病历摘要:患者,男,60岁,因上腹痛、腹胀、恶心、精神萎靡3天于1996年5月26日入院.患者3天前无明显诱因出现上腹阵发性绞痛伴腹胀恶心及食欲下降,精神萎靡.在当地医院给抗感染止痛治疗未缓解,故来我院诊治.既往健康,嗜好饮酒,每日喝老白干约500ml.查体:T36.5℃,P98次/分,R22次/分,BP21/14kPa.神志清、贫血貌.皮肤无出血点,无皮疹,无蜘蛛痣及肝掌.巩膜轻度黄染.浅表淋巴结不肿大.颈软,甲状腺无肿大.双肺呼吸音清,未闻及干湿罗音及胸膜摩擦音.心界无扩大,心率98次/分,     

高血压,浮肿,贫血,恶心,呕吐

病历摘要吕××,女51岁,工人。高血压22年,浮肿14年,贫血、恶心、呕吐5个月,于1988年3月2日入院。 22年前开始发现高血压,血压在22.7～24/12～13.2kPa,经常口服降血压药物,具体药名不详,14年前突然不明原因全身浮肿,乏力,即以“急性肾小球肾炎”住本厂职工医院治疗,经治疗后浮肿消退,但尿常规检查蛋白一直在(++)～(+++),血压21.3～24/14.7kPa,浮肿时有时无,休息后消退,曾多次住院治疗,缓解后能上班及做家务劳动,     

发热,胸痛,腹痛,贫血

1病例简介患者,女,18岁,主因腰背痛、贫血伴发热10 d,门诊以“肾盂肾炎”于2000-12-28收住院。患者于10 d前因受凉出现“感冒”症状,腰背部疼痛,发热,体温38·0~40·0℃,门诊给予先锋V、甲硝唑、清开灵治疗1周,无效,门诊查血常规,WBC 16·8×109/L,N 0·86,L 0·14,Hb 95 g/L,血沉48 mm/h,尿酮(+),改用头孢曲松钠2·0 g静滴3 d,体温36·0~40·5℃,仍有胸痛、腹痛、肌肉关节痛,初步诊断“发热待查”收住血液科。患者既往体健,无疫源物(区)接触史。入院查体:T 38·0℃、P 86次/m in、R 21次/m in、Bp 127/60 mm Hg(1 mm Hg=0·133 kPa)…     

四肢麻木,口干,腹胀,排尿困难

乏力,少尿,浮肿,黄疸

病历摘要 患者,女,34岁,干部。因间断浮肿1年,加重伴全身皮肤黄染1周于1998年4月10日9时入院。患者缘于1年前无明显诱因出现晨起眼睑浮肿,继而出现少尿(400ml/日)及下肢浮肿,曾在我院及北京某医院化验及肾穿,病理检查诊断为“膜性肾病”应用强的松60mg/日治疗,浮肿逐渐缓解,尿量增加,恢复正常3个月后,强的松逐渐减量,现停用近1个月,眼睑及双下肢浮肿时有复发,休息后减轻。1周前患者因劳累后出现乏力,右上腹不适,巩膜及全身逐渐黄染,自觉发热(体温未测)少尿(约 400ml/日)眼睑及双下肢浮肿加重,曾在基层医院给予先锋Ⅵ(5g/日)消炎治疗3天,无明显疗效,复来我院就医。患者自述两年前体检时发现,HBsAg ( ),但无症状,未进行治疗。否认有其他疾病病史。 体检,T37.8℃,P80次/分,R18次/分,BP16/ 12kPa,发育正常,营养良好,神志清晰,全身皮肤黄染,全身浅表淋巴结未触及肿大,眼睑轻度浮肿,巩膜明显黄染,双肺未见异常,心率96次/分,律规则,心尖部第一心音略减弱,未闻及杂音,腹平坦,右上腹压痛(±),肝肋弓下触及边,质中等, Murphy's征(-),肝区叩痛( ),肠鸣音4次/分,双下肢明显可凹性浮肿,生理反射存在,病理反射未引出。 实验室检查:血常规RBC4.1×10~(12)/L, Hb120g/L,WBC7.5×10~9/L,N0.72,L0.29,PIt225
