Tuberculous osteomyelitis of the mandible: a case report   总被引:1,自引:0,他引:1  
Osteomyelitis of jaws caused by infection with Mycobacterium tuberculosis is uncommon, especially in children. We present a case of tuberculous osteomyelitis in a young child. Its clinical presentation, with features similar to a dento-alveolar abscess, underline the importance of considering it in the differential diagnosis of jaw lesions.
We discuss of the diagnostic techniques, management and preventive measures, and stress the importance of history taking during clinical examination.  相似文献

A case of osteomyelitis of the mandible caused by actinomyces infection is described. Gram stain and cultures of the discharge in fluid thioglycolate medium and blood agar, as well as an analysis of the biochemical properties of the causative organism with API system, indicated the infection was caused by A israelii. Lesions in both hard and soft tissues responded to chemotherapy with cephalexin, minocycline, and aminobenzylpenicillin and drainage of the pus from the cutaneous abscesses.  相似文献

BACKGROUND: Osteomyelitis of the maxillofacial skeleton is rare in developed countries such as Australia. This case report describes the successful surgical treatment of chronic suppurative osteomyelitis (CSO) of the mandible in a 75 year old man. The precipitant factor was thought to be a retained tooth root in the (right) posterior body of the mandible. METHODS: Treatment included a pre-surgical course of antibiotics (clindamycin 300mg, p.o. q.i.d. for two weeks) followed by removal of the retained root, surgical débridement of the affected bone, the intra-oral draining sinus, and resection of the cutaneous sinus tract. Specimens were taken for bacterial cultures and antibiotic sensitivity testing, and the resected tissue sent for histopathological review. RESULTS: On clinical and radiographic review at three months, the patient was well, completely symptom free and the osteomyelitis had fully resolved. CONCLUSION: This case report demonstrates the typical features of CSO. The combination of antibiotic therapy and surgical débridement was effective in the treatment of chronic suppurative osteomyelitis of the mandible utilizing intravenous sedation, and so averting the need for a general anaesthetic.  相似文献

Diffuse sclerosing osteomyelitis (DSO) of the mandible is a chronic condition, the cause of which is not known. Jaw pain, occurring irregularly, is a typical symptom. The aim of the study was to assess the effectiveness of disodium clodronate for relieving pain in patients with DSO. Disodium clodronate is a bisphosphonate used to treat diseases of bone and calcium metabolism. Ten DSO patients experiencing pain received disodium clodronate or placebo intravenously on a randomized double-blind basis. Both minimum (300 mg) and maximum (900 mg) doses were well tolerated. Disodium clodronate administration did not result in better immediate pain relief than placebo administration. However, 6 months after treatment there was a statistically significant difference in pain intensity between the groups, with the disodium clodronate group experiencing significantly less pain.  相似文献

A case of tuberculous osteomyelitis of the mandible in a 76-year-old woman is reported. The patient sought treatment for a periostitis of the left side of the mandible. A purulent discharge was evident. Roentgenographic examination of the mandible revealed bone destruction from the midmandible to the ramus on the left side. The diagnosis was made by microscopic examination of the specimen from the involved bone. This case proved to be primary tuberculous osteomyelitis caused by the absence of the primary focus.  相似文献

A report of Garre's osteomyelitis of the mandible associated with a fistula is presented. Elimination of pulpal periapical infection through endodontic therapy was shown to be an effective treatment. The total bone healing was observed one year later.  相似文献

A dwarf suffering from pycnodysostosis was treated for 2 episodes of chronic osteomyelitis of the mandible occurring at an interval of 6 years. Pycnodysostostis, a variant of osteopetrosis, occurs only in dwarfs and is characterised by increased density of bone which may lead to increased susceptibility to fracture and inflammatory disease.  相似文献

The incidence of tuberculosis is increasing worldwide. The disease has a variable mode of presentation and therefore diagnosis is not easy. Although a rare occurrence, the differential diagnosis of tuberculous osteomyelitis must always lurk in the deepest recesses of the dental clinician's mind when routine therapy fails to bring about an improvement in lesions of the jaw. We present an unusual case of tuberculous osteomyelitis in a 4-year-old boy in whom pulmonary tuberculosis was detected only subsequently and emphasize the role of history taking, diagnostic techniques, and management.  相似文献

A 45-year-old Nigerian housewife with chronic osteomyelitis associated with osteopetrosis is described and discussed. Haematological and radiographical investigations on routine admission led to the secondary diagnosis of osteopetrosis. Osteopetrosis is a rare hereditary bone disorder presenting with variable clinical features and is characterized by an increase in bone density. The main clinical findings of the benign-type of osteopetrosis (autosomal dominant) are fractures and osteomyelitis of the mandible, leading to the detection of the disease. We report a case of autosomal dominant osteopetrosis with secondary mandibular osteomyelitis. Clinical and radiological patterns and treatment evaluation were assessed. The patient was referred for evaluation and treatment of an acute abscess in the left side of the face and osteomyelitis of the mandible. Several imaging studies were performed to assess the lesion and the adjacent bone and soft tissue structures. Treatment consisted of intravenous antibiotic therapy, debridement of the necrotic bone and sequestrum, and extra-oral drainage of the abscess, with submandibular access. Healing was uneventful.  相似文献
