Three children with known primary brain neoplasms and leptomeningeal disease were evaluated with MR imaging. Two of the patients had medulloblastoma and one had pineoblastoma. The presence of leptomeningeal tumor spread was established by positive CSF cytopathology in conjunction with compatible contrast-enhanced CT findings. Contrast-enhanced CT, nonenhanced MR, and Gd-DTPA-enhanced MR studies were then compared. In two cases, leptomeningeal lesions were seen better with Gd-DTPA-enhanced MR than with contrast-enhanced CT. In all three cases, Gd-DTPA MR imaging revealed lesions that were not identified on noncontrast MR. Gd-DTPA-enhanced MR imaging is useful when searching for intracranial leptomeningeal tumor deposits in pediatric patients at risk for this condition.  相似文献

MR imaging in the diagnosis of pancreatic disease   总被引:2,自引:0,他引:2  
MR imaging examinations of 20 patients with normal pancreas and of 38 patients with suspected pancreatic disease were analyzed retrospectively to evaluate the ability of MR imaging to depict the normal and abnormal pancreas, establish MR criteria for various pancreatic diseases, determine if MR imaging can distinguish among various pancreatic diseases, and compare the usefulness of MR imaging with CT. In all 20 patients with normal pancreas and in 34 of the 38 patients with suspected pancreatic disease excellent or good evaluation of the pancreas was achieved. A 0.35-T magnet (Diasonics MT/S) was used, and both T1- and T2-weighted images were needed. T1 and T2 relaxation times and MR signal intensities showed no specific pattern to allow consistent differentiation between normal and diseased pancreatic tissue or to distinguish between tumor and inflammation. In the 29 patients in whom MR and CT images could be compared, MR imaging and CT provided equivalent information in 20 (69%). In 4 patients (14%), MR imaging added information, and in 5 patients (17%) of cases, MR imaging yielded less information than CT. MR imaging of the pancreas was found to be superior to CT in selected instances, such as in the staging of pancreatic neoplasms (n = 4) and in the evaluation of pancreatic disease after surgery (n = 3). We conclude that, at present, MR imaging should not be used as the screening method for pancreatic disease but should be reserved as an adjunct when the information provided by CT is insufficient.  相似文献

Eighty-one pediatric patients with a variety of spinal disorders, including suspected dysrhaphism, scoliosis, neoplasia, and neurofibromatosis, underwent magnetic resonance (MR) imaging. The results were retrospectively compared with those of myelography followed by computed tomography (CT) and surgery. In patients with dysrhaphism, most abnormalities, including hydromyelia, inclusion tumors, and sites of cord tether, were demonstrated with MR imaging. Diastematomyelia and small hydromyelic cavities were indistinguishable on routine coronal and sagittal T1-weighted images; axial images with T2 weighting were optimal for this differentiation. MR imaging did not enable direct visualization of a thickened filum or evaluation of tethering with a thin, dorsally positioned neural placode. Congenital or severe scoliosis required lengthy studies with multiple planes of imaging or myelography and CT. Milder curvatures were readily evaluated with MR imaging, and neoplastic lesions, with the exception of intrathecal tumor seeding, were adequately defined.  相似文献

PURPOSE: To compare state-of-the-art magnetic resonance (MR) imaging with single-phase helical computed tomography (CT) in abdominal screening for extrahepatic disease in patients with proved malignancy. MATERIALS AND METHODS: Fifty-seven patients with known malignancy underwent abdominal contrast material-enhanced helical CT and MR imaging from 1994 through 1997. Prospective interpretations of CT scans and MR images were used to assess each modality's sensitivity in depicting malignant extrahepatic tumor at 17 anatomic sites. Imaging findings were compared with surgical results in all patients. RESULTS: Helical CT depicted 101 (66%) of 154 surgically confirmed extrahepatic tumor sites; MR imaging depicted 139 (90%) (P < .001). MR imaging depicted tumor in more patients at 11 of the 17 anatomic sites; at six sites, MR imaging and helical CT were equivalent. MR imaging showed significantly greater depiction of extrahepatic tumor for the peritoneum (P < .05), bowel (P < .01), and mesentery (P < .05). False-negative interpretations would have altered patient care had the extrahepatic tumor remained undetected in 13 patients for helical CT and in six patients for MR imaging. CONCLUSION: State-of-the-art MR imaging can be used for effective abdominal screening for extrahepatic tumor in patients with malignancy. Compared with single-phase helical CT, MR imaging depicted more sites of extrahepatic tumor and was particularly advantageous for the peritoneum, mesentery, and bowel.  相似文献

The pediatric parotid gland and periparotid region are subject to a variety of lesions and are most often evaluated with ultrasonography (US), contrast material-enhanced computed tomography (CT), and magnetic resonance (MR) imaging. US may be used to assess the size of the parotid gland, distinguish diffuse from focal disease, assess vascularity and adjacent vascular structures, distinguish cystic from solid lesions, and guide fine-needle aspiration. However, further evaluation with CT or MR imaging may be needed to better define the nature and extent of disease. CT is the imaging modality of choice for most pediatric parotid disease (including acute inflammation, abscess, calculi, and major salivary duct obstruction) and most solid masses and may obviate sedation. However, a mass associated with facial nerve symptoms should be evaluated with MR imaging because it is the only modality that can consistently demonstrate the facial nerve. Findings at US, CT, and MR imaging allow localization of parotid lesions and may suggest a specific cause. Clinical information, familiarity with normal parotid anatomy at various stages of its development, and knowledge of the imaging characteristics of parotid and periparotid lesions are essential for appropriate radiologic evaluation. This information can be used to guide therapy and plan a surgical approach.  相似文献

The purpose of this study was to compare the performance of magnetic resonance (MR) imaging using currently available techniques with contrast-enhanced single-phase helical computed tomography (CT) in depicting extrahepatic disease in patients with malignancy. At two institutions, 164 patients with known or suspected malignancy underwent abdominal imaging with contrast-enhanced helical CT and MR imaging. The prospective interpretations of the CT scans and MR examinations were used to assess the sensitivity of each imaging test in detecting benign and malignant extrahepatic disease at 17 anatomic sites. Imaging findings were compared with results of surgery in 57 patients and with the combined results of image-guided biopsy, follow-up cross-sectional imaging studies, other concurrent imaging tests, and clinical follow-up. For the 164 patients, helical CT scans depicted 221 of 316 (70%) sites of proven extrahepatic tumor compared with 288 sites (91%) (P < 0.0001) for MR imaging. For the 57 patients who underwent exploratory laparotomy, helical CT scans depicted 101 of 154 (66%) findings of surgically confirmed extrahepatic tumor compared with MR imaging, which depicted 139 sites (90%) (P < 0.0001). Anatomic sites at which MR imaging showed a significantly greater detection of extrahepatic tumor included the peritoneum, bowel, and vascular and osseous structures. For depiction of benign extrahepatic disease, there was no significant difference between helical CT and MR imaging. MR imaging, using currently available T1-weighted, RARE T2-weighted, and gadolinium-enhanced imaging, is effective in depicting extrahepatic disease in patients with malignancy. Compared with single-phase helical CT scanning, MR imaging shows an advantage in depicting tumor involving the peritoneum, omentum, bowel, and osseous and vascular structures.  相似文献

Twenty-three pediatric patients with white matter abnormalities on MR images were evaluated retrospectively to assess the contribution of MR compared with CT in diagnosing these conditions. In addition, the MR findings in major categories of white matter diseases were analyzed for sensitivity in detecting the presence of an abnormality. White matter disease categories included demyelinating disease (five cases), dysmyelinating disease (eight cases), developmental white matter abnormalities (four cases), and white matter abnormalities of unknown origin (idiopathic) (six cases), as seen on long TR images. We found that MR is not more sensitive than CT in detecting disease in the demyelinating or dysmyelinating categories, although it is more sensitive than CT in detecting the degree of disease present. In cases of developmental delay, MR is distinctly more useful than CT in demonstrating abnormalities of myelination. And in the idiopathic group, MR detected the presence of focal white matter abnormalities on long TR images in children with neurologic complaints and normal CT. MR may serve to redefine and broaden the spectrum of reported imaging abnormalities in pediatric patients.  相似文献

Recently, the application of intravenous gadolinium-diethylenetriamine pentaacetic acid (Gd-DTPA) has been shown to improve the detection of intradural extramedullary spinal disease in adults. The ability of Gd-DTPA enhanced magnetic resonance (MR) to detect intradural extramedullary spinal metastases was studied in pediatric brain tumor patients. Spinal MR images before and after intravenous injection of Gd-DTPA were analyzed retrospectively in eight pediatric patients with known intracranial neoplasms and clinically suspected subarachnoid tumor seedings. Contrast enhanced spinal MR was compared with CT myelography in four of these patients. In our pediatric population Gd-DTPA enhanced images revealed tumor seeding not appreciable on noncontrast images. Although CT myelography has been the accepted standard investigation in the evaluation of suspected spinal metastases in children, we found that contrast enhanced MR is equal or superior in sensitivity to CT myelography. Spinal MR also provided information not obtainable via CT myelography. In the future, Gd-DTPA enhanced spinal MR should be considered in the initial evaluation of suspected subarachnoid spinal metastases in pediatric patients with known primary brain tumors.  相似文献

Twenty patients with biopsy-proven osteogenic (11 cases) or Ewing's (nine cases) sarcoma were evaluated by MR imaging on a 0.15-T resistive unit to determine the value of MR in the diagnosis and treatment of these two neoplasms and to develop the best protocol for MR imaging. In all 20 cases, MR identified tumor spread into bone marrow, and it was superior to CT in five cases. Extension of tumor into the soft tissues adjacent to bone was shown better by MR than CT in six cases. Improved anatomic information from MR is the result of the ability to image in the axial, coronal, and sagittal planes. Compared with CT, MR identifies cortical disease but has inferior spatial resolution and defines calcium poorly. MR can be used to monitor tumor response to chemotherapy, and the relationship of tumor to adjacent vasculature can be determined without the use of contrast agents. Two pulse sequences are necessary for maximum display of disease, since, in general, tumor involvement of the bone marrow is best assessed on T1-weighted sequences, and tumor involvement of the soft tissue is best seen on T2-weighted sequences. Additional information about bone-marrow involvement, soft-tissue tumor extent, and the relationship of tumor to blood vessels makes MR a valuable adjunct to CT in the evaluation of these neoplasms.  相似文献

Surface coil MR imaging is an effective technique for evaluating orbital pathology. T1 and T2 weighting provides information not obtainable by CT and results in better characterization of some lesions. MR appears to be most useful for characterizing ocular pathology, specifically for differentiating retinoblastoma from benign diseases (Coat's disease and PHPV). MR can also be used to separate ocular tumors from associated subretinal fluid. With MR imaging, the signal intensities of lymphoma differ from those of benign inflammatory or infiltrative conditions such as pseudotumor or thyroid ophthalmopathy. Mucoceles, too, have a specific signal intensity pattern with T1 and T2 weighting. CT appears to be superior to MR for demonstrating densely calcified lesions or lesions of bone.  相似文献

We report Langerhans cell histiocytosis involving the sphenoid sinus and extending to the orbital apex. MR and CT imaging, particularly with contrast, both were helpful in defining the extent of the lesion before treatment. MR was a useful examination for follow-up after treatment. Langerhans cell histiocytosis should be considered in the differential diagnosis of lesions of this region in the pediatric age group.  相似文献

BACKGROUND AND PURPOSE: Complex CSF diseases may be underdiagnosed or poorly understood on conventional CT or MR imaging. Although intrathecal CT cisternography with water-soluble iodinated contrast medium has been used, very few studies have dealt with the intrathecal use of gadopentate dimeglumine (Gd-DTPA), though it appears superior to CT. We report our experience with the intrathecal use of Gd-DTPA for MR cisternography and ventriculography in pediatric patients referred for study and treatment of complex CSF-related diseases. MATERIALS AND METHODS: Ten patients (range, 1 month-16 years of age) were selected after we obtained specific informed consent. Intrathecal gadolinium injection was performed via transfontanelle ventriculostomy, ventriculoperitoneal shunt reservoir, or lumbar puncture. Cases included spontaneous CSF leaks (n=1), complex traumatic frontoethmoidal fractures with suspected CSF leak (n=2), multiloculated congenital or acquired hydrocephalus (n=3), intraventricular tumor (n=1), suspected postoperative arachnoiditis (n=1), complex midline defect (n=1), and acquired orbital meningoencephalocele (n=1). RESULTS: No patient showed biologic, behavioral, or neurologic alterations. In complex hydrocephalus or intraventricular cysts, ventriculography Gd-DTPA MR imaging helped to differentiate isolation of a ventricle or noncommunicating cyst in all 4 patients. In suspected posttraumatic CSF leaks, the procedure established with precision the place of the leak in 1 patient and excluded it in the other. In 1 patient who underwent surgery for spinal cord neoplasm, the procedure excluded arachnoiditis. In the other 3 patients with complex CSF-related diseases, the procedure showed distinctive radiologic findings for the understanding and treatment of the disease. Altogether, in 8 patients, imaging findings influenced or changed clinical decisions and surgical planning. CONCLUSIONS: Our preliminary results showed no side effects and potential useful clinical applications in the evaluation of CNS diseases involving the ventricular system or the subarachnoid space in selected pediatric patients.  相似文献

Extranodal lesions in Hodgkin disease may develop and spread to virtually any organ system, simulating other neoplastic or infectious diseases. It is important to determine whether extranodal involvement represents a primary manifestation or dissemination of systemic disease, which has a poorer prognosis. Computed tomography (CT) is the preferred modality, although ultrasonography and magnetic resonance (MR) imaging may also be helpful. CT is superior to conventional radiography in assessing chest disease, although MR imaging is more sensitive than CT in detecting chest wall involvement. CT is preferred for evaluating hepatic lymphoma and has proved particularly valuable in diagnosing gastric lymphoma and detecting renal or perirenal masses. CT and MR imaging are equally effective in detecting brain Hodgkin disease; however, the latter is superior in the detection of extracerebral tumor deposits in the subdural or epidural space. MR imaging is also preferred for evaluating meningeal and spinal cord involvement. Both MR imaging and CT allow excellent assessment of bone texture and accurate analysis of tumoral bone invasion, but MR imaging is superior in demonstrating bone marrow infiltration, and CT is superior in delineating the extent of cortical bone destruction. In the future, metabolic positron emission tomography may provide more information about extranodal lymphoma than do the current imaging modalities.  相似文献

PURPOSE: To determine the optimal imaging modality for diagnosis and staging of ovarian cancer. MATERIALS AND METHODS: Two hundred eighty women suspected to have ovarian cancer were enrolled in a prospective study before surgery. Doppler ultrasonography (US), computed tomography (CT), and magnetic resonance (MR) imaging were used to evaluate the mass; conventional US, CT, and MR imaging were used to stage spread. RESULTS: All three modalities had high accuracy (0.91) for the overall diagnosis of malignancy. In the ovaries, the accuracy of MR imaging (0.91) was higher than that of CT and significantly higher than that of Doppler US (0.78). In the extraovarian pelvis and in the abdomen, conventional US, CT, and MR imaging had similar accuracies (0.87-0.95). In differentiation of disease confined to the pelvis from abdominal spread, the specificity of conventional US (96%) was higher than that of CT and significantly higher than that of MR imaging (88%), whereas the sensitivities of MR imaging (98%) and CT (92%) were significantly higher than that of conventional US (75%). CONCLUSION: MR imaging is superior to Doppler US and CT in diagnosis of malignant ovarian masses. There is little variation among conventional US, CT, and MR imaging as regards staging.  相似文献

Magnetic resonance (MR) imaging characteristics of pineal germinomas are described in seven patients imaged with MR and computed tomography (CT). In patients with symptoms of an enlarging process in the quadrigeminal plate cistern, MR imaging was as sensitive as CT scanning in detecting the mass. MR imaging did not detect a normal-sized, calcified neoplastic gland. Germinoma, germinoma with embryonal cell carcinoma elements, and pineoblastoma demonstrated different MR signal characteristics. Although direct coronal and sagittal MR images were useful in defining the relationship of the tumor to the posterior third ventricle, Sylvian aqueduct, and vein of Galen, the ease, rapidity, and sensitivity of CT scanning suggest that CT should remain the modality of choice for initial evaluation and screening of the pineal region, especially in the younger pediatric population, in whom detection of calcification may provide the only clue of an abnormality.  相似文献

Rhabdoid tumor of the kidney (RTK) is a rare pediatric malignancy. Its imaging features by ultrasound and CT have been well described; however, to our knowledge, there are only a handful of case reports that describe the MR appearance of rhabdoid tumor. We report the MR appearance of a RTK in an 18-month-old boy, review the literature on its imaging features, and discuss the differential diagnosis of solid renal tumors in children.  相似文献

Computed tomography (CT) and magnetic resonance (MR) imaging have become invaluable imaging modalities in the diagnosis of diseases involving the lower urinary tract. Both CT and MR imaging are able to accurately stage bladder carcinoma, with MR imaging able to distinguish between superficial and deep muscle invasion of tumor. CT and MR are also the studies of choice for evaluating retroperitoneal fibrosis, which often affects the urinary tract; MR imaging is often able to detect the presence of active inflammation and occasionally rule out a malignant cause. MR imaging holds promise for the evaluation of stress urinary incontinence and urethral disease. Although diseases of the distal ureter continue to be most accurately diagnosed by intravenous urography and retrograde studies, CT and MR imaging may serve a helpful secondary role.  相似文献

BACKGROUND: The choice of imaging before liver surgery is debated regarding the use of magnetic resonance (MR) imaging, computed tomography (CT), and positron emission tomography (PET). No studies have compared contrast-enhanced PET/CT with superparamagnetic iron oxide (SPIO)-enhanced MR imaging. PURPOSE: To compare PET/CT with superparamagnetic iron oxide (SPIO)-enhanced MR imaging, PET, and CT in the detection of liver metastases (LM) and extrahepatic tumor from colorectal cancer (CRC). MATERIAL AND METHODS: Thirty-five patients with suspected LM underwent PET/CT with a contrast-enhanced CT protocol and SPIO-enhanced MR imaging. Readers independently analyzed images from MR imaging, PET/CT, and the CT part and PET part of the PET/CT study. Imaging findings were compared with surgical and histological findings. RESULTS: Lesion-by-lesion sensitivity and accuracy for liver lesions was 54% and 77% for PET alone, 66% and 83% for PET/CT, 82% and 82% for SPIO-enhanced MR imaging, and 89% and 77% for CT alone, respectively. CT and SPIO-enhanced MR imaging were less specific but significantly more sensitive than PET (P<0.0001). For extrahepatic tumor, sensitivity and specificity was 83% and 96% for PET/CT and 58% and 87% for CT, respectively. CONCLUSION: CT and SPIO-enhanced MR imaging are more sensitive but less specific than PET in the detection of LM. PET/CT can detect more patients with extrahepatic tumor than CT alone.  相似文献

The spectrum of orbital lesions occurring in childhood is wide, including a variety of both benign and malignant disorders. Although physical examination and fundoscopy may aid in establishing the diagnosis of retro-ocular lesions, imaging remains a critical step in the evaluation of the pediatric orbit. Ultrasonography, CT, and MR imaging are the primary modalities for the evaluation of the diseased orbit, and careful observation of the characteristic radiological features usually leads to correct diagnosis; however, some of the lesions look very similar and are difficult to differentiate from each other. The purpose of this article is to review the common and unusual entities that may involve the pediatric orbit, to describe the radiological features, and to evaluate the efficacy of US, CT, and MRI in the diagnosis and management of these conditions.  相似文献

目的分析研究眶壁转移瘤的CT和MRI特点,提高诊断准确率.方法回顾性分析23例经手术病理证实的眶壁转移瘤的CT和MRI表现,成人组16例,儿童组7例.所有病例均行CT平扫与MR平扫和增强扫描.结果16例成年患者CT表现均为溶骨性骨质破坏和眼眶不规则肿块,9例骨质破坏区邻近的颅内可见扁平不规则肿块;16例MRI表现为骨质破坏区邻近的眼眶和颅内均可见略长T1、略长T2信号的不规则软组织影,增强后呈明显强化.7例儿童患者CT表现均为溶骨性骨质破坏和眼眶不规则肿块,其中2例可见垂直针状高密度影,3例骨质破坏区邻近的颅内可见扁平不规则软组织肿块;MRI显示7例骨质破坏区邻近的眼眶内和6例颅内可见略长T1、略长T2信号不规则的软组织影,其中1例为双侧,增强后呈明显强化;7例双侧眶骨、4例斜坡和双侧岩尖及1例双侧颞骨鳞部骨髓腔呈略长T1、略长T2信号影,增强后呈明显强化.结论眶壁转移瘤的CT和MRI表现有一些特点,有助于诊断和鉴别诊断.  相似文献
