We report a case of Penicillium chrysogenum endophthalmitis after penetrating ocular trauma in a 9-year-old child, describing the initial management and the therapeutic adaptation after biopsy culture. After a review of endophthalmitis treatment, we discuss mycotic endophthalmitis treatment and recommend the use of intravitreal antibiotics. In this case, we used amphotericin B to treat the fungal disorder with success.  相似文献

PURPOSE: To describe a postkeratoplasty endophthalmitis owing to Alcaligenes faecalis that resolved with medical management. METHODS: Case report and review of literature. RESULTS: To best of our knowledge, postkeratoplasty endophthalmitis caused by A. faecalis has not been reported in literature. The source of infection was found to be the donor tissue. The organism isolated was resistant to gentamicin, which was the only antibiotic present in the storage medium (McCarey and Kaufman medium). The patient was treated aggressively with topical, systemic and intravitreal antibiotics. The endophthalmitis resolved completely within two weeks. CONCLUSION: A. faecalis is a gram negative, non-spore forming rod that can cause endophthalmitis if donor tissue is contaminated with the organism. Clinical suspicion, review of bacterial culture and sensitivity report from donor tissue, anterior chamber and vitreous taps, along with early therapy with appropriate antibiotics can lead to resolution of infection.  相似文献

A case of bilateral metastatic Staphylococcus aureus endophthalmitis is presented. It evolved over a two-week period, causing some diagnostic confusion. Treatment with fusidic acid and benzylpenicillin resulted in the retention of useful vision in one eye and good vision in the other.  相似文献

Metastatic candida endophthalmitis presented as a unilateral panuveitis in an apparently healthy drug addict. Diagnosis was confirmed by vitrectomy. The eye made an excellent response to treatment with the new antifungal drug, ketoconazole, and 5-fluorocytosine. We describe the first case of successfully treated exogenous aspergillus endophthalmitis following penetrating injury and primary scleral wound repair. After repeated vitreous surgery with multiple intravitreous instillations of amphotericin B combined with intravenous antifungal therapy the patient's visual acuity was 6/18. A vigorous approach in the management of mycotic endophthalmitis is emphasised and the role of closed vitrectomy discussed.  相似文献

Recurrent post-cataract endophthalmitis is a well-recognized postoperative complication that has been attributed to various organisms and different mechanisms. To our knowledge, there is no case of recurrent postoperative endophthalmitis reported where the organism was found to be sequestered in the posterior capsule, escaping total eradication and thus producing the recurrence. The following is a case report of such recurrent postoperative endophthalmitis caused by Enterococcus faecalis sequestered in the posterior capsule.  相似文献

We report three cases of endophthalmitis following cataract extraction. Enterococcus faecalis was the causative organism in all of them. Initial visual acuity was only light perception in all cases, final visual acuity was negative light perception in two cases, and light perception in one case. After initial improvement, inflammation was exacerbated in one patient and evisceration realized. The capsular bag was examined using scanning electron microscopy and it was shown that enterococci were adherent to the capsular bag, four months after the onset of the infection. Enterococcus faecalis can be the causative agent both of an acute and a chronic form of post operative endophthalmitis. Experimental studies on rabbit eyes could explain the poor prognosis of these endophtalmitis and focused on bacterial toxins. The contamination appears to be post operative, because Enterococcus faecalis is not a commensal agent of the conjunctiva. So we have to inform our patients about some hygienic habits, to prevent from this devastating complication of cataract surgery.  相似文献

BACKGROUND: Endogenous fungal endophthalmitis (EFE) is a rare intraocular infection that has recently increased in incidence over the last few decades. The most common causative organism of endogenous endophthalmitis is Candida albicans. Treatment generally is initiated with an antifungal drug systemically and, depending on the ocular findings, may also include a vitrectomy with the injection of an intraocular antifungal agent. Specific predisposing factors may make a patient more at risk for development of an ocular infection. CASE REPORT: We present a case of presumed Candida endogenous endophthalmitis in an 83-year-old white man with a Candida albicans urinary tract infection and describe the general characteristics, treatment, and management of this condition. CONCLUSION: Endogenous fungal endophthalmitis is a potentially blinding condition that can signal an underlying systemic infection. Without a dilated fundus examination in the presence of ocular inflammation, the condition easily can be misdiagnosed. Fungal infection may lead to retinal and vitreal lesions that can be visually debilitating if left untreated. Prognosis for visual outcome is dependent on timely diagnosis and initiation of treatment.  相似文献

We report a case of endogenous aspergillus endophthalmitis. This infection occurred in a young immunocompromised boy of 6 years old. The localisation of the chorioretinitis was unusual because out of the posterior area. The evolution was favorable with recovering of the visual acuity under general treatment and intravitreous injections.  相似文献

AIM: To identify instrument holding archetypes used by experienced surgeons in order to develop a universal language and set of validated techniques that can be utilized in manual small incision cataract surgery (MSICS) curricula. METHODS: Experienced cataract surgeons performed five MSICS steps (scleral incision, scleral tunnel, side port, corneal tunnel, and capsulorhexis) in a wet lab to record surgeon hand positions. Images and videos were taken during each step to identify validated hand position archetypes. RESULTS: For each MSICS step, one or two major archetypes and key modifying variables were observed, including Tripod for scleral incision, Tripod-thumb bottom for sceral tunnel, Underhand-index to thumb grip for side port, Index-contact tripod for corneal entry, and Tripod-forceps for capsulorhexis. Key differences were noted in thumb placement and number of fingers supporting the instrument, and modifying variables included index finger curvature and amount of flexion. CONCLUSION: Identification of optimal hand positions and development of a formal nomenclature has the potential to help trainees adopt hand positions in an informed manner, influence instrument design, and improve surgical outcomes.  相似文献

BACKGROUND: Postoperative bacterial endophthalmitis is caused by the patient's endogenous flora in most cases (80 %). Pasteurella multocida (PM) is a Gram-negative coccobacillus found in the upper respiratory tract of dogs and cats and is very rarely implicated in postoperative endophthalmitis. HISTORY AND SIGNS: We describe a case of PM endophthalmitis that developed after cataract surgery. THERAPY AND OUTCOME: Cultures of both the conjunctiva and the aqueous humor were positive for PM. Topical, intravitreous and intravenous antibiotics were administered. Despite treatment, the outcome was unfavourable and complicated by a corneal perforation. CONCLUSIONS: The prognosis of postoperative PM endophthalmitis remains poor, despite adequate treatment of the infection. A history of recent pet exposure should alert physicians to this possible aetiological factor.  相似文献
