目的 评价Tenax Fiber White玻璃纤维桩加全瓷冠修复前牙残根残冠的临床效果.方法 对42例64颗已行完善根管治疗的前牙残根残冠进行玻璃纤维桩核加全瓷冠修复.按照牙体预备后余留牙本质高度分为3组,A组余留牙体高度大于等于2 mm,B组高度小于2 mm并大于或等于1mm,C组高度小于1 mm.治疗完成后1、6、12、24个月复查,评价修复体完整性,边缘适合度,牙周组织健康状况及根尖周健康状况等疗效.结果 95.3%的修复体保持完整.有2例修复体发生桩脱位,其中1例伴有崩瓷,为咬硬物所致,1例根管粗大.另有1例发生纤维桩折断,牙龈充血.余留牙体在0~1 mm组无箍效应,修复失败率增加(P<0.05).全瓷冠类型无明显影响.其他所有修复体边缘适合性良好,色泽稳定,牙龈状况良好.X线片未见根尖异常,未见根折或桩折.结论 纤维桩全瓷冠修复效果除了与牙本质肩领高度有关,还与余留牙本质的性状有关,临床选择应综合考虑.在符合适应证的情况下,Tenax Fiber White玻璃纤维桩和全瓷冠修复可获得良好修复效果.  相似文献

目的    评价双重固化高强度复合树脂水门汀和树脂加强型玻璃离子水门汀黏固纤维桩的临床效果。方法     选择2007年在沈阳沈河金英武口腔诊所就诊,拟采用纤维桩修复的70例前磨牙或第一磨牙缺损患者的96颗患牙,随机分为2组,每组48颗,分别用不同的黏结剂(Ⅰ组:双重固化高强度复合树脂水门汀;Ⅱ组:树脂加强型玻璃离子水门汀)黏固纤维桩,制备树脂桩核,全冠修复。随访2年,观察修复效果。结果    Ⅰ组4颗失败,1颗为上颌第二前磨牙纤维桩折断,另3颗为上颌第一前磨牙纤维桩脱落,成功率为91.7%;Ⅱ组2颗失败, 1颗为上颌第一前磨牙,另1颗为下颌第一前磨牙,均为纤维桩折断,成功率为95.8%。两组治疗成功率比较差异无统计学意义(P>0.05)。结论    2种黏结剂用于临床黏固纤维桩均表现出良好的临床效果。  相似文献

STATEMENT OF PROBLEM: Very little is known about the resistance to fracture of endodontically treated teeth restored with newly developed esthetic post systems. Purpose. This in vitro study compared the effect of 1 titanium and 3 esthetic post systems on the fracture resistance and fracture patterns of crowned, endodontically treated teeth. MATERIAL AND METHODS: A total of 40 recently extracted human maxillary canines with their crowns removed were endodontically treated. Four groups of 10 specimens were formed. Teeth were restored with titanium, quartz fiber, glass fiber, and zirconia posts and numbered as groups 1, 2, 3, and 4, respectively. All posts were cemented with Single Bond dental adhesive system and dual-polymerizing RelyX ARC adhesive resin cement. All teeth were restored with composite cores, and metal crowns were fabricated and cemented with glass ionomer cement. Each specimen was embedded in acrylic resin and then secured in a universal load-testing machine. A compressive load was applied at a 130-degree angle to the long axis of the tooth until fracture, at a crosshead speed of 1 mm/min. One-way analysis of variance and a Tukey test were used to determine the significance of the failure loads between groups (P<.001). A non-parametric chi(2) test was conducted for evaluation of the mode of failure (P<.001). RESULTS: The mean failure loads (kg) were 66.95, 91.20, 75.90, and 78.91 for groups 1 to 4, respectively. Teeth restored with quartz fiber posts (group 2) exhibited significantly higher resistance to fracture (P<.001) than the other 3 groups. Teeth restored with glass fiber and zirconia posts (groups 3 and 4) were statistically similar (P>.05). Fractures that would allow repair of the tooth were observed in groups 2 and 3, whereas unrestorable, catastrophic fractures were observed in groups 1 and 4 (P<.001). CONCLUSION: Within the limitations of this study, significantly higher failure loads were recorded for root canal treated teeth restored with quartz fiber posts. Fractures that would allow repeated repair were observed in teeth restored with quartz fiber and glass fiber posts.  相似文献

Objectives

This study aims to evaluate the fracture resistance of endodontically treated anterior teeth restored with crowns made of composite or ceramic and retained without the use of a post (endocrowns) or with posts of 5 mm (short) and 10 mm in length (long).

Material and methods

Forty-eight intact maxillary incisors were selected for the study. After endodontic treatment, the crowns were sectioned 2 mm coronally to the cementoenamel junction provided with a ferrule of 2 mm. The roots were randomly divided into six groups (n?=?8) according to the post length and type of coronary restoration. The crowns were fabricated with the chairside economical restoration of esthetic ceramics system. Group 1 was restored with a 10-mm glass fiber post, composite core, and a full-coverage ceramic crown (LPCer); group 2, with a 5-mm glass fiber post, composite core, and a full-coverage ceramic crown (SPCer); group 3, with a 10-mm glass fiber post, composite core, and a full-coverage composite crown (LPCpr); group 4, with a 5-mm glass fiber post, composite core, and a full-coverage composite crown (SPCpr); and groups 5 (EndoCer) and 6 (EndoCpr) were restored with ceramic and composite endocrowns, respectively. The teeth were then thermomechanically loaded in a chewing machine. After fatigue, the specimens were loaded to fracture. Data were analyzed with ANOVA and chi-square test. Mode of failure was defined as repairable or non-repairable.

Results

Presence of post, post length, and crown material had no significant effect on the fracture resistance. Groups restored with endocrowns presented a higher number of repairable fractures in respect to the other groups.

Conclusions

Presence of a post had no effect on the restorations’ fracture strength.

Clinical relevance

Although this in vitro study has some limitations in respect to its clinical relevance, the restoration of largely destroyed anterior teeth with the use of an endocrown or a short glass fiber post might have advantages over a large glass fiber post.  相似文献

The aims of this in-vitro investigation were to compare the fracture resistance and the failure modes of endodontically treated teeth restored with glass fibre-reinforced posts with those of teeth restored with titanium-alloy posts. A total of 60 single-rooted human mandibular premolars were endodontically treated. The teeth were divided into two experimental and one control group. Post spaces 9mm long were prepared in the roots of the experimental groups in which glass fibre-reinforced posts (Group A) and titanium-alloy posts (Group B) were cemented. In the control group (Group C), no post was inserted. The specimens were stored in normal saline for a period of three weeks before being intermittently loaded at an angle of 30 degrees degrees to the long axis of the tooth at a frequency of two loads of 40N per second. Log-rank test used for the overall analysis revealed that there was no significant difference of fracture resistance between teeth restored with glass fibre-reinforced posts (Group A) and titanium-alloy posts (Group B). The survival of the control group was found to be significantly inferior to that of the experimental groups. There was no significant difference in the number of failures between the two experimental groups. There was significantly more core and post failure for the glass fibre-reinforced posts, root and core failure for the titanium-alloy posts and core failure for the control group. The results suggest that post failures are more frequent in teeth restored with quartz fibre posts and root fractures are more frequent in teeth restored with titanium posts.  相似文献

STATEMENT OF PROBLEM: Little information exists regarding the outcome of crown build-ups on endodontically treated teeth restored with metal-ceramic crowns or with only a direct-placed composite. PURPOSE: The aim of this study was to evaluate the clinical success rate of endodontically treated premolars restored with fiber posts and direct composite restorations and compare that treatment with a similar treatment of full-coverage with metal-ceramic crowns. MATERIAL AND METHODS: Subjects included in this study had one maxillary or mandibular premolar for which endodontic treatment and crown build up was indicated and met specific inclusion/exclusion criteria. Only premolars with Class II carious lesions and preserved cusp structure were included. Subjects were randomly assigned to 1 of the following 2 experimental groups: (1) teeth endodontically treated and restored with adhesive techniques and composite or (2) teeth endodontically treated, restored with adhesive techniques and composite, and then restored with full-coverage metal-ceramic crowns. Sixty teeth were included in the first group and 57 in the second. All restorations were performed by one operator. Causes of failure were categorized as root fracture, post fracture, post decementation, clinical and/or radiographic evidence of marginal gap between tooth and restoration, and clinical and/or radiographic evidence of secondary caries contiguous with restoration margins. Subjects were examined for the listed clinical and radiographic causes of failure by 2 calibrated examiners at intervals of 1, 2, and 3 years. Exact 95% confidence intervals for the difference between the 2 experimental groups were calculated. RESULTS: At the 1-year recall, no failures were reported. The only failure modes observed at 2 and 3 years were decementations of posts and clinical and/or radiographic evidence of marginal gap between tooth and restoration. There was no difference in the failure frequencies of the 2 groups (95% confidence interval, -17.5 to 12.6). There was no difference between the number of failures caused by post decementations and the presence of marginal gaps observed in the 2 groups (95% confidence intervals, -9.7 to 16.2 and -17.8 to 9.27). CONCLUSION: Within the limitations of this study, the results upheld the research hypothesis that the clinical success rates of endodontically treated premolars restored with fiber posts and direct composite restorations after 3 years of service were equivalent to a similar treatment of full coverage with metal-ceramic crowns.  相似文献

目的:比较计算机辅助设计和计算机辅助制造(CAD/CAM)氧化锆全瓷桩、玻璃纤维桩修复上前牙残冠的临床效果。方法???随机选择18~55岁患者的40颗上前牙残冠为研究对象,在修复前行完善的根管治疗,其中CAD/CAM全瓷桩、纤维桩各20件,均采用CAD/CAM氧化锆全瓷冠修复。于修复完成后2年复查修复体、牙根及牙周情况。结果??全瓷桩组有1例全瓷冠崩瓷,1例桩核折断及1例牙龈炎症;纤维桩组有1例桩核松动脱落,1例桩核折断,2例牙龈炎症。2组患者的患牙均未出现根折和继发性根尖周炎。修复的临床效果无显著性差异(P>0.05)。结论??对于上前牙残冠的氧化锆全瓷冠修复,CAD/CAM全瓷桩与玻璃纤维桩在固位性,抗根折能力和牙周、牙髓疾病控制方面无显著性差异,均可取得较好的临床效果。  相似文献

目的 比较全瓷桩核与金属桩核的临床适应证与修复效果。方法 分别制作全瓷桩核 铸造全瓷冠、桥,金属桩核 金属烤瓷冠、桥,天然牙 铸造全瓷冠、桥各50个,比较三者的适应范围,用Shade eye电脑比色仪测定、比较三者的Shade值,临床复查修复体的颜色、松动脱落与折裂折断情况。结果 全瓷桩核 铸造全瓷冠桥与金属桩核修 金属烤瓷冠桥在颜色上有差异,与天然牙 全瓷冠桥无差异;全瓷桩核的折断率与金属桩核无统计学差异,但全瓷桩核在临床上有严格的适应证。结论 全瓷桩核有良好的美学效果和较好的强度。  相似文献

Purpose: This controlled clinical trial aimed to compare the 3-year outcomes of glass fiber posts and composite cores with gold alloy-based posts and cores for the restoration of endodontically treated teeth. Materials and Methods: One hundred forty-four patients in need of 205 restorations on endodontically treated teeth were selected and followed for 7 to 37 months (mean: 21 ± 9 months). The teeth were primarily stratified based on the remaining tissue available to restore the tooth core with or without a post. Then, randomization allocated the teeth to either test group 1 (prefabricated glass fiber posts), test group 2 (custom-made glass fiber posts), or test group 3 (composite cores without posts). The control group consisted of gold alloy-based posts and cores. All posts/cores were covered with all-ceramic single crowns. Failures were either absolute, such as root fractures or irreparable fractures of the post/core, or relative, such as loss of post retention or reparable fractures of the core. Success and survival probability lifetime curves, corrected for clustering, were drawn for the entire data set. Results: The recall rate at 3 years was 97.1%. Absolute failures consisted of two root fractures and one endodontic failure, while relative failures included three instances of retention loss of the post/core and one post fracture. Because of the low number of events, no statistical tests were performed. The success and survival probabilities over all groups together at 3 years amounted to 91.7% and 97.2%, respectively. Conclusions: After being followed for up to 3 years, both cast gold and composite post and core systems performed well clinically. 目的 比较两种不同桩核系统修复的离体牙和无桩修复的离体牙,在冷热循环实验和疲劳实验后牙根的微裂纹情况.方法 30颗因牙周病拔除的上颌中切牙,在釉牙骨质界上2mm截去牙冠,分成3组,每组10颗牙,分别用镍铬合金铸造桩核+金属烤瓷冠(金属桩组)、石英纤维桩+树脂核+金属烤瓷冠(纤维桩组)、无桩金属烤瓷冠修复(无桩组),进行冷热循环实验和疲劳实验,显微镜下观察牙根的微裂纹数,并记录数值,使用SPSS17.0软件进行统计分析.结果 金属桩组牙根的微裂纹数大于纤维桩组和无桩组,差异具有统计学意义.金属桩组牙根四个面之间的微裂纹数差异无统计学意义.结论 金属桩核修复的离体牙牙根的微裂纹数多于纤维桩修复和无桩修复的离体牙牙根.  相似文献

STATEMENT OF PROBLEM: There is insufficient knowledge of the strength of all-ceramic crowns bonded to natural teeth to warrant the use of all-ceramic crowns in place of metal-ceramic crowns. PURPOSE: The purpose of this study was to evaluate and compare fracture resistance of crowns made of 3 different types of 2 all-ceramic crown systems-0.4-mm and 0.6-mm aluminum oxide coping crowns and zirconia ceramic coping crowns-and metal-ceramic crowns. MATERIAL AND METHODS: Forty intact, noncarious human maxillary central incisors were divided into 4 groups (n=10): Group MCC (control), metal-ceramic crown (JRVT High Noble Alloy); Group AC4, crown with 0.4-mm aluminum oxide coping (Procera AllCeram); Group AC6, crown with 0.6-mm aluminum oxide coping (Procera AllCeram); and Group ZC6, crown with 0.6-mm zirconia ceramic coping (Procera AllZirkon). Teeth were prepared for complete-coverage all-ceramic crowns so that a final dimension of 5.5 +/- 0.5 mm was achieved incisocervically, mesiodistally, and faciolingually. A 1.0-mm deep shoulder finish line was used with a rounded internal line angle. All restorations were treated with bonding agent (Clearfil SE Bond) and luted with phosphate-monomer-modified adhesive cement (Panavia 21). Fracture strength was tested with a universal testing machine at a crosshead speed of 2 mm per minute with an angle of 30 degrees to the long axis of the tooth after restorations were stored in 100% relative humidity of a normal saline solution for 7 days. The mode of fracture was examined visually. Means were calculated and analyzed with 1-way ANOVA and Tukey's HSD (alpha=.05). RESULTS: The means of fracture strength were: Group MCC, 405 +/- 130 N; Group AC4, 447 +/- 123 N; Group AC6, 476 +/- 174 N; and Group ZC6, 381 +/- 166 N. There was no significant difference between groups ( P =.501). The mode of failure for all specimens was fracture of the natural tooth. CONCLUSIONS: There was no significant difference in the fracture strength of the teeth restored with all-ceramic crowns with 0.4- and 0.6-mm aluminum oxide copings, 0.6-mm zirconia ceramic copings, and metal ceramic crowns.  相似文献

目的探讨Crystaleye自动比色仪在玻璃渗透氧化铝全瓷冠修复中比色的临床效果。方法选择2009年3月至2010年3月于中国医科大学口腔医学院修复科就诊的前牙玻璃渗透氧化铝全瓷冠修复的患者119例(318颗患牙),随机分为对照组(157颗患牙)和试验组(161颗患牙),分别采用Vitapan 3D比色板和Crystaleye自动比色仪进行比色。修复后,用Crystaleye自动比色仪对各组修复体和比色参照的天然牙进行测色,计算每组修复体与天然牙的总色差(△E)和患者满意率。结果对照组和试验组的△E分别为(2.14±0.66)NBS、(1.78±0.52)NBS,差异有统计学意义(P<0.05)。对照组患者满意率为66.24%,试验组患者满意率为80.12%,差异有统计学意义(P<0.05)。结论使用Crystaleye自动比色仪能更好地指导玻璃渗透氧化铝全瓷冠的颜色复制,提高临床修复效果。  相似文献

OBJECTIVE: To investigate the fracture strength and pattern of failure of teeth with weakened roots reconstructed by different procedures. METHODS: In an in vitro study root posts were placed in 50 endodontically treated canines, divided into 5 groups (n=10) as follows: cast metallic post; glass fibre post with smaller diameter than the root canal; glass fibre post with smaller diameter than the root canal + glass fibre strips; glass fibre post with smaller diameter than the root canal + accessory glass fibre posts; anatomical post (glass fibre post with smaller diameter than the root canal, relined with low viscosity composite resin). Posts were luted with resin cement and the coronal portion of posts was constructed with composite resin. Metallic crowns were cemented on the posts. Specimens were submitted to compressive load in a universal testing machine. Fracture strength values of each group were compared. RESULTS: Fracture strength values were for Groups 1-5 respectively: 1087.06; 745.69; 775.41; 920.64; 876.12kgf, with significant differences between Groups 1 and 2 and between Groups 1 and 3 (p<0.05). Observed patterns of fracture were: Group 1 - 100% of roots fractured; Groups 2 and 4 - variable fracture modes; Group 3 - 60% of fractures occurred in the cervical root third; Group 5 - 50% of failures occurred in the coronal portion of the post. CONCLUSIONS: The fracture strength of teeth with cast metallic posts, teeth with anatomical posts or teeth with glass fibre posts combined with accessory posts was similar. All teeth restored with cast metallic posts presented fractures and were unfavourable to maintenance of the remaining tooth structure. Teeth with fibre posts (Groups 2 to 5) presented variable fracture modes; however, the maximum percentage of unfavourable fractures was 30%.  相似文献

OBJECTIVE: The purpose of this in vitro investigation was to evaluate the fracture strength of endodontically treated teeth restored with different all-ceramic posts and cores. METHOD AND MATERIALS: Seventy-two endodontically treated human maxillary central incisors were divided into 3 groups with 24 teeth each and restored with 1 of the following methods: prefabricated zirconia ceramic posts and resin composite buildups; zirconia ceramic posts and copy-milled alumina ceramic cores (2-piece technique), or copy-milled all-ceramic posts and cores made from a zirconia-reinforced glass-infiltrated alumina ceramic (1-piece technique). Each group was divided into 3 subgroups of 8 samples each, consisting of teeth that did not receive crown restorations and were stored for 60 days, teeth restored with all-ceramic crowns and stored for 60 days, and teeth restored with all-ceramic crowns and subjected to 1.2 million loading cycles with 30 N in a chewing simulator. RESULTS: The mean fracture strengths ranged from 205 to 522 N. Prefabricated zirconia posts and resin composite buildups showed a statistically significantly lower fracture strength after chewing simulation than teeth restored with prefabricated zirconia posts and alumina ceramic cores. CONCLUSION: All-ceramic posts and cores made with the 2-piece technique showed promising fracture strengths under the tested conditions.  相似文献

AIM: The aim of the study was to evaluate the influence of different posts and types of cementation on the fracture load and fracture mode of crowned, endodontically treated premolars with class II cavities in an ex vivo setting. METHODS: Forty-eight single-rooted human premolars were endodontically treated and prepared with standardized MO (mesio-occlusal) cavities and a circular chamfer preparation. Eight teeth each received either no posts or were restored with screws (BKS), glass fiber posts (DentinPost), or quartz fiber posts (DT Light SL). Sixteen teeth were restored with zirconium dioxide posts (CeraPost). BKS-screws and eight zirconium dioxide posts were cemented conventionally with glass ionomer cement; Panavia F was used for all others. The specimens were restored with composite cores and crowns cast from a non-precious metal. Eight sound premolars served as the controls. After thermomechanical fatigue testing, the samples were loaded until fracture occurred at an angle of 45 degrees. All specimens were evaluated for fracture lines. RESULTS: The sound teeth showed the significant highest fracture load (792.50+/-210.01 N). Conventionally cemented zirconium dioxide posts showed the lowest fracture load (327.00+/-45.84 N); the highest fracture load occurred with quartz fiber posts (421.75+/-90.19 N). Only the difference between these two groups was statistically significant. With glass fiber posts and conventionally cemented zirconia posts, restored teeth failed mostly in an "unfavourable" mode. CONCLUSIONS: With respect to the fracture load, there was no statistical difference between the restoration of non-vital premolars with class II cavities with crowns and posts or crowns alone.  相似文献

目的 探讨玻璃纤维桩树脂核加全瓷冠修复前牙残冠残根的临床疗效。方法 选择86例120颗前牙残冠残根,随机分为治疗组43例64颗和对照组43例56颗,治疗组患者采用玻璃纤维桩树脂核加全瓷冠修复,对照组患者采用铸造金属桩核加镍铬烤瓷冠修复。采用美国加州牙科协会的烤瓷冠修复临床标准进行疗效评价。结果 治疗组成功率为96.8%,对照组成功率为83.9%。结论 玻璃纤维桩和全瓷冠是修复前牙残冠残根的理想材料。  相似文献

The aim of this in vitro study was to evaluate the fracture resistance of upper and lower incisors restored with glass fiber reinforced posts (FRP). There were 32 upper and 32 lower incisors endodontically treated and FRPs were cemented, using pretreated (Rocatec) and non-pretreated posts. Crowns were fabricated and cemented with Ketac-cem or Panavia. Additionally, eight upper and lower incisors with intact natural crowns were used as control groups. Mandibular incisors restored with FRPs attain fracture strengths comparable to those of natural teeth. The fracture strength of all teeth was increased by using Rocatec. Cementation of the crowns using Panavia only increased the fracture strength in upper incisors. Rocatec used to pretreat the posts and crowns cemented using an adhesive cement appear to reduce the risk of clinical failure.  相似文献

目的比较两种纤维桩修复磨牙缺损的临床效果。方法选取磨牙残根残冠患者84例共120颗患牙,经过完善的根管治疗后,随机分为2组,A组42例患者共58颗患牙,置入石英纤维桩;B组42例患者共62颗患牙,采用玻璃纤维桩。两组患者均采用全冠恢复牙冠外形。随访36个月,观察2组患牙的修复效果,并用SPSS13.0软件对2组修复体的成功率进行χ2检验。结果 A组成功率96.6%,B组成功率为93.5%,差异无统计学意义(P〉0.05)。A组无桩折断发生,B组3颗患牙发生桩折断,2组桩折断的发生率差异无统计学意义(P〉0.05)。结论石英纤维桩与玻璃纤维桩对磨牙缺损均能取得良好的修复效果。  相似文献

PURPOSE: The purpose of this study was to compare fracture strengths of teeth restored with cast metal and ceramic dowel and cores supporting all-ceramic crowns. MATERIALS AND METHODS: Eighteen recently extracted teeth were randomly divided into 3 equal groups. Metal dowel and cores (MDC) were fabricated for 1 group using a base metal alloy, and served as the control. The other groups received In-Ceram Spinell (ICS) and IPS Empress 2 (IPS) dowel and cores. IPS Empress 1 crowns were fabricated for all samples. Each tooth was subjected to a shear force at a crosshead speed of 2 mm/min on a universal testing machine until failure occurred. RESULTS: The mean fracture strengths and standard deviations of MDC, ICS, and IPS restorations were 1101 N (361), 832 N (288), and 1137 N (367), respectively. The differences between groups were insignificant (p < 0.05). Fractures of the all-ceramic crowns were observed in the MDC group, whereas the failure of ceramic dowel and core restorations was associated with root fracture at the cervical region.
