knee Documentation Committee and Lysholm knee scores. There were no differences between the study groups preoperatively. For an average of 14 months of follow-up (range 12–20 months), 30 patients of the double-bundle group and 29 patients of the single-bundle group were available (91%). At the follow-up, the rotational stability, as evaluated by pivot shift test, was significantly better in the double-bundle group than in the single-bundle group. However, in anterior stability of the knee, there was no significant difference between the groups. None of the patients in the double-bundle group had graft failure, while four patients in the single-bundle group had. In addition, knee scores were equal at the follow-up, and all the results were significantly better at the follow-up than preoperatively, in both groups.  相似文献

Changes in the femoral and tibial bone tunnel were studied prospectively after arthroscopic ACL reconstruction with quadruple hamstring autograft. To determine whether tunnel enlargement can be decreased by fixing the graft close to the joint line having a stiffer fixation construct we compared "anatomical" (one absorbable interference screw femorally, and bicortical fixation with two absorbable interference screws tibially) and extracortical fixation techniques (Endobutton femorally, and two no. 6 Ethibond sutures over a suture washer tibially). Over a 2-year period we evaluated 60 patients clinically (IKDC scale, Cincinnati Knee Score, KT-1000) and radiographically (confirmed by MRI). The operated knee was radiographed immediately postoperatively and 6 and 24 months postoperatively. The femoral and tibial bone tunnel diameter was measured on anteroposterior and lateral images, and the tunnel area was calculated and compared to the initial area calculated from the perioperative drill size. In the "anatomical" group the immediately postoperative bone tunnel area was 75% larger than the initial tunnel area, after 6 months it was increased another 31%, and between 6 and 24 months it remained basically unchanged. In the "extracortical" group there was no significant enlargement immediately postoperatively, but after 6 months it was 65% larger than the initial area of drill and graft size, and between 6 and 24 months it decreased to 47%. There was no correlation between the amount of tunnel enlargement and clinical scores or KT-1000 measurement. Arthroscopic ACL reconstruction with quadruple hamstring autograft is associated with bone tunnel enlargement. Using a purely extracortical fixation technique thus significantly increased the tibial and femoral tunnel area during the first 6 postoperative months, while it decreased slightly thereafter. The insertion of large interference screws apparently not only compresses the graft in the bone tunnel but also significantly enlarges the bone tunnel itself. The immediate enlargement at the time of the operation is followed by a reduced further enlargement at 6 months and then stabilization. Tunnel widening did not influence clinical outcome over a 2-year period.  相似文献

Purpose

The aim of this study was to evaluate the clinical outcome and differences in anterior–posterior laxity of ACL reconstruction using a bioabsorbable interference screw for femoral graft fixation when compared to femoral bioabsorbable cross pin fixation.

Methods

Clinical outcome was evaluated among 59 patients 1?year after arthroscopic ACL reconstruction with hamstrings graft in a prospective, non-randomised study. In 31 cases, femoral fixation of the graft was performed using a bioabsorbable interference screw. In 28 cases, two bioabsorbable cross pins were used for femoral fixation. Patients were evaluated using Tegner, Lysholm and Marshall scores, the visual analogue scale for pain and KT-1000 arthrometer measurement.

Results

No significant difference (P?≥?0.05) was observed at follow-up for the knee scores. The average Tegner score was 5.83 points (±2.00) for the interference screw fixation and 5.83 points (±1.24) for the cross pin fixation; the average Lysholm score was 93.58 (±5.79) to 92.72 (±6.34) points; and the average Marshall score 46.72 (±2.4) to 47.30 (±2.35) points. No significant difference was found for the visual analogue scale for pain. KT-1000 arthrometer measurement revealed a significant (P?<?0.05) difference in the mean side-to-side anterior translation at all applied forces. At 67?N, the mean difference was 1.53?mm (±1.24) in the interference screw group and 0.47?mm (±1.18) in the cross pin group (P?<?0.05). At 89?N, the mean differences were 1.85?mm (±1.29) versus 0.59?mm (±1.59), respectively, (P?<?0.05), and maximum manual displacements were 2.02?mm (±1.26) versus 1.22?mm (1.18; P?<?0.05).

Conclusions

In ACL reconstruction with hamstrings graft, similar clinical results are obtained for the use of bioabsorbable cross pins when compared to bioabsorbable interference screws for femoral fixation. Cross pin fixation was superior with regard to the anteroposterior laxity as measured with KT-1000.  相似文献

The effect of the graft placement on the clinical outcome of patients after anterior cruciate ligament (ACL) reconstruction has been studied sparsely. We conducted a prospective follow-up of 140 patients who underwent an arthroscopic ACL reconstruction with a hamstring graft. One hundred and four of them (74%) could be examined at the 2-year follow-up. Clinical examination included Lysholm, Tegner, and International Knee Documentation Committee rating scores, arthrometric anterior–posterior knee laxity assessment, and muscle strength assessments. The graft placement was measured from lateral radiographs using a system recommended for measuring the attachment positions of the cruciate ligaments as well as a method called ‘the sumscore of the graft placement’, which takes into account both the femoral and the tibial graft placements simultaneously. The sumscore was smaller in knees with normal anterior–posterior knee laxity in the Lachman test (P = 0.002) and normal rotational knee laxity in the pivot shift test (P = 0.01) than in those with abnormal laxity. The tibial graft placement was more anterior when the Lachman test was normal (P = 0.04). The Lysholm score was better when the femoral graft placement was more posterior (r = −0.20, P = 0.04). The optimal femoral graft placement was between 25 and 29% of length of the femoral condyle along the Blumensaat’s line from posterior to anterior. The optimal tibial graft placement was between 32 and 37% of the length of the tibial plateau from the anterior corner, and the optimal sumscore was between 61 and 66. The sumscore and its components (the femoral and tibial graft placements) showed a clear association with the clinical outcome of the patients. The best outcome was achieved when the sumscore was small; that is the graft placement showed posterior enough in the femur, and anterior enough in the tibia.  相似文献

Tendon–bone incorporation of a tendon graft within the bone tunnel is of priority concern when using for anterior cruciate ligament (ACL) reconstruction. Superior healing process and stronger healing strength can be achieved when periosteum is sutured on the tendon inserted into a bone tunnel. We applied this idea to ACL reconstruction for enhancing tendon graft–bone tunnel healing. This is a prospective clinical outcome study with this surgical technique at minimal 2 years follow-up. Periosteum-enveloping hamstring tendon graft has been used in 68 patients. Data from 62 patients who had been followed up completely were analyzed. All patients suffered from a grade 3 or higher grade of Lachman and anterior drawer test with a positive pivot-shift test. Clinical assessments included the Lysholm knee scores, International Knee Documentation Committee (IKDC) scores, KT-1000 instrumented testing, thigh muscle assessment, and radiographic evaluation. The median Lysholm knee score was 59 (40–70) and 94 (60–100) points (P<0.01) before and after surgery. After reconstruction, 81% of patients were able to return to moderate or strenuous activity. Four (6%) patients were found to exhibit grade 2 or more ligament laxity. Complete range of motion could be achieved in 86% of patients. Three patients (5%) had positive pivot shift. Finally, 92% of patients were assessed as normal or nearly normal rating by IKDC guideline. Bone tunnels enlargement of more than 1 mm was identified in 5% of femoral tunnels and 6% of tibial tunnels. The study shows that a satisfactory result can be achieved with the periosteum-enveloping hamstring tendon graft in ACL reconstruction. Periosteum can be easily harvested at the proximal tibia from a routine incision for hamstring tendon harvesting. Besides the potential for improving tendon–bone healing, enveloped periosteum may help to seal the intra-articular tunnel opening in the early postoperative period, and thus avoid synovial fluid reflux into the tunnel. ACL reconstruction with bone patellar tendon bone (BPTB) grafts has been shown to produce dependable results. Recently, reconstructions with double-looped semitendinosus gracilis (DLSG) grafts have become common. The prevailing opinion is that ACL reconstruction with patellar tendon graft produces a more stable knee with more anterior knee pain than DLSG grafts, while the functional results and knee scores are similar. The present study evaluates BPTB grafts fixed with metallic interference screws and DLSG grafts fixed with Bone Mulch Screw on the femur and WasherLoc fixation on the tibia. All else being the same, there is no difference in the outcome between the two grafts and fixation methods. This is a prospective randomized multicenter study. A total of 115 patients with isolated ACL ruptures were randomized to either reconstruction with BPTB grafts fixed with metal interference screws (58 patients) or DLSG grafts (57 patients) fixed with Bone Mulch Screws and WasherLoc Screws. Follow-up was at one and two years; the latter by an independent observer. At two years, one ACL revision had been performed in each group. Eight patients in the DLSG group and one in the BPTB group underwent meniscus surgery in the follow-up period (P = 0.014). Mean Lysholm score at the two year follow-up was 91 (SD ± 10.3) in the DLSG group and also 91 (SD ± 10.2) in the BPTB group. Mean KT-1000 at two years was 1.5 mm in the BPTB group and 1.8 mm in the DLSG group (n.s.). At two years, four patients in the BPTB group and three in the DLSG group had a Lachman test grade 2 or 3 (n.s.). More patients in the BPTB group had pain at the lower pole of the patella (P = 0.04). Peak flexion torque and total flexion work were lower in the DLSG group at one year (P = 0.003 and P = 0.000) and total flexion work also at two years (P = 0.05). BPTB ACL reconstruction fixed with interference screws and DLSG fixed with Bone Mulch Screws on the femur and WasherLoc Screws on the tibia produce satisfactory and nearly identical outcomes. Among our patients in the DLSG group, flexion strength was lower, and more patients underwent meniscus surgery in the follow-up period. The BPTB group has more anterior knee pain.  相似文献

The aim of the study is to compare the results after arthroscopic anterior cruciate ligament (ACL) reconstruction using central-third, bone-patellar tendon-bone (BTB group) (n=28) and four-strand semitendinosus/gracilis (ST/G group) (n=31) autografts in female patients. The type of study was non-randomised prospective consecutive series. A consecutive series of 61 female patients, all with unilateral ACL ruptures, was included in the study. In both groups, interference screw fixation of the graft was used at both ends, and 59/61 (97%) of the patients returned for the follow-up examination after a period of 26 (23–31) months. The pre-operative assessments in both groups were similar in terms of the Lysholm score, KT–1000 measurements, one-leg-hop test, and knee-walking test. At the 2-year follow-up, the knee-walking test was significantly worse in the BTB group than in the ST/G group (P=0.003). Furthermore, the knee-walking test was significantly worse at follow-up than pre-operatively in the BTB group (P<0.005). The corresponding finding was not made in the ST/G group. A reduction in knee laxity compared with the pre-operative assessments was found in both groups. No significant difference in the post-operative knee laxity measurement was found between the groups. A significant increase in activity level and subjective scores was found in both groups compared with pre-operative values, without any significant differences between the groups. Two years after ACL reconstruction, the groups displayed no significant differences in terms of functional outcome and knee laxity. However, the use of ST/G autografts rendered significantly less discomfort during the knee-walking test than the use of BTB autografts.  相似文献
