A 6-year-old boy with coronary artery fistula emptying into the left ventricle was reported. An attempt was made to outline clinical manifestations of this disease which have not yet been fully described, based on the experience of the present case and a review of 14 previously reported cases. It is concluded that clinical signs and symptoms are not specific for this anomaly and retrograde aortography is required to establish the definite diagnosis of the lesion. Authors' views are expounded as to the location of maximal intensity of the murmur, genesis of the systolic murmur and electrocardiographic features.  相似文献

A rare case of left coronary artery--pulmonary artery fistula is reported. Transoesophageal echocardiography was capable of precisely demonstrating the origin, the course and the drainage site of the fistula. In contrast, transthoracic echocardiography could visualise a drainage flow in the pulmonary artery only. In conclusion, transoesophageal echocardiography may be helpful in the diagnosis or exclusion of the form of coronary artery fistula.  相似文献

The treatment of a rarely described variant of tetralogy ofFallot with pulmonary atresia is reported. The case is uniquebecause the patient remained relatively pink although the pulmonaryblood flow came solely from a fistula between the left coronaryartery and the main pulmonary artery. The diagnostic and surgicalimplications are discussed and the literature briefly reviewed.The diagnosis should be borne in mind in patients who have pulmonaryatresia with ventricular septal defect and a continuous murmur.  相似文献

Summary This paper reports two patients presenting a combination of coronary artery-pulmonary artery fistula and pulmonary atresia with ventricular septal defect. A confluent central pulmonary artery arose from the proximal portion of the left coronary artery in both patients. The fistulous segment between the coronary artery and the central pulmonary artery was large, and no coronary arterial stenosis was found in either patient. Besides this fistulous origin of the pulmonary blood supply, large collateral vessels originated from the descending aorta in both patients. Neither patient has shown findings of myocardial ischemia, at least in consecutive rest and exercise electrocardiograms. Both patients were successfully operated, at the ages of 3 and 8 years, respectively. The clinical and embryological implications of this rare malformation are briefly reviewed.  相似文献

Coronary artery fistulae are rare congenital or acquired connections between the coronary vessels and the cardiac chambers or other vascular structures. We present two consecutive cases of coronary fistulae between the proximal left anterior descending artery (LAD) and the main pulmonary artery. Both cases where admitted with history of acute coronary syndromes and had multivessel coronary disease along with coronary pulmonary fistulae. The two cases were managed by coronary artery bypass grafting (CABG) and repair of the fistulae.  相似文献
