Incidence data from the Shanghai (People's Republic of China) Cancer Registry were used to assess the temporal trends of three major female cancers during 1972–89. Rates for cancers of the breast, corpus uteri and, to a lesser extent, ovary rose over the study period. The increases in breast and ovarian cancer were most pronounced among women under age 50, whereas those for corpus uteri cancer were restricted generally to those aged 55 to 69 years. When considered by cohort year of birth, risk of breast and ovarian cancers rose among women born since 1925 and 1935, respectively, but little evidence of cohort effect was apparent for corpus uteri cancer. Potential explanations for these patterns are explored.  相似文献

目的对1972~1999年上海市区常见的女性生殖系统恶性肿瘤的发病率进行统计,分析其发病趋势及变化原因,为防治措施的制定提供依据.方法根据上海市肿瘤发病登记处收集的1972~1999年的上海市区卵巢癌、宫颈癌、宫体癌和不明部位子宫癌的病例资料和相应年份的人口资料,分别计算各年龄组的年龄别发病率.并采用直接法计算世界人口标化发病率,对数线性回归法计算标化率的年变化率(Annual percentage change,APC),并对病例数进行加权计算.结果1972~1999年上海市肿瘤登记处共登记卵巢癌6106例、宫颈癌8063例,宫体癌3 933例和不明部位子宫癌1 312例.28年来,宫体癌和卵巢癌的标化发病率呈上升趋势,分别从1972~1974年的2.49/10万和4.77/10万上升至1996~1999年的4.75/10万和6.88/10万,年增长率分别为3.0%和2.0%.同期宫颈癌的标化发病率从26.66/10万快速下降至2.18/10万,年下降率达10.5%.不明部位子宫癌的标化发病率亦呈下降趋势(P<0.01).宫体癌以55~64和65~74岁组发病率上升最快,年增长率分别为2.5%和3.3%.卵巢癌各年龄组的发病率均有上升趋势,年变化率都在1.0%以上.宫颈癌发病率下降最快的年龄组是45~54和55~64岁组,25~34和35~44岁组的发病率在近几年有升高趋势.结论上述肿瘤的发病率及年龄别发病率的变化趋势提示,上海女性生活方式和环境因素的改变可能是导致这种变化的重要原因.  相似文献

Objectives: Despite the fact that socioeconomic status (SES) has been shown to have important implications in health related issues, population-based cancer registries in the United States do not routinely collect SES information. This study presents a model to estimate the SES of cancer patients in the registry database.Methods: At the Los Angeles Cancer Surveillance Program (CSP), we developed a model to estimate each cancer patient's SES from aggregate measurements of the census tract of residence (n = 1,640) at time of diagnosis. We then applied the SES estimates to observe the relationship between SES and risk of cancers of the female breast and reproductive organs including cancers of the ovary, cervix uteri, and corpus uteri. The analyses were performed on the cumulative records (n = 127,819) of cancer patients diagnosed between 1972 and 1992 in Los Angeles County, California, for the mutually exclusive racial/ethnic groups of non-Hispanic Whites, Hispanic Whites, Blacks, Asians, and persons of other ethnic groups.Results: We found SES is positively associated with female breast cancer, ovarian cancer, and cancer of the corpus uteri, but inversely associated with cervical cancer. These SES trends were quite consistent across age groups among non-Hispanic White women. Variations by race/ethnicity in the SES patterns were also found, with Asians exhibiting little association.Conclusions: Our model of measuring SES is sufficiently sensitive to capture the trends. Adopting the aggregate approach to measure SES in population-based registry data appears to be useful.  相似文献

Systematic analysis of mortality trends of cervix and corpus uteri cancers is difficult in Italy, as in many other countries, because of the poor specification of uterine cancer subsites in official death statistics. The aim of this article is to propose a method for the analysis of uterine cancers mortality based on high quality incidence and prevalence data from population‐based cancer registries. The method assumes that the excess mortality of cancer patients, compared to death rates expected in the general population, is attributable to the specific cancer. The method is applied to estimate mortality trends for cancers of cervix, corpus and uterus as whole, during the period 1987–1999, in an area covered by 8 Italian cancer registries. Official mortality rates for the 2 subsites were about 60% lower than excess mortality rates, due to the very high proportion of deaths attributed to not specified subsite. Age adjusted cervical cancer excess mortality rates decreased from 3.7 to 2.7 × 100,000 women. Excess mortality for corpus uteri cancer remained approximately stable between 3 and 3.3 × 100,000 women in the period 1990–1999. The results support the efficacy of organized screening in reducing cervical cancer mortality. The same method can be used to assess mortality rates for every cancer entity identifiable in cancer registries data, not otherwise available from official death records. © 2008 Wiley‐Liss, Inc.  相似文献

Analysing time trends in mortality from cancers of the cervix and corpus uteri using routine data sources (such as the World Health Organistion mortality database) involves two major problems: deaths certified as “uterus, unspecified site”, and the presence of a combined category comprising unspecified and corpus uteri cancer deaths. To avoid misleading interpretations, the unspecified and the misclassified data must be incorporated into the analysis to produce rates that allow meaningful comparisons between populations and over time. Reallocation methods based on age- and time-specific distributions of cervix and corpus uteri cancer are applied to the unspecified deaths, while for those in the combined category, the age- and time-specific distributions of unspecified and corpus uteri cancer are considered. Adjustments of the general strategies for reallocation were developed to take into account the different quality of the data. Results from eight European countries with different degrees of coding precision are presented. The reallocation methods bring the cervix and corpus uteri mortality trends more in line with the trends for countries with more precise data while keeping the country-specific characteristics. In addition, the methods ensured the availability of time trends for corpus uteri cancer in women age 50 years and older, which were completely missing without reallocation. We propose generally applicable reallocation methods that allow valid time trend analysis of cervix and corpus uteri cancer mortality using datasets of varying precision. Our results show that any sensible analysis of time trends must involve procedures for correcting for unspecified and misclassified uterine cancer deaths. The association between cancers of the breast, endometrium and ovary is reviewed, using mainly population-based data from the cancer registries in Denmark and Connecticut, U.S.A. Breast cancer patients had an approximately three-fold increased risk of developing a cancer in the contralateral breast. The risk of breast cancer was also elevated following cancers of the corpus uteri and ovary, with relative risk (RR) estimates of about 1.5 from 1 to 4 years after the diagnosis of the first primary cancer. An increased risk of cancer of the corpus uteri subsequent to breast cancer was found in Connecticut, but not in Denmark. After an ovarian cancer, the risk of cancer of the corpus uteri was also elevated (RR = 1.6–2.3). An increased risk of ovarian cancer was observed subsequent to breast cancer (RR = 1.3–1.7), whereas the ovarian cancer risk decreased with time since the diagnosis of a cancer of the corpus uteri, probably reflecting treatment involving oophorectomy.  相似文献

Italian death certification data from 1955 to 1979 for total cancer mortality and 30 cancer sites in the population aged 25 to 74, were analyzed using a log-linear Poisson model to isolate the effects of birth cohort, calendar period of death, and age. The most frequent cohort pattern was characterized by increases up to the generations born between 1920 and 1930, followed by stabilization or a slight decrease. This pattern was evident for total cancer mortality in men, and for several common sites, including larynx, lung, esophagus, bladder, female breast, and ovary. Only four sites (pancreas, pleura, intestines in both sexes, and kidney in men) showed cohort values still rising in more recent generations. Stable cohort and period of death curves were observed for cancers of the prostate and testis, whereas trends were steadily going down for neoplasms of the stomach, and (cervix) uteri. Finally, there were a few discontinuous trends (e.g., in the case of brain neoplasms, leukemias, and lymphomas), which probably reflect different effects of improvements in diagnosis and/or treatment. Period of death values increased for lung and other tobacco related sites (chiefly in males) and, up to the early 1970s, for a few other common sites, including intestines, and the female breast. Downward trends over the calendar period were evident for cancers of the stomach and of the (cervix) uteri. Therefore, total cancer mortality trends over the calendar period of death were moderately increasing for men, and slightly decreasing for women.  相似文献

Background: The objective of this study WAS to describe cancer incidence rates and trends among THE HongKong population for the period 1983-2008. Methods: Incident cases and population data from 1983 to 2008 wereobtained from the Hong Kong Cancer Registry and the Census and Statistics Department, respectively. Agestandardizedincidence rates (ASIR) were estimated and joinpoint regression was applied to detect significantchanges in cancer morbidity. Results: For all cancers combined, the ASIR showed declining trends (1.37%in men, 0.94% in women), this also being the case for cancers of lung, liver, nasopharynx, stomach, bladder,oesophagus for both genders and cervix cancer for women. With cancer of thyroid, prostate, male colorectal,corpus uteri, ovary and female breast cancer an increase was evident throughout the period. The incidence forleukemia showed a stable trend since early 1990s, following an earlier decrease. Conclusions: Although overallcancer incidence rates and certain cancers showed declining trends, incidence trends for colorectal, thyroid andsex-related cancers continue to rise. These trends in cancer morbidity can be used as an important resource toplan and develop effective programs aimed at the control and prevention of the spread of cancer amongst theHong Kong population. It is particularly useful in allowing projection of future burdens on the society with theincrease in certain cancer incidences.  相似文献

The magnitude of socio-economic differences in health varies between societies, and over time within societies. Even in the Nordic countries, where socio-economic differences are not as striking as elsewhere, such differences have been observed. We have studied social class variation among 45- to 69-year-old Finns during 1971-1995 in the incidence of cancers of the breast, ovary, corpus and cervix uteri, vulva and vagina, by means of a computerised record linkage of the Finnish Cancer Registry and the 1970 Population Census, which included social class data. Cancers of cervix uteri (both invasive and in situ) and vagina were associated with low social class. Cancers of the breast (both in men and women) were most common in high social classes throughout the whole observation period 1971-1995, whereas for cancer of the corpus uteri, the positive social class association disappeared in the early 1980s. For cancer of the vulva and ovary, no clear differences by social class were observed. We believe that socio-economic differences usually point to life styles or life conditions (e.g., reproductive patterns, viral infections, diet, physical activity, prevalence of overweight and obesity, smoking and alcohol consumption or combinations of these factors) that may be risk factors for the specific cancers studied.  相似文献

The Seoul cancer registry was established in 1991. Cancer is a notifiable disease, and registration of cases is done by passive and active methods. The registry contributed survival data for 56 cancer sites or types registered during 1993-1997. Follow-up information has been gleaned predominantly by passive methods with median follow-up ranging between 5-82 months for various cancers. The proportion with histologically verified diagnosis for different cancers ranged between 23-99%; death certificates only (DCOs) comprised 0-67%; 33-100% of total registered cases were included for survival analysis. The top-ranking cancers on 5-year age-standardized relative survival rates were testis and placenta (95%), thyroid (93%), non-melanoma skin (93%), corpus uteri (79%), renal pelvis (77%), cervix (76%), Hodgkin lymphoma (75%), breast (74%) and prostate (74%). Five-year relative survival by age group showed a decreasing trend with increasing age groups for cancers of the small intestine, colon, gall bladder, cervix, corpus uteri, ovary, kidney, urinary bladder and thyroid, or was fluctuating for other cancers.  相似文献

Data from 10 Italian population-based cancer registries were used to estimate the prevalence of female tumors of the breast and genital tract. The total prevalence, expressed in number per 100,000, was highest for breast cancer (1,117), followed by cancer of the corpus (264) and cervix uteri (146), ovary (110), and vagina and vulva (23). For all tumors the prevalence increased with age at diagnosis. The cancer prevalence was divided into intervals from diagnosis, expressing different health needs in terms of therapy and intensity of clinical follow-up. For all tumors considered, 1-year prevalence was higher than 1-2-year prevalence, reflecting a high death risk due to perioperative mortality and to the proportion of patients diagnosed at advanced stages. The prevalence decreased in the following intervals considered. Noticeable geographic variability was observed in the prevalence across Italy, with higher proportions being registered in the northern-central regions than in the South. The two extreme 0-5-year prevalence figures (per 100,000) were: for breast cancer 568 (Genova) and 259 (Ragusa); for corpus uteri cancer 94 (Romagna) and 21 (Latina); for cervix uteri cancer 63 (Romagna) and 26 (Latina); for ovarian cancer 49 (Parma) and 21 (Latina); for cancer of the vagina and vulva 17 (Genova) and 5 (Ragusa). This variability depends mainly on incidence and on the proportion of elderly in the general population. From 1987 to 1992 there was an increase in the prevalence of tumors of the breast, ovary and vagina and vulva, especially in the elderly. The prevalence of corpus uteri cancer decreased slightly in the elderly only, whereas that of cervix uteri cancer decreased at all ages.  相似文献

[目的]探讨上海市杨浦区妇女乳腺与生殖器官癌症死亡现状及流行特征,为今后妇女癌症防治提供一定的参考依据。[方法]收集1988年至2007年杨浦区共20年的肿瘤死亡监测资料,分析女性乳腺癌与主要生殖器官癌症死亡情况。[结果]1988年到2007年杨浦区妇女乳腺癌与主要生殖器官癌症死亡率分别是乳腺癌12.53/10万、宫颈癌2.15/10万、子宫体癌和未特指部位子宫恶性肿瘤2.69/10万、卵巢癌4.68/10万、外生殖器癌0.25/10万。乳腺癌、宫颈癌、子宫体癌和未特指部位子宫恶性肿瘤死亡主要以70岁以上老年人群为主,卵巢癌死亡以50岁以上人群为主。[结论]加强杨浦区女性乳腺与生殖器官的保健.减少女性乳腺与生殖器官癌症的死亡.仍是今后杨浦区妇女保健工作和癌症防治工作的重要内容。  相似文献

Toward a comparison of survival in American and European cancer patients   总被引:11,自引:0,他引:11  
BACKGROUND: Only recently have extensive population-based cancer survival data become available in Europe, providing an opportunity to compare survival in Europe and the United States. METHODS: The authors considered 12 cancers: lung, breast, stomach, colon, rectum, melanoma, cervix uteri, corpus uteri, ovary, prostate, Hodgkin disease, and non-Hodgkin lymphoma. The authors analyzed 738,076 European and 282,398 U.S. patients, whose disease was diagnosed in 1985-1989, obtained from 41 EUROCARE cancer registries in 17 countries and 9 U.S. SEER registries. Relative survival was estimated to correct for competing causes of mortality. RESULTS: Europeans had significantly lower survival rates than U.S. patients for most cancers. Differences in 5-year relative survival rates were higher for prostate (56% vs. 81%), skin melanoma (76% vs. 86%), colon (47% vs. 60%), rectum (43% vs. 57%), breast (73% vs. 82%), and corpus uteri (73% vs. 83%). Survival declined with increasing age at diagnosis for most cancers in both the U.S. and Europe but was more marked in Europe. CONCLUSIONS: Survival for most major cancers was worse in Europe than the U.S. especially for older patients. Differences in data collection, analysis, and quality apparently had only marginal influences on survival rate differences. Further research is required to clarify the reasons for the survival rate differences.  相似文献

The present study is aimed to compare survival and prognostic changes over time between elderly (70–84 years) and middle-aged cancer patients (55–69 years). We considered seven cancer sites (stomach, colon, breast, cervix and corpus uteri, ovary and prostate) and all cancers combined (but excluding prostate and non-melanoma skin cancers). Five-year relative survival was estimated for cohorts of patients diagnosed in 1988–1999 in a pool of 51 European populations covered by cancer registries. Furthermore, we applied the period-analysis method to more recent incidence data from 32 cancer registries to provide 1- and 5-year relative survival estimates for the period of follow-up 2000–2002.A significant survival improvement was observed from 1988 to 1999 for all cancers combined and for every cancer site, except cervical cancer. However, survival increased at a slower rate in the elderly, so that the gap between younger and older patients widened, particularly for prostate cancer in men and for all considered cancers except cervical cancer in women. For breast and prostate cancers, the increasing gap was likely attributable to a larger use of, respectively, mammographic screening and PSA test in middle-aged with respect to the elderly. In the period analysis of the most recent data, relative survival was much higher in middle-aged patients than in the elderly. The differences were higher for breast and gynaecological cancers, and for prostate cancer. Most of this age gap was due to a very large difference in survival after the 1st year following the diagnosis. Differences were much smaller for conditional 5-year relative survival among patients who had already survived the first year.The increase of survival in elderly men is encouraging but the lesser improvement in women and, in particular, the widening gap for breast cancer suggest that many barriers still delay access to care and that enhanced prevention and clinical management remain major issues.  相似文献

Digoxin is a phyto-estrogen capable of inducing hormonal effects. Use has been associated with increased risk of breast cancer, an estrogen-sensitive malignancy. The incidence of corpus uteri (uterus) cancer is also strongly increased with exposure to estrogens. Therefore, we evaluated whether digoxin use might also increase its incidence. In all women in Denmark, we identified digoxin users from 1995 through 2008 using a nationwide pharmacy registry system. Cancer occurrence was obtained from Danish Cancer Registry. Relative risk was determined using incidence risk ratios (RR) and 95% confidence intervals (CIs) relative to non-users after adjustment for age- and calendar-time. For ovarian and cervical cancers, RRs in users and non-users were similarly evaluated, these cancers representing gynecological cancers with weak or no associations to estrogen exposure. Of 2.1 million women, 104,648 (4.9%) had digoxin exposure and 137,493 6.5% had exposure to angina drugs but not digoxin during the study period. For uterus cancer, the RR was increased in current digoxin users (1.48, 95% CI: 1.32-1.65; N = 350). Incidence was marginally increased in former users. For ovary and cervix cancers, RRs in current digoxin users were 1.06 (95% CI: 0.92-1.22; N = 207) and 1.00 (95% CI: 0.79-1.25; N = 81), respectively. We examined risks in women using angina drugs but not digoxin to determine whether being under cardiac care affected risk. Among women using angina drugs only, RRs for uterus, ovary or cervix cancers were not statistically significant. We conclude that women currently using digoxin, a phyto-estrogen, have an increased risk of developing uterus cancers.  相似文献

The proportion with histologically verified diagnosis for different cancers ranged between 16-100%; death certificates only (DCOs) comprised 0-51%; 49-100% of total registered cases were included for the survival analysis. The top-ranking cancers on 5-year age-standardized relative survival rates were testis (98%), thyroid (90%), ureter (87%), adrenal gland (86%), nonmelanoma skin (83%), corpus uteri (82%), Hodgkin lymphoma (81%), breast and cervix (74%). Five-year relative survival by age group showed a decreasing trend with increasing age groups for cancers of the stomach, small intestine, colon, gall bladder, larynx, lung, breast, cervix and ovary, and was fluctuating for other cancers.  相似文献

Incidence rates for a number of cancers in urban Shanghai, China, have been changing markedly. Herein we update the trends using population-based data from the Shanghai Cancer Registry for 1972-1994. During 1993-1994, cancers of the lung, stomach, and liver were the 3 leading forms among men, with age-adjusted (world standard) incidence rates of 50.9, 39.2, and 26.5 per 100,000 person-years, respectively, followed by cancers of the colon (12.4) and esophagus (10.0). Among women, cancers of the breast (27.5), stomach (19.1), and lung (17.7) were the most common tumors, followed by cancers of the colon (11.3) and liver (9.4). Over the 23-year period, the rate for all cancers combined, excluding non-melanoma skin cancer, decreased from 247.5 to 215.2 among men and from 173.6 to 154.0 among women. However, trends for individual forms of cancer varied considerably. Rates doubled for cancers of the colon and biliary tract in both sexes, and they increased substantially for cancers of the brain and nervous system, kidney, pancreas, prostate, corpus uteri, female breast, and ovary, and for non-Hodgkin's lymphoma. Rates for cancers of the lung and rectum changed little. Rates declined by at least one-half for cancers of the esophagus and cervix, with notable decreases also for cancers of the stomach and liver. Some of these trends may reflect variations in diagnostic or screening practices, although changes in lifestyle and other environmental exposures are likely to play important roles. Further epidemiologic research in China is needed to identify risk factors influencing the cancer incidence trends.  相似文献

In this article, we analyzed trends in incidence rates of the major cancer sites for a 14‐year period, 1993–2006, in the Sousse region localized in the centre of Tunisia. Five‐year age‐specific rates, crude incidence rates (CR), world age‐standardized rates (ASR), percent change (PC) and annual percent change (APC) were calculated using annual data on population size and its estimated age structure. A total of 6,975 incident cases of cancer were registered, with a male to‐female sex ratio of 1.4:1. ASRs showed stable trends (?0.1% in males, and +1.0% in females). The leading cancer sites in rank were lung, breast, lymphoma, colon‐rectum, bladder, prostate, leukemia, stomach and cervix uteri. For males, the incidence rates of lung, bladder and prostate cancers remained stable over time. While, cancers of colon‐rectum showed a marked increase in incidence (APC: +4.8%; 95% CI: 1.2%, 8.4%) and non‐Hodgkin's lymphoma (NHL) showed a notable decline (APC: ?4.4%; 95% CI: ?8.2, ?0.6). For females, cancers of the breast (APC: +2.2%; 95% CI: 0.4%, 4.0%) and corpus uteri (APC: +7.4%; 95% CI: 2.8%, 12.0%) showed a marked increase in incidence during the study period, while the cervix uteri cancer decreased significantly (APC: ?6.1%; 95% CI: ?9.2%, ?3.0%). The results underline the increasing importance of cancer as a cause of mortality and morbidity in Tunisia. Our findings justify the need to develop effective program aiming at the control and prevention of the spread of cancer amongst Tunisian population.  相似文献

This study concerns the survival of European patients diagnosed between 1978 and 1989 with cancer of corpus and cervix uteri and ovary. Variations in survival in relation to age, country and period of diagnosis were examined. Data from the EUROCARE study were supplied by population-based cancer registries in 17 countries to a common protocol. Five years after diagnosis, relative survival rates were 75, 62 and 35% for cancers of the endometrium, cervix and ovary, respectively. Survival decreased markedly with age. The decrease was especially evident for ovarian cancer, which declined from 65% (15–45 years) to 18% (75+ years). In 1985–1989 there were important inter-country differences in survival for European women with gynaecological cancers: Eastern European countries were characterised by low 5-year relative survival whilst in Sweden, Austria, The Netherlands and Switzerland survival was generally higher than for other European countries. From 1978–1989, 5-year relative survival improved slightly for cervical cancer and improved more among the oldest patients. Prognosis also improved slightly for patients with ovarian tumours and this increase (around 20%) was concentrated among patients between 15 and 64 years of age. Intercountry differences in survival did not in general reduce over time, although for ovarian cancer survival differences narrowed probably in relation to the more widespread use of more effective chemotherapy. Intercountry and time differences in survival for cervical cancer are almost certainly related to variations in the effectiveness of cervical screening programmes. For corpus uteri cancer there was no improvement in survival over the period of this study and intercountry survival differences for this cancer are probably related to differences in patient management.  相似文献

目的根据中国各省市登记处上报的2015年肿瘤登记数据,估算2015年中国子宫体癌发病与死亡情况。方法收集整理中国501个登记处上报的2015年肿瘤登记数据,对数据进行质量审核和评估,将符合标准的368个登记处数据纳入分析,按地区(城市、农村、东部、中部和西部)和年龄等因素分层,计算子宫体癌的发病率和死亡率,结合2015年全国人口数据估算全国子宫体癌发病和死亡数据。标化率分别采用2000年全国普查人口和Segi′s世界标准人口计算。结果2015年纳入分析的368个登记处覆盖309553499人口,占全国2015年年末人口的22.52%。估计全国子宫体癌新发病例数约6.89万,发病率为10.28/10万,中标率为6.86/10万,世标率为6.66/10万。城市地区发病率(11.35/10万)高于农村地区(8.90/10万),东部地区发病率(12.12/10万)高于中部地区(9.94/10万)和西部地区(8.25/10万)。估计2015年全国因子宫体癌死亡病例数约1.60万,死亡率为2.39/10万,中标率为1.49/10万,世标率为1.47/10万。城市地区死亡率(2.40/10万)与农村地区(2.39/10万)相似;中部地区死亡率(2.55/10万)高于东部地区(2.32/10万)和西部地区(2.31/10万)。结论中国子宫体癌发病不断上升且有年轻化趋势,威胁女性健康,疾病负担逐渐加重。中国子宫体癌发病存在城乡和区域差异,应有针对性地开展防控工作。  相似文献
