We have assessed the usefulness of dobutamine infusion for the diagnosis of coronary artery disease by using two-dimensional echocardiography and 12-lead electrocardiogram. Dobutamine was infused at incremental doses (up to a maximum of 40 micrograms kg-1 min-1) in 52 patients with chest pain; all the patients underwent coronary angiography; significant coronary artery disease was quantitatively defined as greater than or equal to 50% diameter stenosis. Thirty-six patients were on betablockers. The test was considered positive when new regional wall motion abnormalities appeared during dobutamine infusion. No significant side effects occurred in any patient during the test. Transient wall motion abnormalities were detected in 20 of 37 patients with coronary artery disease (sensitivity = 54%); ischaemic ST segment changes were present on ECG in nine patients (sensitivity = 24%). Dobutamine stress echocardiography was negative in 12 of 15 patients with coronary artery diameter stenosis less than 50% (specificity = 80%). Exercise electrocardiography (ECG) was performed in 35 of these 52 patients. Maximum heart rate and systolic blood pressure were significantly higher during exercise than during dobutamine stress test (127 +/- 23 vs 99 +/- 24 beats min-1, P less than 0.0001; 179 +/- 25 vs 152 +/- 30 mmHg, P less than 0.0001). The exercise ECG test was positive in 12 of the 26 patients with significant coronary artery disease (sensitivity = 46%), and dobutamine stress echocardiography in 16 (sensitivity = 62%). Dobutamine stress echocardiography test is a safe and feasible diagnostic test for the noninvasive diagnosis of coronary artery disease and can be performed in patients unable to exercise.(ABSTRACT TRUNCATED AT 250 WORDS)  相似文献

Forty-two patients with known stable coronary artery disease, referred for coronary angiography, were examined with technetium-99m-hexakis-2-methoxy-2-methylpropyl-isonitrile (MIBI) tomography combined with a high-dose dipyridamole infusion (0.7 mg/kg) and handgrip stress. MIBI tomography was unable to show coronary artery disease only in 2 patients, thus yielding a sensitivity figure of 95%. MIBI tomography correctly identified 27 (82%) of 33 stenotic lesions (greater than or equal to 50% diameter stenosis) of the left anterior descending artery, 17 (61%) of 28 of those of the left circumflex artery, and 28 (90%) of 31 of those of the right coronary artery. The overall vessel sensitivity was 78%. The computed lumen diameter stenoses were more advanced in cases detected than in those not detected with MIBI tomography: 87 +/- 14 vs 76 +/- 14% (p less than 0.01). The 50 to 69% stenoses did not show any tendency to produce less positive findings than those with greater than or equal to 70% stenoses. In the subgroup of 21 patients who also presented for thallium scintigraphy, the overall diseased vessel identification rate was 76% for thallium tomography and 83% for MIBI tomography (p = not significant). Minor noncardiac side effects related to the dipyridamole-handgrip test occurred only in 5% of 63 study sessions. A high-dose dipyridamole combined with isometric exercise is a safe stress method, and when used during scintigraphy, MIBI tomography is at least as efficient a tool as thallium tomography in detecting diseased vessel territories in patients in coronary artery disease.  相似文献

We assessed the accuracy of early dobutamine stress echocardiography to detect infarct-related coronary artery and multivessel disease in patients with first Q wave myocardial infarction after withdrawal of cardioactive drugs. Dobutamine-atropine echocardiography was performed in 91 consecutive patients (mean age 59+/-6 years) 7+/-4 days after myocardial infarction. Dobutamine was infused at incremental doses of 5, 10, 20, 30 to 40 microg/kg/min each one dose for 3 min. Peak heart rate was 134+/-17 bpm. All patients underwent coronary angiography before discharge. Sensitivity, specificity and accuracy of ischemic and biphasic response to detect residual stenosis of infarct-related coronary artery were 70, 92 and 73%, respectively. The sensitivity, specificity and accuracy of ischemic or biphasic response were similar in the vascular territories of left anterior descending (74, 86 and 75%, respectively), right (67, 100 and 70%, respectively) and circumflex coronary arteries (64, 100, and 69%, respectively). Sensitivity, specificity and accuracy of heterozonal wall motion abnormalities for multivessel coronary artery disease were 64, 82 and 76%, respectively. Dobutamine stress echocardiography is sensitive and specific in detecting residual coronary stenosis and multivessel disease in patients with first Q-wave myocardial infarction. We have assessed the value of dobutamine stress echocardiography for the diagnosis of myocardial ischemia in 20 consecutive patients referred for chest pain, without a previous myocardial infarction and with a normal left ventricular wall motion at rest. The test was considered positive when wall motion abnormalities appeared during dobutamine infusion (from 5 to a maximum of 40 micrograms/Kg/min). The results were compared to the % diameter stenosis (%DS) quantitatively measured on coronary arteriography. "Significant" coronary artery disease was defined as greater than or equal to 50% DS. No significant side effects occurred in any patients during the test. Transient wall motion abnormalities were detected in 8 of the 12 patients with significant coronary artery disease (sensitivity = 66%) and in 1 of the 8 patients without significant coronary artery disease (specificity = 88%). All the patients with false-negative dobutamine stress echocardiography had distal stenosis or stenosis in a collateral vessel; moreover, all the patients with true-positive dobutamine stress echocardiography had proximal (7 patients) or middle (1 patient) stenosis. The results of this study show that dobutamine stress echocardiography is a safe and feasible test for the noninvasive diagnosis of myocardial ischemia.  相似文献

Objectives. The purpose of this study was to determine the predictive value of quantitative coronary angiography in the assessment of the functional significance of coronary stenosis as judged from the development of left ventricular wall motion abnormalities during dobutamine-atropine stress echocardiography.Background. Coronary angiography is the reference method for assessment of the accuracy of noninvasive diagnostic imaging techniques to detect the presence of significant coronary stenosis. However, use of arbitrary cutoff criteria for the interpretation of angiographic data may considerably influence the true diagnostic accuracy of the technique investigated.Methods. Thirty-four patients without previous myocardial infarction and with single-vessel coronary stenosis were studied with both quantitative angiography and dobutamine-atropine stress echocardiography. Two different techniques of quantitative angiographic analysis—edge detection and videodensitometry—were used for measurement of minimal lumen diameter, percent diameter stenosis and percent area stenosis. Two-dimensional echocardiographic images were collected during incremental doses of intravenous dobutamine and later analyzed using a 16-segment left ventricular model. Angiographic cutoff criteria were derived from receiver-operating curves to define the functional significance of coronary stenosis on the basis of dobutamineatropine stress echocardiography.Results. The angiographic cutoff values with the best predictive value for the development of left ventricular wall motion abnormalities during dobutamine-atropine stress echocardiography were minimal lumen diameter of 1.07 mm, percent diameter stenosis of 52% and percent area stenosis of 75%. Minimal lumen diameter was found to have the best predictive value for a positive dobutamine stress test (odds ratio 51, sensitivity 94%, specificity 75%).Conclusions. Automated quantitative angiographic measurement of animal lumen diameter is a practical and useful index for determining both the anatomic and functional significance of coronary stenosis, and a value of 1.07 mm is the best predictor for a positive dobutamine stress test.  相似文献

Objectives. This study evaluated both the relation between dobutamine-induced wall motion abnormalities and the physiologic and morphologic features of epicardial coronary artery stenoses and the impact of the extent of the area at risk on the sensitivity of dobutamine echocardiography.Background. The accuracy of dobutamine echocardiography has traditionally been assessed by comparing results with stenosis geometry. Myocardial fractional flow reserve is a functional index of coronary stenosis severity that takes into account both anterograde and collateral flow and may therefore be a more appropriate standard for comparison.Methods. Seventy-five patients with normal left ventricular function, good echocardiographic images and an isolated corenary stenosis underwent, within 6 h, debutamine echocardiography, quantitative coronary angiography and intracoronary pressure measurements. Myocardial fractional flow reserve was calculated as the ratio of mean hyperemic distal coronary to aortic pressure.Results. The degree of dobutamine-induced dyssynergy correlated significantly with percent diameter stenosis (r = 0.68), area stenosis (r = 0.68) and minimal lumen diameter (r = 0.60) and markedly better with myocardial fractional flow reserve (r = −0.77). However, marked dispersion of the individual data was observed. The sensitivity of dobutamine echocardiography in detecting lesions with a minimal lumen diameter ≤1 mm and diameter stenosis ≥50% was 83% and 80%, respectively. All but one patient with a myocardial fractional flow reserve >0.75 had a normal stress test result. Among patients with a myocardial fractional flow reserve ≤0.75, the sensitivity of dobutamine echocardiography was significantly lower for lesions in vessels with a reference diameter ≤2.6 mm than for lesions in larger vessels (58% vs. 90%, p = 0.008).Conclusions. 1) The magnitude of wall motion abnormalities induced by dobutamine infusion correlates with angiographic and, more closely, with functional indexes of stenosis severity, even though a wide scatter is observed. 2) In patients with a functionally significant stenosis, the amount of myocardium at risk is a critical determinant of the accuracy of debutamine echocardiography.  相似文献

Objectives. This study sought to assess the value of quantitative coronary arteriography in predicting an ischemic response at exercise echocardiography and technetium-99m 2-methoxy isobutyl isonitrile (mibi) single-photon emission computed tomography (SPECT) in patients with single-vessel disease of the left anterior descending coronary artery.

Background. The relation between severity of coronary stenosis and ischemic response to exercise echocardiography and perfusion scintigraphy in patients with single-vessel left anterior descending coronary artery disease is not well established.

Methods. Thirty-one patients without a previous myocardial infarction who had isolated stenosis of varying degrees in the proximal or midportion of the left anterior descending coronary artery were studied. Quantitative arteriographic analysis was used for measurements of percent diameter stenosis and minimal lumen diameter. Exercise-induced wall motion abnormalities by echocardiography and transient perfusion defects by mibi SPECT were considered a positive response. The analysis of sensitivity/specificity and receiver operating characteristic curves was applied to establish the diagnostic power of quantitative coronary arteriography to predict an ischemic response to exercise echocardiography and mibi SPECT.

Results. The “best” angiographic cutoff values for predicting a positive exercise echocardiographic and scintigraphic response were similar (diameter stenosis 52%, minimal lumen diameter 1.12 mm for echocardiography; diameter stenosis 49%, minimal lumen diameter 1.20 mm for SPECT). However, the sensitivity/specificity at the cross point was slightly higher (even if not statistically significant) for echocardiography than for SPECT, both for diameter stenosis (81% vs. 67%) and minimal lumen diameter (81% vs. 74%), suggesting that quantitative coronary arteriographic measurements are more closely related to echocardiographic than scintigraphic exercise test results.

Conclusions. The functional significance of a proximal/mid-left anterior descending coronary artery stenosis measured by quantitative coronary arteriography is slightly better related to echocardiographic than scintigraphic markers of exercise-induced myocardial ischemia.  相似文献

Transient increase in diastolic wall thickness (pseudohypertrophy) during pacing stress echocardiography has been reported in normal myocardium. To evaluate the occurrence of pseudohypertrophy and to investigate the contribution of myocardial ischemia on its production during pacing and dobutamine stress echocardiography, we produced a physiologically significant coronary stenosis in 14 open chest dogs. The stenosis in the circumflex artery was measured by quantitative coronary angiography (range: 50% to 89% reduction in luminal diameter), and no resting segmental wall-motion abnormalities were observed by epicardial echocardiography (short-axis, papillary level). In each study, dobutamine (5-40 microg/kg/min) and pacing (up to 260 beats/min) were performed randomly. Positivity of stress echocardiography tests was quantitatively determined by a significant (P < 0.05) reduction or failure to increase in absolute and percent systolic wall thickening in the myocardial area supplied by the stenotic artery as compared to the left anterior descending (LAD) artery-related areas. Diastolic wall thickness and left ventricular diastolic area were compared before and after each stress test in the circumflex and LAD artery-related regions. Pseudohypertrophy was observed in 57% and 86% of dogs for pacing and dobutamine, respectively, in the circumflex region, and in 50% and 64% in the LAD region. Despite its increased incidence in the circumflex region, the augmented diastolic wall thickness did not correlate with coronary stenosis severity or stress test positivity, but correlated inversely with changes in left ventricular diastolic area. In addition, it correlated directly with changes in heart rate only for pacing. In conclusion, pseudohypertrophy was a frequent finding during pacing and dobutamine stress echocardiography tests but was not related to myocardial ischemia in this animal model.  相似文献

To compare the diagnostic value of dobutamine stress echocardiographywith dipyridamole thallium-201 single-photon emission computedtomography (SPECT) in detecting coronary artery disease (CAD),we performed both tests on 54 patients who also underwent coronaryarteriography. Dobutamine was infused at an incremental regimenof 5,10,20,30 and 40 µg. kg-1. min-1. Dipyridamole wasinfused at a rate of 0.14 mg. kg-1. min-1 over 4 min. Dobutaminestress echocardiography detected 40 (93%) and SPECT 42 (98%,P=ns) of the 43 patients with significant CAD, defined as (greaterthan or equal) 50% diameter stenosis. The specificity was 73%(8 of 11) for both tests. The sensitivity for detecting individualcoronary artery stenosis with dobutamine stress echocardiographywas 81% (30 of 37) for the left anterior descending artery,75% (24 of 32) for the right coronary artery, and 61% (17 of28) for the left circumflex artery. For SPECT it was 89%, 97%(P>0.05 vs dobutamine stress echocardiography) and 75%, respectively. Among the 97 stenotic coronary arteries, 17 had mild to moderatestenosis (50%-69% diameter stenosis) and 80 had severe stenosis($$70% diameter stenosis). With dobutamine stress echocardiography,53% of the arteries with mild to moderate stenosis were identifiedvs 78% of those with severe stenosis (P<0.05). With SPECT,the sensitivity was 82% (14 of 17) in mild to moderate stenosisand 89% (71 of 80) in severe stenosis (P=ns). No major sideeffects occurred during either test. Thus, both dobutamine stressand SPECT are highly sensitive for detection and localizationof CAD. However, the sensitivity of dobutamine stress is affectedby the level of stenosis severity.  相似文献

Background Dobutamine stress echocardiography has become an accepted simpleand inexpensive method of detecting coronary artery disease.In this pharmacological stress test, particular attention hasbeen paid to transient systolic wall motion abnormalities. Ourgroup has noted an abnormal diastolic ventricular septal motion,a ‘diastolic notch’, during dobutamine stress echocardiographythat has not been previously described. Methods and Results To find out whether this anomalous septal motion is relatedto coronary artery disease we have analysed the stress studiesof 125 patients (69 men, age 61±9 years) with chest pain,no previous myocardial infarction and no left bundle branchblock, who underwent a dobutamine stress test. Dobutamine wasinfused up to 40µ.kg.–1min–1in 3min stages.A positive stress test was defined as the appearance of transientasynergy. Dobutamine time was the time from the infusion ofdobutamine to the appearance of transient asynergy. The diastolicnotch time was the time at which diastolic notch was first detected.Diastolic notch was detected in 21 patients with single coronaryartery disease, 19 of whom had a severe left anterior descendingartery stenosis. Diastolic notch was present in 19 out of 27patients (70%) with single left anterior descending stenosis.Twenty-six out of 44 patients with multivessel coronary arterydisease had evidence of a diastolic notch and 20 of these 26had severe stenosis of the left anterior descending artery.Finally, all three patients with left main coronary artery diseasehad a diastolic notch while no patient with angiographicallynormal coronary arteries had this sign. In patients with a diastolicnotch and a positive dobutamine stress test, diastolic notchtime was shorter than dobutamine time (9±4min vs 11±3min,P<0·05). Conclusions In patients without previous myocardial infarction and withoutleft bundle branch block (1) the appearance of a septal diastolicnotch during dobutamine stress echocardiography is very specificfor the presence of coronary artery disease; (2) the detectionof diastolic notch is mostly related to the existence of severeleft anterior descending artery stenosis; (3) diastolic notchprecedes the development of ventricular asynergy.  相似文献

Objectives. This study was designed to characterize the clinical, echocardiographic and angiographic findings in patients who have regional wall motion abnormalities predictive of coronary artery disease on dobutamine stress echocardiograms, although coronary angiography reveals no critical stenoses.Background. The specificity of dobutamine stress echocardiography has been reported to be lower than its sensitivity; the sources of false positive findings on dobutamine stress echocardiograms have not been previously defined.Methods. Clinical and echocardiographic characteristics were retrospectively reviewed for patients who had both a dobutamine stress echocardiogram indicate of coronary artery disease on the basis of wall motion abnormalities and <50% stenoses reported on coronary angiography performed within 6 weeks of the echocardiogram. A 16-segment model was used to perform wall motion scoring. Angiograms were independently reviewed, and stenosis severity was quantified with the use of digital calipers.Results. Thirty-nine (11.4%) of 342 studies met criteria for false positive test results, which occurred predominantly in women (72%, p < 0.601). Regional wall motion abnormalities were evident more often in the posterior circulation (62%), and 65% of them were limited to the basal segments. Twelve (28%) of 43 wall motion abnormalities were associated with coronary stenoses of at least intermediate grade (lumen diameter 40.3% to 68.1%). Abnormalities confined to basal segments of the posterior circulation were unlikely to have associated coronary lesions (p = 0.03).Conclusions. False positive findings on dobutamine stress echocardiograms tend to involve small wall motion abnormalities that are frequently located in basal segments of the posterior myocardial circulation. Approximately one third of false positive results occurred in patients with intermediate-grade coronary stenoses, and these studies may reflect true inducible ischemia. Additional sources of false positive study results may include poor endocardial visualization and abnormal motion dye to tethering to the fibrous skeleton of the heart. Altered echocardiographic diagnostic criteria may be appropriate for small wall motion abnormalities confined to basal segments of the posterior circulation.  相似文献

Objectives. This study attempted to determine the safety and accuracy of dobutamine stress echocardiograpby for detection of coronary artery disease in patients with dilated cardiomyopathy.Background. Detection of regional wall motion abnormalities at rest does not reliably distinguish ischemic front nonischemic cardiomyopathy. Previous studies have shown that dobutamine stress echocardiography safely and accurately identifies coronary artery disease in patients without dilated cardiomyopathy.Methods. Seventy patients with dilated cardiomyopathy under-went dobutamine stress echocardiography. Echocardiograms were obtained at baseline and at low (5 to 10 μg/kg body weight per min) and peak doses of dobutamine. Rest and stress left ventricular wall motion scores were derived from analysis of regional wall motion. Fifty-four subjects underwent coronary angiography.Results. Dobutamine infusion was terminated after achievement of the target heart rate or maximal protocol dose in 49 patients (70%), ischemia in 12 (17%), arrhythmia in 4 (6%) and side efects in 5 (7%). No patient had prolonged ischemia or sustained arrhythmia. Of those with angiographic studies, 40 had significant coronary artery disease (≥50% diameter stenosis). Use of the change in global wall motion score index from low to peak dose resulted in a sensitivity of 83% for dobutamine stress echocardiography and a specificity of 71% for detection of coronary artery disease. Sensitivity for detection of triple-,double-and single-vessel disease was 100%, 83% and 69%, respectively.Conclusions. Dobutamine stress echocardiography safely provides diagnostic information in patients with dilated cardiomyopathy. This technique has high sensitivity for multivessel coronary artery disease but only moderate specificity.  相似文献

Combined proximal left anterior descending and proximal left circumflex artery stenoses greater than or equal to 70% have been referred to as "left main equivalent" lesions. We compared the survival rates of medically treated patients who have this type of coronary anatomic characteristics with the survival rates of patients who have left main coronary artery stenoses greater than or equal to 70% by use of a stratified life table approach and a Cox regression model. Comparison of the patients with left main coronary artery stenoses with those who have left main equivalent lesions by use of life table analysis and three different calculations of patient exposure time revealed a poorer prognosis for the patients who had left main coronary artery disease (p less than or equal to .04 for all three methods). The stepwise Cox analysis also determined that patients who had left main artery stenoses had a significantly poorer prognosis than patients who had left main equivalent coronary disease (p = .002), even after consideration of important baseline variables known to affect survival rates. We then compared the patients who had combined proximal left anterior descending and proximal left circumflex artery disease with patients who had combined stenoses greater than or equal to 70% in the nonproximal left anterior descending stenosis influenced survival rates. The 5 year to determine if location of the left anterior descending stenosis influenced survival rates. The 5 year survival rate was not as high for the patients who had proximal left anterior descending artery disease (55% vs 70%, p = .001). In conclusion, combined proximal left anterior descending and proximal left circumflex artery disease identifies a high-risk (as determined by angiography) patient subset.(ABSTRACT TRUNCATED AT 250 WORDS)  相似文献

To determine whether compensatory enlargement of atherosclerotic coronary arteries occurs and to what degree it affects the angiographic assessment of coronary artery disease, we performed postmortem coronary angiography of 30 human hearts with suspected coronary artery disease and studied 70 histologic cross sections of the proximal left anterior descending artery and proximal right coronary artery. Angiographic and morphometric analyses of 50 stenoses in proximal and middle sections of the left anterior descending artery, right coronary artery, and left circumflex artery were performed. The control group of 10 human hearts without suspected coronary artery disease was evaluated in the same way. For this purpose, coronary arteries were filled with a methylmethacrylic radiopaque resin at a pressure of 100 mm Hg and closely embedded in a methylmethacrylic resin by use of which shrinkage and mechanical artifacts could be avoided. The area circumscribed by the internal elastic lamina was taken as a measure of the area of the arterial lumen if no plaque had been present. The angiographic and corresponding morphometric degree of stenosis was assessed. A significant correlation (r = 0.85, p less than or equal to 0.0001) was found between the internal elastic lamina area and the area of the plaque (lesion area), suggesting that coronary arteries may enlarge as lesion area increases. With the morphometric degree of stenosis, the expected anatomic diminution of the coronary artery was abolished (r = 0.79, p less than or equal to 0.0001), indicating compensatory enlargement in atherosclerotic segments. Accordingly, the degree of stenosis assessed from in vitro angiograms was underestimated. Compensatory coronary enlargement of the stenotic segment was the main reason for angiographic underestimation. The underestimation factor of up to 3.50 for very mild stenoses decreased to 1.37 at an angiographic degree of 50% area stenosis and 30% diameter stenosis.(ABSTRACT TRUNCATED AT 250 WORDS)  相似文献

Background: Following the first attempts to detect myocardial ischemia with two-dimensional echocardiography stress testing, pharmacologic stress using dobutamine infusion has become an alternative to echocardiography exercise testing for evaluation of coronary artery disease. It has been shown that stress echocardiography has a diagnostic accuracy similar to that of an exercise thallium test. Other studies, however, indicated that radionuclide myocardial perfusion imaging was more sensitive than exercise or pharmacologic stress echocardiography for detection of ischemia or jeopardized myocardium. Hypothesis: The aim of the present study was to determine the ability of dobutamine stress echocardiography in comparison with thallium-201 scintigraphy to identify multivessel disease and the presence of myocardial scar and ischemia in 60 consecutive patients who suffered a first myocardial infarction (MI). Methods: Patients were evaluated by coronary angiography and ventriculography, thallium-201 (201Tl) tomographic scintigraphy, and dobutamine echocardiography within 3 months of a first MI. Forty-seven had Q-wave MI and 13 had non-Q-wave MI. Eleven patients were excluded from final analysis—7 because of failure to achieve target heart rate in spite of the use of atropine, and 4 because of high blood pressure following the infusion of dobutamine. Results: Dobutamine echocardiography showed an overall sensitivity of 43% for detection of coronary artery lesions of 50–74% diameter stenosis and 201Tl scintigraphy showed a sensitivity of 71%. For detection of lesions of ≥75% diameter stenosis, dobutamine echocardiography showed a sensitivity of 52% and 201Tl a sensitivity of 70%. Overall agreement between wall motion and myocardial perfusion for detection of necrosis and/or ischemia in the infarct area was 40.4% with a kappa coefficient of 0.09 (p = 0.13). For detection of ischemic myocardium outside the infarct zone, overall agreement was 78.6% with a kappa coefficient of 0.49 (p<0.0001). Conclusion: Dobutamine echocardiography results showed a lower sensitivity than myocardial perfusion images in predicting multivessel coronary artery disease, and there was poor agreement between both methods in identifying necrosis or ischemia.  相似文献

OBJECTIVE: To investigate the diagnostic accuracy, electrocardiogram and hemodynamic effects and safety of dobutamine stress echocardiography. SUBJECTS: Sixty-one patients with suspected coronary artery disease. All patients underwent coronary arteriography. MAIN RESULTS: The sensitivity of dobutamine stress echocardiography in diagnosis of coronary artery disease in the whole group was 91%. In patients with left anterior descending it was 97%; right 85%; circumflex 76%; three vessel 100%; two vessel 95%; single vessel 77%. Specificity, positive predictive value and accuracy of dobutamine stress echocardiography in diagnosis of coronary disease (whole group) was 57, 94 and 87%, respectively. ST depression of more than 1 mm occurred in 11 patients, ST elevation of more than 1 mm in three patients, T wave inversion in one and T normalization in nine. Significant differences of the effects of beta-blockers were noted on the peak effects of dobutamine as follows: heart rate increase of 46 +/- 22 versus 20 +/- 13 beats/min (P less than 0.0001); systolic pressure increase of 4 +/- 26 versus 22 +/- 19 mmHg (P less than 0.01); diastolic pressure decrease of 18 +/- 16 versus 10 +/- 12 mmHg (P less than 0.03) for patients without or with beta-blockers, respectively. Unifocal ventricular premature beats were noted in 10 patients, atrial premature beats in five and ventricular couplets in one. Angina occurred in 11 patients. Atypical chest pain occurred in seven patients, tingling in 11 and nausea in four. Thirty-six patients were totally asymptomatic. CONCLUSIONS: In this population with high prevalence (85%) of coronary artery disease, dobutamine stress echocardiography had high sensitivity and positive predictive value for coronary disease detection particularly in patients with left anterior descending or three vessel disease. The specificity and accuracy were not as good, but this may reflect the small number of normal patients. Dobutamine was well tolerated and conveniently administered.  相似文献

The ability of radionuclide variables obtained at rest and at peak exercise to discriminate the number of stenosed (greater than or equal to 70% luminal diameter narrowing) major coronary arteries was evaluated in 296 patients undergoing supine exercise radionuclide ventriculography. Stepwise linear discriminant analysis of the data from the first 200 patients identified a significant (p less than 0.001) discriminatory combination. Application of this function to the remaining 96 patients provided correct classification of arteriographically determined zero, one, two, and three stenosed arteries in 59%, 18%, 14%, and 60% of cases, respectively. The discriminant function classified minimal stenoses (zero or one artery) and multivessel stenoses (two or three arteries) correctly by arteriography in two thirds of cases in each group. Arteriographic presence of three stenoses was unlikely in those classified as having no stenosis, and absence of stenosis was rare in those classified as having three stenoses. Exercise radionuclide ventriculography is most helpful in identifying minimal and multivessel coronary disease rather than number of stenosed major coronary arteries.  相似文献

BACKGROUND. Two-dimensional echocardiography performed during dobutamine infusion has been proposed as a potentially useful method for detecting coronary artery disease. However, the safety and diagnostic value of dobutamine stress echocardiography has not been established. METHODS AND RESULTS. In this study, echocardiograms were recorded during step-wise infusion of dobutamine to a maximum dose of 30 micrograms/kg/min in 103 patients who also underwent quantitative coronary angiography. The echocardiograms were digitally stored and displayed in a format that allowed simultaneous analysis of rest and stress images. Development of a new abnormality in regional function was used as an early end point for the dobutamine infusion. No patient had a symptomatic arrhythmia or complications from stress-induced ischemia. Significant coronary artery disease (greater than or equal to 50% diameter stenosis) was present in 35 of 55 patients who had normal echocardiograms at rest. The sensitivity and specificity of dobutamine-induced wall motion abnormalities for coronary artery disease was 89% (31 of 35) and 85% (17 of 20), respectively. The sensitivity was 81% (17 of 21) in those with one-vessel disease and 100% (14 of 14) in those with multivessel or left main disease. Forty-one of 48 patients with abnormal echocardiograms at baseline had localized rest wall motion abnormalities. Fifteen had coronary artery disease confined to regions that had abnormal rest wall motion, and 26 had disease remote from these regions. Thirteen of 15 patients (87%) without remote disease did not develop remote stress-induced abnormalities, and 21 of 26 (81%) who had remote disease developed corresponding abnormalities. CONCLUSIONS. Echocardiography combined with dobutamine infusion is a safe and accurate method for detecting coronary artery disease and for predicting the extent of disease in those who have localized rest wall motion abnormalities.  相似文献

AIMS: To compare long-axis function and wall motion analysis for the detection of significant coronary artery stenoses in patients with single and multivessel disease. METHODS AND RESULTS: We performed dobutamine stress echocardiography in 67 subjects, 14 with normal coronary anatomy, and 53 with significant coronary disease. A blunted increase in mean long-axis shortening of <0.25 cm was the best discriminator for coronary artery disease (sensitivity 85%, specificity 81%). Using this threshold, long axis function gave a sensitivity of 88% and specificity 89% for the detection of coronary artery disease in patients with normal resting wall motion while wall motion abnormality analysis had a sensitivity 73% and specificity 94%. Of 26 patients with a resting wall motion abnormality, 14 (54%) had multivessel disease. Long axis function detected multivessel disease in 12 of these (sensitivity 86%) compared with nine (sensitivity 64%) for wall motion analysis. CONCLUSION: Long axis function provides a promising, quantitative adjunct to wall motion analysis for the detection of coronary ischaemia using dobutamine stress echocardiography in patients with single and multivessel disease and with resting wall motion abnormalities.  相似文献

BACKGROUND: New techniques to evaluate coronary artery disease, such as calculation of myocardial fractional flow reserve (FFR) with a guidewire and pressure transducer, provide a functional assessment of coronary lesions. The present study was designed to determine the correlation between FFR and dobutamine stress echocardiography in patients with moderately severe coronary stenosis in order to judge the usefulness of FFR for commonly encountered clinical problems. METHODS AND RESULTS: We studied 21 patients with 23 moderately severe coronary artery stenoses on angiography. The FFR was calculated and dobutamine stress echocardiography was performed to detect ischemia. Of the 16 stenoses with a negative FFR (> or = 0.75), dobutamine echocardiography also was negative. In the seven stenoses with a positive FFR (< 0.75), dobutamine echocardiography was positive in three. The efficacy of FFR in detecting ischemia that was confirmed with stress echocardiography was sensitivity 100%, specificity 80%, positive and negative predictive value 42.8%, and 100%, respectively, with a global predictive value 82.6%. A moderate degree of correlation was found between the two diagnostic tests (kappa [kappa] = 0.51). CONCLUSIONS: FFR correlates moderately well with dobutamine stress echocardiography in the assessment of moderately severe lesions in patients for whom coronary arteriography is usually indicated. However, its high negative predictive value makes FFR a useful aid in reaching clinical decisions promptly in the hemodynamics laboratory.  相似文献
