苏州地区14994例儿童呼吸道感染细菌病原学特点   总被引:8,自引:1,他引:7  
目的 了解儿童呼吸道感染的细菌病原学特点。方法 收集2005 年11 月至2014 年10 月连续9 年因呼吸道感染入住苏州大学附属儿童医院的14 994 例儿童的病例资料进行回顾性分析。结果 14 994 份呼吸道感染患儿的痰标本中,细菌培养阳性3 947 份,总阳性检出率为26.32%。第1 位细菌病原为肺炎链球菌(12.79%),其次为流感嗜血杆菌(5.02%)、卡他莫拉菌(2.91%)。在不同年份、不同季节、不同年龄段儿童细菌检出率差异有统计学意义(P<0.01)。入院前未使用过抗菌药物的患儿痰细菌培养阳性率高于使用过抗菌药物的患儿(P<0.01)。院外病程<1 个月组、1~3 个月组及>3 个月组患儿痰培养细菌检出率差异有统计学意义(P<0.05)。肺炎链球菌、卡他莫拉菌、鲍曼不动杆菌的阳性检出率随病程的延长呈上升趋势(P<0.05)。结论 肺炎链球菌是引起儿童呼吸道感染的最常见细菌病原,其次为流感嗜血杆菌和卡他莫拉菌。各种细菌在不同的年份、不同的季节、不同的年龄阳性检出率有差异。疾病病程与院外抗菌药物使用对细菌的阳性检出率有影响。  相似文献

目的 探讨营养性缺铁性贫血(IDA) 的临床特点,分析影响贫血及其程度的危险因素,为预防和临床诊治提供参考依据。方法 回顾性分析372例IDA患儿临床资料,调查其发病及贫血程度的影响因素。结果 372例患儿中,男女比例为2.72:1;6月龄至2岁患儿占79.9%;早产儿占30.7%;出生体重< 2.5 kg者占22.9%,≥2.5 kg者占77.1%;自然分娩者占36.0%,剖宫产64.0%;未适时添加辅食者占79.3%;既往患过下呼吸道或反复上呼吸道感染、腹泻、有外伤、手术史者占46.2%。单因素分析显示年龄、出生体重、孕周、是否适时添加辅食、既往有上述病史与贫血严重程度有关。多因素分析显示,出生体重、既往有上述病史者是贫血严重程度的主要危险因素。结论 IDA高发于6月龄至2岁儿童;喂养不当仍是导致缺铁性贫血的主要原因之一;贫血严重程度主要与出生体重、既往有下呼吸道或反复上呼吸道感染、腹泻、外伤、手术史密切相关。  相似文献

目的 研究下呼吸道感染患儿病原体特点,为临床医生合理使用抗生素提供依据。方法 选取2017年1月至2018年6月因下呼吸道感染住院且接受纤维支气管镜治疗的108例患儿为研究对象,收集其支气管肺泡灌洗液,通过多重实时荧光PCR检测其病原体。结果 在108例患儿中,检测出病原体85例(78.7%),其中单一病原体感染检出52例(48.1%),多重病原体感染检出33例(30.6%)。肺炎支原体检出率最高,共检出38例(35.2%),其中36~ < 72月龄患儿检出率最高;其次为肺炎链球菌及流感嗜血杆菌,各检出29例(26.9%),其中肺炎链球菌主要集中于24月龄以下患儿。检出率较低的为鲍曼不动杆菌、白色念珠菌及肺炎克雷伯杆菌,各检出3例(2.8%)。在31例支气管肺炎患儿中,流感嗜血杆菌检出率最高(9例,29%)。在34例大叶性肺炎患儿中,肺炎支原体检出率最高(22例,65%)。在22例支气管异物合并支气管肺炎患儿中,肺炎链球菌检出率最高(10例,45%)。结论 在下呼吸道感染患儿中,肺炎支原体检出率最高,其次为肺炎链球菌及流感嗜血杆菌。不同年龄、不同类型下呼吸道感染患儿的病原体检出率存在差异。  相似文献

目的 了解复旦大学附属儿科医院4年中呼吸道感染患儿呼吸道标本中化脓性链球菌、肺炎链球菌、金黄色葡萄球菌、流感嗜血杆菌和卡他莫拉菌耐药率变化。方法 以2006至2009年在复旦大学附属儿科医院诊断为呼吸道感染的患儿为研究对象,采集咽拭子或痰标本行细菌培养。细菌鉴定采用NCCLS/CLSI 2006至2009年标准及英国BSAC标准。采用琼脂扩散法(KB)对5种常见细菌进行药物敏感实验,结果参照CLSI 2006至2009年标准,肺炎链球菌青霉素药物敏感实验采用E-test检测。结果 2006至2009年呼吸道标本中共分离到细菌3 169株,其中化脓性链球菌1 225株、肺炎链球菌341株、金黄色葡萄球菌297株、流感嗜血杆菌248株、卡他莫拉菌237株。①化脓性链球菌在上呼吸道细菌中构成比为74.5%~78.8%,流感嗜血杆菌和卡他莫拉菌在上下呼吸道细菌中的构成比有逐年上升趋势(P<0.05)。②2006至2009年,化脓性链球菌对红霉素耐药率为86.8%~94.7%,未发现青霉素耐药株;青霉素不敏感肺炎链球菌比例为1.4%~20.8%,4年间差异有统计学意义(P=0.007),肺炎链球菌对红霉素耐药率为91.1%~97.9%;金黄色葡萄球菌对红霉素耐药率为50.0%~54.9%,耐甲氧西林金黄色葡萄球菌比例为7.7%~11.4%;上述3种细菌对万古霉素均敏感。流感嗜血杆菌和卡他莫拉菌β-内酰胺酶阳性率分别为17.0%~31.3%和83.1%~100%,流感嗜血杆菌对头孢呋辛耐药率较低,卡他莫拉菌对环丙沙星和阿莫西林/克拉维酸高度敏感。结论 化脓性链球菌和肺炎链球菌对青霉素高度敏感。化脓性链球菌、肺炎链球菌及金黄色葡萄球菌对红霉素高度耐药。产酶的流感嗜血杆菌对第2代头孢菌素高度敏感,卡他莫拉菌对喹诺酮类和酶抑制剂复合制剂高度敏感。  相似文献

Growing antimicrobial resistance among Streptococcus pneumoniae, Haemophilus influenzae and Moraxella catarrhalis is raising major concern worldwide. Strains of S. pneumoniae, H. influenzae and M. catarrhalis isolated from children with respiratory tract as well as invasive infection in a South-Western region of Germany between 1993 and 2002 were tested for susceptibility to common antibiotics including penicillins, cephalosporins and macrolides. A total of 2,362 S. pneumoniae, 2,501 H. influenzae, and 1,982 M. catarrhalis isolates were tested. Only two S. pneumoniae strains were found to be highly resistant to penicillin. The overall rate of intermediate resistance to penicillin was 3.5%. There was a significant increase in erythromycin resistance from 5% in 1993 to 12.2% in 2002 ( P =0.001). No increase in ampicillin resistance was observed for H. influenzae over time. The rate of cefaclor resistance, however, increased from 4.5% to 11.8% ( P <0.0001). Furthermore, a massive increase in erythromycin resistance from 26% to 40% was observed ( P <0.0001). The vast majority of M. catarrhalis isolates were -lactamase positive, the minimal inhibitory concentration to ampicillin, however, exceeded only in 3% of all strains the cut-off of 1.5 mg/l. The erythromycin resistance rate of M. catarrhalis was 0.3%. Conclusion:There is still a low rate of penicillin/ampicillin resistance in S. pneumoniae, and H. influenzae, but an alarming increase in resistance to erythromycin, and in H. influenzae a significant increase in cefaclor resistance was observed over the 10-year period in South-Western Germany.  相似文献

This study aimed to establish the vaginal introitus microbial flora in girls with and without symptoms of vulvovaginitis, and to present the distribution of isolated microorganisms by age groups in girls with vulvovaginitis. We enrolled 500 girls with vulvovaginitis symptoms, aged 2-12 years, referred by their pediatricians for microbiological examination of the vaginal introitus swabs, and 30 age-matched asymptomatic girls. 目的 了解儿童呼吸道感染的病原菌种类、分布特点和耐药情况,为临床提供病原学诊断及合理使用抗菌药物的依据。方法 选取2016年1月至2018年12月因呼吸道感染就诊的15 047例患儿为研究对象,采集患儿痰标本送检,通过Phoenix-100全自动微生物鉴定药敏系统对分离的病原菌进行鉴定和药敏分析。结果 从17 174份送检的痰标本中检出阳性标本2 395份,阳性率为13.95%;共分离出2 584株病原菌,其中革兰阴性菌1 577株(61.03%),革兰阳性菌967株(37.42%),真菌40株(1.55%)。最常见的病原菌依次为流感嗜血杆菌(33.90%)、肺炎链球菌(33.55%)、卡他莫拉菌(19.20%)和金黄色葡萄球菌(3.64%)。2 331例感染阳性患儿中有251例混合感染,以流感嗜血杆菌合并肺炎链球菌感染发生率最高。病原菌检出率以冬春季较高,夏秋季较低(P < 0.05)。各年龄组患儿病原菌检出率比较差异有统计学意义(P < 0.05),其中1个月~ < 1岁组患儿病原菌检出率最高。肺炎链球菌和金黄色葡萄球菌对万古霉素、利奈唑胺和替考拉宁敏感率均为100%,流感嗜血杆菌除对氨苄西林、复方新诺明和头孢呋辛敏感率较低,对其他药物敏感率均较高。结论 儿童呼吸道感染病原菌以流感嗜血杆菌、肺炎链球菌和卡他莫拉菌为主,且常见混合型感染。不同季节、不同年龄段病原菌检出率不同。不同病原菌耐药性有不同特点,临床应依据药敏结果合理选用抗生素。  相似文献

Bronchiectasis in children without cystic fibrosis is most common in socioeconomically disadvantaged communities. Recurrent pneumonia in early childhood and defective pulmonary defences are important risk factors. These help establish a 'vicious cycle' of impaired mucociliary clearance, infection, airway inflammation and progressive lung injury. Haemophilus influenzae, Streptococcus pneumoniae, Moraxella catarrhalis and Pseudomonas aeruginosa are the main infecting pathogens. H. influenzae predominates across all ages, while P. aeruginosa is found in older children with advanced disease. It is uncertain whether viruses and upper airway commensal bacteria play an important aetiological role. Overall, the microbiological data are limited however and there are difficulties obtaining reliable respiratory specimens from young children. Bronchiectasis is a complex disorder resulting from susceptibility to pulmonary infection and poorly regulated respiratory innate and adaptive immunity. Airway inflammatory responses are excessive and persist, even once infection is cleared. Improved specimen collection, molecular techniques and biomarkers are needed to enhance management.  相似文献

Different microorganisms not commonly found in the female genital tract have been implicated as causative agents for amniotic fluid infections. Eikenella corrodens is part of the normal flora in the human oral cavity and upper respiratory tract. It is most commonly found in the clinical setting of polymicrobial infections involving human bite wounds, head and neck infections, and periodontitis. On rare occasions, E. corrodens has been reported as a cause of gynecologic infections. There are only 3 reports of documented E. corrodens chorioamnionitis of the placenta resulting in preterm labor, one of which is associated with early neonatal demise. We report a case of preterm delivery of twin infants at 23 weeks of gestation with documented E. corrodens infection in 1 twin who was not resuscitated.  相似文献

Background  Bacteremia is a major pediatric health care problem despite the availability of new modalities in the management of this disease. The aim of the present study was to determine the incidence and pattern of bacteremia in pediatric group at a tertiary hospital in Riyadh, Saudi Arabia. Methods  This retrospective study was conducted at the Department of Pediatrics, College of Medicine, King Khalid University Hospital, Riyadh in the period of January 2003 to January 2005. Positive culture was found in 259 patients aged below 15 years with a total of 8244 admissions in the period. Results  The highest incidence of bacteremia was found in patients aged less than 1 year (57.9%), and the majority of patients (30.5%) were infants aged less than 1 month. Staphylococcus aureus was the most common isolated pathogen (18.7%). Prematurity was associated with 13.2% of the cases, and respiratory tract infection (10.1%) and fever (76.1%) were chief complaints. Conclusions   Staphylococcus aureus is the most common isolated pathogen. The most common primary infections are respiratory tract infection and septic meningitis. Klebsiella pneumoniae and E. coli are the most common isolated Gram-negative organisms.  相似文献

Staphylococcus saprophyticus commonly causes urinary tract infection (UTI) in young women: in males it is found mainly in the elderly. In this studyS. saprophyticus UTI occurred in 59 children (45 girls and 14 boys) below 16 years of age, of whom 20 were less than 13 years of age. The common presentation was dysuria and flank or back pain whereas fever 38.5°C was rare. Radiological investigation performed in 63% of the children revealed no anomalies of importance. Children withS. saprophyticus UTI appear to constitute a group with a low frequency of urinary tract anomalies and with a low risk of UTI caused by other bacteria.  相似文献

To identify risk factors associated with hospitalization for acute lower respiratory tract illness, 102 children less than 2 years of age admitted to four Atlanta metropolitan area hospitals between December 1984 and June 1985 with the diagnosis of lower respiratory tract illness were studied. The most common causative agent associated with illness was respiratory syncytial virus, followed by other respiratory viruses, Haemophilus influenzae, and Streptococcus pneumoniae. The 102 case-patients were compared with 199 age- and sex-matched controls. A parent or guardian for each patient and control was interviewed by telephone regarding demographic data, care outside the home, breast-feeding, previous medical history, allergies, and smoking and illness in household members. Five factors were associated with lower respiratory tract illness in both a univariate analysis and a multiple logistic regression model (P less than .05). These factors were the number of people sleeping in the same room with the child, a lack of immunization the month before the patient was hospitalized, prematurity, a history of allergy, and regular attendance in a day-care center (more than six children in attendance). Care received outside of the home in a day-care home (less than or equal to six children in attendance) was not associated with lower respiratory tract illness. The suggestion made by our study and other studies was that for children less than 2 years of age, care outside of the home is an important risk factor for acquiring lower respiratory tract illness, as well as other infectious diseases, and that this risk can be reduced by using a day-care home instead of a day-care center.  相似文献

A case of necrotizing pneumonia secondary toFusobacterium necrophorum is reported. This anaerobic infection commonly originates in the upper respiratory tract and is often accompanied by multiple system disease due to hematogenous seeding. When the lungs are involved, diffuse necrotizing pneumonia with pleural effusions and cavitation results. The course is prolonged, and the diagnosis is frequently delayed. With appropriate antibiotics, the prognosis is good.  相似文献

Accepted 31 December 1996The aim of the study was to define the respiratory morbidity caused by lymphocytic interstitial pneumonitis (LIP) in children with vertically acquired HIV infection. A retrospective case note review was performed on 95 children attending three London hospitals. Clinical and radiological evidence of LIP, acute lower respiratory tract infections, and chronic lung disease was obtained using a structured protocol. A diagnosis of LIP had been made in 33%, and an acute admission due to acute lower respiratory tract infection had occurred in 42% of all children (despite 99% taking regular co-trimoxazole prophylaxis). Admission rates because of acute lower respiratory tract infection were significantly higher in the LIP group (0.38 admissions/child year) than in the non-LIP group (0.17 admissions/child year) (p = 0.0002). Encapsulated bacteria (Streptococcus pneumoniae, Haemophilus influenzae) were most frequently isolated. Improved methods of prevention of acute lower respiratory tract infection may help to reduce the severe respiratory morbidity seen in children with LIP and HIV infection.  相似文献

A regional surveillance study was carried out in children with recurrent acute otitis media (AOM) to determine the antimicrobial susceptibility of three common AOM pathogens. Susceptibility to relevant antimicrobial agents was determined on 149 Streptococcus pneumoniae, 246 Haemophilus influenzae and 119 Moraxella catarrhalis strains isolated between January 1999 and January 2002, either from the nasopharynx or middle ear of 74 children with recurrent AOM, the majority (77%) being otitis-prone. Overall pneumococcal resistance to penicillin was 9.4% (6.7% penicillin-intermediate resistant, 2.7% penicillin-resistant), whereas cotrimoxazole and erythromycin resistance accounted for 25.5% and 38.9% respectively. The prevalence of antimicrobial-non-susceptible S. pneumoniae was the highest in middle ear isolates (P<0.05) and in otitis-prone children (P<0.01). Moreover, otitis-prone children harboured significantly more pneumococci resistant to at least two antimicrobial agents (24.3% versus 7.4%; P<0.01). No patient age related variation was observed. Five serogoups (6, 19, 23, 14 and 9) covered by the 7-valent pneumococcal conjugate vaccine, constituted most of the antibiotic resistant pneumococci. Among nasopharyngeal and middle ear H. influenzae isolates, 17.1% were resistant to ampicillin and 16.3% to cotrimoxazole. For M. catarrhalis, 92.4% of all isolates was ampicillin-resistant. Conclusion: this study confirms international and national differences in antimicrobial susceptibility profiles of three acute otitis media pathogens with relatively favourable antibiotic resistance rates in Belgian children with frequent acute otitis media. This at risk population of otitis-prone children is shown to harbour more antimicrobial resistant and multidrug resistant pneumococci. OBJECTIVES: To assess risk factors for nasopharyngeal carriage of Streptococcus pneumoniae, Haemophilus influenzae and Moraxella catarrhalis in a large sample of healthy children. METHODS: In this point prevalence survey nasopharyngeal specimens were obtained from 1723 healthy children, ages 1 to 7 years, attending day-care centers or schools in 18 Italian cities. Written questionnaires for obtaining information about the demographics and medical history of the children were completed by the parents in the presence of a pediatrician. RESULTS: The overall carrier rate of respiratory pathogens was 17.9% (S. pneumoniae, 3.5%; H. influenzae, 11.9%; M. catarrhalis, 4.1%). Only 5% of S. pneumoniae strains were penicillin-resistant whereas approximately 40% were erythromycin-resistant. Beta-lactamase production was found in 5.8% of H. influenzae and 88.7% of M. catarrhalis isolates. By multivariate analysis age (< or = 3 years), having older siblings, a history of prolonged full-time day-care attendance and living in a rural area significantly influenced the odds of carrying nasopharyngeal respiratory pathogens, particularly in children ages 1 to 5 years. Sex, breastfeeding, passive smoking and recent upper respiratory tract infections were not significant variables. Antibiotic treatment in the previous 3 months did not affect nasopharyngeal carriage, whereas repeated treatments with a macrolide were associated with carriage of S. pneumoniae. CONCLUSIONS: Our results suggest that there is a strong and long term relationship between exposure to large numbers of children in the first years of life and nasopharyngeal carriage of all respiratory pathogens. In addition antimicrobial restrictive guidelines should be tailored to local microbiologic sceneries.  相似文献

Mycoplasma pneumoniae is an important pathogen which causes nervous system disorders during or after the course of a respiratory tract infection. The exact pathogenic mechanism which causes neurological disorders still remains unknown. Although meningoencephalitis and acute disseminated encephalomyelitis are common complications, there are few cases of acute transverse myelitis and isolated abducens nerve palsy associated with M. pneumoniae infection in childhood. The association between ocular myasthenia gravis and M. pneumoniae infection has not been described before. Here, we describe five patients with different nervous system complications associated with M. pneumoniae infection and discuss the pathological features of central nervous system involvement.  相似文献

Objective

Urinary tract is one of the most common sources of infection in children under the age of two years. Many known and unknown risk factors predispose to this important disease in children. This study was conducted to determine whether using a specific type of diaper plays a role in urinary tract infection (UTI) in girls under the age of 2 years.

Methods

This case control study was performed in hospitalized children; girls with their first urinary tract infection were selected as cases, and those admitted for other reasons comprised the control group. Two groups were matched for age (±1 month), and other known risk factors for UTI. Type of diapers (superabsorbent, standard disposable and washable cotton), used for these children during six months, from October 2007 to March 2008, were compared in both groups.

Findings

59 matched pair infant girls less than 2 years were selected. It was revealed that in cases with UTI superabsorbent diapers were used more frequently than in controls (Odds ratio=3.29, P-value=0.005) There were no significant differences in other factors like number of diapers used per day, the time between defecation and diaper change, mothers’ educational level, level of family income and mother''s occupation.

Conclusion

The use of superabsorbent diapers could be a risk factor for urinary tract infection in infant girls.  相似文献
