College students engage in risky alcohol use within a variety of contexts, including specific celebratory events. Student intentions and peer perceptions predict alcohol use; however, how these factors affect specific celebratory drinking may vary from typical alcohol use. The current study sought to better understand event-specific drinking among college students during St. Patrick’s Day, as compared to Spring Break. Undergraduate students (N = 82) at a campus with a unique traditional celebration of St. Patrick’s Day were surveyed. At Time 1, participants were asked to indicate how much alcohol they intended to drink and how much alcohol they expected other students to drink during St. Patrick’s Day and Spring Break. At Time 2, students reported on actual alcohol consumption during both events. Results indicated that participants reported greater intent to consume, expectation of peer consumption, and actual alcohol consumption during St. Patrick’s Day as compared to Spring Break. Neither sensation seeking nor impulsivity predicted alcohol use during either event. Findings are discussed in the context of understanding, preventing, and intervening with event-specific drinking among college students.  相似文献

Exposure to a variety of toxins and/or infectious agents leads to disease, degeneration and death, often characterised by circumstances in which cells or tissues do not merely die and cease to function but may be more or less entirely obliterated. It is then legitimate to ask the question as to whether, despite the many kinds of agent involved, there may be at least some unifying mechanisms of such cell death and destruction. I summarise the evidence that in a great many cases, one underlying mechanism, providing major stresses of this type, entails continuing and autocatalytic production (based on positive feedback mechanisms) of hydroxyl radicals via Fenton chemistry involving poorly liganded iron, leading to cell death via apoptosis (probably including via pathways induced by changes in the NF-κB system). While every pathway is in some sense connected to every other one, I highlight the literature evidence suggesting that the degenerative effects of many diseases and toxicological insults converge on iron dysregulation. This highlights specifically the role of iron metabolism, and the detailed speciation of iron, in chemical and other toxicology, and has significant implications for the use of iron chelating substances (probably in partnership with appropriate anti-oxidants) as nutritional or therapeutic agents in inhibiting both the progression of these mainly degenerative diseases and the sequelae of both chronic and acute toxin exposure. The complexity of biochemical networks, especially those involving autocatalytic behaviour and positive feedbacks, means that multiple interventions (e.g. of iron chelators plus antioxidants) are likely to prove most effective. A variety of systems biology approaches, that I summarise, can predict both the mechanisms involved in these cell death pathways and the optimal sites of action for nutritional or pharmacological interventions.  相似文献

Introduction: As the most common major neurocognitive disorder, Alzheimer’s disease (AD) will play an increasingly important role both socially and financially as the population ages. Approved treatments for AD are symptomatic in nature and show modest improvements in cognition and global functioning among patients with AD.

Areas Covered: This article focuses on the pharmacokinetics, pharmacodynamics, efficacy, and safety of the transdermal patch form of the cholinesterase inhibitor rivastigmine. The rivastigmine transdermal system is approved for the treatment of patients with mild, moderate, and severe AD. Three randomized trials have shown the rivastigmine patch to be efficacious and tolerable across all stages of AD.

Expert Opinion: The rivastigmine patch offers several advantages over the capsule form, including decreased peak to trough plasma fluctuations, reduced rates of nausea and vomiting, better treatment adherence, higher probability of reaching the target dose, ease of administration, and greater satisfaction among caregivers. These factors may be especially important in patients with severe AD, in which patients are more vulnerable to adverse side effects from higher doses. While the patch is more expensive than generic therapies, patient populations that may benefit from the patch include those that are particularly sensitive to GI side effects, have chronic gastrointestinal problems, have difficulty swallowing medications, or have failed to respond with high doses of other generic options.  相似文献

Background: Alcohol craving is common among adolescents, stronger among those with more alcohol-related problems, and predicts drinking levels in their daily lives. Yet, the conditions that predict momentary changes in craving in real time among adolescents remain unclear. Objectives: This study examined the interactive effects of momentary risk-taking propensity and affect on adolescents’ alcohol craving by leveraging ecological momentary assessment (EMA) methods. Methods: Participants were 29 adolescents ages 15–19?years (55% female; 69% White; 10% Black; 17% Hispanic); 45% met criteria for alcohol dependence. Following a laboratory session that captured self-report and behavioral assessments, including the well-established Balloon Analog Risk Task (BART), participants completed multiple daily assessments of alcohol craving, positive and negative affect, and risk propensity for approximately one week. Momentary risk propensity was captured in real-world settings via an EMA behavioral task (“Balloon Game”). Results: Mixed-effects models with EMA reports (Level 1) nested within participants (Level 2) revealed the majority (74%) of variability in “Balloon Game” performance was due to within-person, momentary, fluctuations. Greater momentary positive affect predicted increased alcohol craving, but only when participants exhibited heightened risk-taking propensity. Negative affect did not influence the relation between momentary risk-taking and craving. Conclusions/Importance: Momentary fluctuations in positive affect predicted acute increases in craving but only in moments when adolescents demonstrated higher levels of risk-taking propensity, as captured with an EMA-delivered behavioral task. Momentary risk-taking assessments offer new avenues to substantiate dominant theories on the driving mechanisms of craving and alcohol use among adolescents.  相似文献

Abstract

Aim: This article examines the impact of new psychoactive substances (NPS) on drug service interventions using a case study of professional practitioners in South East England. We assess how professionals seek to develop an innovative approach towards providing ‘sensible drug information.’

Methodology: The research methods include observations, and individual and collective ethnographic interviews with 13 professionals who work with young people across the region.

Results: The article theorises sensible because it is a key element in contemporary drug education with a harm reduction approach. Therefore, we take up this challenge and use the ideas of Gilles Deleuze, which according to Mazzei and McCoy ‘prompts the possibilities of new questions and different ways of thinking research’. We identify a series of drug intersectionalities between ‘traditional’ illegal drugs and NPS and through social class differences between young affluent and more socially disenfranchised drug users. This article assesses the delivery of ‘sensible drug information’ as part of a harm reduction approach, which may not always be supported by other agencies. In responding to these challenges we explore Deleuze’s ideas as a foundation for ‘sensible’ drug information which incorporates Matza?s theory of drift, to explain young people?s changing pattern of drug consumption.  相似文献

This study investigated interrelations between life events, psychosocial distress, self-efficacy, and substance use among patients manifesting substance use disorders. Gender differences in these interrelations were also explored. Patients (N = 352) were recruited during 2008 and 2009 from 16 Norwegian facilities. These patients completed a questionnaire with validated measurement instruments. Interrelations were investigated by SEM analysis. The results suggested that negative life events facilitated substance use and psychological distress. Positive life events were associated with self-efficacy, but weakly related to substance use. The results supported the notion that males are more prone to use substances when faced upon negative life events.  相似文献

BackgroundThe U.S. Food and Drug Administration (FDA) restricts dispensing of mifepristone for medication abortion to certified health care providers at clinical facilities, thus prohibiting pharmacist dispensing. Allowing mifepristone dispensing by pharmacists could improve access to medication abortion.ObjectiveTo assess the feasibility of pharmacists dispensing mifepristone to patients who have undergone evaluation for eligibility and counseling for medication abortion by a clinician.MethodsBefore providing a study training on medication abortion, we administered baseline surveys to pharmacists who participated in a multisite mifepristone-dispensing intervention. The survey assessed medication abortion knowledge—using a 15-item score—and perceptions about the benefits and challenges of the model. We administered follow-up surveys in the study’s final month that also assessed the pharmacists’ satisfaction and experiences with mifepristone dispensing. To investigate the association of the study intervention with the pharmacists’ knowledge, perceptions, and experiences dispensing mifepristone, we conducted multivariable linear regression analyses using generalized estimating equation models, accounting for clustering by individual.ResultsAmong the 72 pharmacists invited from 6 pharmacies, 47 (65%) completed the baseline surveys, and 56 (78%) received training. At the study’s end (mean 18 months later), 43 of the 56 pharmacists who received training (77%) completed the follow-up surveys. At follow-up, 36 (83%) respondents were very or somewhat satisfied with mifepristone dispensing, and 24 (56%) reported experiencing no challenges dispensing mifepristone. Four (6%) of the 72 pharmacists invited objected to participating in mifepristone dispensing. In regression analyses, average knowledge scores, perceived ease of implementation, and level of support for the pharmacist-dispensing model were higher at follow-up (P < 0.001).ConclusionMost pharmacists were willing to be trained, dispensed mifepristone with few challenges when given the opportunity, were satisfied with the model, and had higher knowledge levels at follow-up. Our findings support removal of FDA’s restriction on pharmacist dispensing of mifepristone.  相似文献

Background: There is a wealth of research on motives for alcohol consumption among young people. However, little is known about motives to avoid alcohol in this population. Objectives: The study purpose was to explore what influences young adults’ decisions to avoid alcohol and their motives to avoid alcohol. Methods: Face-to-face, semi-structured, one-on-one interviews were conducted in 2015 with young adults (n?=?30, Mage = 21.13?years, SD = 2.05) living in Australia who did not consume alcohol regularly. Interviews were analyzed using thematic content analysis. Results: Thematic analysis resulted in seven themes: being in control; avoiding negative health consequences; taste; socialization influences; being left out; peer pressure; strategies to curb excessive alcohol consumption. Conclusions/Importance: Findings from the present study contribute to the literature in identifying coping strategies that participants adopted when faced with questions concerning their abstinence. The data provide evidence that, even in a minority, strong identities and beliefs appear to be a robust means to counteract pressure to conform to the social norm to consume alcohol. Findings may inform the (1) development of youth-centered interventions that target values and social norms to help build resistance to pressures to consume alcohol from peers and the wider community and (2) creation of opportunities and promotion of activities that are fun and alcohol free.  相似文献
