BACKGROUND: A high tension in anterior cruciate ligament grafts affects both graft and knee functional performance. Clinical observations suggest that impingement of the graft against the posterior cruciate ligament might cause high graft tensions. Also, meniscal injury has been well documented in association with damage in the anterior cruciate ligament. METHODS: In this paper, we present the results obtained in a three-dimensional finite element model of the human knee, corresponding to different aspects of anterior cruciate ligament reconstruction with bone-patellar tendon-bone grafts. This model was used to investigate the effect of the angle in the coronal plane of femoral and tibial tunnels. Firstly, graft tension was computed in a knee moved from 0 degrees to 60 degrees of flexion and the results were compared with experimental ones obtained by other authors. Secondly, the resulting kinematics under an anterior load of 134 N was compared to that of the intact knee. FINDINGS: The obtained results showed that the closest anterior tibial translation to that of the intact knee was obtained with femoral and tibial tunnels with angles of 60 degrees. In this same case, a lower graft tension was also obtained. The results demonstrated noticeable increases in the meniscal stresses after anterior cruciate ligament reconstruction. INTERPRETATION: Our results showed that impingement only depends on the femoral tunnel angle. On the contrary, laxity principally depends on the tibial tunnel angle. The angle of the femoral tunnel affects the graft tension while the tibial tunnel affects laxity, meniscal stresses and strains.  相似文献

BACKGROUND: The anterior cruciate is the most frequent knee ligament to be totally disrupted. Surgical reconstruction of the anterior cruciate ligament is a common practice to treat the disability or chronical instability of knees due to anterior cruciate ligament insufficiency. Some of the factors that influence the success or failure of the anterior cruciate ligament reconstruction are the integrity of secondary restraints, the preoperative laxity of the knee, the status of the articular and meniscal cartilages, the selection of the graft material, the surgical technique, the graft tension and the postoperative rehabilitation. METHODS: In this paper we present and discuss the results obtained with a three-dimensional finite element model of the human knee joint corresponding to different aspects of human anterior cruciate ligament reconstruction. In particular, this model was used to investigate the effect of graft stiffness and graft tensioning on the knee joint biomechanics. The initial graft tension was set to 0, 20, 40 or 60 N with the knee at 0 degrees , 30 degrees and 60 degrees of flexion. Three different stiffnesses corresponding to those of patellar tendon, gracilis and quadrupled semitendinosus grafts were analyzed. FINDINGS: The resulting kinematics in each of these cases under an anterior load of 134 N was compared to that of the intact knee. The obtained results showed that, after reconstruction, the closest anterior tibial translation to that of the intact knee is obtained with a bone-patellar tendon-bone graft with a pretension of 60 N. INTERPRETATION: However, this initial tension produces an important additional stress in the graft during the knee movement. This may cause problems in revascularization and remodelling during the postoperative healing process. A lower pretension of about 40 N should therefore be recommended in the present conditions.  相似文献

OBJECTIVE: To biomechanically evaluate interference screw fixation of the doubled flexor tendon graft in anterior cruciate ligament reconstruction using cyclic elongation. DESIGN: Biomechanical properties of the interference screw fixation of the flexor tendons were compared with those of three standard fixation techniques which had been commonly performed in anterior cruciate ligament reconstruction. BACKGROUND: The interference screw fixation of the flexor tendon graft has attracted notice because of various possible advantages. METHODS: Forty fresh frozen porcine hind limbs were divided into four groups of ten knees each. Anterior cruciate ligament reconstruction was carried out in each group using one of four different procedures. For each group, five femur-graft-tibia complexes underwent submaximal cyclic elongation of 5000 cycles after initial tension of 80 N was applied. Then, tensile testing was performed in the same manner for the complex with a tensile tester. The remaining five complexes were examined in the same tensile test without applying any cyclic elongation. RESULTS: The initial tension was more rapidly relaxed by cyclic elongation in the flexor tendon graft fixed with interference screws than in the bone-patellar tendon-bone graft fixed with two standard techniques. After cyclic elongation, while the ultimate failure load of the former was significantly lower than the latter, the linear stiffness of the former was significantly higher than the flexor tendon graft fixed with sutures. CONCLUSION: The present study has clarified that the advantage of the interference fixation for the doubled flexor tendon graft is the high linear stiffness of the FGT complex, and the disadvantage of this screw is the low ultimate failure load of the FGT complex. RELEVANCE: The present study has suggested that vigorous activities should not be permitted for the patients in the early period after anterior cruciate ligament reconstruction using this fixation technique, because of its low ultimate failure load.  相似文献

背景:重建膝关节前交叉韧带的移植物包括自体移植物、同种异体移植物和人工韧带3种,关于移植物的选择一直存在诸多争议。目的:回顾分析深低温处理对异种骨-髌腱-骨重建膝前交叉韧带排斥反应的影响。方法:以"骨-髌腱-骨、膝前交叉韧带、排斥反应"为中文关键词,以"bone-patellartendon-bone、anterior cruciate ligament、reject reaction"为英文关键词,采用计算机检索中国期刊全文数据库、PubMed数据库(1995-01/2011-11)相关文章。纳入骨-髌腱-骨重建膝前交叉韧带等相关的文章,排除重复研究或Meta分析类文章。结果与结论:共检索到76篇,按纳入和排除标准对文献进行筛选,共纳入16篇文章。文章从运动中膝关节交叉韧带损伤治疗方法,交叉韧带损伤重建,移植物体的选择和对比,移植物体排斥反应,异种骨-髌腱-骨移植免疫反应等方面的内容进行了叙述。研究表明,深低温处理能有效降低异种骨-髌腱-骨免疫原性,对于抑制排斥反应的发生具有积极作用。  相似文献

BackgroundA question as to the effect of the graft choice on rotational kinematics after anterior cruciate ligament reconstruction remains unclear. The purpose of this study was to determine if there were any differences in rotational alignment of the knee after anterior cruciate ligament (ACL) reconstruction using bone-patellar tendon-bone (BTB) graft compared to that using semitendinosus-gracilis (STG) graft.MethodsTen patients with BTB graft and 10 patients with STG graft were assessed at 3 months after ACL reconstruction. We compared the 6 degrees-of-freedom alignment of the tibiofemoral joint in the fully extended position between knees reconstructed with BTB graft and STG graft using the uninvolved knee as a control.FindingsThe BTB graft group showed no difference in knee alignment between the ACL reconstructed knee and uninvolved knee, whereas the STG graft group showed greater external rotation in the ACL reconstructed knee than in the uninvolved knee (median values, 8.4° vs 5.8°; p = 0.022).InterpretationACL reconstruction with the STG graft leads to increased external tibial rotation. Our findings suggest that clinicians should make an effort to prevent increased external tibial rotation during the rehabilitation process in patients with STG graft.  相似文献

背景:交叉韧带损伤是膝关节常见的运动损伤,可引起膝关节的疼痛和关节不稳,常伴有关节松弛、半月板损伤等症状出现。目的:分析膝关节人工交叉韧带的特点以及在膝关节交叉韧带损伤治疗中的效果评价。方法:膝关节交叉韧带发生断裂时,需要通过外科方法重建韧带功能,应用生物材料进行交叉韧带的重建,可以使患者获得较少的病废,并且允许患者膝关节在治疗后立即活动,有利于患者的康复。交叉韧带的移植材料包括自体韧带材料、同种异体韧带材料和人工韧带材料。结果与结论:膝关节交叉韧带修复重建的自体材料有骨-髌腱-骨和半腱肌移植,骨-髌腱-骨移植的并发症较多,半腱肌与股薄肌腱取材后会影响关节的功能。异体韧带材料主要包括骨-髌腱-骨和股四头肌腱-骨,异体材料的不足之处是杀灭细菌和病毒不完全。人工交叉韧带材料分为永久性韧带材料、支架韧带材料和框架韧带材料,LARS人工韧带移植治疗交叉韧带损伤的疗效较为理想,与自体韧带移植的近期疗效相比,差异有显著性意义(P〈0.05),与自体韧带移植的远期疗效比较无差异。无论是自体、同种异体还是人工韧带材料,每种材料都有各自的优缺点,需要根据患者的自身情况和病情来决定,人工韧带材料为交叉韧带移植提供了新的材料选择,随着生物材料学和生物医学工程学的发展,人工韧带材料一定会成为韧带材料的良好替代物。  相似文献

背景:传统前交叉韧带重建造模腱骨界面愈合缓慢,造模时间较长。目的:通过在腱骨界面注入以纤维蛋白胶为载体的重组人骨形态发生蛋白2复合物,建立造模周期更短、更完善的兔膝关节前交叉韧带重建动物模型。方法:取成年新西兰兔同侧半腱肌肌腱作为自体移植材料,建立双侧前交叉韧带重建动物模型,建模后随机分为模型组、纤维蛋白胶组,空白对照组和正常组。模型组在移植腱骨隧道界面注射填充以纤维蛋白胶作为载体的重组人骨形态发生蛋白2,纤维蛋白胶组在重建术后腱骨界面仅填充纤维蛋白胶,空白对照组在重建术后腱骨界面不作任何填充,正常组则不予手术,保留正常的前交叉韧带。各组分别于术后第4和8周取材,进行生物力学检测。结果与结论:模型组最大载荷和刚度在术后4和8周与纤维蛋白胶组和空白对照组相比均增强(P〈0.01)。由此可见,实验成功建立了双侧前交叉韧带重建动物模型,以纤维蛋白胶作为载体的重组人骨形态发生蛋白2可以在术后早期提高腱骨界面的最大载荷和刚度,促进了腱骨界面的愈合,缩短了重建动物模型的实验周期。  相似文献

Most surgeons agree that postoperative rehabilitation is an important part of the final outcome after anterior cruciate ligament (ACL) reconstruction. With the aim of a full return of knee function in terms of stability, strength, range of motion and neuromuscular control different methods of rehabilitation are performed in different phases of graft healing. A review of the literature of prospective and randomized studies revealed the following information for the rehabilitation after ACL reconstruction with a bone-patellar tendon-bone (BPTB) autograft: a brace is not required. Full weight bearing reduces anterior knee pain and has no negative effect. Neuromuscular electrical stimulation increases muscle strength and improves kinematics. Cold therapy has no positive effect. Early joint motion can avoid the negative effect of immobilization. The literature on rehabilitation after ACL reconstruction with hamstrings is not sufficient for a consensus opinion.  相似文献

背景:目前同种异体组织移植成为韧带移植重建的一种选择,同种异体骨-髌腱-骨可作为外侧副韧带重建的一种选择。目的:观察采用同种异体骨-髌腱-骨移植重建陈旧性外侧副韧带损伤的临床效果。方法:选择2008-04/2009-12解放军第101医院骨科收治的Ⅲ度陈旧性外侧副韧带损伤患者11例,均采用同种异体髌腱解剖移植重建外侧副韧带。结果与结论:11例患者均获得随访,随访时间12~15个月。切口均Ⅰ期愈合,关节活动范围移植重建后较移植前显著增大(P<0.05)。Lysholm评分末次随访时较移植前显著增加(P<0.05)。移植前内翻应力位X射线平片显示患膝外侧关节间隙较对侧平均增加8~20(12.9±3.5)mm,移植后6个月为-2~3(1.0±1.9)mm,与移植前相比差异有显著性意义(P<0.05),与健侧相比差异无显著性意义(P>0.05)。提示应用同种异体骨-髌腱-骨对陈旧性外侧副韧带损伤进行重建,能够有效恢复膝关节内翻稳定性,对其他软组织重建提供保护作用。  相似文献

目的 介绍一种关节镜下后交叉韧带(Posterior Cruciate Ligament,简称PCL)重建术并评估其近期临床治疗效果。方法 中1/3骨—髌韧带—骨组织(Bone—Patellar Tendon—Bone Graft,简称BPB)移植重建膝关节后交又韧带,用于治疗后交叉韧带损伤后关节不稳定。手术在关节镜下经前方进路完成。结果 18例患者平均随访13.1个月。Larson评分由术前61分提高到93分。Lysholm评分由术前55分提高到91分。所有患膝术前后抽屉试验及Lachman试验均阳性,术后仅2例抽屉试验弱阳性,1例Lachman弱阳性。所有患者主观评价膝关节功能明显增进,对治疗效果满意。结论 在关节镜直视下,能准确定位后交叉韧带解剖止点,钻制骨遂道,植入移植组织。该技术具有不切开关节囊,损伤小、关节粘连率低的优点。  相似文献

OBJECTIVE: To investigate the extent of coupling between the anterior and posterior cruciate ligaments as well as the role of the posterior cruciate ligament in the knee joint response under anterior femoral force at different flexion angles. DESIGN: A developed finite element model of the tibiofemoral joint is used to perform non-linear elastostatic analyses. BACKGROUND: The structural properties of the posterior cruciate ligament subsequent to an injury (either left untreated or replaced by a graft) would likely change, an event that alters the function of not only the ligament itself but also the other intact cruciate ligament and the entire joint. METHODS: The model consists of two bony structures and their articular cartilage layers, menisci and four principal ligaments. Under 100 N anterior femoral load at different flexion angles from 0 degrees to 90 degrees, kinematics, forces in ligaments and contact forces in the fully unconstrained joint were computed in intact cases and following alterations in joint ligaments. RESULTS: Collateral ligaments were the primary structures to resist the force at full extension under 100 N anterior femoral load with a moderate contribution from the posterior cruciate ligament. With joint flexion up to 90 degrees, however, force in the posterior cruciate ligament substantially increased whereas that in collateral ligaments diminished. CONCLUSIONS: A remarkable coupling was found between the posterior cruciate ligament and the anterior cruciate ligament in flexion; a structural alteration in one of them significantly influenced the mechanical role of both ligaments and not just the one affected. A tauter or stiffer ligament increased the force in both ligaments while an excessive laxity or rupture in one diminished forces in both. RELEVANCE: Alterations in ligament stiffness or initial tautness during reconstruction surgery or following injuries markedly influence the normal role of both cruciate ligaments. Consideration of cruciate ligaments coupled together rather than in isolation should be the rule in the management of ligament injuries towards a successful long-term outcome.  相似文献

目的:总结近年对膝关节韧带的生物力学与人工韧带的研究进展,强调膝关节的损伤类型,膝关节前后交叉韧带修复的主要方法及进展.资料来源:以human knee cruciate ligament,biomechanics,xenogeneic ligamentous,reconstruction为检索词,检索PubMed数据库(2008-01/2009-07):以膝关节交叉韧带,生物力学,重建,人工韧带,康复为检索词,检索中国期刊全文数据库(2008-01/2009-07).文献检索语种限制为英文和中文.资料选择:纳入膝关节及人工韧带相关的内容.排除已被证实落后的研究及重复性研究.结局评价指标:①膝关节的生物力学特征.②膝关节韧带损伤的情况.③膝关节韧带的修复与重建方法.结果:计算机初检得到526篇文献,根据纳入排除标准,对膝关节损伤生物材料及其生物力学进行分析.常见的膝关节损伤主要是韧带损伤,为恢复其功能主要采用手术重建,其中利用自体中1/3骨-髌韧带-骨作为移植材料的挤压螺钉内固定重建术,已成为重建前交叉韧带的标准方法.羟基磷灰石是一类具有良好生物相容性的生物活性材料,羟基磷灰石生物活性陶瓷植入体内不仅安全、无毒,还能传导骨生长,即新骨可以从羟基磷灰石植入体与原骨结合处沿着植入体表面或内部贯通性孔隙攀附生长,能与组织在界面上形成化学键性结合.结论:对膝关节及膝关节韧带进行生物力学分析是防止和治疗膝关节损伤及疾病的基础.具有表面羟基磷灰石涂层无螺纹的假体松动率最低,是最适宜的假体置换材料.  相似文献

BACKGROUND: The Bone Mulch screw/WasherLoc fixation system has attracted notice because of its possible advantages. The purpose of the present study was to compare the biomechanical properties of this fixation system for the double-looped flexor tendon graft with those of two standard fixation techniques that had been commonly performed in anterior cruciate ligament reconstruction. METHODS: Anterior cruciate ligament reconstruction was carried out in each group using one of three different procedures (n=14 for each procedure). For each group, seven femur-graft-tibia complexes underwent submaximal cyclic displacement of 5000 cycles after an initial tension of 20N was applied. Then, tensile testing was performed for the complex at a single position, 45 degrees of knee flexion, in which the longitudinal axis of the graft coincided with the axis of the bone tunnels. The remaining seven complexes in each group were examined in the same tensile test without applying any cyclic displacement. FINDINGS: At the 5000th cycle of the displacement, the peak load of the complex with the Bone Mulch screw/WasherLoc system was significantly higher than that with the Endobutton technique (P<0.0001). After 5000 cycles of displacement, the initial stiffness and the linear stiffness of the complex with the Bone Mulch screw/WasherLoc system were significantly higher than those with the double-looped tendon graft and the Endobutton technique (P<0.0001 for both comparisons), while those with the Bone Mulch screw/WasherLoc system were significantly lower than those with the patellar tendon graft with interference screws (initial stiffness: P=0.0004, linear stiffness: P=0.0007). INTERPRETATION: The present study has clarified that the Bone Mulch screw/WasherLoc system provides high stiffness to the complex for the double-looped flexor tendon graft.  相似文献

背景:前交叉韧带重建移植物的选择以及重建后移植物的组织学转归一直是前交叉韧带修复与重建领域的一个重要的研究课题.目的:就前交叉韧带的结构,移植物的选择问题进行综述.方法:以Anterior cruciate ligament、implant、reconstruction为检索词,检索Medline数据库1980-01/2010-01相关文献.纳入标准:①前交叉韧带损伤重建手术相关文献.②前交叉韧带移植物选择相关文献.排除标准:①较陈旧的文献.②重复研究.共收集250篇关于关节镜下前交叉韧带重建的文献,排除发表时间较早、重复及类似研究,纳入33篇符合标准的文献.结果与结论:目前,前交叉韧带断裂主要治疗方法为关节镜和关节镜辅助下前交叉韧带重建术.前交叉韧带重建术临床应用的移植物很多,包括自体移植物、同种异体移植物、异种异体移植物、生物材料、人工材料和组织工程学移植物.应用自体半腱肌腱和股薄肌腱或者应用自体骨-髌腱(中1/3)-骨(BPTB)重建前交叉韧带的方法是国内外重建前交叉韧带的主要方法.  相似文献

Background

Anterior cruciate ligament reconstruction does not necessarily restore normal knee movement. Increased tibial rotation has previously been noted during pivoting activities and may be due to the orientation of the anterior cruciate ligament graft associated with traditional single bundle reconstruction techniques. Recent research has shown that it is possible to limit rotation during level walking using a single bundle anterior cruciate ligament reconstruction. This study evaluated rotational knee kinematics during a pivot task in a group of patients who had undergone anterior cruciate ligament reconstruction using a single bundle technique and compared the findings to a normal control group.

Methods

In 27 anterior cruciate ligament reconstruction and 25 control participants, internal–external rotation was measured during a descend stairs and pivot task in a gait laboratory.

Findings

Results showed that the anterior cruciate ligament reconstruction patients had less internal tibial rotation (for both range of rotation and maximum rotation) than the control participants (effect size = 0.7).

Interpretation

These results suggest that it is possible to limit rotation after anterior cruciate ligament reconstruction using a single bundle technique, even during a pivoting movement that places a high rotational load at the knee joint. The positioning of the femoral tunnel in a more anatomical position may be responsible for the reduced tibial rotation.  相似文献

BACKGROUND: Intra-articular techniques (single and double bundles) are the most widely used procedures for the anterior cruciate ligament reconstruction. Lemaire introduced in 1967 the extra-articular techniques, and combined intra-articular and extra-articular reconstruction, to better restore the stability of the knee. However, the effectiveness of these procedures (intra-articular, extra-articular combined or not with intra-articular) seems to be controversial. METHODS: In the present study, we developed numerical models of a knee joint to evaluate the effects of these different procedures on the kinematics and biomechanics of the knee during an internal rotation test. Six cases were simulated: intact anterior cruciate ligament, intra-articular reconstructed anterior cruciate ligament (single and double bundles), extra-articular reconstructed anterior cruciate ligament alone, and combination of extra- and intra-articular reconstructions. The loading condition was an internal tibial torque of 2 N m at 0 degrees, 15 degrees, 30 degrees and 45 degrees of knee flexion. Internal rotation of the tibia and forces within the grafts and the ligaments were calculated. FINDINGS: This study showed that both single and double bundles intra-articular reconstructions restore similar internal rotation control and biomechanics of the soft structures as the intact anterior cruciate ligament situation. On the other hand, our results indicate that extra-articular reconstruction reduces appreciably the internal rotation and modifies the charges distribution in the soft structures when compared to the intact anterior cruciate ligament. INTERPRETATION: The extra-articular procedure alters the kinematics of the knee, which might overconstraint the ligaments and the femorotibial joints, leading to the failure of the anterior cruciate ligament reconstruction.  相似文献

背景:关节镜下前交叉韧带重建可以有效地解决前交叉韧带损伤引起的膝关节不稳症状,而双骨道(胫骨单骨道-股骨单骨道)与三骨道(胫骨单骨道-股骨双骨道)前交叉韧带重建是目前较为常用的重建方式。目的:探讨双骨道与三骨道前交叉韧带双束重建对膝关节稳定性的影响。方法:选用8具新鲜正常成人尸体膝关节标本,分别进行双骨道与三骨道前交叉韧带双束重建,然后在MTS-809生物力学测试系统上测试膝关节在胫前加载(134N)以及胫骨旋转加载(5Nm内旋)下屈曲0°,15°,30°,60°,90°时的膝关节稳定性。结果与结论:①胫前加载:在所测的5个角度下,两重建组的胫前位移较前交叉韧带完整组均增大,但差异无显著性意义(P>0.05);双骨道重建组较三骨道重建组胫前位移增大,但差异无显著性意义(P>0.05)。②旋转加载:在所测的5个角度下,前交叉韧带完整组的胫前位移最小,双骨道重建组与三骨道重建组比较,在膝关节屈曲0°,15°和90°时,差异无显著性意义(P>0.05),在膝关节屈曲30°和60°时,三骨道重建组胫前位移小于双骨道重建组,差异有显著性意义(P<0.05);三骨道重建组与前交叉韧带完整组比较,差异无显著性意义(P>0.05)。提示双骨道与三骨道前交叉韧带双束重建均可促进膝关节前后及旋转稳定性的恢复,三骨道前交叉韧带双束重建与双骨道前交叉韧带双束重建相比,显示了更好恢复膝关节旋转稳定性的作用。  相似文献

目的探讨膝关节镜下异体骨-髌腱-骨前交叉韧带(ACL)重建术后早期功能锻炼的方法,最大限度地减少合并症及功能障碍等不良反应。方法对8例前交叉韧带损伤患者实施异体骨-髌腱前交叉韧带重建术,术后通过使用可调式支具外固定、被动活动器(CPM)、被动及主动功能锻炼等康复护理。结果8例术后均获随访,时间为27~77个月,平均41个月。按Lukianov的ACL重建术后功能评定标准,优8例。随访结果显示,术前7例Lanchman试验阳性,术后均阴性;术前6例前抽屉试验阳性,术后均呈阴性;术前6例有明显膝关节不稳定感,术后均消失;术后8例功能锻炼后膝关节活动度可达0~90°。结论ACL重建术后早期进行综合康复训练明显有利于膝关节功能的恢复。  相似文献

背景;间歇性负压被证实可以促进软组织修复及骨愈合,但其对交叉韧带重建后腱一骨愈合的影响尚未见报道。目的:观察间歇性负压对兔前交叉韧带重建后腱一骨愈合及肌腱移植物生物力学的影响。方法:取24只新西兰大白兔制各自体半腱肌前交叉韧带重建模型,随机取一侧后腿作为负压侧,负压侧关节通过引流管接微型负压吸引器,并维持低强度、间歇性负压;对侧后腿作为对照,接普通引流管。5d后两侧同时拔除吸引管。造模后6周,取关节液检测白细胞介素113的表达水平;取股骨一韧带一胫骨复合体行肌腱移植物拉力测定和腱一骨界面组织学观察。结果与结论:1只兔关节感染,最终23只兔进入结果分析。拉力测定结果显示,负压组完全断裂所需拉力显著大于对照组(P〈0.05)。组织学观察结果发现,负压组成骨细胞数目显著多于对照组,差异有显著性意义(P〈0.01)。关节液检测结果提示负压组关节滑液中自细胞介素113含量低于对照组,差异有显著性意义(P〈0.01)。提示间歇性负压可能在前交叉韧带重建后腱一骨愈合、肌腱移植物的塑性过程中扮演着积极作用。  相似文献
