The presence of lymph node metastasis is an important prognostic factor for patients with esophageal cancer. Accurate assessment of lymph nodes in thoracic esophageal carcinoma is essential for selecting appropriate treatment and forecasting disease progression. Positron emission tomography combined with computed tomography (PET/CT) is becoming an important tool in the workup of esophageal carcinoma. Here, we evaluated the effectiveness of the maximum standardized uptake value (SUVmax) in assessing lymph node metastasis in esophageal squamous cell carcinoma (ESCC) prior to surgery. Fifty-nine surgical patients with pathologically confirmed thoracic ESCC were retrospectively studied. These patients underwent radical esophagectomy with pathologic evaluation of lymph nodes. They all had 18F-FDG PET/CT scans in their preoperative staging procedures. None had a prior history of cancer. The pathologic status and PET/CT SUVmax of lymph nodes were collected to calculate the receiver operating characteristic (ROC) curve and to determine the best cutoff value of the PET/CT SUVmax to distinguish benign from malignant lymph nodes. Lymph node data from 27 others were used for the validation. A total of 323 lymph nodes including 39 metastatic lymph nodes were evaluated in the training cohort, and 117 lymph nodes including 32 metastatic lymph nodes were evaluated in the validation cohort. The cutoff point of the SUVmax for lymph nodes was 4.1, as calculated by ROC curve (sensitivity, 80%; specificity, 92%; accuracy, 90%). When this cutoff value was applied to the validation cohort, a sensitivity, a specificity, and an accuracy of 81%, 88%, and 86%, respectively, were obtained. These results suggest that the SUVmax of lymph nodes predicts malignancy. Indeed, when an SUVmax of 4.1 was used instead of 2.5, FDG-PET/ CT was more accurate in assessing nodal metastasis.  相似文献

Objective To investigate the prognostic factors and influence of the number of lymph node metastases on survival and UICC-TNM classification in patients with thoracic esophageal cancer after curative resection. Methods From 1985 to 1990, 1224 patients were surgically treated for thoracic esophageal cancer. The patients who died within 30 days after operation were not included in this study. Fifteen factors possibly influencing survival of these patients were selected and analyzed. A multivariate analysis of these individual variables was performed by Cox proportional hazard model. According to the n, mher of lymph node metastases (0, 1 and ≥ 2), a new modification of the TNM classification was suggested: stage Ⅱ a (T2N0M0 and T3N0M0), stage Ⅱb [T1N1M0 and T2N1 (1) M0], stage Ⅲ a [T2N1 (2)M0 and T3N1 (1)M0] and stage Ⅲ b [T3N1 (2)M0 and T4NanyMO]. Results According to multivariate analysis, lymph node metastases, depth of invasion, location of tumor, histological classification and length of the tumor were of prognostic significance (P < 0.01). There was obvious correlation between the rate of lymph node metastasis and the depth of invasion, length of tumor and grade of differentiation. The 5-year survival rate of the patients with 0, 1 and ≥2 positive metastatic lymph nodes was 59.1%, 32.0% and 8.9%, respectively. The 5-year survival rate of the patients with stage T2N1M0 and stage T3N1M0 was significantly higher in those with only one lymph node involved than in those with two or more lymph nodes involved (43.1% vs. 18.0% and 28.0% vs. 9.6%, P<0.01). The 5-year survival rate of the modified stage Ⅱa, Ⅱb, Ⅲa and Ⅲb was56.5%, 43.9%, 25.6% and 11.1%, respectively, with a statistically significant difference among different stages (P < 0. 01). Conclusion The lymph node metastasis is the most important prognostic factor for thoracic esophageal cancer after resection. The major influencing factors of lymph node metastasis are the depth of invasion, length of tumor and grade of differentiation. Therefore, the lymphadenectomy along with esophngectomy and subsequently combined modality therapy against lymph node metastasis is necessary to improve the S-year survival rate. Objective To evaluate the clinical value of double contrast-enhanced uhrasonography using oral and intravenous contrast agents in preoperative staging of gastric cancer.Methods Sixty-two patients with biopsy-proven gastric cancer were enrolled into this study,and were examined by double contrast-enhanced gastric uhrasonography preoperatively.The results were compared with postoperative pathologic findings.Results The accuracy of oral contrast-enhanced gastric uhrasonography and double contrast-enhanced ultrasonography in determining the T stage of gastric cancer was 72.9%(T1:66.7%,T2:60.0%,T3:76.9%,T4:71.4%)and 88.1%(T1:66.7%,T2:80.0%,T3:89.7%,T4:100%),respectively,with a statistically significant difference between the two methods(P =0.036).The sensitivity,specificity,accuracy and Youden index of oral contrast-enhanced gastric ultrasonography and double contrast-enhanced uhrasonography in assessment of lymph node metastasis were 74.5%,66.7%,72.9%,and 0.41 versus 89.4%,75.0%,86.4%,0.76,respectively.No significant difference in the accuracy of assessment for lymph node metastasis was observed(P > 0.06).Conclusion Double contrastenhanced ultrasonography is useful for preoperative staging of gastric cancer,especially for T staging.  相似文献

目的对比微探头超声内镜(MCUS)及CT在胸段食管癌术前进行T、N分期中的应用价值,探讨MCUS判断淋巴结转移的标准。方法对35例初治胸段食管癌患者分别进行MCUS扫描和CT扫描,并同时分别进行T、N分期;以术后病理为金标准,比较在对淋巴结转移的判断中,本研究试行的3种标准的准确率(即真实性)、灵敏度、特异度、阳性及阴性预测值等参数,判断其临床应用价值,并且,就MCUS扫描对淋巴结转移的判断标准进行探讨。结果本组35例中,MCUS的T分期准确率(85.7%)高于CT(45.7%),差异有统计学意义(P=0.000);MCUS的N分期中,两种标准的准确率(分别为85.7%及80.0%)均略高于CT(74.3%),但差异无统计学意义(P〉0,05)。应用淋巴结短径及短长轴比例(S/L)结合的方法,研究结果显示,检测的灵敏度有所提高。结论MCUS在胸段食管癌术前分期中有重要作用,其T分期准确率明显高于传统CT扫描。以淋巴结短径与S/L结合进行分析,有望提高判断淋巴结转移的准确性。  相似文献

背景与目的:食管癌术前临床分期对选择合理治疗方案至关重要,超声内镜(endoscopicultrasonography,EUS)对食管癌T、N分期诊断准确率高,目前已成为国外食管癌治疗前的常规诊断方法。本研究对食管癌超声内镜检查与手术病理结果进行对照研究,以评价其在食管癌术前临床分期中应用价值。方法:对72例食管癌患者行术前EUS检查及术后病理检查,按UICC(1997)TNM分期标准分期,将EUS分期结果和手术病理分期结果进行对照研究。结果:食管癌术前EUS检查T、N分期的诊断准确率分别为80.6%(58/72)、88.9%(64/72)。T0期诊断准确率为50.0%(2/4),T1期为75.0%(3/4),T2期为70.6%(12/17),T3期为90.0%(36/40),T4期为71.4%(5/7);N0期诊断准确率为92.3%(36/39),N1期为84.8%(28/33),其中纵隔淋巴结和腹部淋巴结诊断准确率分别为90.0%和64.5%。EUST分期、N分期结果与病理分期结果有显著相关性(r=0.687,P<0.001;r=0.776,P<0.001)。EUS检查判断分期不准确的22例中,T分期14例,其中8例难以鉴别T2、T3;N分期8例,其中5例为难以鉴别炎性淋巴结和转移淋巴结。结论:EUS对食管癌术前T、N分期诊断准确率较高,其中对T3期食管癌和纵隔淋巴结诊断价值尤其高;准确鉴别T2和T3分期、转移淋巴结和炎性淋巴结及正确诊断腹部淋巴结转移是提高EUS诊断率的重点和难点。  相似文献

目的:食管癌术前临床分期对选择合理治疗方案至关重要,微探头超声内镜(mini-probe endoscopic ultrasonography ,MCUS)对食管癌T、N 分期诊断准确率高,目前已成为国外食管癌治疗前的常规诊断和分期方法,本研究比较多层螺旋CT(multi-slice computer tomography,MSCT)与MCUS对食管癌患者术前评估的价值以及对手术方案预测的意义。方法:前瞻性纳入明确诊断为食管癌158 例患者。随机分为MSCT组(A 组)和MCUS组(B 组)各79例,术前均行X 线钡餐、消化道内镜活检外,再分别接受MSCT检查和MCUS检查,据此进行术前分期和手术方案的预测,并与术后病理分期和实际手术方案比较。结果:实际纳入152 例,A 组(75例)和B 组(77例)间基线特征差异无统计学显著性(P>0.05)。 术前T 分期准确度A 组60.00% ,B 组84.41% ,两组间差异有显著性统计学意义(P=0.001)。 术前N 分期准确度A 组77.33% ,B 组83.12% ,两组比较差异无统计学意义(P=0.370)。对手术方案预测的准确度A 组73.33% ,B 组89.61% ,两组间有统计学差异(P=0.010)。 结论:MCUS对食管癌术前分期、手术方案的预测准确度高于MSCT,从而为临床选择合理治疗方案具有一定的指导意义,建议有条件的医院对食管癌患者进行常规MCU检查。   相似文献

背景与目的:胃癌严重影响人类健康,术前准确T分期及淋巴结转移的预测有利于患者治疗方案的选择及预后。该研究旨在探讨3.0 T磁共振(magnetic resonance imaging, MRI)平扫结合弥散加权成像(diffusion-weighted imaging,DWI)对术前胃癌T分期及淋巴结转移的诊断价值。方法:该研究前瞻性开展了胃癌3.0 MRI平扫+DWI成像研究,其中35例胃癌患者被作为研究对象。这些患者均在术前进行T分期及淋巴结良恶性的评估,并与手术病理结果相对照。MRI结合DWI与手术病理T分期结果一致性分析采用Kappa检验。在MRI上分别测量淋巴结的的长径、短径、表观扩散系数(apparent diffusion coefficient, ADC)值、相对肌肉ADC (relative of muscle’s ADC,rADCm)值(rADCm =ADC淋巴结值/ADC左侧竖脊肌值)以及相对原发灶ADC(relative of primary tumor’s ADC,rADCp)值(rADCp =ADC淋巴结值/ADC胃癌原发灶值)等指标。采用独立样本t检验比较胃癌转移淋巴结与非转移淋巴结各项观测指标间有无统计学差异,利用ROC曲线下面积的大小评估各项指标对胃癌淋巴结转移的诊断效能。结果:35例患者MRI结合DWI术前 T分期准确率分别为:T1期75%,T2期100%;T3期76.47%;T4期75.00%。良恶性淋巴结的长径、短径、ADC值、rADCm值及rADCp之间差异均有统计学意义(P<0.05)。受试者工作特征(receiver operating characteristic, ROC)曲线显示rADCm值曲线下面积最大,诊断效能最高。淋巴结长径、短径、ADC值、rADCm值和rADCp值的最佳截断点分别为9.55 mm、6.05 mm、0.934×10-3mm2/s、0.60和1.083,灵敏度与特异度分别为59.00%和73.10%、59.00%和69.80%、82.60%和88.50%、83.70%和84.60%、78.20%和80.80%。结论:MRI结合DWI对胃癌术前T分期及淋巴结良恶性的评估具有重要指导意义。  相似文献

目的:通过多层螺旋CT薄层多平面重建图像所见与病理结果对照,探讨转移淋巴结的诊断标准。方法以病理诊断为金标准,回顾性分析208例胸段食管癌患者淋巴结的术前多层螺旋CT多平面重建图像,测量淋巴结的长径、短径,计算出直径比(短径/长径,将淋巴结直径比≥0.7视为圆形淋巴结),同时观察淋巴结有无环形强化、成簇分布、边缘模糊等影像学特征,分析比较各项指标的诊断价值。结果以短径≥1.0 cm作为转移淋巴结的诊断标准,敏感度为23.6%,特异度为98.6%,约登指数为0.22。以短径≥0.8 cm作为转移淋巴结诊断标准,对于短径<0.8 cm的淋巴结运用直径比≥0.7作为诊断转移的标准,诊断效能有了明显的提高,敏感度为83.8%,特异度为82.9%,约登指数为0.67。以淋巴结有“环形强化”或“边缘模糊”征象作为诊断标准的阳性预测值分别为62.2%、89.4%。结论用较小的短径作为转移淋巴结的诊断标准,能提高诊断效能;直径比有助于提高CT术前诊断转移淋巴结的价值;边缘模糊、环形强化可作为诊断转移淋巴结的参考标准。  相似文献

PURPOSE: To evaluate the role of endoscopic ultrasonography (EUS) in the initial staging and follow-up of esophageal cancer on the basis of a review of the published literature. METHODS: Articles published from 1985 to 2005 were searched and reviewed using the following keywords: "esophageal cancer staging," "endoscopic ultrasound," and "endoscopic ultrasonography." RESULTS: For initial anatomic staging, EUS results have consistently shown more than 80% accuracy compared with surgical pathology for depth of tumor invasion (T). Accuracy increased with higher stage, and was >90% for T3 cancer. EUS results have shown accuracy in the range of 75% for initial staging of regional lymph nodes (N). EUS has been invariably more accurate than computed tomography for T and N staging. EUS is limited for staging distant metastases (M), and therefore EUS is usually performed after a body imaging modality such as computed tomography or positron emission tomography. Pathologic staging can be achieved at EUS using fine-needle aspiration (FNA) to obtain cytology from suspect Ns. FNA has had greatest efficacy in confirming celiac axis lymph node metastases with more than 90% accuracy. EUS is inaccurate for staging after radiation and chemotherapy because of inability to distinguish inflammation and fibrosis from residual cancer, but a more than 50% decrease in tumor cross-sectional area or diameter has been found to correlate with treatment response. CONCLUSION: EUS has a central role in the initial anatomic staging of esophageal cancer because of its high accuracy in determining the extent of locoregional disease. EUS is inaccurate for staging after radiation therapy and chemotherapy, but can be useful in assessing treatment response.  相似文献

The purpose of the present study was to assess the contribution of simultaneous functional/anatomical imaging using integrated 18F-fluorodeoxyglucose-positron emission tomography/computed tomography (FDG-PET/CT), compared with PET alone for the evaluation of initial lymph node staging in esophageal cancer. We studied 167 consecutive patients with thoracic esophageal squamous cell carcinoma (SCC) who had radical esophagectomy performed between January 1999 and April 2007. For individual nodal group evaluation, PET/CT showed 46.0% sensitivity (p<0.05 vs. PET), 99.4% specificity, 95.1% accuracy (p<0.05 vs. PET), 87.0% positive and 95.5% negative predictive values. PET showed 32.9% sensitivity, 98.9% specificity, 93.1% accuracy, 74.7% positive predictive value and 93.9% negative predictive value. Thus, the sensitivity and accuracy of PET/CT were significantly higher than those of PET. Comparisons between CT, PET and PET/CT in detecting lymph node metastasis by each region showed that PET/CT had a higher sensitivity in lower thoracic regions than PET and CT (p<0.05 vs. CT and PET). Lymph node staging (N0 vs. N1) was not significantly different, but staging per lymph nodal group was significantly better with PET/CT. Integrated PET/CT imaging with co-registration of anatomic and functional imaging data is useful in the initial lymph node staging of patients with operable esophageal cancer compared with PET alone.  相似文献

背景与目的:直肠癌术前分期对选择合理治疗方案和判断预后至关重要。传统的计算机断层扫描(computed tomography,CT)对直肠癌术前分期存在争议,本研究旨在探讨多层螺旋CT(multislice spiral CT,MSCT)对直肠癌术前分期的诊断价值。方法:中山大学肿瘤防治中心2006年3月至2007年2月,经病理证实的直肠癌患者87例,所有患者术前行MSCT平扫及增强扫描,由两位放射科医生独立评价肿瘤的部位、大小、侵犯范围(T)、淋巴结转移(N)及远处转移(M)情况,做出诊断及TNM分期,并与术后病理对照,评价准确性、灵敏度及特异度。结果:MSCT检出了全部87例直肠癌,对直肠癌TNM分期总的准确性为81.6%(71/87)。T、N、M期准确性分别为94.3%(82/87)、82.8%(72/87)、98.9%(86/87)。≤T2、T3、T4期灵敏度分别为90.5%、91.3%、97.7%,特异度分别为98.5%、94.2%、97.7%。N0、N1、N2期灵敏度分别为92.9%、72.0%、82.4%,特异度分别为88.9%、88.5%、91.7%。远处转移的患者仅1例因肝脏转移灶<5mm而漏诊。结论:MSCT能较准确地判断直肠癌的侵犯范围、淋巴结转移及远处转移,是非常有价值的术前分期方法。  相似文献

目的 探讨纵隔镜技术评估非小细胞肺癌(NSCLC)术前纵隔淋巴结状态(是否存在转移)的临床应用策略.方法 2000年10月至2007年6月,对临床连续收治的经病理确诊的临床分期为Ⅰ~Ⅲ期的NSCLC患者152例,分别采用CT和纵隔镜技术评估纵隔淋巴结状态.根据纵隔淋巴结最终病理结果,计算CT下纵隔肺门淋巴结阴性NSCLC的纵隔镜检查阳性率和实际纵隔淋巴结转移发生率.以患者性别、年龄、肿瘤部位、病理类型、肿瘤T分期、肿瘤类型(中央型或外周型)、CT下纵隔淋巴结大小和血清癌胚抗原(CEA)水平等作为预测因子,进行纵隔淋巴结转移危险因素的单因素和多因素分析.结果 69例CT下纵隔肺门淋巴结阴性NSCLC,纵隔镜检查阳性8例,阳性率为11.6%;实际纵隔淋巴结转移14例,发生率为20.1%.62例临床Ⅰ期(cT1~2NOMO)NSCLC,纵隔镜检查阳性7例,阳性率为11.3%;实际纵隔淋巴结转移12例,发生率为19.4%.对全部152例NSCLC患者纵隔淋巴结转移危险因素的分析结果显示,病理类型和CT下纵隔淋巴结大小是纵隔淋巴结转移的独立危险因素.对69例CT下纵隔肺门淋巴结阴性NSCLC患者纵隔淋巴结转移危险因素的分析结果显示,病理类型是纵隔淋巴结转移的独立危险因素.结论 对于CT下纵隔淋巴结短径≥1 cm的NSCLC患者,术前必须进行纵隔镜检查;对于腺癌患者,即使是CT下纵隔肺门淋巴结短径<1 cm,术前也应该进行纵隔镜检查.  相似文献
