Leiomyosarcoma of the renal vein is a rare tumor of complex diagnosis. We presented a case of renal vein leiomyosarcoma detected in a routine study. The primary treatment was complete surgical removal of the mass. In cases where surgical removal is not possible the prognosis is poor, with high rates of local recurrence and distant spread.  相似文献

Leiomyosarcomas arising from the renal vein are rare tumors: only 13 cases are found in the literature. We report a case in a sixty-one-year-old woman who presented with back pain and hematuria. She underwent resection of the tumor and an ipsilateral nephrectomy. The tumor recurred locally, and the patient died thirty months after diagnosis. Review of the cases reported in the literature reveals the aggressive nature of these tumors and the need for a combined therapeutic approach.  相似文献

Summary Primary malignant tumours of the great saphenous vein are extremely rare with only 15 cases reported in the English literature since 1919. We report a patient who survived 5.5 years after treatment with surgery and chemotherapy before dying from lung metastases.
Résumé Les tumeurs malignes de la veine saphène interne sont très rares. Depuis 1919, quinze cas seulement en ont été publiés dans la littérature de langue anglaise. Les auteurs en rapportent un cas qui, malgré des métastases pulmonaires traitées par chimiothérapie, a eu une survie de 5 ans et demi.

Leiomyosarcoma of the iliac vein is an uncommon tumor. We report a case of a 63-year-old Japanese woman with leiomyosarcoma of the right external iliac vein. The patient complained of right inguinal pain and swelling. Computed tomography demonstrated a mass surrounding the right external iliac artery and vein. Metastases in the lungs and liver were found. Complete resection of the tumor along with the involved vessels was performed. Polytetrafluoroethylene grafts were used to reconstruct the vessels. Pathological examination revealed leiomyosarcoma of the external iliac vein. Although the prognosis of leiomyosarcoma is poor, en bloc tumor resection is the treatment of choice.  相似文献

Five cases of leiomyosarcoma of blood vessels—three venous, two arterial—are reported. Treatment was surgical alone in two patients with involvement of the internal mammary artery and the long saphenous vein. The other three were treated initially by surgery, followed by radiation therapy, and surgery aimed at either total or partial removal of metastases. Two patients are alive and well, free of disease. Two patients have died of their disease despite all modalities of treatment, and one patient has a life-threatening metastases and is not expected to survive.  相似文献

Aneurysms of the portal venous system are increasingly reported in the past five years. Congenital weakness of the venous wall, trauma, pancreatitis and portal hypertension are possible etiologies. Surgical intervention is indicated in case of symptomatic aneurysms with or without progressive expansion of the aneurysm diameter. The treatment of asymptomatic splenic vein aneurysms remains debated. We report the case of an asymptomatic and uncomplicated splenic vein aneurysm for which a conservative approach was advocated with regular follow-up by means of Doppler ultrasonography. After six years of follow-up the aneurysm diameter has not changed and no complications were observed.  相似文献

Primary leiomyosarcoma of venous origin is a rare but frequently lethal disease. Clinical symptoms can be different, often not much evident or absolutely absent. Preoperative diagnosis is difficult also with modern imaging and only pathological examination can identify this malignant tumor. Successful therapy requires early surgery but the long-term survival is poor. A case of leiomyosarcoma arising from veins has been presented.  相似文献

Isolated splenic vein thrombosis may lead to a specific clinical presentation, namely, bleeding from isolated gastric varices (which are difficult to diagnose), splenomegaly, and normal liver function. The most common cause is pancreatic disease. To exclude associated portal venous thrombosis, angiography is essential prior to a splenectomy, which is curative of the variceal hemorrhage. Splenic vein thrombosis may also be noted as an incidental finding during angiography. Although the natural history is uncertain, a splenectomy is probably not indicated until a hemorrhage occurs.  相似文献

Isolated splenic vein thrombosis is being recognized more frequently as a complication of pancreatic disease and as the cause of gastrointestinal hemorrhage in patients without liver disease. The increased incidence reflects advances in diagnostic radiology and a higher index of suspicion for the diagnosis. Splenic vein thrombosis should be suspected in: (a) a patient with a history of pancreatitis and gastrointestinal blood loss; (b) a patient with splenomegaly in the absence of portal hypertension, cirrhosis, or hematologic disease; and (c) in the setting of isolated gastric varices. Celiac angiography has replaced splenoportography as the definitive diagnostic tool for splenic vein thrombosis and is indicated prior to operation for suspected portal hypertension or for complications of pancreatitis. The importance of making the diagnosis lies in differentiating the lesion from the more common case of hepatic and extrahepatic portal hypertension. The treatment of choice is splenectomy.
Resumen La trombosis aislada de la vena esplénica viene siendo reconocida con mayor frecuencia como consecuencia de enfermedad pancreática y como causa de hemorragia gastrointestinal en pacientes libres de enfermedad hepática. La aumentada incidencia es un reflejo de los avances en radiología y de un más alto índice de sospecha en el proceso diagnóstico.Se debe sospechar la presencia de trombosis de la vena esplénica en: (a) un paciente con historia de pancreatitis y de sangrado gastrointestinal; (b) un paciente con esplenomegalia en ausencia de hipertensión portal, cirrosis o enfermedad hematológica, y (c) en presencia de várices gástricas aisladas.La angiografía celiaca ha suplantado a la esplenoportografía como método definitivo de diagnóstico en casos de trombosis de la vena esplénica, y se halla indicado antes de la operación por sospecha de hipertensión portai y por complicaciones de la pancreatitis. La importancia de hacer el diagnóstico reside en diferenciar esta lesión de otras causas más comunes de hipertensión portal hepática y extrahepática. El tratamiento de elección es la esplenectomía.

Résumé La thrombose isolée de la veine splénique est reconnue avec une plus grande fréquence qu'elle complique une affection pancréatique ou qu'elle soit la cause d'une hémorragie digestive chez des malades dont le foie est normal. Ce fait reflète les progrès dans le diagnostic radiologique et dans la connaissance de sa relative fréquence.La thrombose de la veine splénique doit être envisagée: (a) chez un malade qui présente une histoire de pancréatite ou d'une hémorragie gastrointestinale; (b) chez un sujet porteur d'une splénomégalie en l'absence d'hypertension portale, de cirrhose ou de maladie hématologique; et (c) en présence de varices gastriques isolées.L'angiographie coeliaque a pris la place de la splénoportographie pour porter le diagnostic; elle est indiquée avant toute opération pour hypertension portale ou pancréatite compliquée. Cette exploration capitale permet de distinguer la thrombose de la veine splénique de causes plus courantes à l'origine de l'hypertension portale d'origine hépatique ou extrahépatique. Le traitement de choix est la splénectomie.
