梭形细胞血管内皮瘤一例

患者女,43岁。因右下肢皮下结节1年于2013年7月来我院就诊。患者1年前右下肢出现一花生米大蓝青色结节,无明显自觉症状,结节生长缓慢,无发热等全身症状。患者否认既往患有其他皮肤疾病,否认家族成员有类似病史及遗传病史……     

Vascular spindle cell lipoma

Spindle cell lipoma is a neoplasm of adipose tissue that primarily affects posterior necks and shoulders of men in their fifth to seventh decades. It consists of a mixture of adipose tissue and spindle-shaped cells that could be misinterpreted as sarcoma. A clinical and pathological study of two patients, both men, with vascular variants of spindle cell lipoma is presented. These tumors were unusual clinically, in that one caused pain on motion and the other was painful to palpation. On microscopic examination, their vascularity was augmented over previously described examples of spindle cell lipoma.     

Coexistence of spindle cell lipoma and ordinary lipoma

We report a patient with a spindle cell lipoma on the nape and three ordinary lipomas on the abdomen and extremities. The coexistence of spindle cell lipoma and ordinary lipoma in a single patient is rare. Abundant CD34-positive spindle cells and mast cells were found in the spindle cell lipoma, but in the ordinary lipomas, only a small number of CD34-positive spindle cells were found in the interstitial connective tissue and no mast cells were seen. Because mast cells are known to stimulate mesenchymal cell proliferation and collagen production, mast cell infiltration may be a trigger for the proliferation of CD34-positive spindle cells, leading to the conversion of ordinary lipoma into spindle cell lipoma.     

Pedunculated cutaneous spindle cell/pleomorphic lipoma

Spindle cell/pleomorphic lipoma is an infrequently seen benign adipose tissue tumor. This tumor, mostly arising from the subcutaneous tissue, usually affects male patients and occurs in back, shoulders, head and neck area. It is rarely localized to dermis. Cutaneous spindle cell/pleomorphic lipoma differs from its subcutaneous counterpart by wider anatomical distribution, female predilection and infiltrative margins. We here present a pedunculated type, cutaneous pleomorphic lipoma case localized to the nasolabial region of 59‐year‐old man. Gurel D, Karg? A, Lebe B. Infiltrating intramuscular spindle cell lipoma of the face

Spindle cell lipoma is a benign lipomatous tumor, which usually arises on the back of the neck, shoulder or upper back of males in the third to seventh decade of life. We report herein an unusual infiltrating intramuscular spindle cell lipoma arising in the nose of a 53-year-old man. The patient presented with a 0.5-cm 'cyst' of the nose, just above the right alar crease, which was removed. Four years later, the lesion recurred and was re-excised. Histologically, a proliferation of mature adipocytes, ropey collagen fibers and spindle cells within a myxoid stroma was present in the subcutaneous tissue and infiltrated between skeletal muscle fibers. Nine cases of intramuscular spindle cell lipoma with histological examination have previously been reported and have involved the oral cavity and muscles of the extremities. To our knowledge, this is the first report of an infiltrating intramuscular spindle cell lipoma arising on the face.

患者男,95岁,因双上肢出现红色结节1年余,于2016年8月23日来广州医科大学附属第二医院皮肤科就诊。患者于1年前双上肢无明显诱因出现红色结节,皮损进行性增大且数量增多,无疼痛、瘙痒,未予治疗。既往体健,否认高血压、冠心病等慢性病史,否认外伤史及手术史。家族成员中无类似病史……
