目的探讨HB-H-6树脂血浆灌流治疗黄疸犬动物模型的有效性和安全性.方法 HB-H-6树脂经过筛、酸、碱、乙醇、冷热水和高压灭菌处理,黄疸犬动物模型采用胆总管-腔静脉搭桥术,灌流方式采用血浆灌流.结果总胆红素下降82%,直接胆红素下降76%,间接胆红素下降97%,对蛋白质没有明显吸附,对血常规、凝血指标、血气分析和钠、氯、磷没有明显影响;对钾、钙和镁吸附明显.结论 HB-H-6树脂对胆素菜素吸附率高,对治疗黄疸犬动物模型安全有效,但对钾、钙和镁吸附明显,应注意补充. 相似文献

目的探讨血浆胆红素吸附(PBA)联合小剂量血浆置换(PE)治疗严重肝衰竭的临床疗效。方法选取2016年3月至2017年3月该院ICU收治的100例严重肝衰竭患者作为研究对象,按随机数字表法将其分为PE治疗组和联合组,每组各50例,检测两组总胆红素(TBIL)、直接胆红素(DBIL)、谷丙转氨酶(ALT)、凝血酶原时间(PT)、凝血酶原时间活动度(PTA)、清蛋白(ALB)、总胆固醇(TC)、C反应蛋白(CRP)和血氨(NH3)水平,观察治疗疗效和不良反应发生情况。结果联合组治疗后联合组ALT、AST、TBIL和NH3水平明显下降,PT明显缩短,ALB、PTA、TC明显上升,与治疗前比较差异有统计学意义(P0.05)。联合组治疗有效15例,有效率为30.0%;PE组中治疗有效6例,有效率为12.0%;联合组治疗有效率明显优于PE组(P0.05)。联合组治疗后出现1例低血压、1例低烧,不良反应总发生率为4.0%;PE组治疗后出现5例低血压、6例不同程度出血、8例发热、7例寒战,不良反应总发生率为52.0%;联合组不良反应总发生率明显低于PE组,差异有统计学意义(P0.05)。结论PBA联合小剂量PE治疗严重肝衰竭时血浆用量少且安全可靠,有效降低了不良反应发生率,为临床治疗提供了更多选择。 相似文献

目的探讨血浆置换与胆红素吸附治疗盯衰竭的临床疗效和安全性。方法将25例肝衰竭的患者随机分为两组,吸附组(13例)采用血浆胆红素吸附治疗,置换组(12例)采用血浆置换治疗,血浆置换量(2500±500)m1/次。比较两组治疗前后胆红素、血浆白蛋白、凝血因子X、凝血功能、肿瘤坏死因子α(tumornecrosis factor-α,TNF-α)、白细胞介素1(inter]eukin-1,IL-1)的浓度变化,临床症状改善情况,同时观察治疗后不良反应及存活率。结果吸附组及置换组治疗后,胆红素、TNF-α和IL-1β均明显降低(均P〈0.01),吸附组血浆白蛋白、凝血因子X轻度下降,置换组凝血因子X无降低。吸附组无一例发生出血、休克、过敏等严重并发症,置换组1例患者出现过敏性皮疹,1例发热,对症处理后均好转。结论血浆胆红素吸附能有效降低血清胆红素水平,降低炎症因子水平,改善肝衰竭患者预后,与血浆置换比较,血浆胆红素吸附无明显不良反应,不受血浆用量限制,无输血感染风险。 相似文献

目的比较几种吸附剂对肝衰竭患者血浆总胆红素、胆汁酸及血氨的吸附作用。方法收集4名肝衰竭患者血浆,将每位患者血浆分为6份,分别用以下6种吸附材料进行血浆灌注:丽珠HA330吸附树脂(HA330组);用人血白蛋白包被的活性炭(活性炭+HSA组);AB-8大孔吸附树脂(AB8组);用人血白蛋白包被经化学处理的AB-8大孔吸附树脂(化学处理的AB-8+HSA组);人血白蛋白包被乙基纤维素(EC+HSA组);用人血白蛋白包被经化学处理的乙基纤维素(化学处理的EC+HSA组)。观察各组吸附剂对总胆红血素、胆汁酸和血氨的吸附效果并进行比较。结果除EC+HSA组外其余各组对总胆红素、胆汁酸的吸附均有效(P〈0.05),其中化学处理的EC+HSA组对总胆红素的吸附率最高(10.82±1.24)%,活性炭+HSA组对胆汁酸吸附率最高(9.85±1.50)%;各组对血氨的吸附均有效(P〈0.05),化学处理的EC+HGA组的吸附率最高(33.83±2.51)%。结论化学处理的EC+HSA组对总胆红素、血氨、胆汁酸有较好的吸附效果。 相似文献

目的:探讨双重血浆分子吸附治疗(DPMAS)对高胆红素血症患者皮肤瘙痒状况的影响 。方法:选取我院 2016年4月至2020年3月进行DPMAS治疗患者20例共进行治疗81次,治疗过程密切监测病情变化,治疗前和停止治疗后采用皮肤瘙痒评分表评估患者皮肤瘙痒程度和匹兹堡睡眠指数量表评估患者睡眠质量。结果:DPMAS治疗前、停止治疗后总胆红素之间具有统计学意义(P<0.05),治疗停止后瘙痒程度评分较治疗前具有统计学意义(P<0.01),患者睡眠质量量表除催眠药物成份外,其他各成份均具有统计学意义(P<0.05或P<0.01);。结论:双重血浆分子吸附治疗不仅能够有效清除胆红素,减轻患者皮肤瘙痒程度,同时能够改善患者睡眠质量。 相似文献

目的:探讨个体化血浆流速对双重血浆分子吸附系统(DPMAS)治疗肝衰竭合并高胆红素血症病人的疗效。方法:采用回顾性研究,选取我院2021年1月—2022年2月18例肝衰竭病人为研究对象(共进行61例次DPMAS治疗),根据血浆流速分为两组,20~30 mL/min组14例,>30~50 mL/min组4例。观察两组不同时间点和时间段总胆红素清除率、治疗前后血压变化、总有效率及并发症发生率。结果:两组DPMAS治疗单位时间内总胆红素清除效率比较,差异有统计学意义(P<0.05);两组DPMAS治疗总时间段内总胆红素清除率比较,差异有统计学意义(P<0.05);DPMAS治疗血浆流速分组治疗后总有效率、并发症发生率比较差异无统计学意义(P>0.05)。结论:DPMAS治疗血浆流速推荐>30~50 mL/min, 1 h清除总胆红素效率最高,治疗时间建议2.5~3.0 h。 相似文献

目的观察不同血浆吸附容积在双重血浆分子吸附系统治疗高胆红素血症患者中的临床疗效。方法分析于上海中医药大学附属第七人民医院行双重血浆分子吸附系统治疗的40例高胆红素血症患者,根据单次治疗的血浆吸附容积分组,比较各组患者的血常规、凝血功能、C反应蛋白、胆红素清除率等指标。结果40例患者共行60次双重血浆分子吸附系统治疗,其中显效4例,有效50例,无效6例,总有效率为90%。单次治疗的血浆吸附容积C组(1.0 L≤血浆吸附容积<1.4 L)或E组(1.8 L≤血浆吸附容积<2.2 L)相较于B组(0.6 L≤血浆吸附容积<1.0 L)胆红素下降率更显著(t=-2.403,P=0.023;t=-2.405,P=0.032)。C组患者治疗后总胆红素差值高于B组(t=-2.068,P=0.048)。治疗后,患者凝血酶原时间和活化部分凝血活酶时间延长(t=-3.662,P=0.001;t=-2.828,P=0.008),国际标准化比值升高(t=-3.092,P=0.004),血浆纤维蛋白原降低(t=4.786,P<0.001),血小板计数减少(t=4.963,P<0.001),C反应蛋白下降(t=3.088,P=0.004)。结论双重血浆分子吸附系统单次治疗的血浆吸附容积达到1.0~1.4 L时胆红素的清除效果最具优势,延长吸附柱的治疗时间仍存在一定的吸附能力,可以增加胆红素的清除效率,推荐单次治疗血浆吸附容积为3.5~5.5倍个体血浆容积。 相似文献

背景:人工肝治疗重型肝炎要达到有效的治疗效果,必须具有吸附性能特异性强、吸附率高、血液相容性好等特性的吸附材料.目的:观察8种新型吸附剂对重型肝炎患者血浆中的胆红素和细胞因子的吸附性能.设计:对比观察.单位:天津医科大学研究生院,天津海河医院,天津市第三中心医院和南开大学高分子化学研究所.对象:全部血浆来自2004-11/2005-11天津市第三中心医院住院的重型肝炎患者30例.患者对治疗和实验均知情同意且该实验经医院伦理委员会批准.将上述患者随机分为2组:实验1组10例和实验2组20例.两组总胆红素水平分别为(377.3±147.5),(327.6± 140.1)μmol/L.方法:①吸附剂:实验选用的壳聚糖由山东青岛利中壳聚糖厂生产,相对分子质量97 000,脱乙酰度85%.1#~3#吸附剂为以质量浓度1%,3%,5%相对分子量为600的聚乙二醇为致孔剂制备的壳聚糖,4#为胺基化壳聚糖,5#为苯乙烯/二乙烯苯聚合物,6#为后交连苯乙烯/二乙烯苯聚合物,7#为壳聚糖-苯乙烯/二乙烯苯聚合物,8#为壳聚糖-后交连苯乙烯/二乙烯苯聚合物.②指标检测:第一步:收集实验1组重型肝炎患者的血浆各 3 mL,以8种不同的吸附剂各1 mL进行吸附.采用钒酸盐氧化法测定吸附前后血浆中总胆红素、直接胆红素、间接胆红素浓度,计算血浆吸附率,筛选出两种吸附率较高的吸附剂.第二步:用以上筛选出吸附率较好的两种吸附剂各1 mL对实验组患者血浆(各3 mL)进行胆红素和白细胞介素6、肿瘤坏死因子α的吸附实验,后两者的测定采用ELISA法.主要观察指标:吸附前后血浆中胆红素的浓度和细胞因子水平.结果:①第一步实验显示4#和5#吸附剂吸附后患者血浆总胆红素、直接胆红素、间接胆红素浓度明显降低,差异有显著性意义(P < 0.01);其余6种吸附剂吸附前后比较,差异无显著性意义(P > 0.05).②第二步实验显示,4#和5#吸附剂吸附后实验2组患者血浆总胆红素、直接胆红素、间接胆红素浓度和白细胞介素6 和肿瘤坏死因子α水平均明显下降,差异有显著性意义(P < 0.01).4#吸附剂吸附后前述5项指标低于5#吸附剂,差异有显著性意义(P < 0.01).结论:胺基化壳聚糖体外吸附重型肝炎患者血浆中胆红素、细胞因子效果有明显优势. 相似文献

背景:胆红素吸附柱在临床上用于清除患者体内高浓度的胆红素,在使用过程中与血液有大面积长时间的接触,对其血液相容性的评价具有重要意义。目的:评价一个树脂类胆红素吸附柱的血液相容性。方法:将待测胆红素吸附柱及对照已经上市胆红素吸附柱中的树脂微粒置于硅化玻璃管中,以空硅化玻璃管作为空白对照。向3组硅化玻璃管中加入人新鲜血浆,于37℃隔水培养箱的旋转培养器上以30r/min的速率旋转,3min后吸取出血浆,用于凝血酶原时间、活化部分凝血活酶时间、纤维蛋白原含量及总补体活性CH50的检测,实验重复3次。结果与结论:与空白对照组比较,对照组与实验组凝血酶原时间、活化部分凝血活酶时间延长,纤维蛋白原含量减少,CH50减小,但随着树脂微粒与新鲜血浆接触次数的增多,这种变化越来越小。与对照组比较,实验组凝血酶原时间、活化部分凝血活酶时间、纤维蛋白原含量的3次检测数值更接近于空白对照组;实验组CH50在3次检测中先大于对照组,再与对照组相当,最后小于对照组。表明:①随着与新鲜血浆接触次数的增多,树脂微粒的血液相容性得到改善。②待测胆红素吸附柱中树脂微粒对凝血系统的影响小于对照,对补体系统的影响在预吸附过程中小于对照,但其相容性的改善慢于对照。 相似文献

Group A and group C meningoccal polysaccharide vaccines were evaluated in infants. No significant local or systemic reactions were observed with 908 doses of vaccine given to 396 infants between 3 and 12 mo of age. The antibody response varied with the age of the infant, vaccine dose, molecular weight of vaccine, prior immunization with vaccine, and prior exposure to naturally occurring cross-reactive antigens. Only 7% of 3-mo-old infants had detectable antibody responses to primary immunization with 5-200 mug of A vaccine, presumably because of suppressive effects of high concentrations of maternal anti-A. More than 90% of 7- and 12-mo-old infants responded to A vaccine, achieving geometric mean anti-A concentrations of 0.38 and 0.98 mug/ml, respectively. The dose-response curve was flat between 10 and 200 mug of A vaccine. Geometric mean anti-A concentrations of 2.51 and 4.00 mug/ml were induced in 7- and 12-mo-old infants by booster injections of A vaccine. Approximately 90% of 3-mo-old infants had detectable antibody responses to primary immunization with C vaccine. The 100-mug dose appeared to be optimal, resulting in geometric mean anti-C concentrations of 0.49, 1.55, and 2.64 mug/ml in 3-, 7-, and 12-mo-old infants, respectively. Significant booster responses were not observed with C vaccine. Indeed, except for the 10-mug dose, booster injections of C vaccine in 7- and 12-mo-old infants resulted in lower anti-C concentrations than did primary immunizations. 相似文献

目的探讨不同胆红素浓度对血浆凝血酶原时间(PT)与活化部分凝血活酶时间(APTT)检测结果的影响,进而得出校正公式。方法对不同TBIL浓度的质控血浆进行血浆PT和APTT的测定,并用二次回归方程得出校正公式。结果当胆红素浓度低于20μmol/L时,对血浆PT和APTT检测结果的影响差异无统计学意义(P〉0.05);随着胆红素浓度的增高,PT和APTT的检测结果随之升高。结论胆红素浓度轻度增高的血浆对PT结果的影响不大,但胆红素浓度明显升高的血浆可能会对PT和APTT检测结果产生干扰。 相似文献

At present, phosphate removal and recovery from wastewater is gaining wide attention due to the dual issues of eutrophication, caused by the increased production of algae, and universal phosphorus scarcity. In this study, a layered zinc hydroxide (LZH) was synthesized by a simple precipitation method and characterized via various techniques. Experiments investigating the effect of contact time, pH, LZH dose, initial phosphate concentration, and co-existing ions on phosphate adsorption were conducted. LZH exhibited a high phosphate adsorption capacity (135.4 mg g−1) at a neutral pH. More than 50% of phosphate was removed within the first 60 s of contact time at an initial phosphate concentration of 5 mg L−1. Phosphate removal using the as-prepared LZH adsorbent was also tested in real treated sewage effluent reducing the residual phosphate amount to levels inhibiting to the growth of algae. Furthermore, phosphate desorption from LZH was investigated using acetic acid and sodium hydroxide regenerants which showed to be very effective for phosphate recovery.This study demonstrates a novel LZH adsorbent synthesized, characterized and applied for high phosphate removal and recovery from wastewater. 相似文献

In this study, porous electrospinning polyacrylonitrile nanofiber (PPAN) surface functionalization with amine groups is studied for methyl orange (MO) dye removal from aqueous solution. A series of adsorption experiments were carried out to investigate the influence of initial solution pH value, contact time, initial solution concentration, and adsorption temperature on the adsorption performance. The experimental results showed that the removal of MO on these PPAN-PEI and PPAN-TEPA nanofibrous mats was a pH-dependent process with the maximum adsorption capacity at the initial solution pH of 3, and that the PPAN-PEI and PPAN-TEPA nanofibrous mats could be regenerated successfully after 4 recycling processes. The adsorption equilibrium data were all fitted well to the Langmuir isotherm equation, with maximum adsorption capacity of 1414.52 mg g−1 and 1221.09 mg g−1 for PPAN-PEI and PPAN-TEPA, respectively. The kinetic study indicated that the adsorption of MO could be well fitted by the pseudo-second-order equation and Weber–Morris model. Thermodynamic parameters such as free energy, enthalpy, and entropy of adsorption of the MO were also evaluated, and the results showed that the adsorption was a spontaneous exothermic adsorption process.Amino functionalized porous polyacrylonitrile electrospun nanofibers were fabricated, which have good adsorption performance for MO in an acidic environment. 相似文献

In this work, starch-derived spherical carbon (HC) was prepared by hydrothermal carbonization and further activated with microwave assistance to obtain the target activated carbon (HMAC). The samples were characterized by methods of N2 adsorption–desorption, Brunauer–Emmett–Teller analysis, Fourier transform infrared spectroscopy, X-ray photoelectron spectroscopy, elemental analysis and scanning electron microscopy. Moreover, HMAC has a high BET surface area of 616.8 m2 g−1. The effects of initial naphthalene concentration, contact time, temperature, and pH of the naphthalene adsorbed on HC and HMAC were investigated systematically. The HMAC exhibits higher capability for naphthalene removal than HC, and the equilibrium adsorption quantity of HMAC was 223.03 mg g−1 at 303 K. The kinetic data revealed that the equilibrium time for naphthalene adsorption on samples was achieved at 40 min. The adsorption process of HC and HMAC for naphthalene both followed the pseudo-second-order kinetic and Freundlich isotherm models. Additionally, H-bond and π–π interactions were proposed to be involved in the adsorption process. An increasing adsorption amount of naphthalene onto HC and HMAC was observed when the pH value varied from 2 to 10. The HMAC can be successfully regenerated and maintained sorption performance after three cycles. This study revealed that HMAC obtained by hydrothermal synthesis combined with microwave-assisted activation has a promising application in the field of naphthalene removal.Hydrothermal synthesis and microwave-assisted activation of starch-derived carbon as a high performance adsorbent for naphthalene removal from aqueous solution. 相似文献

BackgroundClinically significant variation has been reported within and between plasma and whole blood total bilirubin methods used to identify neonates for whom clinical intervention for hyperbilirubinemia may be required.ObjectiveTo evaluate total bilirubin measurements between the Radiometer whole blood co-oximeter and plasma bilirubin methods from Roche Diagnostics and Ortho Clinical Diagnostics using neonatal specimens.MethodsTotal bilirubin levels were analyzed by whole blood co-oximetry (Radiometer® ABL90). Specimens were centrifuged and plasma analyzed for total bilirubin with a diazo method (Roche Cobas® C-601) and a reflectance spectrophotometric BuBc dry film method (Ortho Clinical Diagnostics VITROS® 350). Results were evaluated by regression, Bland-Altman comparisons and t-tests.ResultsThe patient correlation study yielded the following regression equations in μmol/L: a) Radiometer = 1.03 Roche – 3.5 μmol/L b) Radiometer = 0.98 Ortho – 5.7 μmol/L c) Roche = 0.97 Ortho – 2.4 μmol/L. The mean bias over the range of total bilirubin levels examined was − 1.0 μmol/L for the Radiometer versus the Roche (p ≤ 0.305); − 4.4 μmol/L for the Radiometer versus Ortho (p ≤ 0.005) and − 4.4 μmol/L for the Roche versus Ortho (p ≤ 0.002).ConclusionsWhole blood total bilirubin measurement using the Radiometer ABL90 blood gas analyzer provides accurate and precise results compared to the Roche plasma diazo method. Compared to the reflectance spectrophotometric method, results are precise and had a small but statistically significant bias of − 4.4 μmol/L. 相似文献
