Rightward deviation on line bisection is considered one of the most classic clinical signs of unilateral visual neglect--a cognitive disorder of spatial processing that commonly follows right brain damage. Recently, short-term adaptation to wedge prisms has been shown to significantly reduce neglect on this and other conventional diagnostic tasks. Our previous study has shown that visuomotor adaptation in normals produces a similar pattern of directional bias on a line bisection task. Based on the good working knowledge of how neglect patients perform on different versions of the standard diagnostic task, we showed here that using leftward-deviating prisms in normals, it is possible to produce: (1) a reliable bias on line bisection, (2) a rightward specific deviation, (3) a modulation of rightward deviation, which depends on the relative spatial location of the target lines and (4) a line length effect. A final experiment confirmed that these after-effects are specific to prism adaptation rather than passive prism exposure. Collectively, these findings confirm that adaptation to left-deviating prisms in normals produces a reliable right-sided bias and as shown by a previous visuospatial judgement task, these findings cannot be adequately explained by the symmetric sensori-motor effects of prism adaptation. Taken together with the improvement of spatial neglect shown by right-deviating prisms only, the present study suggests that low level sensori-motor adaptations play a greater role in right hemisphere organisation for spatial cognition than previously thought.  相似文献

Unilateral neglect has been recently shown to be improved following a short period of adaptation to wedge prisms. The present study investigates whether visuo-motor adaptation can generate a bias in normals on line bisection tasks classically used to assess unilateral neglect. Our results show that adaptation to left-deviating prisms induces a stronger rightward bias for the perceptual than the motor line bisection task. This bias is in the same direction as the one observed in unilateral neglect. No significant effect is produced by adaptation to right-deviating prisms. Our data confirm that the plasticity of inter-sensory and sensori-motor coordinations affects higher levels of space representation. These asymmetric results may reflect the inherent bias of the brain's structural organisation and provide an empirical explanation for the left-sided predominance of unilateral neglect.  相似文献

IntroductionUnilateral visuospatial deficits have been observed in children with brain damage. While the effectiveness of prism adaptation for treating unilateral neglect in adult stroke patients has been demonstrated previously, the usefulness of prism adaptation in a pediatric population is still unknown. The present study aims at evaluating the feasibility of prism adaptation in children with unilateral brain lesion and comparing the validity of a game procedure designed for child-friendly paediatric intervention, with the ecological task used for prism adaptation in adult patients.MethodsTwenty-one children with unilateral brain lesion randomly were assigned to a prism group wearing prismatic glasses, or a control group wearing neutral glasses during a bimanual task intervention. All children performed two different bimanual tasks on randomly assigned consecutive days: ecological tasks or game tasks. The efficacy of prism adaptation was measured by assessing its after-effects with visual open loop pointing (visuoproprioceptive test) and subjective straight-ahead pointing (proprioceptive test).ResultsGame tasks and ecological tasks produced similar after-effects. Prismatic glasses elicited a significant shift of visuospatial coordinates which was not observed in the control group.ConclusionPrism adaptation performed with game tasks seems an effective procedure to obtain after-effects in children with unilateral brain lesion. The usefulness of repetitive prism adaptation sessions as a therapeutic intervention in children with visuospatial deficits and/or neglect, should be investigated in future studies.  相似文献

Visuomotor adaptation to rightward displacing optical prisms is known to induce temporary improvements in the symptoms of left visual neglect. We report a 74 year-old woman with severe and chronic neglect of nine months duration, who underwent three weekly sessions of prism adaptation. Substantial improvements were obtained on tests of visual neglect (cancellation, copying and bisection). Improvement was also observed on a spatial judgement task, with no explicit visual component, in which CS was required to locate the centre of a haptically explored circle. These observations confirm that brief periods of prism exposure can benefit even chronic neglect disorders. Moreover, the improvement observed on the haptic task supports the belief that this procedure can influence higher levels of spatial representation.  相似文献

Patients with unilateral spatial neglect due to right-hemisphere lesions typically fail to attend to and explore left-sided stimulus objects. It has been postulated that in right-brain damaged (RBD) patients an ipsilesional displacement of the egocentric frame of reference (ER), whether visual or tactile, may be responsible for a contralesional supramodal spatial bias causing their left neglect behavior. However, this hypothesis had been proposed without testing, in the same patients, the position of the ER or their performance in the visual and tactile modalities. Thus, the aim of the present study was to test the hypothesis that an ipsilateral shift of the ER is responsible for a supramodal spatial bias in neglect.For this purpose, a within-subject design is required. Consequently, 12 left neglect patients and 12 control subjects were asked to perform a proprioceptive straight-ahead pointing task while blindfolded, as well as visual and tactile bisection tasks.In the left neglect patients, we found:no systematic deviation of the ER on the ipsilesional right side;a significant rightward bias in visual bisection, and normal performance in tactile bisection;no correlation among the three tasks;that only visual bisection correlated with the severity of neglect.These results are discussed with regard to the egocentric and attentional hypothesis of neglect.  相似文献

A short period of prism adaptation (PA) has been shown to reduce spatial neglect symptoms. Recent evidence suggests that the positive effects of PA might be restricted to visually guided actions, with PA having little effect on perception. However, the majority of studies have adopted a concurrent exposure technique that fosters the development of a change in felt arm position (proprioceptive straight ahead, PSA). Few studies have used terminal exposure that promotes a change in the perceived visual direction (visual straight ahead, VSA). The positive effects of PA might appear to be primarily action based because studies have adopted an exposure technique that promotes a change in proprioception. Here, we compare the effects of the two exposure types on a perceptual and a manual line bisection task in healthy young adults. Before and after seven minutes of exposure to leftward displacing prisms we measured performance on two line bisection tasks (manual and perceptual) and perceived straight ahead (PSA and VSA). During the exposure period participants made pointing movements while the view of their pointing arm was either (i) restricted to the second half of the pointing movement (concurrent exposure) or (ii) restricted to the final part of the pointing movement (terminal exposure). In line with the previous research, concurrent exposure produced a large shift in PSA and a shift on the manual line bisection task. Interestingly, terminal exposure produced a large shift in VSA and a shift in performance on the perceptual line bisection task. Our results shed light on the underlying mechanisms of prism-induced neglect recovery and help to address an apparent discrepancy within the literature.  相似文献

Prism adaptation may alleviate some symptoms of spatial neglect. However, the mechanism through which this technique works is still unclear. This study investigated whether prism adaptation differentially affects dysfunction in perceptual-attentional 'where' bias versus motor-intentional 'aiming' bias. Five neglect patients performed a line bisection task in which lines were viewed under both normal and right-left reversed viewing conditions, allowing for the fractionation of 'where' and 'aiming' spatial bias components. After two consecutive days of prism adaptation, participants demonstrated a significant improvement in 'aiming' spatial bias, with no effect on 'where' spatial bias. These findings suggest that prism adaptation may primarily affect motor-intentional 'aiming' bias in poststroke spatial neglect patients.  相似文献

Recent research has shown that prism adaptation alleviates some of the symptoms of neglect. Although prism adaptation can aid patients with neglect, the mechanisms underlying these benefits remain largely unknown. One way in which prisms may work is by altering attentional orienting mechanisms known to be impaired in neglect. To investigate this hypothesis, we tested four right brain damaged patients (two with neglect) on a reflexive covert attention task before and after rightward prism adaptation and compared them with a group of healthy controls who underwent sham prism adaptation. Results demonstrated that rightward prism adaptation reduced both the rightward attentional bias, and the disengage deficit in patients with right brain damage irrespective of the presence of neglect.  相似文献

We asked 16 right-brain damaged patients with left unilateral neglect, 9 right-brain damaged patients without neglect and 11 healthy control subjects to bisect horizontal lines or to extend them rightward or leftward. Although in the line bisection task patients with neglect showed the typical rightward deviation of the subjective line midpoint, they did not show any significant difference when compared to the other two experimental groups in both rightward and leftward line extension tasks. These results suggest that neglect for the left side of space is usually observed in tasks requiring an automatic lateral orienting of attention, but not in tasks in which this lateral orienting is submitted to a continuous intentional control.  相似文献

OBJECTIVES: To clarify the mechanisms of left unilateral spatial neglect found in the bisection of lines after cueing to the left end point and to determine whether neglect occurs for the mental representation of a line. METHODS: A new representational bisection task was developed to eliminate the influence of the right segment of the physical line that would attract attention. Eight patients with typical left unilateral spatial neglect underwent line and representational bisection tasks on a computer display with a touch panel. In the line bisection with cueing, they bisected a line after touching the left end point. In the representational bisection, the patients were presented with a line until they touched the left end point. On the blank display, they pointed to the subjective midpoint of the erased line. The performances of the two bisection tasks were compared when the length and position of stimulus lines were varied. RESULTS: The rightward errors in the representational bisection were greater than or equivalent to those in the line bisection with cueing. The effect of line length in which the errors became greater for the longer lines was equally found in the line bisection with cueing and the representational bisection. This was confirmed in the condition where the right end point was placed at a fixed position and the line length was varied. CONCLUSIONS: After cueing to the left end point, rightward bisection errors of patients with neglect are not caused by overattention to the right segment of the physical line. Left neglect occurs mainly for the mental representation formed at the time of cueing or seeing the whole extent of a line.  相似文献

A failure to report or respond to stimuli presented in a portion of space is termed hemispatial neglect. Line bisection and line cancellation are two of the tests used most commonly to assess for neglect. Perhaps, because neglect can be induced by a variety of deficits, neither of these tests used alone is as sensitive as both used together. Hence, the primary purpose of this study was to assess the sensitivity, reliability and validity of a new test called the Character-line Bisection Task (CLBT) that combines features of both the bisection and cancellation tests. Since local attention and language are primarily mediated by and activate the left hemisphere, our second goal was to learn if the CLBT and especially the letter version induce a greater rightward bias than the solid-line bisection task. Eighty patients with acute right hemisphere stroke and 81 controls performed the CLBT that consists of two subtests, the Letter-line and Star-line Bisection tasks. All subjects also completed four conventional tests for neglect (Standard solid-line bisection, line cancellation, Star Cancellation, and figure copying). In the bisection tasks both patients and controls bisected to the right with the CLBT than with the solid-line bisection task, suggesting the CLBT induces asymmetrical hemispheric activation. This enhanced rightward deviation with the CLBT was the same for the Letter-line and Star-line Bisection tasks. In regard to sensitivity, we defined the presence of neglect syndrome based on a total score derived from performance of controls on all six tests. This total score detected 55 (68.8%) patients with neglect. Within this group, the Letter-line and Star-line tasks diagnosed neglect in 50 and 48 patients, respectively, resulting in the highest sensitivities (90.9, 87.3%) of the six tests. Thus, the CLBTs demonstrated higher sensitivities than the other commonly used neglect tests and these new tests can be useful for the detection and quantification of unilateral neglect.  相似文献

Previous studies have shown that adaptation to rightward displacing prisms improves performance of neglect patients on visuo-manual (VM) tasks such as line cancellation, figure copying, and line bisection [Nature 395 (1998) 166]. The present study further evaluated the effect of prism adaptation (PA) on neglect symptoms by investigating: (a) the range of beneficial effects on common visuo-spatial deficits as well as less frequent phenomena like neglect dyslexia; (b) the duration of improvement following a single exposure to the right optical deviation; (c) the extent to which visuo-spatial performance can be comparatively ameliorated in VM tasks and visuo-verbal (VV) tasks (i.e. involving or not the adapted arm, respectively) and (d) the presence and duration of the manual visuo-motor bias induced by the prismatic adaptation (i.e. the after-effect). We investigated these issues in a group of neglect patients with right hemispheric damage who were also affected by neglect dyslexia. Following a single, brief prismatic adaptation the results showed that (a) several visuo-spatial abilities, including accuracy in reading single words and non-words, considerably improved, (b) the amelioration was long-lasting, continuing for at least 24h, (c) the presence, amount, and duration of neglect amelioration was not limited to VM tasks, but extended to VV tasks and (d) the presence and duration of the after-effect induced by prismatic adaptation remarkably paralleled the presence and duration of the improvement of neglect symptoms. These findings clearly demonstrate that beneficial effects induced by a single PA are very long-lasting and spread over a wide range of visuo-spatial deficits, independent of the type of response required. In addition, our results strongly suggest that the process of adaptation, as revealed by the presence of a visuo-motor after-effect, might be essential for establishing amelioration. In light of its characteristics, the prismatic adaptation technique should be a priority tool for the rehabilitation of the multifaceted hemispatial neglect syndrome.  相似文献

Six patients with unilateral right hemisphere damage are reported. Each patient performed two tasks that are traditionally regarded as diagnostic for left (peripersonal) visuospatial neglect: target cancellation and horizontal line bisection. Two patients were unimpaired on both tasks, and two were impaired on both. The two remaining patients showed a classic (and reliable) double-dissociation between the tasks. One of the patients who scored within normal limits on both cancellation and bisection showed left sided omissions on representational drawing. We argue that these results question the validity of any unitary concept of unilateral visuo-spatial neglect in peripersonal space.  相似文献

Patients with right hemisphere lesions often show left spatial neglect and the typical rightward deviation in horizontal line bisection. Previous studies have shown that sensory stimulation modulates line bisection. A less well-known but promising sensory stimulation method is galvanic vestibular stimulation (GVS). This non-invasive technique leads to activation of the vestibular cortices and adjacent cortical areas in the temporo-parietal cortex via polarization effects of the vestibular nerves. This is accomplished by application of weak direct currents, delivered by two electrodes attached to the mastoids. Despite the relative benefits of GVS its effects on line bisection have not yet been studied in neglect patients. Thus, the present study investigated the impact of GVS on performance in a modified line bisection task in right-brain damaged patients with versus without leftsided visual neglect. In neglect patients, but not in control patients, left-cathodal and right-cathodal GVS significantly reduced the rightward line bisection error as compared to Baseline (without GVS) and sham stimulation. A larger decrease of the rightward line bisection error was observed during right-cathodal GVS. Sham stimulation showed no specific effects on line bisection. The beneficial effects of GVS might be due to activation of preserved structures of the lesioned right posterior parietal cortex which is known to be involved in line bisection.  相似文献

Past research associated unilateral neglect with a systematic ipsilesional shift of the perceived position of the body midline; however, this was not confirmed by recent experiments. We used the constant stimuli method to control for potential artifacts intrinsic to the techniques used in previous studies. Body midline perception was measured in the visual and proprioceptive modalities in ten patients with left unilateral neglect, ten control patients and ten normal subjects and compared with a visual line bisection task, also using the constant stimuli method. Neglect patients showed a significant rightward bias in the line bisection task, but no consistent directional bias either in the proprioceptive or in the visual body midline task. These results clearly counter the association between neglect and an ipsilesional shift of the body midline. However, in the body midline tasks neglect patients made more errors in judgement on both sides of their subjective midline, both with respect to the control groups and with respect to the line bisection task. This may imply that a specific impairment of body-centered representations is indeed present and manifests as a non directional increase in response variability, rather than as a systematic shift. It is suggested that body- and object-related tasks (such as line bisection) may be processed by independent cognitive computations. This interpretation is discussed with reference to a recent neuroimaging study investigating the same kinds of tasks.  相似文献

Whether an attentional gradient favouring the ipsilesional side is responsible for the line bisection errors in visual neglect is uncertain. We explored this by using a conjunction-search task on the right side of a computer screen to bias attention while healthy subjects performed line bisection. The first experiment used a probe detection task to confirm that the conjunction-search task created a rightward attentional gradient, as manifest in response times, detection rates, and fixation patterns. In the second experiment subjects performed line bisection with or without a simultaneous conjunction-search task. Fixation patterns in the latter condition were biased rightwards as in visual neglect, and bisection also showed a rightward bias, though modest. A third experiment using the probe detection task again showed that the attentional gradient induced by the conjunction-search task was reduced when subjects also performed line bisection, perhaps explaining the modest effects on bisection bias. Finally, an experiment with briefly viewed pre-bisected lines produced similar results, showing that the small size of the bisection bias was not due to an unlimited view allowing deployment of attentional resources to counteract the conjunction-search task's attentional gradient. These results show that an attentional gradient induced in healthy subjects can produce visual neglect-like visual scanning and a rightward shift of perceived line midpoint, but the modest size of this shift points to limitations of this physiological model in simulating the pathologic effects of visual neglect.  相似文献

Hemineglect was assessed in 34 patients with right-hemisphere stroke using a letter-cancellation task and a line bisection task. No significant correlation (r = .39) was found between scores on the two tests. Ten patients who showed neglect on the cancellation task but performed normally on line bisection had frontal or deep lesions. Eleven patients with posterior lesions deviated rightward on line bisection; several of these had minimal or no cancellation deficit. A nonmotor task involving judgment of a bisected line was also performed abnormally by six patients with line bisection shift, suggesting that such shift does not result from a motor response asymmetry. We propose that separable components of the neglect syndrome may be associated with damage to discrete areas of the nondominant hemisphere.  相似文献

Visuomotor prism adaptation has been found to induce a lateral bias of spatial attention in chronic hemispatial neglect patients. Here, two experiments were conducted to explore the effects of 10° prism adaptation on visual search tasks and standard visual inattention tests. Baselines and intervention effects were measured on separate days for all patients. The first experiment explored whether prism adaptation affects performance on a time restricted visual search task (maximum 3500 ms presentation followed by visual and auditory feedback). No positive effects of prism adaptation were found on accuracy in visual search nor on traditional neglect tests. These results accord well with previous studies showing that increased cognitive load can lead to prism de-adaptation or unchanged performance following prism adaptation. Response times in visual search became faster following intervention but this was not the case for the standard neglect tests. In the second experiment, the same single-featured search task was used, but the participants had unlimited search time and received no feedback on their response. This time, the patients showed accuracy improvements in visual search and all four on regular neglect tests. Therapeutic effects lasted for at least 90-120 min. Response times on all tasks became faster after prism adaptation. The results are consistent with studies showing effects of prism adaptation on neuropsychological neglect tests and other attentional tasks that are not speeded or time restricted, where feedback is not provided, or are performed following non-feedback-based tasks. The current findings show that prism adaptation improves visual search in neglect and that these beneficial effects can disappear with feedback.  相似文献

Prism adaptation improves visual and haptic manifestations of left neglect, and can induce a small but reliable simulation of left visual neglect in normal individuals. Here, we present two experiments in which the effects of prism adaptation on the representation of space were explored. In Experiment 1, normal subjects were required to locate the centre of a haptically explored circle, before and after adaptation to leftward displacing prisms. In Experiment 2, a visual circle centring task was used. In both tasks, prism adaptation induced a significant rightward shift of performance. In addition, in both experiments, three classical measures of visuo-manual adaptation were taken: the visual shift, the proprioceptive shift and the total shift. The effects found on the haptic and visual tasks did not correlate with any of these measures. This suggests that the effects of prism adaptation on the circle centring tasks did not depend directly on the sensorimotor consequences of the adaptation. These results imply that prism adaptation can affect noetic levels of space representation in normal subjects, supporting the hypothesis that this low-level sensorimotor intervention can exert a bottom-up structuring influence on higher levels of cognitive integration.  相似文献
