The pulsed Doppler technique was used to record the flow velocity patterns in the ductus arteriosus and the pulmonary artery in 26 patients with either isolated or complicated patent ductus arteriosus (PDA). In all patients, abnormal Doppler signals indicating left-to-right (L-R) or right-to-left shunt flow or both could be obtained at the site of the ductus arteriosus. These Doppler flow patterns determined within the ductus coincided with the direction of ductal flow seen on the contrast two-dimensional echocardiogram. No Doppler signals of shunt flow were demonstrated in any of 42 control subjects. The peak, mean, and diastolic velocities of the L-R shunt flow within the ductus were measured from the ductal flow velocity profiles. With the Doppler-derived measurements of the mean and diastolic velocities, patients with normal pulmonary arterial pressure and those with evidence of pulmonary hypertension could be correctly identified. In addition, the mean velocity of the diastolic antegrade flow portion obtained from the proximal left pulmonary artery, which was related to ductal L-R shunting, was measured in 16 patients with isolated PDA. This Doppler flow determinant showed a good linear correlation with the L-R shunt ratio determined by Fick's method (r = .88, p less than .01). Our technique permits the noninvasive evaluation of shunt flow dynamics in patients with PDA.  相似文献

Nonoperative closure of patent ductus arteriosus (PDA) by means of Ivalon plugs (according to the technique of Porstmann) was performed in 101 patients. Sixty-five patients were symptomatic, the Q p/Q s ratio exceeded 1.5 in 56 patients, and pulmonary hypertension (mean pulmonary artery pressure greater than 20 mmHg) was present in 50 patients. In 100/101 patients the PDA could be closed successfully. Ninety-nine patients were without any evidence of residual left-to-right shunt. In one patient a hemodynamically insignificant left-to-right shunt was found with color Doppler echocardiography. Complications were pulmonary embolism due to plug dislocation in two patients (12th and 14th patient; 2 and 7 weeks after the procedure, respectively). One of these patients underwent elective surgery with patch closure of the ductus and removal of the embolized plug. In the other patient the ductus was successfully closed with a second larger plug while the first plug was left in a peripheral pulmonary artery. Surgical revision of the femoral artery was required in six and blood transfusion in two patients. Deep venous thrombosis developed in two patients. During follow-up (total follow-up time more than 200 patient years) no late complications were observed. In conclusion, transfemoral catheter closure of PDA by means of Ivalon plugs is an effective method. It is applicable to adolescents and adults with a low complication rate. The ductus can be closed without residual left-to-right shunt. Long-term results are excellent.  相似文献

Twenty-nine patients with a patent ductus arteriosus (PDA) in isolation (n = 17) or in combination with other lesions (n = 12) underwent simultaneous hemodynamic assessment and evaluation of PDA flow velocity by the Doppler method. The accuracy with which Doppler velocity across the PDA predicted pulmonary arterial pressure and the influence of PDA size and shape on the Doppler velocity-pressure relationship were examined. Seventy percent had a cone-shaped PDA (narrowest at the pulmonary artery end), and the remainder were tubular. Narrowest PDA diameter ranged from 1.5 to 9 mm (mean 3.5 mm). Peak systolic and mean pulmonary arterial pressure ranged from 10 to 116 and 8 to 72 mm Hg, respectively. Twenty-one patients (group 1) had left-to-right shunting only. The following variables showed significant correlation in this group: peak instantaneous systolic aortic-to-main pulmonary arterial (MPA) pressure gradient and maximum Doppler velocity across the PDA (slope = 1.03, SEE = 13 mm Hg, r = .94, p less than .001), mean aortic-to-MPA pressure gradient and mean Doppler velocity (slope = 1.06, SEE = 10 mm Hg, r = .95, p less than .001), and end diastolic aortic-to-MPA pressure gradient and minimum Doppler velocity (slope = 1.12, SEE = 8 mm Hg, r = .96, p less than .001). Eight patients (group 2) had bidirectional shunting. In this group peak instantaneous aortic-to-MPA pressure gradient significantly correlated with maximum Doppler velocity measured from the left-to-right shunt (slope = .70, SEE = 2 mm Hg, r = .92, p less than .002) and mean pressure gradient correlated with mean Doppler velocity (slope = .83, SEE = 3 mm Hg, r = .78, p less than .003). Right-to-left Doppler velocities showed no correlation with pressures. In six patients with pulmonary hypertension Doppler velocity changes accurately predicted the effect of pulmonary vasodilation on pulmonary arterial pressure. Doppler velocity of PDA flow reliably predicts pulmonary arterial pressure over a wide range of pressures and PDA shapes and sizes.  相似文献

We used pulsed Doppler ultrasound to determine the flow dynamics of the aorta and patent ductus arteriosus (PDA) in 21 infants (ages 1 to 16 days) with PDA and aortic atresia (n = 15) or aortic stenosis (n = 6). The flow within the PDA was recorded in 19, and was right-to-left during systole in all. There was a diastolic left-to-right PDA shunt in 11 patients with aortic atresia and in three with aortic stenosis, and the shunt was associated with large (3 to 11 mm in diameter) interatrial communications. In two patients with aortic atresia and three with aortic stenosis, however, the diastolic PDA shunt was from right to left, and the interatrial communications were small (0 to 2 mm in diameter). The right-to-left diastolic PDA shunting may be best explained by the relative pulmonary (high with left-sided inflow obstruction and a small interatrial communication) and systemic resistances. All patients with aortic atresia and three with aortic stenosis had retrograde systolic flow in the transverse aortic arch, probably resulting from inadequate left ventricular output. Antegrade diastolic flow in the transverse aortic arch toward the PDA was observed in all infants with aortic atresia and a left-to-right PDA shunt. Ascending aortic flow was recorded in 11 patients with aortic atresia, and was retrograde during diastole in each, the result of coronary perfusion. Application of pulsed Doppler ultrasound can lead to a better understanding of the hemodynamics and physiology of patients with congenital cardiovascular disease.  相似文献

Long-standing left-to-right shunting across a patent ductus arteriosus (PDA) can result in Eisenmenger syndrome. In this report, we present echocardiographic findings of a 27-year-old female patient with pulmonary hypertension. In diagnostic work-up especially Doppler findings of the pulmonary artery suggested the presence of a reversed PDA as a cause of pulmonary hypertension. The diagnosis was confirmed by contrast study and computed tomography.  相似文献

Thirty-two patients from 3 Pediatric Cardiological Centres underwent attempted endoluminal transcatheter closure of a patent ductus arteriosus. The patients' age ranged from 8 months to 67 years, but there were only 2 adults (18 and 67 years) in this series. All patients had a murmur which was continuous in 28 cases and systolic in 4 cases. Nine patients had left ventricular hypertrophy on the electrocardiogram and radiological cardiomegaly. Closure of the patent ductus was attempted by Rashkind's technique using a double umbrella of 12 mm diameter for ductus less than 4 mm diameter (26 cases), and an umbrella of 17 mm diameter when the ductus was wider (6 cases). The immediate results judged by retrograde aortography showed total occlusion in 12 patients and incomplete closure in 17 others with persistence of a minimal left-to-right shunt. In 3 cases, embolisation of the umbrella into the left pulmonary artery (2 cases) or right pulmonary artery (1 case), required thoracotomy to extract the umbrella and at the same time section-suture of the ductus arteriosus. Color Doppler echocardiographic follow-up showed the disappearance of the residual shunt in seven children.  相似文献

Pulsed Doppler echocardiography (PDE) from the suprasternal approach was used to assess flow characteristics of ductus arteriosus (DA) in 145 infants (aged 1 day to 6 months) with major congenital heart disease. Direct ductal Doppler interrogation was possible in 138 patients and serial studies before and after medical treatment were performed in 28 infants. According to pulmonary artery pressure and associated heart lesions, 3 ductal shunting patterns were identified. An isolated left-to-right shunt, observed in isolated DA or in right ventricular outflow tract obstruction, was characterized by a continuous flow with a peak velocity in late systole. An isolated right-to-left shunt, observed in persistent fetal circulation and aortic arch abnormalities, was characterized by a continuous flow with a peak velocity in early systole. In patients with a bidirectional ductal shunt, the right-to-left shunt always occurred in systole and the left-to-right shunt began in late systole and extended into diastole. A systolic right-to-left shunt always corresponded to the presence of significant pulmonary hypertension. Ductal flow changes could be documented after prostaglandin E1 therapy in patients with ductus-dependent heart disease or after tolazoline therapy in patients with persistent fetal circulation. Thus, PDE with direct ductal Doppler interrogation is an important complement to the echocardiographic evaluation of DA. It is a safe noninvasive approach to ductal shunt and permits convenient evaluation of the effects of drugs on pulmonary artery resistance (tolazoline) and ductal patency (prostaglandin E1).  相似文献

Postnatal circulatory adaptations were studied with Doppler echocardiographic measures of flow velocity in the main pulmonary artery (PA) and ascending aorta (Ao) in 45 normal full-term neonates at 5 hours and at 27 hours after birth. PA flow velocity integral (FVI) was measured as the area under the systolic flow velocity curve and reflected total systemic flow in the presence of a left-to-right shunt through the ductus arteriosus. This index increased from 5 and 27 hours age, while Ao FVI, reflecting total pulmonary flow, remained unchanged. Evidence of a left-to-right ductal shunt demonstrated as diastolic retrograde flow in the main PA was detected in 42 neonates at 5 hours and in only four subjects after 27 hours of age, indicating a patent ductus arteriosus at 5 hours of age and its subsequent closure. In the PA, acceleration time (AT) increased while pre-ejection period to ejection time ratio (PEP/ET) decreased from 5 to 27 hours of age, reflecting the physiologic fall in PA pressure. In the Ao, FVI, AT, and PEP/ET remained unchanged, suggesting little change in left ventricular function.  相似文献

Serial Doppler echocardiographic examinations were performed in 10 normal neonates (0.3-4.0 hrs after birth). The flow patterns through the ductus arteriosus were evaluated using Doppler color flow imaging, pulsed Doppler echocardiography and continuous-wave Doppler echocardiography. At the initial examination, flow through the ductus arteriosus was clearly visualized in all the neonates using Doppler color flow imaging. The ductal flow patterns were categorized as follows: 1. Systolic blue color (right-to-left shunt flow) and diastolic red color (left-to-right shunt flow) in four neonates (group 1). 2. Systolic blue color and diastolic mosaic colors in four neonates (group 2). 3. Continuous mosaic colors in two neonates (group 3). Using pulsed Doppler echocardiography, the systolic right-to-left ductal shunt flow in the groups 1 and 2 was triangular in shape beginning in early systole. The diastolic left-to-right shunt flow was box-like in shape beginning late in systole and lasting long in diastole in the group 1. In the group 2, the diastolic flow showed a wide spectrum (turbulent flow). In the group 3, the flow through the ductus arteriosus had a continuous wide spectrum (turbulent flow). Mosaic or turbulent ductal flow of a left-to-right ductal shunt had high velocities by continuous-wave Doppler echocardiography. Serial examinations revealed that the ductal flow pattern observed in the group 1 changed to the flow pattern observed in the group 2, and then to that of the group 3 with increasing diastolic ductal flow velocities. The estimated aorto-pulmonary pressure gradient according to the simplified Bernoulli equation (delta p = 4V2) using a maximum diastolic left-to-right ductal shunt velocity increased within 12 hrs after birth. It was concluded that bidirectional ductal shunts may be observed in most normal neonates (8/10). With increasing diastolic velocities the bidirectional ductal flows changed to the pattern of a continuous left-to-right shunt. The bidirectional ductal shunt is considered due to physiologic pulmonary hypertension of the newborn and due to less conduction time from the pulmonary valve to the pulmonary end of the ductus than from the aortic valve to the aortic end of the ductus. Analysis of the flow through the ductus provides informations about the neonatal circulatory adaptation, especially in the early neonatal period.  相似文献

Recent reports suggest reopening of the patent ductus arteriosus (PDA) after complete occlusion with three-loop Gianturco coils. We hypothesize that five-loop coils may produce a larger thrombus than three-loop coils, which will result in no or less probability of recanalization of PDA during follow-up. This study is designed to test this hypothesis. Follow-up echocardiographic and Doppler data of 30 patients who underwent five-loop coil occlusion of small to medium-sized PDA during a 33-month period ending December 1998 were examined. Thirty patients had no residual shunt on echo Doppler study on the day following the procedure and were followed for 6 to 30 months (median, 12) after coil implantation. At the last follow-up study, none of the patients had a residual shunt and left atrial size decreased. Careful pulsed, continuous wave, and color Doppler interrogation of left/main pulmonary artery junction and proximal descending aorta did not reveal any evidence for obstruction. The follow-up data suggest that complete occlusion of small- to medium-sized PDAs is feasible with five-loop coils without evidence for recanalization at a mean follow-up of 12 months. Much longer (2 to 5 years) follow-up data may be necessary to confirm these observations. We speculate that a greater degree of thrombosis is produced within the ductus by the five-loop coils, which in turn may be responsible for lack of shunt recurrence. We recommend use of five-loop instead of three-loop coils for transcatheter occlusion of small- to medium-sized PDAs.  相似文献

A persistent ductus arteriosus was occluded with an ivalon-plug introduced through the femoral artery in 14 patients over 60 years old (61 to 70 years). Two patients had previous surgery. The maximum left-to-right shunt was 53%, the maximum pulmonary artery pressure 54/24 mm Hg. In all patients ductus occlusion was successful and permanent. Bleeding at the puncture site in two patients could be stopped by manual compression. During a follow-up period of up to 5 years there were no instances of recanalization.  相似文献

Eighteen infants and children with isolated ductus arteriosus and various hemodynamic states were examined with the Doppler color flow mapping technique to study the flow patterns of the patent ductus and its adjacent structures. Direct visualization of the ductus was achieved in all patients. All ductal flow was shunting left to right from the descending aorta to the pulmonary artery even in the presence of significant elevation of pulmonary artery pressure. However, transient bidirectional shunting was observed in two patients on serial studies. Disturbed systolic and diastolic flows were demonstrated within the ductus in each case. The blood flows in the main pulmonary artery consisted of three distinguishable areas: 1) a characteristic high velocity turbulent retrograde ductal jet throughout the entire cardiac cycle, located in the anterolateral aspect of the main pulmonary artery; 2) nonspecific low velocity retrograde late systolic and early diastolic flow, located in the posteromedial aspect of the main pulmonary artery; and 3) low velocity forward systolic and diastolic flow, occupying the remaining area of the pulmonary artery. The blood flows in the descending aorta near the ductal orifice consisted of disturbed systolic and diastolic flows in a reversed direction. By providing detailed real time blood flow information with simultaneous imaging of the ductus, Doppler color flow mapping greatly facilitates the detection of a small ductal shunt. This technique also allows detection of a bidirectional or right to left ductal shunt.  相似文献

OBJECTIVES: This study was designed to assess the safety, efficacy, medium-term outcome, and complications of patent ductus arteriosus (PDA) stenting in duct-dependent pulmonary circulation. BACKGROUND: Patent ductus arteriosus stenting has been proposed as an alternative to surgical shunt on account of postoperative morbidity and complications of surgical shunting. METHODS: Between April 2000 and February 2003, 69 patients with duct-dependent pulmonary circulation underwent cardiac catheterization with the intent of PDA stenting as first palliative procedure. Patients with critical pulmonary stenosis and pulmonary atresia with intact ventricular septum post-radiofrequency valvotomy who had PDA stenting were excluded. Thirteen more patients were excluded because of branch pulmonary artery (PA) stenosis. The follow-up was by clinical examination, echocardiography, and repeat cardiac catheterization at six to nine months following the procedure. RESULTS: Patent ductus arteriosus stenting was successful in 51 patients (91.1%) and failed in 5 patients (8.9%). The mean narrowest PDA diameter was 1.9 +/- 0.6 mm. The mean procedure and fluoroscopy time were 95.7 min and 29.4 min, respectively. In one patient the stent dislodged and migrated to the left femoral artery and another patient developed transient intravascular hemolysis. There was no procedure-related mortality. Three patients (5.9%) died one day to two months after the procedure. At follow-up (3.2 months to 2.4 years), 8 patients developed significant stent stenosis requiring reintervention. Seven patients developed worsening of preexisting branch PA stenosis. The freedom from reintervention was 89% and 55% at 6 months and 1 year, respectively. CONCLUSIONS: Patent ductus arteriosus stenting is an attractive alternative to surgical shunt in a majority of patients with duct-dependent circulation. An absolute contraindication to this technique is the presence of branch pulmonary stenosis.  相似文献

To characterize the ductus arteriosus shunt after birth, 53 normal newborn infants (36-41 weeks gestation), appropriate in size for gestational age, were examined using two dimensional Doppler echocardiography directed continuous Doppler for evidence of patent ductus arteriosus. The infants were examined within six hours of birth, and every six-eight hours thereafter until ductus arteriosus shunt could no longer be detected. In 51/53 infants, an adequate examination was possible and ductus arteriosus was detected in every infant using the standard precordial approach. Using two dimensional Doppler echocardiography a yellow-orange-red jet, sometimes blue in the central area, directed at the lateral wall of the pulmonary artery was recorded; using continuous Doppler a diastolic or continuous spectral flow into the main pulmonary artery was recorded. Ductus arteriosus shunt could no longer be detected in 7 infants 12 hours after birth, in 26 infants 12-24 hours after birth, in 11 infants 24-36 hours after birth, in 5 infants 36-48 hours after birth and in 2 infants 48-60 hours after birth. A diastolic spectral Doppler flow was present in 38 infants and became continuous before duct closure in 27 infants. In 13 infants it was continuous at first examination and until the ductus arteriosus closure. Both diastolic or continuous spectral Doppler flow could present a flat waveform profile, or a protodiastolic or protosystolic peak velocity. The shunt peak velocity increased significantly with the age i.e. (1.5 +/- 0.7 m/sec mean and SD-), at first examination, vs 2.3 +/- 0.6 m/sec at last examination before ductus arteriosus closure (p less than 0.001).(ABSTRACT TRUNCATED AT 250 WORDS)  相似文献

Serial two-dimensional Doppler echocardiography was performed in 22 normal neonates (2-9 hours after birth). A left-to-right shunt through the foramen ovale and a shunt through the ductus arteriosus were evaluated. Interatrial shunts were observed as transseptal jets in 16 of 22 neonates on initial examination and resolved nine to 26 hours after birth in 13 neonates. In the remaining three neonates the shunts were observed more than six days and resolved by two months of age. Shunts through the ductus arteriosus were observed in all neonates examined as diastolic or continuous flows toward the transducer in the main pulmonary artery at the initial examination. The ductal flow resolved simultaneously with (3/13) or after (10/13) the disappearance of interatrial flow. With two-dimensional echocardiography, the enlarged left atrium and rightward deviation of the interatrial septum were observed in the neonates with interatrial shunts. This was confirmed by the immobile septum primum and the increased ratio of the left atrial to aortic diameters (LA/Ao ratio) on the M-mode echo. The interatrial septum became mobile and the LA/Ao ratio decreased after disappearance of the interatrial shunts. It was speculated that in neonates with large ductal shunts, large pulmonary venous flow enters into the less compliant left ventricle and raises the left ventricular end-diastolic pressure. Then, increased left atrial pressure distends the interatrial septum and causes left-to-right interatrial shunts via the stretched foramen ovale. This shunt is considered one of the physiological phenomena occurring in the early neonatal period.  相似文献

目的评价Amplatzer封堵器治疗动脉导管未闭(PDA)合并重度肺动脉高压(SPH)的初步疗效.方法对12例(女9例,男3例)PDA合并SPH患者实施封堵治疗.其中10例采用Amplatzer动脉导管未闭封堵器,2例采用Amplatzer房间隔缺损封堵器.结果全组12例PDA封堵器均放置成功.11例PDA封堵后30分至1小时肺动脉收缩压、肺动脉平均压均明显降低.1例封堵术后即刻肺动脉压无变化.术后36小时彩色多普勒估测肺动脉压明显下降,封堵术后30分降主动脉造影,无残余分流9例,微量残余分流2例,少量残余分流1例.全组术后24~48小时彩色多普勒检查,动脉水平左向右分流均完全消失.无重要并发症发生.随访1~24个月(平均8个月),患者症状改善,11例心脏缩小,无1例发生再通.结论采用Amplatzer法封堵治疗PDA合并SPH,近期疗效满意.  相似文献

目的 评价应用国产封堵器封堵治疗巨大动脉导管未闭(PDA)的疗效.方法 全组17例,男性8例,女性9例,年龄15~43(24.5±11.1)岁.主动脉造影示PDA最窄腰部内径为18~26(25.5±1.8)mm,需采用腰部直径20mm以上的封堵器方可操作成功.术后3d及3个月后复查超声心动图.结果 术前超声诊断,17例中6例合并重度肺动脉高压,17例患者中14例(其中5例合并重度肺动脉高压)封堵成功(操作和临床成功),成功率为82%(14/17).16例选用国产特制PDA封堵器,1例26 mm ASD因封堵器位置不能牢固地“站稳”而放弃封堵治疗;1例严重肺动脉高压且左向右分流为主存在少量右向左分流,经封堵试验为阻力性肺动脉高压不适宜治疗;1例术后第2天发生封堵器脱落入降主动脉,经捕获器成功取出体外,但死于误吸(迷走反射引起恶心呕吐)导致的严重肺部感染及感染性休克.封堵成功14例,封堵器直径为22~30(26.5±2.7)mm,术后即刻降主动脉造影检查示9例微量残余分流,均为封堵器中间存在云雾状分流,10 min后重复造影云雾状分流消失.术后3d及术后3个月超声心动图示患者所有左心内径增大均明显回缩,未发现残余分流和导管再通.结论 巨大PDA在可能的范围内尽量争取介入治疗,成功后较普通PDA血液动力学恢复更明显,可获得较好的收益/风险比.  相似文献

To develop a new method for noninvasive measuring of the pulmonary artery pressure (PAP) in patient ductus arteriosus (PDA), left and right heart catheterization (Cath) and continuous-wave Doppler echocardiography (CWD) were performed simultaneously in 30 cases with PDA. The results showed that there was high correlation of the systolic peak pressure gradient (delta pp), the end-diastolic pressure gradient (delta Pd) and the mean pressure gradient (delta Pm), across PDA measured with the two techniques (r = 0.99, 0.96 and 0.98, respectively). The CWD-derived pulmonary artery systolic (PASP), diastolic (PADP) and mean (PAMP) pressure, which were estimated by subtracting delta Pp, delta Pd and delta Pm from the brachial artery systolic, diastolic and mean pressure, correlated well with the corresponding PAP measured with Cath (r = 0.92, 0.89 and 0.94, respectively). It is concluded that CWD offers a reliable technique for estimating shunt pressure gradients and PAP in patients with PDA.  相似文献

目的探讨老年动脉导管未闭(PDA)患者彩色多普勒超声心动图(CDE)特征。方法选择老年PDA患者25例,应用彩色多普勒血流显像(CDFI)分流束血流信号估测PDA肺动脉端直径,与心血管造影对比判断其准确性;连续波多普勒测量三尖瓣最大反流速度峰值,与心导管对比判断其准确性。结果 M型超声和二维超声心动图显示左心房(100%)、左心室(88.0%)内径不同程度增大,室间隔与左心室后壁运动幅度增大。所有患者CDFI均显示过动脉导管左向右五彩镶嵌分流束血流信号。PDA肺动脉端直径与心血管造影呈正相关(r=0.71,P<0.01);连续波多普勒估测肺动脉压力与心导管呈正相关(r=0.63,P<0.01)。合并二尖瓣关闭不全占88.0%、主动脉瓣关闭不全占80.0%、三尖瓣关闭不全占60.0%、肺动脉瓣关闭不全占28.0%。老年PDA患者合并肺动脉高压明显增多,占60.0%。女性(96.0%)明显多于男性(4.0%)。结论老年PDA患者的CDE特征明显,有特异性诊断价值。  相似文献

PURPOSE: To describe the radiographic appearance of the Gianturco and the Cook detachable coils and present the clinical results in patients who underwent transcatheter closure of patent ductus arteriosus. MATERIALS AND METHODS: Between January 1994 and June 1997, eighty-two patients underwent closure of patent ductus arteriosus (PDA) using either Gianturco or Cook detachable coils. The chest x-ray and echocardiography of all patients were reviewed and the following parameters were evaluated: 1) the size of the heart (cardiothoracic ratio), 2) the position and the type of the coils in the postero-anterior and the lateral projection, 3) the number of coils used, 4) the existence of residual ductal flow, 5) Doppler velocity in the left pulmonary artery. RESULTS: Complete occlusion was achieved in 94%, and cardio-thoracic ratio regressed from 0.57 to 0.53 (p < 0.01), after a mean follow-up of 1.2 years. The identification of the different coils on the chest radiograph was successful in only 47% of cases, difficulties arising especially, when multiple coils were used. In 55 patients (67%) the coil position was judged to be optimal, in 27 patients (33%) suboptimal. The latter correlates with the presence of residual shunt. Multiple coils correlated more with a left pulmonary artery flow velocity exceeding 1.5 m/s. CONCLUSION: Coil-occlusion of patent ductus arteriosus is effective and leads to reduced cardio-thoracic ratio. Radiographic coil identification is possible but may be difficult if multiple coils are deployed. Suboptimal coil position led more often to residual PDA shunt. Multiple coils are more commonly associated with increased LPA velocities, but hemodynamic significant obstruction to flow is rare.  相似文献
