Self-cured hydrocephalus following ventriculitis--clinical experience and experimental consideration (author's transl)]

Dynamics of CSF circulation is often altered in the presence of intracranial pathology especially in hydrocephalus. Most neurosurgeons acknowledge that the status of hydrocephalus often changes regardless of the type of treatment. In the follow-up of hydrocephalic children who under-went shunting operation, not a few hydrocephalic children have been doing well in spite of shunt malfunction for a long period. Interestingly enough, we have experienced the children who need no longer shunt after shunt infection complicated by ventriculitis. These cases differ from so-called spontaneously "arrested hydrocephalus", in that the latter develops in not severe hydrocephalus during the course of natural history or after shunt not complicated by serious infection. Consequently, we define these cases "self-cured hydrocephalus" following ventriculitis. Recently we experienced a similar case as mentioned above at the Kyoto University Hospital. We reported the case and made some inquiry into pathogenesis of "self-cured hydrocephalus". We hope that further inventigation of the similar cases along with experimental studies might give us a key to establish a radical treatment for hydrocephalus.

78 patients were operated on from 1st July to 31th December 1976 according to the method of Roux-en-Y-loop under several indications concerning primary and secondary reconstructive procedures in abdominal surgery. The experiences made are good. We recommend this method.

The cancerologically correct operation for nephroblastoma (author's transl)]

The following methodology is suitable for increasing the radicality of and the safety of the vessels during a transperitoneal operation for nephroblastoma and simultaneously for sparing the necessity of postoperative radiation: 1. primary exposure of the renal vascular cross, the vena cava, and the aorta through incision of the plica duodeno jejunalis together with mobilization of the duodenum to the right, the mesocolon to the left; 2. primary ligature of the a. renalis in front of the v. renalis (no problems on either side); 3. systematic retroperitoneal en bloc lymphodissection from the a. mesenterica superior to the forking of the aorta, laterally to the diaphragm, probative at the adnexa; 4. subsequent lateral release of the left-sided tumorous kidney, medial release of the right-sided tumorous kidney after mobilization of the cecum.     

The new iglesias resectoscope (author's transl)]

The new Iglesias resectoscope with continuous irrigation has been described. Advantages of this instrument include: no interruption of the TUR, better endoscopic vision, decreased and equally strong intravesical pressure, shorter operating time, less bleeding, easier teaching and performing of transurethral resections.

The Balloon-Trokar-Catheter can be recommended as a simple and quick method for a suprapubic urinary diversion (cystostomy). The instrument and its application are described.
