Relationships between early motor development and language and reading skills were studied in 154 children, of whom 75 had familial risk of dyslexia (37 females, 38 males; at-risk group) and 79 constituted a control group (32 females, 47 males). Motor development was assessed by a structured parental questionnaire during the child's first year of life. Vocabulary and inflectional morphology skills were used as early indicators of language skills at 3 years 6 months and 5 years or 5 years 6 months of age, and reading speed was used as a later indicator of reading skills at 7 years of age. The same subgroups as in our earlier study (in which the cluster analysis was described) were used in this study. The three subgroups of the control group were 'fast motor development', 'slow fine motor development', and 'slow gross motor development', and the two subgroups of the at-risk group were 'slow motor development' and 'fast motor development'. A significant difference was found between the development of expressive language skills. Children with familial risk of dyslexia and slow motor development had a smaller vocabulary with poorer inflectional skills than the other children. They were also slower in their reading speed at the end of the first grade at the age of 7 years. Two different associations are discussed, namely the connection between early motor development and language development, and the connection between early motor development and reading speed.  相似文献

Specific language impairment (SLI) diagnosed in the pre-school years is frequently associated with reading and writing difficulties at school age. The nature of this relationship is unclear, despite the availability of a large number of studies, mostly on English speaking children. Phonological processing deficits have been considered the prominent cause of both difficulties. However recent findings in both children with SLI and in children with reading difficulties are not easily accommodated within a single dimensional model explaining the relationship between oral and written language deficits.Our study focuses on the long-term reading and spelling outcome in relation to preschool oral language skills in a group of Italian adolescents with a documented history of SLI.Sixteen Italian adolescents diagnosed as SLI at our Hospital in the pre-school years and 32 normal controls were submitted to an extensive assessment of oral and written language skills. At a group level SLI adolescents had weak oral and written language skills in almost all tests.Results show that reading difficulties have some features in common with those of Italian developmental dyslexics but also have distinct characteristics, since reading accuracy and written comprehension, usually relatively spared in Italian developmental dyslexics, were impaired in adolescents with SLI.Longitudinal analyses showed that expressive morpho-syntactic and lexical abilities at pre-school age were the oral language skills that best predicted reading and spelling outcomes in adolescents with SLI.However, also children with severe phonological impairment in the absence of other oral language deficits showed later literacy difficulties, although less severe and mainly limited to reading accuracy.Our study supports the notion that there is a complex relationship between oral and written language difficulties which may change at different developmental time points, not captured by a single deficit model, but best conceptualized considering multiple interactions between language skills and literacy abilities.  相似文献

This study used the Peabody Individual Achievement Test (PIAT) to assess reading and spelling skills in a normal control group (n = 100) and in patients having cerebral damage in the left hemisphere (LH; n = 86) or right hemisphere (RH; n = 76). Relative to the normals, both brain-damaged groups were impaired on PIAT Reading Comprehension and Spelling. Only the LH group was impaired on PIAT Reading Recognition. PIAT performances were strongly related to extent of brain damage within the left hemisphere, but not within the right. Similarly, whereas reading and spelling deficits were associated with temporal- and occipital-lobe involvement in the LH group, presence or absence of specific lobe involvement was unrelated to PIAT performance in the RH group. Relationships also were assessed between PIAT performances and demographic variables and the Average Impairment Rating from the Halstead-Reitan Battery.  相似文献

Alzheimer's disease (AD) and cerebrovascular disorders are the leading causes of dementia in our ageing population. Given that the progression of neuropathological changes in the brains of AD patients initiates several years, and even decades, before the diagnosis of dementia, a great effort has been made to identify potentially modifiable factors that contribute to the pathogenesis of sporadic late-onset AD. Among these factors, cerebrovascular disease and microvascular alterations seem to bilaterally interact with the underlying AD pathology, affecting the progression of cognitive deficits. In addition, cerebrovascular dysfunction has emerged as an early event in AD, encompassing changes in virtually all cell types of the neurovascular unit, including bone marrow-derived cells, astrocytes, pericytes, vascular smooth muscle cells and endothelial cells. In this review, we discuss recent studies implicating cerebrovascular factors in the pathogenesis of AD. We also discuss how the impairment of mechanisms of brain regeneration, such as neurogenesis and angiogenesis, might be related to the vascular dysfunction. Finally, we briefly discuss several therapeutic options targeting the vascular system, which might represent an interesting strategy for preventing or delaying the onset of dementia in AD.  相似文献

“Late talkers” is a term used in the scientific field of atypical language development to describe toddlers who exhibit delay in expressive language skills, although they do have intact receptive skills. This article provides an overview of the literature on late talking toddlers. Specifically, it underscores the risk factors for late talking as well as the parenting and individual characteristics of this group of children. It also presents the association between expressive language delay and the behavioral and socio-emotional development of late talkers, and the language outcomes of late talking toddlers at a later point in development. Our review culminates with recommendations and intervention guidelines for clinicians.  相似文献

Background

Otitis media (OM) leads to significant reduction in the hearing sensitivity. The reduced auditory input, if in the early years of life when the auditory neural system is still maturing, may adversely influence the structural as well as functional development of the system. Past research has reported abnormalities in both the structure and function of brainstem nuclei following auditory deprivation, but, it has not necessarily focused on children who had OM in their first year of life. It can also be said that if auditory processing is affected at the brainstem level because of early onset OM (reduced auditory input in the crucial periods of neural development), then, it may be said that auditory processing is also affected at the cortical level because it receives distorted input from the brainstem. Therefore, the purpose of this study was to document the effects of early onset OM on auditory processing, if any, at the brainstem as well as at cortical levels. A related purpose of the study was to investigate the persistence of the effects of early onset OM, if any, on auditory processing.

Methods

A cross sectional approach and a standard group comparison design was used in the study. Thirty children, who had OM between 6 and 12 months of age and who were in the age range of 3.1 – 5.6 years participated in the study. Children with OM were divided into 3 groups based on their age. Click evoked auditory brainstem responses (ABRs) and late latency responses (LLRs) were recorded from these children, and the responses were compared with those from age and gender matched normal children without any history of OM. The data from the 2 groups was statistically analyzed through independent t test. Pearson's Product Moment correlation was computed to examine the relationship between results of ABR and LLR in children with early onset OM.

Results

The mean central conduction time was significantly increased and the mean amplitude of wave I and III of ABRs was significantly reduced in children with early onset OM compared to normal children. Also, the latency of all LLR waves was significantly less in children with early onset OM than in normal children. However, significant differences in mean values of either ABR or LLR (latencies or interwave intervals as the case may be) were observed only in 3-year old children. There was a significant, but negative association between central conduction time and latency of LLRs.

Conclusion

OM in the first year of life leads to negative effects on brainstem signal processing even if it has occurred only for a short duration (maximum of 3 months). In such a situation, auditory cortical structures probably show compensatory changes through central gain to offset the prolonged central conduction time. Although the results of the present study showed that the negative effects of early onset OM (occurring in the first year of life) on auditory processing disappeared by the time the children were 4.1 years, there is need for longitudinal studies on this to confirm the findings.  相似文献

Comparisons were also made between the speech and lexical development of children with cleft lip and palate and noncleft children at 21 months of age. The participants included 30 children; 15 with cleft lip and palate and 15 noncleft children. Results revealed differences between the groups for several measures of speech and lexical development at 21 months. For the children with cleft palate, correlational analyses suggested that true stop production, both immediately before and after palatal surgery, was positively correlated with a majority of the speech production measures at 21 months. At postsurgery/13 months, true stop production was related to later vocabulary development, and size of true consonant inventory was related to all measure of speech production and one measure of lexical development at 21 months. For the noncleft group, true canonical babbling ratio at 13 months was the only measure that was significantly correlated with any of the speech and/or language measures at 21 months. Background: Experimental studies of short-term memory and working memory (WM) in aphasia fail to discriminate cognitive impairments of different aphasia types—non-fluent, Broca-type aphasia and fluent, Wernicke-type aphasia. However, based on the varying fundamental features of these two aphasia syndromes, the potentially different underlying mechanisms of impairment and scant preliminary evidence of varying cognitive deficits, a differential relationship between cognitive function and language processing in these two groups can be predicted.

Aims: The current study investigates the hypothesis concerning the differential impact of cognitive impairments in individuals with fluent versus non-fluent aphasia types.

Methods & Procedures: Participants with fluent (n = 19) and non-fluent (n = 16) aphasia and participants without brain damage (n = 36) were presented with an eye-tracking WM task. Additionally, individuals with aphasia completed two language comprehension tasks.

Outcomes & Results: Results revealed significant decrease in WM capacity in individuals with aphasia compared with participants without brain damage. The two aphasia groups performed similarly on the WM and language tasks. Furthermore, for participants with non-fluent aphasia, it was revealed that WM makes a significant contribution to language comprehension, while for fluent individuals this relationship was not significant.

Conclusions: Overall, the present data support the claim that there are cognitive deficits in aphasia and that these cognitive deficits tend to exacerbate the language impairments of persons with non-fluent aphasia types. The results are discussed in the context of varying mechanisms of impairment in different types of aphasia. The present findings have important implications both for the assessment and the treatment of individuals with aphasia and for understanding the nature of aphasia.  相似文献

BackgroundGesture precedes vocabulary development and may be an early marker of later language impairment.AimsUsing data from the National Institute of Child Health and Human Development Study of Early Child Care and Youth Development, this study examined the contribution of children’s (N = 1064) early gestures and early vocabularies to their risk of language impairment in preschool years.Methods and proceduresAt age 15 months, maternal reports on children’s use of gestures and vocabulary comprehension and production skills were measured using the MacArthur Communicative Development Inventories. At age 3 and 4.5 years, children’s language skills were assessed using the Reynell Developmental Language Scale and Preschool Language Scale-3, respectively.Outcomes and resultsAfter controlling for child, maternal, and family sociodemographic factors, children at later risk for language impairment were found to exhibit significantly less early gesture use and vocabulary skills relative to their typically developing peers. Early use of gestures was also significantly correlated with early vocabulary skills.Conclusions and implicationsThe effect of early gesture on children’s later risk of language impairment was indirect and mediated by early vocabulary production. Early gesture may have the potential to serve as an early diagnostic tool and play a role in early intervention.  相似文献

It is clear that family/parental peer and individual risk factors that appear early in life contribute to increased susceptibility to addiction. This study aimed to determine the relationship between those risk factors, the development of opiate addiction, and the development of psychological maladjustment in addicts. A total of 252 subjects were selected from in and around Baltimore, aged less than 40 years, and 12 or older at the onset of opiate addiction. There were 342 controls from the same neighbourhood, matched for age, race and place of reidence, but free from opiate addiction. Ten risk factors in the five domains of family disruption, peer deviance, personal deviance, psychological symptoms and lack of protective factors, were studied. The addicts and the non-addicts had different risk profiles comparing black and white people, whilst a significant differentiation was found for each race: white addicts scored significantly higher than non-white on all of the risk factors, whereas black addicts scored significantly higher than black non-addicts on risk factors denoting peer school misbehaviour and fighting, personal drug abuse and behavioural deviance. The study has implications for both the course and treatment of drug addiction. The findings suggest that as well as being a causal agent, drug addiction is a concomitant feature of a variety of problems. Copyright © 1997 Whurr Publishers Ltd.  相似文献

This study documents a patient, SA, with an impairment of semantic memory arising as a result of Semantic Dementia (Pick's disease). The patient is impaired at deriving semantic knowledge from both words and pictures. However, his ability to derive semantic knowledge of countries is relatively spared compared to concrete nouns and famous people. The presence of a semantic deficit was used to investigate the role of semantics in reading and spelling. Several novel cueing/priming paradigms are reported which suggest that SA is able to use partial semantic knowledge to constrain his reading and spelling. These results are broadly consistent with the 'summation hypothesis' [27] and suggest that normal reading and spelling may take place by integrating both semantic information and knowledge of direct orthography-phonology correspondences.  相似文献

OBJECTIVE: The objective of this study is to determine the association between established cardiovascular risk factors and depression among older men. METHODS: The authors conducted a cross-sectional study of a community-representative sample of 5,439 men aged 71-89 years. Cardiovascular disease and risk factors assessed included history of diabetes, hypertension, angina, myocardial infarction, and stroke; current smoking; total cholesterol and fractions; triglycerides; total plasma homocysteine; and MTHFR677 genotype. Depression was defined by a Geriatric Depression Scale 15 items score of 7 or greater. RESULTS: A complete data set was available for 4,204 men, of whom 212 were depressed (5%). Men who were depressed reported higher frequency of diabetes (23.1% versus 13.2%), angina (30.2% versus 20.4%), myocardial infarction (26.2% versus 16.0%), and stroke (23.6% versus 9.1%) than nondepressed men. Participants with depression were also more likely to have plasma homocysteine above 15 mumol/L (39.1% versus 25.5%) and high triglycerides (32.1% versus 20.9%) than nondepressed subjects. Depressed older men were also more likely to be active smokers (9.9% versus 4.8%). The other factors measured in the study were not significantly associated with depression. Estimation of the population-attributable fraction (PAF) after logistic regression showed that high plasma homocysteine had the highest PAF for depression (PAF:15%, 95% confidence interval [95% CI]: 5%-23%) followed by high triglycerides (PAF: 11%, 95% CI: 2%-18%), angina (PAF: 9%, 95% CI: 2%-15%), stroke (PAF: 8%, 95% CI: 3%-13%), diabetes (PAF: 7%, 95% CI: 1%-13%), myocardial infarction (PAF: 5%, 95% CI: 0%-11%), and smoking (PAF: 5%, 95% CI: 1%-9%). CONCLUSIONS: High plasma homocysteine and triglycerides appear to account for a considerable proportion of cases of depression in older men. The successful management of these risk factors may contribute to decrease the prevalence of depression in later life.  相似文献

目的探讨影响颅脑损伤去骨瓣减压术后硬膜下积液发生的独立危险因素。方法回顾性分析2012年2月至2014年2月149例颅脑损伤去骨瓣减压术患者的临床资料,其中术后发生硬膜下积液47例(积液组),未发生硬膜下积液102例(非积液组)。对其危险因素进行单因素分析和多因素Logistic回顾分析。结果两组患者入院时GCS评分、手术时机、血肿位置、血肿量、基底池受压、中线结构位移≥10 mm、蛛网膜下腔出血及脑积水发生率差异有统计学意义(P〈0.05);多因素Logistic回归分析结果显示,入院时GCS评分≤5分、血肿量〉40 ml、中线结构位移≥10 mm、蛛网膜下腔出血、脑积水、基底池受压是颅脑损伤去骨瓣减压术后硬膜外积液的独立危险因素。结论损伤严重、血肿量大、CT表现为基底池受压和中线结构位移≥10 mm、并发蛛网膜下腔出血及脑积水的颅脑损伤患者在去骨瓣减压术后发生硬膜下积液的风险较高,应给予重视。  相似文献

In the etiology of dementia several genetic and various environmental factors are assumed to interact. Most epidemiological studies on risk factors have focused on "constitutional" factors like age, sex, birth order, dementia in the family and "life style" factors like smoking and alcohol consumption. Only few studies have investigated the role of psychosocial factors at work and during leisure time on the risk to develop dementia. Our paper gives a systematic review of all published results of controlled studies about psychosocial risk factors. A positive association has been observed between dementia and living alone, having no close social ties, not participating in social and leisure activities, and never having married. Recent studies have found that Alzheimer's disease in particular is negatively associated with diversity of activities and intensity of intellectual activities and positively associated with psychosocial inactivity, unproductive working style, living with a dominant spouse and with physical inactivity.  相似文献

BackgroundThis investigation addresses the question of whether there exists a significant discrepancy in the cognitive abilities of children with cerebral palsy (CP) who, despite the presence of age appropriate communication and language, have reading and spelling delays.AimsWe wanted to discover whether there was a relationship between the phonological and visual perceptual abilities of children with CP and their progress in reading and spelling.Methods and proceduresFifteen children with CP (aged between 6:9 years and 11:6 years) were assessed on reading and spelling; communication and language; non-verbal reasoning; phonological processing; and visual perception.Outcomes and resultsTen of the children had very weak reading and spelling skills. Five children had (mostly) age appropriate scores of reading and spelling. No differences were found between these two groups in non-verbal reasoning or communication and language. However, phonological abilities, visual sequential memory and perception of visuospatial relationships were found to be related to reading and spelling.Conclusions and implicationsThe findings suggest that children with CP are at risk for reading and spelling delays when they have poor phonological processing, visual sequential memory and perception of visuospatial relationships. The implications of the findings for classroom practice are discussed.  相似文献
