A series of 198 total hip arthroplasties was performed using a porous-coated, hemispheric press-fit cup. One hundred and twenty-seven cups were available for clinical and radiological examination at mean follow-up of 10.6 years. The mean age at the index procedure was 61.2 years. The mean Harris hip score at final follow-up was 89.8. Three cups were revised for aseptic loosening and two liners were changed for eccentric wear and pelvic osteolysis. Nine additional patients showed mild or suspected osteolysis. Two cups were rated "fibrous" stable. There was no correlation between additional screw fixation of the press-fit cup and osteolysis or revision.  相似文献

We reviewed 123 second-generation uncemented total hip replacements performed on 115 patients by a single surgeon between 1993 and 1994. The acetabular component used in all cases was a fully porous-coated threaded hemispheric titanium shell (T-Tap ST) with a calcium ion stearate-free, isostatically compression-moulded polyethylene liner. The titanium femoral component used was a Taperloc with a reduced distal stem. No patient was lost to follow-up. Complete clinical and radiological follow-up was obtained for all 123 hips at a mean of 14 years (12 to 16). One femoral component was revised after a fracture, and three acetabular components for aseptic loosening. No additional femoral or acetabular components were judged loose by radiological criteria. Mild proximal femoral osteolysis was identified in two hips and minor acetabular osteolysis was present in four. The mean rate of penetration of the femoral head was 0.036 mm/year (0.000 to 0.227). These findings suggest that refinements in component design may be associated with excellent long-term fixation in cementless primary total hip replacement.  相似文献

This article presents a prospective clinical and radiologic review of 55 total hip replacements in 50 patients in which the cementless Harris-Galante porous-coated acetabular component was used. All patients were 50 years of age or younger. The cups were inserted by 15 different surgeons. The minimum followup was 5 years, with a mean of 7 years. No acetabular component has been revised for loosening, and none have migrated. Three (5.5%) cups have progressive bone-component radiolucent lines, two (3.6%) of which are complete. Six (10.9%) acetabular liners have been replaced for excessive polyethylene wear, and an additional four (7.3%) show sufficient wear to produce obvious radiographic eccentricity of the femoral head. All of the revised liners had 32-mm femoral heads and an initial polyethylene thickness of less than 6 mm. Although the results reflect excellent fixation of this component in the medium term, the incidence of exchange of the polyethylene liner for excessive wear is unacceptably high. The reduction in polyethylene thickness associated with the use of a metal-backed component and a 32-mm articulation was one of the factors contributing to the accelerated wear patterns seen in this series.  相似文献

A retrospective clinical and radiographic analysis was performed on 99 patients (106 hips) undergoing total hip arthroplasty with noncemented Harris-Galante (Zimmer, Warsaw, IN) or Optifix (Smith Nephew Richards, Memphis, TN) acetabular components. There were 94 primary and 12 revision procedures with a minimum follow-up period of 24 months (range, 24–52 months). The cups were evaluated for evidence of vertical and horizontal migration, as well as the presence of radiolucencies at the bone-implant interface. Age, sex, component inclination and medialization, cup coverage, and number of fixation screws used were examined to determine the influence of these factors on the incidence of cup migration or radiolucent line formation. A radiolucent line was present in at least one zone in 60% of the Harris-Galante and 45% of the Optifix cups. Progressive radiolucent lines were noted in two of the Optifix and three of the Harris-Galante components. Two Harris-Galante cups (1.9%) were revised. There were no Optifix cup migrations or evidence of instability in the Optifix or remaining Harris-Galante cups. Although there was a trend toward an increased incidence of radiolucencies in those cups lacking complete coverage, no statistically significant radiographic predictors for failure or impending failure could be determined from this short-term follow-up study. With the exception of the two revisions, all acetabular components performed well clinically over the study period.  相似文献

This study reviewed the results of a cementless anatomical femoral component to give immediate post-operative stability, and with a narrow distal section in order not to contact the femoral cortex in the diaphysis, ensuring exclusively metaphyseal loading. A total of 471 patients (601 hips) who had a total hip replacement between March 1995 and February 2002 were included in the study. There were 297 men and 174 women. The mean age at the time of operation was 52.7 years (28 to 63). Clinical and radiological evaluation were performed at each follow-up. Bone densitometry was carried out on all patients two weeks after operation and at the final follow-up examination. The mean follow-up was 8.8 years (5 to 12). The mean pre-operative Harris hip score was 41 points (16 to 54), which improved to a mean of 96 (68 to 100) at the final follow-up. No patient complained of thigh pain at any stage. No acetabular or femoral osteolysis was observed and no hip required revision for aseptic loosening of either component. Deep infection occurred in two hips (0.3%) which required revision. One hip (0.2%) required revision of the acetabular component for recurrent dislocation. Bone mineral densitometry revealed a minimal bone loss in the proximal femur. This cementless anatomical femoral component with metaphyseal loading but without distal fixation produced satisfactory fixation and encourages proximal femoral loading.  相似文献

Cementless acetabular reconstruction in revision total hip arthroplasty   总被引:6,自引:0,他引:6  
Reconstruction of the failed acetabular component in total hip arthroplasty (THA) can be challenging. Although there are multiple reconstructive options available, a cementless acetabular component inserted with screws has been shown to have good intermediate-term results and is the reconstructive method of choice for the majority of acetabular revisions. We reviewed the results of 138 consecutive acetabular revisions done with a hemispheric, cementless acetabular component that is sintered with Ti fiber metal mesh and was inserted with line-to-line reaming and multiple screws for fixation. At a mean of 15 years, 19 acetabular components had been revised (13.8%); seven were revised for recurrent instability, six were revised for deep periprosthetic infection, five well-fixed cups were removed at the time of femoral revision, and one cup was fixed by fibrous tissue only at the time of femoral revision and was removed. One acetabular component was radiographically loose. Survivorship of the component was 81% at 15 years when revision for any reason was considered as an end point and 96% when revision for loosening or radiographic evidence of loosening was considered as a second end point. Cementless acetabular revision provides durable results at 15 years with a low rate of failure for loosening.  相似文献

The mechanism for the improved wear resistance of cross-linked ultra-high-molecular-weight polyethylene (UHMWPE) remains unclear. This study investigated the effect of cross-linking achieved by gamma irradiation in nitrogen on the tribologic, mechanical, and morphologic properties of UHMWPE. The goal of this study was to relate UHMWPE properties to the wear mechanism in acetabular-bearing inserts. Wear simulation of acetabular liners was followed by detailed characterization of the mechanical behavior and crystalline morphology at the articulating surface. The wear rate was determined to be directly related to the ductility, toughness, and strain-hardening behavior of the UHMWPE. The concept of a plasticity-induced damage layer is introduced to explain the near-surface orientation of the crystalline lamellae observed in the wear-tested acetabular liners. Cross-linking reduces abrasive wear of acetabular components by substantially reducing--but not eliminating--the plasticity-induced damage layer that precedes abrasive wear.  相似文献

目的分析髋臼骨折术后全髋关节置换术的早期临床效果。方法2002年1月至2007年12月,共对18例髋臼骨折后创伤性髋关节关节炎的患者进行了全髋关节置换术,其中13例髋臼骨折做过切开复位内固定术,另5例行闭合复位骨牵引治疗;10例因为既往手术的疤痕粘连,关节置换时需行较广泛的软组织松解;9例在全髋关节置换术时仍有固定用的内植入物,5例术中需要取出内固定;12例术中用取下的股骨头行结构性或颗粒植骨;16例用非骨水泥髋臼,2例用金属加强环与骨水泥固定聚乙烯髋臼。术后随访行x线摄片评估,髋关节功能作Harris评分。结果随访时间6—70个月不等,平均38个月。3例术后发生髋关节周围BrookerⅡ度异位骨化,其中2例行异位骨化切除;1例术中股骨骨折,1例深部感染,1例髋臼松动并移位。最后随访时,12例(67%)患髋没有或仅偶有轻微疼痛,4例中度疼痛,2例疼痛需要助行工具;Harris评分由术前平均43.5分到术后平均84分。结论髋臼骨折后髋关节骨关节炎仍可通过全髋关节置换手术得到有效治疗,但其手术复杂程度与款臼骨折的处理有关,且并发症的发生率比一般的全髋关节置换手术高。  相似文献

343 primary arthroplasties using the AML stem were performed between 1978 and June, 1984 with a mean follow-up of 4.8 years. All had a two-year reading of stem fixation as: 1) bone ingrown or optimum (81%), 2) fibrous encapsulated or suboptimum stable (16%), or 3) unstable (3%). Using a roentgenographic end-point, we performed a 0-8 year survivor analysis of stem fixation. The cumulative survivorship rate (CSR) was 85% at eight years. Among 142 undersized stems, thirteen failed by migration after one year and two by stem fracture, giving a CSR of 71% at eight years. By contrast, no failure was recorded in 201 canal filling stems. Proximal stress shielding occurred only in bone ingrown stems and was slightly progressive. Widening of the intramedullary canal occurred with unstable stems. The clinical results were superior in patients with bone ingrown stems and functional recovery in those cases was equivalent to that of cemented arthroplasty.  相似文献

Metal-on-metal (MoM) total hip arthroplasty (THA) has been introduced in an attempt to reduce the wear rate and the consequent osteolysis around implants. The aim of this study was to present the intermediate to long-term clinical and radiological outcomes and to investigate the metal ion levels in the blood of patients who had undergone primary uncemented MoM THA in our institution. Between July 1997 and November 2003, 166 patients (193 hips), with a mean age of 50 years (range, 18-65 years), underwent primary MoM THA. Clinical data, radiographs, and blood samples were obtained at regular follow-up visits. Cobalt (Co), chromium (Cr), and molybdenum (Mo) ions were measured by inductively coupled plasma-mass spectrometry (ICP-MS) from the patient's whole blood. All patients were prospectively followed for a minimum of 5 years (mean, 7 years; range, 5-11 years). The mean Harris hip score (HHS) and the University of California at Los Angeles (UCLA) activity score at the latest follow-up was 88 ± 11 and 7 ± 1.8 points, respectively. Thirteen hips have been revised. Ten acetabular components had early failure, due to factory manufacturing problems. All other implants have been found stable, with no signs of aseptic loosening. The probability of survival at 11 years, if the hips that were revised due to manufacturing problems were excluded, was 98.4%. The Co and Cr metal ion levels, after increasing significantly during the first 4 to 5 years post-surgery, remained stable, with a tendency to decrease thereafter, but not significantly. During the same follow-up period, Mo ion levels remained stable. In this 5-to-11 year follow-up study of MoM THA patients, excellent survivorship, with low complications rates, was found. Results of longer follow-up studies are necessary to clarify the possible long-term effects of metal ion release.  相似文献

Summary We conducted extensive radiographic and clinical examination of 58 cementless total hip arthroplasties. Forty BIAS femoral stems had heads made of Co-base F 75 alloy and 18 Gustilo-Kyle femoral stems had Ti6A14V alloy heads. The cross-sectional geometry and location of the porous coating pads of the two stem types was identical, the BIAS component being slightly shorter. At the last follow-up, 27.8% of the Gustilo-Kyle femoral components and 2.5% of the BIAS femoral components were unstable. None of the acetabular components in the BIAS group as against 38.9% of the acetabular components in the Gustilo-Kyle group demonstrated wear of more than I mm. All unstable components which did not suffer intraoperative fracture had also acetabular wear of more than 1 mm. The only case with endosteal erosion was seen in a patient with an unstable implant displaying the most extensive wear. Nonparametric statistical analysis showed that the patients with Ti6A14V/polyethylene bearing surfaces had a statistically significantly higher extent of wear and a statistically significant higher incidence of femoral component loosening than patients with CrCoMo/polyethylene bearing surfaces.  相似文献

Seventy-one total hip arthroplasties with a cementless, wedge fit, cobalt chrome femoral component were reviewed in 60 patients at a minimum 10-year followup (mean, 11.5 years). For the femoral component, the mechanical failure rate was 5% and the revision rate for aseptic loosening was 0%. The mean Charnley scores for pain, function, and motion changed from preoperative mean values of 3.0, 2.7, and 3.2 to followup mean values of 5.7, 5.5, and 5.2, respectively. The followup mean Harris hip Score was 91. The incidence of thigh pain was 1.4% at 10-year followup. Ninety-five percent of femoral components showed radiologic evidence of stable, bone ingrowth fixation, whereas loosening was seen in 5% of stems. Despite the high incidence of acetabular osteolysis, no osteolysis was seen on the femoral side distal to the lesser trochanter. Nonmodularity of the femoral component led to unavoidable revision of stably fixed femoral components in seven (9.8%) hips during the revision of a loose socket. Design features (collarless, tapered, wedge fit, and circumferentially porous coated) were thought to be crucial to the superlative results with the cobalt chrome femoral component.  相似文献

Introduction

The outcomes of total hip arthroplasty (THA) for the treatment of posttraumatic arthritis after acetabular fractures were inferior to those after primary non-traumatic THA.

Methods

This study was performed in academic level I trauma center. From January 2011 to December 2014, a consecutive series of 21 patients (9 females), with average age of 56.7 years (range 29–75 years) who had posttraumatic hip joint arthritis after acetabular fractures, were included in our study. All patients underwent cementless THA. The average duration of follow-up was 26 months (range 24–36 months).

Results

At the latest follow-up, all patients could walk independently, thirteen (62%) patients had excellent Harris hip score, five (24%) had good HHS, and 3 (14%) had fair score. WOMAC scale decreased from 63 (range 42–92) to 4 (range 0–19). Two patients (9.5%) had heterotopic bone formation which did not affect the activity of the patients. There were no signs of loosening of the acetabular cups or around the femoral stem.

Conclusion

Cementless THA is an ideal treatment for posttraumatic hip arthritis with anatomic restoration of the hip center to improve the functional results and decrease the incidence of complications and revision rate.

We reviewed the cases of thirty-six patients who had forty cemented total hip replacements with the Harris metal-backed acetabular component. The operations were done between 1972 and 1977, and the duration of follow-up averaged 7.6 years (range, five to 10.8 years). The average age of the patients was forty-four years (range, sixteen to sixty-two years). Aseptic loosening of the acetabular component occurred in three hips (7.5 per cent), and three more sockets were revised for other reasons. Two of the three sockets with aseptic loosening were in the fifteen patients (seventeen hips) who were forty-five years old or younger. The remaining loose cup was in one of the twenty-one patients (twenty-three hips) who were forty-six years old or older. The reduction in the rate of aseptic loosening of the socket in our series, compared with the higher rates reported in similar long-term studies in which other acetabular components were used, supports the conclusion that there is enhanced longevity of acetabular fixation when a metal-backed acetabular component is used in cemented total hip arthroplasty.  相似文献

We report the results of 114 AGC 2000 porous-coated, cementless total knee arthroplasties (TKA) performed consecutively in 102 patients during the period 1984-1986. After 10 years, 58 TKAs in 52 patients were evaluated with patient assessment, Hospital for Special Surgery knee score, weight-bearing radiographs done under fluoroscopic control, and survivorship analysis. All dropouts within the first 9 years were patients dying with a functioning TKA except 1 revision secondary to a supracondylar fracture after 8.5 years. Of the patients, 53 (92%) were satisfied or very satisfied with their TKA, and 55 (95%) of the knees were rated good or excellent. There was no pain in 53 knees, and the median knee flexion was 110 degrees. Six radiolucencies >1 mm were found beneath parts of the tibial component, and 5 radiolucencies were seen beneath the femoral component. None had progressed compared with the 5-year follow-up, and in all cases trabeculae could be seen reaching the prosthetic component. No migrations had occurred since the 5-year follow-up. No obvious joint space reduction was seen. Osteolysis presenting as an isolated cyst was found in 1 knee in the lateral tibial condyle and was not progressive. Two tibial components had been revised because of aseptic loosening and 1 because of septic loosening, all within the first 3 years. No femoral or patellar components were revised. The cumulative prosthesis survival rate after 10 to 11 years was 97%. When pain and radiographic loosening also were considered, the success rate was 87%. Cementless insertion of a nonmodular, porous-coated TKA resulted in a long-term durable bone-prosthesis interface. The flat-on-flat articulation did not result in catastrophic polyethylene wear or osteolysis within the first 10 years.  相似文献

BACKGROUND: This study was undertaken to provide a basis for decision-making when a well-fixed cemented cup is encountered at the time of a revision of a femoral component of a total hip replacement. It may be beneficial to retain the cup in some instances and thus reduce morbidity, complications, blood loss, operative time, and cost. METHODS: All patients who had a revision of the femoral component and retention of an all-polyethylene acetabular component from 1971 to 1996 were identified. Three hundred and seventy-four patients with a total of 395 cemented total hip replacements fit the inclusion criteria. The gender, date of and age at the index surgery, type of acetabular implant, and time of and reason for the revision of the femoral component were reviewed. The time to subsequent cup revision or latest surveillance was noted. The average age (and standard deviation) was 66.5 +/- 11.8 years at the time of the femoral revision, which was performed at an average of 8.0 +/- 4.4 years after the primary arthroplasty. For the purposes of this study, the end point for survival of the acetabular component was cup revision for any reason. RESULTS: At the time of the latest follow-up, at an average of nine years after the femoral revision and 17.3 years after the primary arthroplasty, 342 (86.6%) of the 395 cups remained in situ. Fifty-three cups (13.4%) in fifty-two patients had been revised, at an average of 10.0 +/- 5.7 years after the femoral revision and 16.7 +/- 5.3 years after the primary arthroplasty. The rate of survival of the retained acetabular components was 96.9% at five years, 89.3% at ten years, and 78.7% at fifteen years after the femoral revision and was 95.1% at fifteen years and 87.1% at twenty years after the primary arthroplasty. Increased age (p < 0.0001) and a shorter time-interval (less than 7.5 years) between the primary arthroplasty and the femoral revision (p = 0.05) were significantly associated with an increased likelihood of survival free of cup revision. Femoral head size, acetabular component design, gender, and primary diagnosis did not affect prosthetic survival in this sample of patients. CONCLUSIONS: The likelihood of survival of the unrevised cup was associated with patient age and duration of implantation, with the age of the patient being the more dominant factor predicting failure of the acetabular component.  相似文献
