In a prospective longitudinal study over 2 years and a separate cross-sectional study more than 5 years after operation, we analyzed periprosthetic bone mineral density (BMD) after cementless total hip arthroplasty (THA) (press-fit cementless Spotorno stem, Mecron threaded acetabular cup) by dual-energy X-ray absorptiometry (DEXA). BMD was analyzed in a longitudinal prospective study (n = 53 patients: 29 women, 24 men) and in a separate cross-sectional study (n = 23 patients: 13 women, 10 men) with good clinical outcome (Merle d' Aubigne score > 12). Regions of interest were defined according to Gruen (ROI 1–7) and as netto average ROI (NETAVG I) for the periprosthetic femur, and according to De Lee and Charnley (ROI I-III) and as NETAVG II for the periprosthetic acetabulum. BMD during follow-up was compared with immediate postoperative values of the affected limb. Mean precision error (CV%) was 2.6 ± 0.5% for ROI 1–7 and 1.3 ± 0.9% for ROI I–III. BMD significantly decreased in the periprosthetic femur and acetabulum during the first 3 months after operation. At the femur, BMD (NETAVG I) for women and men, respectively, was 92.4% and 87.5% at 6 months, then 89.4% and 96.2% at 2 years. ROIs around the proximal stem showed the lowest absolute values and decreased most during follow-up (to 79.9% ROI 1 and 68.2% ROI 7, respectively). Mineralization around the cup (NETAVG II), respectively, amounted to 81.1%, 82.6% at 6 months, then 80.1% and 93.8% at 2 years. The medially placed ROI II demineralized most (respectively, 72.1% and 76.7%). More than 5 years after THA, BMD in the femur showed little change, but decreased significantly to 76.4% and 79.1%, respectively, around the cup (NETAVG II). DEXA is a useful method for analyzing changes of mineralization around cup and stem of cementless THA. The results reflect the different stress on the periprosthetic bone after implantation of THA in defined ROIs, supporting earlier reported good clinical results of the Spotorno stem and increased loosening rate of threaded acetabular cups after 5 years. Dose-dependent prevention of early periprosthetic bone loss by alendronate   总被引:1,自引:0,他引:1  
AIM: Periprosthetic bone loss occurs in the first six months after total hip arthroplasty (THA) and is felt to be largely the result of initial operative irritation, immobilization, and stress shielding. This study (a prospective, randomized, open, blinded endpoint evaluation) aims at preventing bone loss around the stem with an oral bisphosphonate. METHOD: 66 healthy subjects with uncemented THA and low lumbar bone mass density (BMD) (negative T score) were treated post-operatively with alendronate as follows: n = 21 with 10 mg/d for 10 weeks (A), n = 21 20 mg/d for 5 weeks (B), n = 24 no treatment for controls (C). The periprosthetic BMD in the Gruen zones (ROI) was measured after the 2nd, 4th, 6th, and 12th month by DEXA as a percentage of the value measured one week after surgery. RESULTS: In C, there was significant bone loss in all ROI during the first months and a deficit of 29 % in ROI 7 following one year. In B, bone loss was completely prevented up to the second month, in ROI 7, a significant difference in comparison to C was registered for the entire year. In A, significant bone loss reduction during 12 months was seen. CONCLUSION: Alendronate, therefore, is capable of preventing initial periprosthetic bone loss. A dosage of 20 mg/d is required initially with daily treatment lasting at least 10 weeks.  相似文献

Background and purpose — Bone remodeling as a response to bone trauma, postoperative immobilization, and device-related bone reactions can lead to loss of bone stock and increase the risk of periprosthetic fracture and aseptic loosening. This study investigates the adaptive bone remodeling of the proximal tibia after uncemented total knee arthroplasty (TKA).

Patients and methods — We performed a 2-year follow up of 53 patients (mean age 62 (38–70) years, 27 of whom were men, who received an uncemented TKA in a randomized controlled trial with bone mineral density (BMD) as secondary endpoint. Patients were randomized to 2 groups of either monoblock (A) or modular (B) polyethylene design. The TKAs were performed using the uncemented Zimmer Nexgen trabecular metal. Measurements of BMD were done postoperatively and after 3, 6, 12, and 24 months. BMD was measured in 3 regions of interest (ROI).

Results and interpretation — In group A statistically significant changes in BMD were seen after 24 months in both the medial and lateral ROI. BMD decreased medially by 15% (p = 0.004) and laterally by 13% (p = 0.01). In group B the BMD changes were limited and after 24 months returned to the preoperative values. The differences in BMD change between groups were statistically significant in both the medial (p = 0.03) and lateral (p = 0.02) ROI. In the distal ROI we found no significant change in BMD in either group. A significantly different bone remodeling pattern of the proximal tibia was seen in the 2 groups with a higher degree of bone loss in the knees that received the monoblock polyethylene design, indicating that the flexible monoblock implant design, previously shown to improve fixation, does not decrease the bone loss of the proximal tibia.

AIM: This prospective study describes the periprosthetic BMD (bone mineral density) after implantation of the cementless Bicontact stem over a period of 4 years. Additionally, the influence of several relevant parameters like age, sex, BMI (body mass index) and the influence of the stem-size on the periprosthetic BMD course were investigated. METHOD: Measurement of the periprosthetic BMD in 36 patients (15 female, 21 male) in a prospective longitudinal study over 4 years with DEXA (dual energy X Ray absorptiometry) of the proximal Gruen zones 1 and 7. RESULTS: We found a good precision of the method in vivo with a CV (coefficient of variation) of 1.04 %. There was a decline of the periprosthetic BMD at the whole proximal stem (ROI 0) of 20 % in the first 6 months. Afterwards there was a significant increase at the lateral stem (ROI 1) of 3.46 %, in contrast no significant change could be found medially, maybe as a result of a more lateral induction of power. The investigated parameters BMI, age and sex had no influence on the relative course of mineralisation. At stem sizes bigger than 15 the decline of BMD was significantly larger than at the stem sizes smaller than 15. CONCLUSION: DEXA is a sufficiently precise method to investigate periprosthetic BMD. The postoperative periprosthetic course of mineralisation and the extent of BMD loss in the first 6 months after the Bicontact stem is similar to other periprosthetic BMD courses of prosthesis with a similar design. The course of mineralisation in "smaller" stems is obviously more auspicious than the course of "bigger" stems.  相似文献

 We sequentially measured the periprosthetic bone mineral density (BMD) of the femur after cementless total hip arthroplasty, using dual-energy X-ray absorptiometry, over a 3-year period. The periprosthetic bone was divided into three regions (proximo-medial, middle, and distal to the prosthetic stem). After the insertion of a fully porous coated stem in 21 patients, the BMD was measured within 3 weeks, and 6, 12, 24, and 36 months after surgery. At 6 months, all zones showed a decrease in BMD relative to the BMD within 3 weeks, but subsequently the BMD was unchanged. The lower the BMD within 3 weeks of surgery, or the lower the body weight, the higher the percent loss of BMD at 6 months. Introduction Total hip arthroplasty changes bone loading conditions in the proximal femur and induces adaptive remodelling of the periprosthetic bone. These remodelling processes depend on many implant-specific qualities, e.g. material and elasticity of the stem. The objective of this study was to investigate the effect of the stem design on periprosthetic bone remodelling after insertion of an anatomic stem with proximal fixation and the direct comparison to a straight stem prosthesis. Materials and methods In a prospective study, the changes in periprosthetic bone mineral density (BMD) after implantation of 68 CTX-S anatomic and 22 PPF straight stem prostheses were assessed in the first post-operative year by means of DEXA and zone analysis by Gruen (Clin Orthop 141:17–27, 1979) “Modes of failure” of cemented stem-type femoral components: a radiographic analysis of loosening.. Furthermore all patients with CTX-S prostheses were monitored in the second post-operative year. The correlation of adaptive bone remodelling and the systemic bone density was also investigated. Results In the distal one-third of the straight stem prosthesis, a clearly greater, although not significant, hypertrophy of the periprosthetic bone was observed. No differences in the extent of bone loss between the two prostheses in the regions of interest (ROI) of the proximal bone were observed. The greatest decrease in BMD was registered in the medial femoral neck in both groups. Bone atrophy decreased progressively as the ROI moved distally, ending in a slight increase in BMD in the distal ROI. No significant changes in periprosthetic BMD occurred in the second post-operative year. A strong positive correlation in the regions with the greatest BMD decrease with the systemic BMD was ascertained. Conclusion After implanting a CTX-S prosthesis, as opposed to PPF prostheses, a different pattern of periprosthetic bone remodelling with a slighter hypertrophy of the distal periprosthetic parts was observed. This implies that the extensive proximal, more physiological bone loading of the anatomic stem as well as the removal of less bone while implanting the stem reduces the negative effects of unphysiological strain distribution and stress shielding. The BMD loss in the medial proximal neck cannot be avoided with this stem design either. The lack of significant BMD changes in the second post-operative year suggests that a stabilisation of bone remodelling processes occurs.  相似文献

We evaluated the effects on bone mineral density (BMD) of a 12-month treatment with goserelin depot, a gonadotropin-releasing hormone agonist, in a group of women with symptomatic uterine myomas requiring hysterectomy. Sixteen women, mean age 45.6±5.0, reporting menorrhagia associated with uterine myomas, candidates for hysterectomy, were scheduled to be treated with goserelin depot for 12 months. BMD was measured at the vertebral (L2–L4) and proximal femur level (femoral neck and trochanter) at the start of therapy and 6, 12, and 18 months later using dual energy X-ray absorptiometry (Hologic QDR 1000/W). The patients were followed for a minimum of 6 months after the end of treatment. Thirteen of the 16 women enrolled completed the treatment and three suspended it after 5, 6, and 7 months, respectively, because of side effects (hot flashes, insomnia, depression). Of the 13 women who completed the treatment, three underwent hysterectomy because of myoma regrowth and the recurrence of symptoms 3–18 months later; four reached the menopause 5–16 months later, and six were all menstruating normally with a follow-up varying from 6 to 18 months. After 12 months of therapy we observed a bone loss at vertebral, femoral neck, and trochanter of 4.4% (P<0.05 versus baseline; P = not significant versus 6 months), 7.5% (P<0.01 versus baseline, P<0.01 versus 6 months), and 7.6% (P<0.001 versus baseline, P<0.05 versus 6 months), respectively. Six months later, BMD increased slightly and not significantly at different sites (0.9% at the spine, and 0.3% at femoral neck, and 1.1% at trochanter). A 12-month treatment with goserelin may avoid the need for hysterectomy in women over 40 with symptomatic myomas. However, this therapy is associated with a marked bone loss which is not significantly reversed at its suspension.  相似文献

Introduction: Resurfacing hip arthroplasty (RHA) has been suggested to provide an alternative to conventional total hip arthroplasty in younger, active patients. It seems to have an ability to conserve the bone mass on the femoral side. Some controversy exists regarding to the possible disadvantages of RHA and some of them are connected to poor femoral bone quality after surgery. Hence we wanted to study the bone mineral density changes 3 and 12 months after RHA. Materials and Methods: A total of 26 patients (22 men and 4 women, 28 hips) underwent a hip resurfacing arthroplasty. The mean age of the patients was 55,2 (range 38-69) years. Bone mineral density (BMD) of the proximal femur was measured by using the dual-energy X-ray absorptiometry (DXA) postoperatively and within 3 and 12 months from surgery. For analysis, we divided the femoral neck area into four equal-sized regions of interest ranging from the prosthesis to the trochanter level. Results: At three months follow-up the BMD changes varied between -5.1% (ROI C) and + 1.9% (ROI A), as compared with the immediate postoperative values. After one year follow-up the BMD changes were + 1.1% in the ROI A, + 5.4% in the ROI B, -3.9% in the ROI C and + 1.3% in the ROI D. The changes in BMD were not statistically significant. Discussion: While there is still much debate and room for additional research in this topic, the results suggest that BMD is conserved in the femoral neck one year after hip resurfacing arthroplasty.  相似文献

Method?The acetabular components were implanted in hydroxyapatite (HA) and HA-removed options. They were implanted in conjunction with a cemented femoral component in 50 female patients who presented with displaced, subcapital, fractures of the neck of the femur. Regions of interest (ROI) were defined according to De Lee and Charnley. BMD during follow-up was compared with immediate postoperative values for the affected limb.

Results?The mean precision error (CV%) was 1.01%, 2.26% and 1.12%, for ROI I, II and III respectively. The mean change in BMD, for both cups, was analyzed. There was no significant difference between the BMD changes induced with the HA- and non-HA-coated cups.

Interpretation?After an initial fall in BMD in all 3 ROI at 6 months, ROI I and ROI II showed return to baseline BMD by 2 years. ROI III showed no significant decrease in BMD beyond 6 months, but did not return to baseline levels. Statistical analysis revealed no significant decrease in BMD in ROI I and ROI II at 2 years, compared with immediate postoperative values. The changes in BMD reflect a pattern of maximally reduced stress in the non-weight-bearing zone (ROI III), with preservation of bone density in weight bearing zones ROI I and ROI II. BackgroundPeriprosthetic bone mineral density (BMD) may influence implant fixation and subsequent loosening. Unicompartmental knee arthroplasty (UKA) restores normal knee kinematics and load distribution to the surrounding bone. We studied the influence of systemic and periprosthetic BMD of the proximal tibia on migration of the tibial component of cemented medial UKA.MethodsThe cohort was dichotomized into a normal BMD group (T-score ≥ −1; n = 37) and a low BMD group (T-score < −1; n = 28) according to World Health Organization criteria. BMD of the proximal tibia and migration of the tibial component were measured with dual X-ray absorptiometry scans and stereoradiographs with 2-year follow-up.ResultsPatients with normal systemic BMD had an 11% to15% higher BMD in all regions of interest (ROIs) compared to patients with low systemic BMD throughout follow-up. Over time, a decrease in periprosthetic BMD in ROI 1-3 was seen for both groups. The operated knees and contralateral knees showed a similar reduction in BMD in all ROIs between preoperative and 24 months.Between 12 and 24 months, the normal BMD group migrated (maximal total point motion) 0.03 mm (95% confidence interval, −0.01, 0.08) and the low BMD group migrated 0.02 mm (95% confidence interval, −0.03, 0.07). Migration over time was not influenced by change in periprosthetic BMD.ConclusionMigration of cemented medial tibial UKA was low until 24 months and was neither affected by preoperative systemic BMD nor affected by postoperative change in periprosthetic BMD. This suggests good long-term fixation despite an index difference in proximal tibial BMD.  相似文献

The influence of circulating sex hormones and gender on the bone mineral density (BMD) in long-term renal transplant recipients needs further investigation. We performed a retrospective analysis of lumbar BMD between 6 years and 20 years after renal transplantation. In 67 patients (47±12 years, 38 male) with a minimum interval of 72 months after transplantation, lumbar BMD measurements (dual energy X-ray absorptiometry) were performed (=complete cohort). Thirty-one patients (=longitudinal cohort) underwent at least three serial BMD measurements (mean follow-up 39±18 months, start at 86±22 months). All patients received prednisolone. In the complete cohort, BMD was significantly reduced in comparison to young healthy (mean T -score –1.33±1.40) and age-matched controls (mean Z -score –0.91±1.45) at 88±31 months ( p <0.05). Osteopenia or osteoporosis were present in two-thirds of patients. In the longitudinal cohort, a mean annual lumbar BMD loss of –0.6±1.9% was detectable equivalent to a –0.03±0.15 reduction of Z -scores per year (regression analysis). Impact of hormonal status: In the complete cohort, postmenopausal status was associated with significantly lower BMD levels compared to men ( p =0.0441). Women and men within the lowest tertile of sex hormone levels (LH, FSH, DHEAS, testosterone, progesterone, estradiol) did not exhibit significant differences in terms of lumbar BMD compared to those in the highest tertile. The mean annual bone loss was statistically indistinguishable between men and women. There was no significant correlation of sex hormone levels and BMD in men and premenopausal women. In postmenopausal women, however, low estradiol and high LH levels correlated with the extent of annual BMD loss ( p <0.05). Our data confirm significantly reduced lumbar T -scores in the very late period after renal transplantation. The lumbar BMD decreased by –0.6±1.9% per year. In postmenopausal long-term renal transplant recipients, low estradiol levels were associated with accelerated bone loss.The data were presented in part at the 37th annual meeting of the American Society of Nephrology, ASN 2004 (Renal Week 2004)  相似文献

We measured the early adaptive bone remodeling of the distal femur prospectively for 1 year after uncemented total knee arthroplasty (TKA) in 29 knees with primary arthrosis. 18 patients were randomized to receive a PCA Modular® femoral component (n 9) or a modified version of the same prosthesis (n 9) with an altered location of the porous coating. the other 11 patients (n 11) formed a consecutive series with the Duracon® femoral component. in the trabecular bone above the femoral component, bone mineral density (BMD) was measured in 2 regions of interest (ROI) anteriorly to the fixation pegs (RO11) and above the pegs (ROI 2), using dual photon absorptiometry (DPA). There were no differences between the Modular component and the modified version regarding the postoperative decrease in BMD. There was a decrease in BMD in both ROI 1 and ROI 2 with all 3 different femoral components, and in both ROIs the highest bone loss rate was observed during the first 3 months after surgery. on average (n 29), a significant bone loss of 44% and 19% in RO11 and ROI 2, respectively, was reached at the 1 -year follow-up, compared to the initial values. A decrease of this magnitude in BMD in the anterior distal femur 1 year after TKA may be an important determinant of periprosthetic fracture and later failure of the femoral component. in this experimental set-up, a modified femoral component with an altered location of the porous coating did not influence the development of bone loss.  相似文献

Purpose

Regenerex is a novel porous titanium construct with a three-dimensional porous structure and biomechanical characteristics close to that of normal trabecular bone. The aim of this study was to evaluate the adaptive bone remodeling of the proximal tibia after uncemented total knee arthroplasty (TKA) using a tibial tray with this novel coating compared to a well-proven standard porous coated (PPS) tibial tray.

Materials

Sixty patients scheduled for TKA were randomized to receive either a Regenerex (n?=?31) or a PPS tibial component (n?=?29). Changes in bone mineral density (BMD) of the proximal tibia were measured at three, six, 12 and 24 months by dual-energy X-ray absorptiometry (DEXA).

Results

In the lateral region (ROI 3), a significant increase in BMD was seen in both groups at three, six, and 12 months after surgery. The relative increase at 12 months was 8.1 % (P?=?0.007) for the PPS group and 6.5 % (P?=?0.002) for the Regenerex group. Positive values were retained at 24 months in both groups. At 24 months BMD in the distal region below the central stem (ROI 1) had decreased in the PPS group by 3.4 % (P?=?0.005) and in the Regenerex group by 2.4 % (P?=?0.17). In the medial region (ROI 2) BMD remained unchanged at all follow-up evaluations in both groups. There were no significant differences between the two groups (P?=?0.45) in any ROI at any follow-up evaluation.

Conclusion

The significant increase in BMD of the lateral proximal tibia plateau with very limited changes medially and distally seen in both implants suggests that the novel porous titanium construct Regenerex and the PPS implant have a pronounced beneficial effect with regard to maintaining periprosthetic BMD in all regions of interest investigated.

Introduction: Joint prosthesis survival is associated with the quality of surrounding bone. Dual-energy X-ray absorptiometry (DXA) is capable to evaluate areal bone mineral density (BMD) around different prosthetic implants, but no studies evaluated periprosthetic bone around total ankle replacement (TAR). Our aim is to determine the precision of the DXA periprosthetic BMD around TAR. Methodology: Short-term precision was evaluated on 15 consecutive patients. Each ankle was scanned 3 times both in the posteroanterior (PA) and lateral views with a dedicated patient positioning protocol. Up to four squared regions of interest (ROIs) were placed in the periprosthetic bone around tibial and talar implants, with an additional ROI to include the calcaneal body in the lateral scan. Coefficient of variation (CV%) and least significant change were calculated according to the International Society for Clinical Densitometry. Results: The lateral projection showed lower mean CV values compared to the PA projection, with an average precision error of 2.21% (lateral scan) compared to 3.34% (PA scans). Overall, the lowest precision error was found at both “global” ROIs (CV = 1.25% on PA and CV = 1.3% on lateral). The highest CV value on PA was found at the medial aspect of talar side (ROI 3; CV = 4.89%), while on the lateral scan the highest CV value was found on the posterior aspect of talar side (ROI 2; CV = 2.99%). Conclusions: We found very good reproducibility BMD values of periprosthetic bone around TAR, that were comparable or even better compared to other studies that evaluated periprosthetic BMD around different prosthetic implants. DXA can be used to precisely monitor bone density around ankle prostheses, despite further long-term longitudinal studies are required to assess the clinical utility of such measurements.  相似文献

Background and purpose Factors that lead to periprosthetic bone loss following total hip arthroplasty (THA) may not only depend on biomechanical implant-related factors, but also on various patient-related factors. We investigated the association between early changes in periprosthetic bone mineral density (BMD) and patient-related factors.Patients and methods 39 female patients underwent cementless THA (ABG II) with ceramic-ceramic bearing surfaces. Periprosthetic BMD in the proximal femur was determined with DXA after surgery and at 3, 6, 12, and 24 months. 27 patient-related factors were analyzed for their value in prediction of periprosthetic bone loss.Results Total periprosthetic BMD was temporarily reduced by 3.7% at 3 months (p < 0.001), by 3.8% at 6 months (p < 0.01), and by 2.6% at 12 months (p < 0.01), but recovered thereafter up to 24 months. Preoperative systemic osteopenia and osteoporosis, but not the local BMD of the operated hip, was predictive of bone loss in Gruen zone 7 (p = 0.04), which was the only region with a statistically significant decrease in BMD (23%, p < 0.001) at 24 months. Preoperative serum markers of bone turnover predicted the early temporary changes of periprosthetic BMD. The other patient-related factors failed to show any association with the periprosthetic BMD changes.Interpretation Female patients with low systemic BMD show greater bone loss in Gruen zone 7 after cementless THA than patients with normal BMD. Systemic DXA screening for osteoporosis in postmenopausal patients before THA could be used to identify patients in need of prophylactic anti-resorptive therapy.  相似文献

Purpose

Periprosthetic bone loss around the femoral stem is frequently found after total hip arthroplasty. We have shown that periprosthetic bone mineral density (BMD) loss using the triple tapered stem is consistently much less in comparison with the straight type component. In this study, we compared periprosthetic BMD change with clinical factors.

Methods

Postoperative dual-energy X-ray absorptiometry was evaluated at follow-up. BMD was determined based on seven Gruen zones. We further compared BMD with clinical examination: body mass index (BMI), age, Harris hip score (HHS) or University of California at Los Angeles (UCLA) activity rating score.

Results

Periprosthetic BMD loss of the triple tapered stem was maintained. Especially, BMD in Gruen zone 1 which was maintained at 96% in comparison with the straight tapered stem. We compared the BMD change with clinical factors. There is no correlation between BMD and BMI, age or HHS. However, we found significant correlation between BMD and UCLA activity rating score in Gruen zones 1 and 2 of the triple tapered stem. Further, the correlation coefficient was increased at 48 months in comparison with 24 months.

Conclusion

The cementless triple tapered stem maintains periprosthetic bone mineral density. Activity may reflect improving periprosthetic bone quality after THA using a triple tapered stem.
