Osseointegration failure is a complex, multifactorial trait shown to concentrate in some treated populations. There has been shown evidence for genetic contribution to dental implant loss. Genetic polymorphisms have been classically considered as genetic risk factors for several diseases and, more recently, for dental implant loss.
Objectives: The purpose of this study was to access clinical factors related to failure process, and to investigate the relationship between a vitamin D receptor (VDR) polymorphism (rs731236, Taq I) and dental implant loss.
Material and methods: Two hundred and seventeen unrelated patients, mean age 51.7±11.3 years, were divided into two groups: (i) control group (C), 137 individuals presenting at least one osseointegrated implant in function for 6 months or more and without any implant loss, and (ii) study group (S), 80 individuals presenting at least one implant loss. After DNA collection and purification, VDR Taq I polymorphism analysis was performed by polymerase chain reaction-restriction fragment length polymorphisms (PCR-RFLP). Differences between C and S, and between healthy (H; n =1232) and lost (L; n =135) implants were accessed.
Results: Positive evidence of association has been detected between implant loss and the following variables: edentulism, implant position, primary stability, and implant length. Cox's regression model showed that primary stability, surgical technique and bone quantity were related to implant survival over time. No association between genotypes or alleles of VDR Taq I polymorphism and implant loss was found between the groups.
Conclusion: It was observed that clinical variables, but not the study polymorphism, were associated with dental implant loss. 相似文献

PURPOSE: Dental implants are currently the aesthetic and functional alternative for tooth replacement. Despite the high success rate shown by longitudinal studies, failures do occur, even in patients who present appropriate clinical conditions. The aim of the present study was to identify factors related to, or determinant of, dental implant loss in patients of the Latin-American Dental Research Institute, Curitiba, PR, Brazil. MATERIALS AND METHODS: Retrospective analysis of 3578 records of patients who had implants placed in this institute during the period of 1996 to 2006 was performed. Beyond records, panoramic and periapical radiographs were analyzed. RESULTS: Of the 3578 individuals implant treated, failures occurred in 126 (3.5%) patients (mean age 52.2 +/- 10.6 years). Men lost more implants (4.5%) than did women (3.1%) (P = 0.05). Most failure occurred before loading (88.2%). Failure was more frequent when the implant was installed in the posterior jaw (58.5%). The main detectable causes of implant loss were evaluated. Most implant losses (75%) did not have an apparent clinical cause. Identified causes were 17.5% iatrogenic conditions (surgical technique, contamination, and/or occlusal trauma), poor bone quality and quantity (3%), peri-implantitis (1%), and 3.5% missing data. CONCLUSIONS: The results obtained in this study suggest that host factors can be contributing to the failure of implants. 相似文献

目的:短种植体具有常规长度种植体所无法替代的优点,但其长期效果国内尚未见报道.本研究即通过评估短种植体修复后五至十年的临床效果,以探讨其临床应用价值.方法:1998年8月至2004年8月在北京大学口腔医学院种植中心共有49位患者植入短种植体共83枚.选取其中临床及x线片检查均满5年及以上共20位患者的38枚种植体,采用Zarb种植体成功标准及Wheeler存留标准评估短种植体的成功率及存留率.结果:38枚种植体中固定修复34枚,活动修复4枚,其中有1枚于种植修复7年后脱落,成功率和存留率分别为94.7%和97.4%.结论:短种植体5年成功率及存留率高,接近常规长度种植体,并可避免植骨手术,减轻病人的创伤及痛苦,修复后使用良好,患者满意度高,具有良好的临床应用价值. 相似文献

目的: 探讨种植体黏膜(dental implant mucosa,DIM)与种植体乳头(dental implant papilla,DIP)水平及周围软组织稳定性的关系。方法: 选择2015年1月—2019年1月于青岛市海慈医院接受口腔种植体修复的患者86例,从治疗起随访至2020年12月,分别于植入物放置即刻(T0)、4周(T1)、2年(T2)和随访结束(T3)时检测边缘骨吸收水平(MBLs)、中颊黏膜水平(MBMLs)、DIP、改良龈沟出血指数(mSBI)、改良出血指数(mBI)植入物稳定性系数(ISQ)水平及之间的相关性。采用SPSS 23.0软件包对数据进行统计学分析。结果: 随访1.0~5.2年,平均(4.79±0.34)年,脱落2例,最终纳入患者84例(95颗种植体)。与T0时相比,T1、T2、T3时mSBI、mPLI、mBI、邻牙接触点的远端骨水平、中骨水平依次显著降低(P<0.05),DIP高度显著升高;T0、T1、T2、T3时,PD、MBLs、MBMLs水平相比差异无统计学意义(P>0.05)。邻牙接触点的远端骨水平、中骨水平可影响DIP高度(P<0.05)。DIP增加49例,丧失46例;T0时,DIP增加、丧失组的ISQ水平均较高;T1时较T0时DIP增加、丧失组的ISQ水平降低(P<0.05);T2、T3时,DIP增加组ISQ值显著大于丧失组(P<0.05)。结论: DIM、DIP水平之间存在依赖性,种植体周围软组织的稳定性取决于角化组织高度和DIP高度。 相似文献

The purpose of this study was to investigate cross-sectional and longitudinal associations between hearing acuity and tooth loss in 1156 US veterans taking part in the Veterans Affairs' Normative Aging (NAS) and Dental Longitudinal (DLS) Studies in the Boston, MA, area. The mean age was 48 years (SD = 8.9), 5.3% were edentulous, and 15.4% had < 17 teeth at baseline. Hearing acuity was determined by puretone, air- and bone-conduction audiometry, and speech discrimination tests at triennial examinations over a 20-year follow-up period. Hearing decline was defined as a change from baseline in the average puretone air-conduction thresholds of ≥ 20 dB at 0.25, 0.5, 1, 2, 3, 4, 6, and 8 kHz. The explanatory variables of interest were change since baseline in dentate status (cut points at < 1, < 17, and < 20 teeth), and in the number of teeth lost (linear). Linear and logistic regression models—which controlled for baseline audiological status, age, air-bone gap, and otoscopic examination at current visit—showed that subjects who went from having ≥ 17 to < 17 teeth had 1.64 times (95% CI, 1.24-2.17) as high odds of having hearing decline as those with no change in their dentate status. For every tooth lost since baseline, there was a 1.04 times as high odds (95% CI, 1.02-1.06) for hearing decline, when additional baseline and time-varying covariates were taken into account in the model. 相似文献

At times, dental implants are placed into sites with a history of periapical pathology. Sometimes the infection is active, and other times the tooth may have been extracted years before implant placement. In either case, the possibility exists for long-term residual cysts or infections that can negatively impact the prognosis of the implant. In this case report, an implant is placed into a healed mandibular ridge several months after extraction of the tooth. A radiolucency was noted on routine radiographic examination 2 years later. Surgical inspection and histology revealed a periapical granuloma with acute and chronic inflammatory cells. After surgical curettage of the site, the patient healed without complication. Implants may develop apical pathology as a result of a preexisting long-term residual infection. 相似文献

The universally accepted concept of delay-loaded dental implants has recently been challenged. This study hypothesizes that early loading (decreased implant healing time) leads to increased bone formation and decreased crestal bone loss. We used 17 minipigs to study implants under a controlled load, with non-loaded implants for comparison. Radiographic and histological assessments were made of the osseointegrated bone changes for 3 healing times (between implant insertion and loading), following 5 months of loading. The effect of loading on crestal bone loss depended on the healing time. Early loading preserved the most crestal bone. Delayed loading had significantly more crestal bone loss compared with the non-loaded controls (2.4 mm vs. 0.64 mm; P < 0.05). The histological assessment and biomechanical analyses of the healing bone suggested that loading and bioactivities of osteoblasts exert a synergistic effect on osseointegration that is likely to support the hypothesis that early loading produces more favorable osseointegration. 相似文献

This paper examines the effect of tobacco use on the failure rates of dental implants. A review of 56 dental implant patients with a total of 187 endosseous dental implants, placed over a four year period, demonstrated a significant association between increased implant failure rates and cigarette smoking with failure rates of 16.6% in smokers compared to 6.9% in non-smokers. Also implant length was shown to be a significant factor with shorter implants (< or = 10 mm) being more susceptible to failure in smokers. A chi-square test was used for data analysis. Current recommendations that should be given to implant patients who smoke are included. 相似文献

Dental implant surgery commonly proceeds in two stages. It is generally accepted that bone loss around implants does not occur at stage-II surgery because implants do not receive mechanical loading. However, early marginal bone loss around implants occasionally does occur during the healing period. Genetic polymorphisms in the interleukin-1 (IL-1) gene have been reported to be important for bone homeostasis and susceptibility to bone disease. We therefore investigated whether the idiopathic early marginal bone loss around implants is related to polymorphisms in the IL-1 gene. We performed a case-control study. Patients demonstrating marginal bone loss around implants at stage-II surgery were designated as the 'marginal bone loss (+)' group and those without bone loss as the 'marginal bone loss (-)' group. Polymorphisms of the IL-1alpha and IL-1beta genes (IL-1A-889, IL-1B-511 and IL-1B+3954) were detected by restriction fragment length polymorphism using NcoI, AvaI and TaqI after polymerase chain reactions. A total of 251 implants were placed in 39 patients. Marginal bone loss was observed in 36 implants. The patients with IL-1B-511 2/2 genotype exhibited a significantly higher occurrence of marginal bone loss than those with IL-1B-511 1/1 or 1/2 genotypes (OR=5.63; 95% CI=1.20-26.42; P=0.033). Multiple logistic regression analyses showed a markedly increased odds ratio (OR=10.86; 95% CI=1.64-71.90) in IL-1B-511 2/2 genotype carriers, while ORs of the other risk factors for bone loss, such as age, smoking status, post-menopausal women and bone quality, remained between 0.44 and 6.20. There was no significant difference in the distributions of the IL-1B+3954 and IL-1 A-889 genotypes between cases and controls. These data suggest that the IL-1B-511 2/2 genotype has a significant association with the incidence of early marginal bone loss around endosseous implants. 相似文献

Objectives: Endosteous dental implants consist in the treatment of choice to replace tooth loss. The knowledge that implant loss tends to cluster in subsets of individuals may indicate that host immune‐inflammatory response is influenced by genetic factors. Interleukin‐1 (IL‐1) is a key mediator of inflammatory processes and functional polymorphisms in IL1 gene could be candidate genetic risk factors to study susceptibility to implant failure. The objective of this study was to investigate the association between IL1B (C‐511T) genetic polymorphism and dental implant loss in a Brazilian population and its influence in the clusterization phenomenon. Material and methods: The sample composed of 277 unrelated, both gender, mean age 53.63 ± 11.14 years individuals, divided into test group – 92 subjects with implant loss, and control group – 185 subjects with no implant loss. Patients' socioeconomic profile and clinical variables were investigated. Genomic DNA from oral mucosa was analyzed by polymerase chain reaction–restriction fragment length polymorphism. Results: There was significant difference between the groups in medical treatment (P=0.040), edentulism (P=0.019), and mean number of placed implants (P=0.001). There was difference between groups with and without implant loss neither considering genotypes (P=0.279) nor alleles (P=0.168) for IL1B (C‐511T) polymorphism. When individuals showing up to one implant failure (n=254) were investigated vs. patients presenting multiple implant loss (n=23), no difference was either observed between groups for genotype (P=0.083) and allele (P=0.838) frequencies. Conclusions: The borderline association of the study polymorphism with implant loss suggests further IL1 haplotype analysis to elucidate the global involvement of IL‐1 proteins in the modulation of the osseointegration process. BACKGROUND: Chronic periodontitis (CP) is characterized by an inflammation in the supporting tissues of the teeth caused primarily by bacterial infection. Interleukin 10 (IL10) is an anti-inflammatory cytokine whose genetic polymorphisms may influence the expression of the protein. Objective: In this study we investigated the hypothesis that single-nucleotide polymorphisms (SNPs) in the promoter of IL10 gene might be related to CP. MATERIALS AND METHODS: DNA was obtained from n=67 CP patients and n=43 control subjects. All studied individuals were non-smokers. The -1087 SNP was investigated by DNA sequencing, and the -819 and -592 SNPs by restriction fragment length polymorphism of PCR products. RESULTS: Frequencies of -819 and -592 SNPs showed differences between the control and CP groups. The ACC haplotype was more prevalent in the control group and the ATA haplotype more prevalent in the CP group. The ATA haplotype seemed to increase susceptibility to CP in women (odds ratio (OR)=2.57). The heterozygous haplotype GCC/ACC was predominant in the control group (OR=8.26; p=0.001). CONCLUSIONS: Specific haplotypes and SNPs in IL10 gene are associated with susceptibility to CP in Brazilian patients. 相似文献
