目的常规拍摄腰椎斜位片观察峡部,但制约因素较多,如肠气、肠内容物较多,常不能满足临床工作的需要。通过与腰椎斜位普通X射线平片比较,探讨数字断层融合技术(DTS)在腰椎斜位摄片中的应用价值。方法选取60例腰椎疼痛患者,其中男性34例,女性26例;年龄19~72岁,平均年龄42.5岁。行斜位普通X射线平片和DTS成像,将2种图像的清晰度、对比度进行比较,分析其相应的表现和诊断。结果 60例腰椎疼痛患者所拍腰椎斜位普通X射线平片与DTS图像的峡部裂比率分别为25.0%、38.3%,可疑峡部裂比率分别为25.0%、3.4%,腰椎斜位普通X射线平片有50.0%未见明显峡部裂,而DTS为58.3%,由此可见,DTS图像质量高于普通成像。结论 DTS成像拍摄腰椎斜位图像清晰度高,避免了临近结构的重叠及肠气、肠内容物等其他因素的限制,辐射剂量低,价格低廉,在实际明确诊断腰椎峡部裂中起到很重要的作用。  相似文献

本文研究了102例国人成年颅锥体角度,计算了颅径与锥体角之间的平均值之相关系数,其中颅长(g-op)、颅长减颅宽(g-op—eu-eu)和锥体角之间的相关系数分别为(-0.288)和(-0.326),表明负相关度比较密切。其结论是。颅长径越长,则锥体角越小,反之亦然。国人锥体角平均度数为52度(精确数为51.635°)。两侧相等者占46.6%,不等者为53.4%,但性差和侧差均不显著,即t值分别为0.74和1.51,P>0.05。计算了从颅长和长、宽差值推算锥体角的回归方程式,其回归系数均为(-1),t值分别为2.99和3.44,P<0.01和0.001。回归方程推算的结果提示:以颅长17厘米或长、宽径差3厘米以及锥体角52°为基数,当颅长或径差比17或3厘米增加1厘米时,则锥体角减小1度,反之应增大1度。  相似文献

目的 :研究经颞骨岩部乙状窦前入路进行岩斜区直接手术的方法 ,寻找出该入路的标志点和颞骨岩部的磨除方法 ,并观察手术的暴露范围和优缺点。方法 :用 15例血管经彩色乳胶灌注的成人尸头标本 ,显微镜下 (× 6~ 2 5 )解剖经颞骨岩部乙状窦前入路。结果 :岩乙状窦交叉点、内淋巴囊裂是磨除颞骨岩部后面的重要标志 ,用内淋巴囊裂可以初步定位前庭小管、总脚和后半规管。此入路能够暴露从鞍背到延髓中上部的区域 ,以暴露中岩斜区最佳。离断内淋巴囊 ,可扩大骨磨除的面积 ,增加手术的暴露。结论 :以岩乙状窦交叉点、内淋巴囊裂为标志为安全磨除颞骨岩部后面提供了保证。经颞骨岩部乙状窦前入路具有手术距离短、视野暴露充分的特点 ,特别适用于骑跨颅中、后窝而以颅后窝为主的岩斜区病变  相似文献

目的 研究骨盆正位与斜位X线片中骶髂关节的放射解剖学特点,为临床诊断骶髂关节病变提供确切的放射解剖学依据。 方法 在男女各1具成人干燥髂骨和骶骨的耳状面上分别均匀涂上钡粉,合拢骨盆胶带固定,拍摄骨盆正位和斜位X线片;再将钡粉洗净,用细铅丝分别沿着骶骨和髂骨耳状面的边缘环绕,合拢骨盆胶带固定,分别拍摄骨盆正位和斜位X线片。观察耳状面和骶髂关节在骨盆正位片与斜位片上的投影变化和形态特征。 结果 在骨盆正位片可清晰的观察到双侧骶髂关节的前间隙和后间隙及耳状面。斜位片可观察到对侧骶髂关节的情况,骶髂滑膜关节位于X线上骶髂关节间隙的中下2/3;女性骶髂关节较男性短。 结论 骶髂关节在骨盆正位和斜位X线片上有其放射解剖学及性别特征,骶髂关节前间隙和后间隙以及耳状面是观察的重点,骨盆正位X线片基本可满足临床需要。  相似文献

背景:退行性颈椎病患者一般存在颈椎曲度改变、颈肩部肌肉力量的不平衡及力量下降。目的:探讨退行性颈椎病矢状位曲度参数与颈肩部肌肉痛阈的相关性。方法:将2022年1月至2023年3月在上海交通大学医学院附属第九人民医院3D打印门诊中心收治的50例退行性颈椎病患者纳入研究,剔除颈椎X射线侧位片不符合测量条件的10例,拍摄颈椎侧位X射线片,测量Borden值、C2-7 Cobb角,并测量双侧上束斜方肌中点和C5-6棘突间旁1 cm处的疼痛阈值;分析退行性颈椎病矢状位曲度参数与颈肩部肌肉痛阈的相关性。结果与结论:(1)颈椎病矢状位曲度Borden值与左侧斜方肌上束痛阈、右侧斜方肌上束痛阈、C5-6棘突间旁左侧1 cm痛阈呈中等正相关关系(r=0.498,P <0.01;r=0.448,P <0.05;r=0.405,P <0.05),与C5-6棘突间旁右侧1 cm痛阈呈弱正相关关系(r=0.322,P <0.05);(2)C2-7 Cobb角与左侧斜方肌痛阈呈弱正...  相似文献

为研究胚胎时期大鼠海马结构的发育状况,用浸泡固定、石蜡切片、混合尼氏染色方法,观察了E14、E16、E18、E22天大鼠胚胎海马结构的发育过程。结果:E14天基本上能区分出海马原基。E16天海马原基背侧与下托部神经上皮延续,腹侧与隔区神经上皮及脉络丛基质相连续;此时海马原基可区分出3部分,即背侧的阿蒙角神经上皮,其下的原始齿状回神经上皮,腹侧的伞部胶质上皮。E18天海马裂形成,将阿蒙角神经上皮与齿状回生发基质区分开来;CA1区锥体层和海马伞开始出现,齿状回外臂和门也开始形成并第一次见向齿状回迁移的细胞。E22天海马裂消失,阿蒙角分子层与齿状回分子层贴在一起;CA1区锥体层增厚,CA3区锥体层出现并穿透门区;阿蒙角神经上皮变成单层结构,中间层变薄,仍见大量逗留的锥体细胞,海马槽形成;齿状回外臂轮廓出现,颗粒细胞呈单层排列,内臂起始部开始形成;CA3区中间层周围及个部附近见大量的细胞正朝门区迁移。研究表明:出生前阿蒙角CA1、CA3区锥体层已经形成,齿状回外臂的轮廓已形成,内臂刚开始出现,其轮廓将于出生后形成。  相似文献

颅容积对鉴别颅骨的性别和人种具有一定的意义。对颅容积的推算,以往多采用颅骨的外测量,回归方程的推算值与实测值,相差在4%以上。为提供更准确的,既可应用于颅骨,也可用于活体或脑的头标本的推算颅容积的回归方程,对33例除去脑的尸头进行了以水和小为为介质测量颅容积,并在正,侧位X-线片上测量了5项颅内径线和正侧位面积,得出了几个回归方程式和常数公式式,其r值为0.83-0.96。  相似文献

目的:探讨骨质疏松时股骨上段髓腔的形态学变化,为老年人工髋关节置换术提供依据。方法:160根股骨,每个标本分别拍照正位、侧位、45°内斜位和45°外斜位的X线片,记录Singh指数,测量股骨上段髓腔,研究Singh指数和股骨上段髓腔的关系。结果:Singh指数为7 ̄5的62个标本中髓腔扩大者6.5%(4),不扩大者93.5%(58);Singh指数为4 ̄1的95个标本中髓腔扩大者67.4%(64),不扩大者32.6%(31);Singh指数越大,股骨上段髓腔越窄,Singh指数越小,股骨上段髓腔越宽大;骨质疏松时股骨髓腔扩大是全方位的,其骨皮质吸收以髓腔内面明显。结论:(1)骨质疏松标本髓腔扩大峡部不明显,假体设计要考虑到峡部不明显时如何使股骨柄远端与股骨骨髓腔形态相匹配,对老年人可将柄下端适当加粗,以适应髓腔变化。(2)对骨质疏松老年人,选择假体前要拍照股骨全长的正、侧位及双斜位X线片,对髓腔形态有较全面的评估,以选择适合的假体。  相似文献

目的 制作颞骨火棉胶冠状位断层连续薄层切片,获取数字化图片库。方法 用2例尸头标本进行螺旋CT扫描后制作断层标本火棉胶包埋后,用大型轮式切片机沿冠状位切片,厚度100μm,每片均摄影并保存于计算机。选取代表性层面的胶片进行HE染色。将断层照片与HE染色照
片和CT片进行对照观察,对代表性层面结构特点进行描述。结果 共获取侧颅底数字化连续冠状位薄层断面标本数据集 2套。断层图像分辨率1920×2560像素,结构毗邻关系显示清晰。与HE照片和CT片进行对照观察,可以更清楚地定位断层结构。结论 火棉胶包埋技术是制作颞骨大切
片,获取高分辨率的数字化图片数据集的理想方法。冠状位对听小骨、Prussak‘s间隙、颈静脉窝、耳蜗与中颅窝的关系,颈内动脉升段及膝部、面神经垂直段、内听道与中颅窝的关系等显示较佳。  相似文献

目的 :观测与岩骨轴向垂直的小脑桥脑角区和岩斜区的断层解剖结构 ,为该区的手术显露提供解剖学资料。方法 :用 5具 (共 10侧 )福尔马林固定的新鲜成人尸头 ,取与岩骨轴向相垂直的乳突内侧缘、茎突内侧缘、颈内动脉外口内侧缘和岩尖内侧缘等颅底外标记点 ,做成小脑桥脑角区和岩斜区的若干断面 ,然后在 8× 4 0倍的手术显微镜下观测其断面形态及毗邻结构。结果 :获得上述解剖标记处的小脑桥脑角区和岩斜区的断层照片及相关解剖结构的测量数据。结论 :此项解剖学研究联系临床实际 ,为临床手术设计和应用提供了一个新的视角和方法。  相似文献

The purpose of this investigation was to measure in vivo fascicle length (L(fas)) and pennation angle (PA) of the long head of the biceps femoris muscle (BF(lh) m.) at different hip and knee angles while the muscle was relaxed using ultrasonography (US). Data were collected from 18 healthy females (23.0 +/- 1.8 years). To validate the measurements of the L(fas) and PA, the US measurements on cadavers (n = 5), embalmed with the hip and knee in anatomical position, were compared to actual fiber lengths and PA from the dissected muscles. US images from volunteer subjects were recorded when the hip joint was positioned at 0 degrees, 45 degrees and 90 degrees of hip flexion, and at each hip position, the knee joint was placed at 0 degrees, 45 degrees and 90 degrees from full extension. The images were digitized to measure L(fas) and PA. Results showed no significant differences between US and direct measurements of the L(fas) and PA on the cadaver BF(lh) m. (p > 0.05). L(fas) and PA changed significantly between the different hip and knee positions in volunteer subjects (p < 0.05). Changes in the L(fas) and PA are more sensitive to changes in hip position with the knee position constant than to changes in knee position with the hip position constant. This difference may be related to the larger muscle moment arm at the hip resulting in greater excursion of the muscle with changing hip position. Based on the changes in L(fas) with changing joint positions, BF(lh) sarcomere length was estimated to occupy a portion of the ascending limb, the plateau and descending limb of the length-tension relationship over the range of motion studied. US scanning is valid and reliable for measuring L(fas) and PA of the BF(lh) m. An understanding of the dynamic nature of muscle architecture will assist in determining effective and efficient clinical evaluation and rehabilitation techniques.  相似文献

OBJECTIVE: To evaluate the effects of habitual physical activity (PA) on the metabolic syndrome (MS) in young adult men and women. RESEARCH METHODS AND PROCEDURES: Cross-sectional PA data were utilized from 249 women and 237 men, aged 18-40 years in the Fels Longitudinal Study. MS components--abdominal circumference (AC), triglycerides (TG), HDL, blood pressure (BP), and fasting glucose (FG)--were dichotomized according to the National Cholesterol Education Program's Adult Treatment Panel III revised criteria. Leisure, sport, work, and total PA scores were calculated using the Baecke Questionnaire of Habitual Physical Activity. Multiple logistic regression modeling assessed the effects of PA, age, smoking, and BMI on MS status. RESULTS: 26.9% of men and 19.3% of women had MS. For men, MS risk was reduced with increases in both total PA [OR = 0.65 (95% CI: 0.47, 0.90)] and sport PA [OR = 0.40 (95% CI: 0.23, 0.70)]. AC, TG, and HDL values also improved with total and sport PA. Among women, the risk for MS was marginally reduced by total PA [OR = 0.72 (95% CI: 0.50, 1.02)] and HDL levels were increased by both total PA [OR = 0.79 (95% CI: 0.63, 0.98)] and sport PA [OR = 0.54 (95% CI: 0.35, 0.84)]. DISCUSSION: Increased total and sport PA reduces risk for the MS in young men, though not as clearly in young women.  相似文献

BackgroundThe estimated worldwide incidence of perioperative or periprocedural anaphylaxis (PA) is between 1 in 1250 and 1 in 20,000 procedures.ObjectiveTo evaluate the incidence of PA in the United States and compare patient characteristics and underlying risk factors using a large national database.MethodsUsing deidentified data from the nationwide inpatient sample from 2005 to 2014, we identified cases of PA through the International Classification of Diseases, Ninth Revision, Clinical Modification codes and conducted a retrospective analysis.ResultsAmong 35,647,347 surgeries and procedures, there were 5458 (0.015%) PA cases identified. The incidence of PA was 15.3 cases per 100,000 procedures. When compared with controls, PA cases had an increased mortality (3.4% vs 1.4%; P < .001), median length of stay (5 vs 3 days; P < .001), and median hospital cost ($45,155 vs $24,734; P < .001). The age group between 18 and 34 years (odds ratio [OR], 1.34; 95% confidence interval [CI], 1.13-1.58; P < .001) and female sex (OR, 1.40; 95% CI, 1.31-1.49; P < .001) were associated with increased odds of PA. Transplant (OR, 3.35; 95% CI, 2.59-4.34; P < .001), hematologic (OR, 1.63; 95% CI, 1.30-2.05; P < .001), vascular (OR, 1.49; 95% CI, 1.30-1.67; P < .001), and cardiac (OR, 1.47; 95% CI, 1.30-1.67; P < .001) procedures were at increased risk for PA. Several comorbidities were associated with PA including chronic pulmonary disease (OR, 1.41; 95% CI, 1.31-1.51; P < .001).ConclusionThe incidence of PA is 1 in 6531 procedures, with a mortality of 1 in 191,652 procedures. PA has worsening outcomes compared with controls. The risk factors of PA include age, sex, procedure type, and comorbidities.  相似文献

Background: Physical inactivity is one major lifestyle risk factor of mild cognitive impairment with ageing.

Aim: To investigate whether or not potential covariates modulate the association between physical activity (PA) and cognitive impairment in older adults.

Subjects and methods: Data from 10?245 Korean older adults (5817 women) were used.

Results: High PA older adults were younger and longer educated and had lower comorbidity and depression than low PA older adults. Compared with low PA men, moderate PA men only had a significantly lower odds-ratio (OR) and 95% confidence interval (95% CI) (OR?=?0.795, 95% CI?=?0.654?~?0.965, p?=?0.021) for having cognitive impairment, even after adjusting for measured covariates, which was no longer significant when additionally adjusted for comorbidity (OR?=?0.862, 95% CI?=?0.707?~?1.051, p?=?0.143). Compared with low PA women, moderate and high PA women had significantly lower risks of cognitive impairment (OR?=?0.830, 95% CI?=?0.712?~?0.969, p?=?0.018 and OR?=?0.784, 95% CI?=?0.651?~?0.943, p?=?0.010, respectively), even after adjusting for the measured covariates including comorbidity, which was no longer significant when additionally adjusted for depression (OR?=?0.897, 95% CI?=?0.776?~?1.049, p?=?0.173 and OR?=?0.919, 95% CI?=?0.761?~?1.111, p?=?0.385, respectively).

Conclusion: These findings suggest that gender differences in the covariates modulate the relationship between physical activity and cognitive decline in older Korean adults.  相似文献

ObjectivesTo determine prevalence, incidence, and factors associated with Pseudomonas aeruginosa (PA) intestinal carriage in residents of long-term care facilities (LTCFs) and to understand the population structure of this pathogen in LTCFs from two European countries.MethodsWe assessed the prevalence of PA intestinal carriage and the incidence of acquisition by collecting fecal samples from 403 residents of 20 LTCFs. We collected 289 environmental samples from sinks and drinking water. Factors associated with carriage and acquisition of intestinal PA were identified. All PA isolates had their antibiotic phenotypic resistance profile determined and their genome sequenced, from which we assessed the population structure of the collection and identified resistance determinants.ResultsWe found a high proportion of residents with PA intestinal carriage (51.6%) over the entire study period. Over the follow-up period, 28.6% of the residents acquired intestinal PA. Older age (OR, 1.29; 95% CI, 1.09–1.52; p = 0.002), urinary incontinence (OR, 2.56; 95% CI, 1.37–4.88; p = 0.003), and male sex (OR, 2.55; 95% CI, 1.05–6.18; p = 0.039) were associated with higher probability of carriage. Wheelchair usage (OR, 4.56; 95% CI, 1.38–15.05; p = 0.013) and a body mass index >25 (OR, 3.71; 95% CI, 1.17–11.82; p = 0.026) were associated with higher risk of PA acquisition. Population structure of our isolates was mainly non-clonal with 112 different STs among the 241 isolates. Most represented STs were high risk clones ST253 (n = 26), ST17 (n = 11), ST244 (n = 11), ST309 (n = 10), and ST395 (n = 10). Most PA isolates (86.3%) were susceptible to antibiotics, with no acquired genes conferring resistance to antipseudomonal agents.DiscussionWe found an unexpected high prevalence of PA intestinal carriage in LTCF residents mainly associated with individual-level factors. Our study revealed a polyclonal PA population structure suggesting that individual acquisition is more frequent than resident-to-resident transmission.  相似文献

BACKGROUND: The study aim was to determine whether moulding the embryo transfer catheter according to the uterocervical angle measured by ultrasound could improve pregnancy and implantation rates. METHODS: Patients were alternately allocated to one of two groups. In the ultrasound-guided group (n = 320), the catheter was moulded according to the uterocervical angle measured by abdominal ultrasound. In controls (n = 320), embryo transfer was performed using the "clinical feel" method. RESULTS: Moulding the embryo transfer catheter according to the uterocervical angle significantly increased clinical pregnancy [(OR = 1.57, 95% CI (1.08-2.27)] and implantation rates [(OR = 1.47, 95% CI (1.10-1.96)] compared with the "clinical feel" method. It also significantly reduced difficult transfers [(OR = 0.25, 95% CI (0.16-0.40)] and blood during transfers [OR = 0.71, 95% CI (0.50-0.99)]. Patients with large angles (>60 degrees ) had significantly lower pregnancy rates compared with those with no angle [OR = 0.36, 95% CI (0.16-0.52)]. CONCLUSIONS: Moulding the embryo transfer catheter according to the uterocervical angle measured by ultrasound increases clinical pregnancy and implantation rates and diminishes the incidence of difficult and bloody transfers.  相似文献

The purpose of this study was to investigate the influence of physical training on subjective vertical perception with the different head positions in order to explore the involving of the neck proprioception. Visual field dependence was assessed using a rod and frame test on women practising judo or dance (international level) or no specific physical activity. Tests were performed with head upright or tilted head to disturb the Z-axis egocentric reference frame. A cluster analysis determined the distribution of visual field independent (VFI) and visual field dependent (VFD) participants. The first result showed no head position effect for the group of judoists (6 degrees +/-5 degrees ; 7 degrees +/-5 degrees ) and dancers (4 degrees +/-2 degrees ; 5 degrees +/-3 degrees ) but a significant effect for untrained participants (5 degrees +/-2 degrees ; 7 degrees +/-3 degrees ): their visual vertical perception was more disturbed with tilted head than with head upright. A variability exists among experts and would necessitate further analysis. The second result showed no influence of the head position for all VFD participants, whereas for VFI participants significant difference between upright and tilted head appeared both for experts (3 degrees +/-1 degrees ; 4 degrees +/-2 degrees ) and untrained participants (3 degrees +/-1 degrees ; 5 degrees +/-2 degrees ). In this research, whatever physical activity level, the VFI participants would mainly use the Z-axis reference frame and rely on proprioceptive information. VFD among experts and VFI among untrained participants suggest that proprioceptive reference frame of neck may not provide alone according the groups an appropriate postural control.  相似文献

The role of antibiotic exposure in the evolution and emergence of resistance is challenging to assess. We used carbapenem-resistant Pseudomonas aeruginosa (PA) phenotypes to assess possible factors that are associated with the occurrence and prognosis of such a phenotype and to examine the possible contribution of antibiotic exposure to the evolution of antimicrobial resistance. We conducted a nested case-control study. Cases were defined as patients from whom carbapenem-resistant ureidopenicillin-sensitive PA (CRUS-PA) was isolated; matched controls were PA patients who did not have isolation of CRUS-PA. We analysed potential predictors of CRUS-PA isolation and assessed their clinical significance (mortality and eventual isolation of pan-resistant PA), taking into account antibiotic exposures. We matched 800 case-control pairs. Case patients were more likely to have been exposed to anti-PA carbapenems (OR = 6.9; 95% CI, 2.5–18.6). This finding did not apply to the administration of other antibiotics. The mortality among CRUS-PA patients was similar to that of the controls (HR, 0.8 95%; CI, 0.6–1.1). Subsequent isolation of pan-resistant PA was more frequent among case patients compared with non-pan-resistant controls (p-value <0.05). Among cases, the risk of eventual pan-resistant PA isolation was increased in ertapenem recipients, only after and not prior to the index specimen date (HR, 1.9, 95%; CI, 1.01–3.4). Therefore we suggest that the CRUS-PA phenotype may represent pan beta-lactam resistance and that antibiotic exposure is associated with evolution of PA resistance phenotypes. We demonstrate a novel association of ertapenem with sequentially appearing PA resistance patterns.  相似文献

Implementing theory-based interventions is an effective way to influence physical activity (PA) behaviour in the population. This meta-analysis aimed to (1) determine the global effect of theory-based randomised controlled trials dedicated to the promotion of PA among adults, (2) measure the actual efficacy of interventions against their theoretical objectives and (3) compare the efficacy of single- versus combined-theory interventions. A systematic search through databases and review articles was carried out. Our results show that theory-based interventions (k = 82) significantly impact the PA behaviour of participants (d = 0.31, 95% CI [0.24, 0.37]). While moderation analyses revealed no efficacy difference between theories, interventions based on a single theory (d = 0.35; 95% CI [0.26, 0.43]) reported a higher impact on PA behaviour than those based on a combination of theories (d = 0.21; 95% CI [0.11, 0.32]). In spite of the global positive effect of theory-based interventions on PA behaviour, further research is required to better identify the specificities, overlaps or complementarities of the components of interventions based on relevant theories.  相似文献

Background: Elevated blood pressure (BP) has been reported in young people and seems to be associated with body mass index (BMI) and reduced physical activity (PA).

Aim: This study sought to analyse the association of BP with BMI, PA intensity patterns and cardiorespiratory fitness (CRF) in youth.

Subjects and methods: A cross-sectional study was carried out including 66 boys (13.91±1.76) and 97 girls (14.02±1.71). Anthropometric characteristics, CRF and BP were assessed during school time and accelerometers were utilized to determine intensity of PA.

Results: The linear regression model, adjusted for age, sex and height, showed that higher values of BMI were associated (p≤0.05) with higher values of systolic blood pressure (SBP) (1.39; 95% CI: 0.56; 2.23) and diastolic blood pressure (DBP) (1.52; 95% CI: 0.68; 2.37). Additionally, SBP, but not DBP, was inversely associated (p≤0.05) with moderate PA (?0.25; 95% CI: ?0.47; ?0.02) and positively correlated with sedentary activities (0.04; 95% CI: 0.00; 0.08).

Conclusion: The present study showed that BMI and time spent in sedentary activities were inversely associated with SBP. However, time spent in moderate PA was positively associated with SBP. BMI was the single predictor of DBP.  相似文献
