A retrospective study of 95 patients was undertaken to compare digital subtraction angiography (DSA) and conventional arteriography of the hand. Eighty patients had conventional angiography and 15 had DSA. In comparison with conventional angiography, DSA is more cost-efficient and facilitates outpatient angiography. It provides images as acceptable as those of conventional angiography. We conclude that intra-arterial DSA is now the procedure of choice for angiographic mapping of the digital arteries.  相似文献

PURPOSE: To assess how often rotational angiography depicts more severe internal carotid arterial stenosis compared with conventional intraarterial digital subtraction angiography (DSA) in two or three projections and how frequently this factor may affect patient treatment. MATERIALS AND METHODS: Rotational angiography (16 or 32 projections) was performed in addition to DSA in 47 stenotic internal carotid arteries (ICAs) in 38 symptomatic patients. ICA stenosis was measured independently at DSA and at rotational angiography with North American Symptomatic Carotid Endarterectomy Trial criteria. The degree of stenosis was categorized as 0%-29%, 30%-49%, 50%-69%, or 70%-99%. RESULTS: In three ICAs, rotational angiography was nondiagnostic. In 28 of the remaining 44 ICAs, the degree of stenosis was categorized similarly with DSA and rotational angiography, whereas with rotational angiography, 15 ICAs were classified one category higher and one ICA was classified two categories higher, owing to the increased number of projections available. Seventy percent to 99% stenosis was demonstrated in 18 ICAs with DSA and in 25 ICAs with rotational angiography. Thus, rotational angiography could have facilitated a change in the optimal treatment (from nonsurgical treatment to carotid arterial endarterectomy) in seven ICAs. CONCLUSION: Compared with DSA in two or three projections, rotational angiography frequently depicts more severe ICA stenosis. This indicates a limitation of DSA in depicting the maximum ICA stenosis.  相似文献

This prospective study compared images obtained with a photostimulable imaging plate with matched images obtained with a conventional screen-film combination in 26 patients undergoing intraoperative arteriography. Diagnostic accuracy of the two techniques was assessed objectively, and image quality was assessed subjectively. In 16 patients (62%), the radiation exposure was reduced by 50% for the imaging plate technique by decreasing the mAs level generally used for the screen-film combination. Because of the dynamic range of the imaging plate system, no repeat examinations were necessary, while 12% of the screen-film studies had to be repeated because of over- or under-penetration. Imaging plate studies required 6% more time for processing than screen-film studies. Receiver-operating-characteristic analysis indicated no difference in diagnostic accuracy between the two imaging techniques. Subjective evaluation also revealed no difference in observer preference for imaging plate or screen-film studies. The imaging plate technique is an excellent alternative to screen-film studies in the operating room.  相似文献

Digital subtraction angiography (DSA) plays an important role in the management of vascular diseases of the lower extremities. A disadvantage is the lack of an automatically moving table top. We used a 1,024 x 1,024 matrix with a large-screen intensifier system and an automated "stepping" facility. In 161 examinations of the arteries of the lower extremity digital peripheral arteriography was performed with and without the subtraction technique. We compared the influence of different iodine concentrations in DA and DSA. Peripheral DA proved to be equal to peripheral DSA in the region of the pelvis, thigh and knee, with no adequate contrasting being obtained merely in the region of the lower leg arteries in about 45%. It is necessary to use contrast medium at a concentration of 300 mg I/ml. The installation of an automated "stepping" facility reduces the amount of contrast medium needed and the exposure time.  相似文献

PURPOSE: To prospectively compare the image quality, sensitivity, and specificity of three-dimensional gadolinium-enhanced magnetic resonance (MR) angiography accelerated by parallel acquisition (ie, fast MR angiography) with MR angiography not accelerated by parallel acquisition (ie, conventional MR angiography) for assessment of aortoiliac and renal arteries, with digital subtraction angiography (DSA) as the reference standard. MATERIALS AND METHODS: The study was approved by the institutional review board; informed consent was obtained from all patients. Forty consecutive patients (33 men, seven women; mean age, 63 years) suspected of having aortoiliac and renal arterial stenoses and thus examined with DSA underwent both fast (mean imaging time, 17 seconds) and conventional (mean imaging time, 29 seconds) MR angiography. The arterial tree was divided into segments for image analysis. Two readers independently evaluated all MR angiograms for image quality, presence of arterial stenosis, and renal arterial variants. Image quality, sensitivity, and specificity were analyzed on per-patient and per-segment bases for multiple comparisons (with Bonferroni correction) and for dependencies between segments (with patient as the primary sample unit). Interobserver agreement was evaluated by using kappa statistics. RESULTS: Overall, the image quality with fast MR angiography was significantly better (P=.001) than that with conventional MR angiography. At per-segment analysis, the image quality of fast MR angiograms of the distal renal artery tended to be better than that of conventional MR angiograms of these vessels. Differences in sensitivity for the detection of arterial stenosis between the two MR angiography techniques were not significant for either reader. Interobserver agreement in the detection of variant renal artery anatomy was excellent with both conventional and fast MR angiography (kappa=1.00). CONCLUSION: Fast MR angiography and conventional MR angiography do not differ significantly in terms of arterial stenosis grading or renal arterial variant detection.  相似文献

PURPOSE: To evaluate the interobserver and intermodality variability of conventional angiography and gadolinium-enhanced magnetic resonance (MR) angiography in the assessment of renal artery stenosis. MATERIALS AND METHODS: Fifty-four patients underwent conventional angiography and gadolinium-enhanced three-dimensional gradient-echo MR angiography. Three angiographers blinded to each other's interpretations and the MR angiographic findings assessed the conventional angiograms for renal artery stenosis. Similarly, three blinded MR imagers evaluated the MR angiograms. RESULTS: Interobserver variability for the degree of renal artery stenosis in the 107 kidneys evaluated was not significantly different between the two modalities. The mean SD of the degree of stenosis was 6.9% at MR angiography versus 7.5% at conventional angiography (alpha < or = .05, P > .05). In 70 kidneys (65%), the average degree of stenosis reported by the readers for the two modalities differed by 10% or less. In 22 cases (21%), the degree of stenosis was overestimated with MR angiography by more than 10% relative to the results of conventional angiography. In 15 cases (14%), the degree of stenosis was underestimated with MR angiography by more than 10%. CONCLUSION: Gadolinium-enhanced MR angiography permits evaluation of renal artery stenosis with an interobserver variability comparable with that of conventional angiography.  相似文献

Central cardiovascular anatomy and function have been evaluated with intravenous digital subtraction angiography (DSA). The subtraction techniques used for studying the left ventricle (LV) were mask mode, time interval difference and functional subtraction. Aside from contrast enhancement, a major use of digital fluoroscopy for cardiac applications has been computer-assisted quantitative analysis of LV dimensions and function. Left ventricular volumes and wall thickness determined from DSA studies have correlated closely with direct left ventriculograms and sonocardiometry measurements in patients and animals, respectively. Measurements of segmental LV contraction with DSA correlated closely with direct left ventriculography in normal patients and patients with coronary artery disease. The sensitivity of intravenous DSA for detecting significant coronary artery disease was increased by performing DSA immediately after increasing the myocardial oxygen demands by atrial pacing. The advantages and disadvantages of DSA in relation to other semi-or non-invasive imaging modalities are discussed.  相似文献

PURPOSE: To obtain the best available estimates of the diagnostic performance of multidetector computed tomographic (CT) angiography compared with that of digital subtraction angiography (DSA) in the assessment of symptomatic lower extremity arterial disease and to identify the most important sources of variation in diagnostic performance between studies. MATERIALS AND METHODS: Reports of studies published from January 2000 through April 2006 in English, German, French, or Spanish were searched for by using the MEDLINE, EMBASE, and Cochrane databases. Studies were included if they allowed construction of 2 x 2 contingency tables for the detection of stenosis of 50% or greater at multidetector CT angiography compared with that at DSA -- the reference standard -- in patients with claudication or critical ischemia. Two observers extracted data about study design, patient characteristics, arterial tracts, and technical protocols. Random-effects summary receiver operating characteristic analysis was performed to examine the influence of these data on diagnostic performance. RESULTS: Of the 70 studies initially identified, 12 were included in which multidetector CT angiography was used to evaluate 9541 arterial segments in 436 patients. The pooled sensitivity and specificity for detecting a stenosis of at least 50% per segment were 92% (95% confidence interval: 89%, 95%) and 93% (95% confidence interval: 91%, 95%), respectively. Three studies provided data about the diagnostic performance of multidetector CT angiography in subdivisions of the arterial tract. The diagnostic performance of multidetector CT angiography in the infrapopliteal tract was lower than but not significantly different from that in the aortoiliac (P > .11) and femoropopliteal (P > .40) tracts. Regression analysis showed that diagnostic performance was not significantly influenced by differences in study characteristics. CONCLUSION: Multidetector CT angiography is an accurate diagnostic test in the assessment of arterial disease (> or =50% stenosis) of the entire lower extremity.  相似文献

In 15 volunteers and 84 patients with clinically suspected peripheral vascular disease, a stepping kinematic imaging platform, a manual retrofit stepping magnetic resonance (MR) imaging table, was used with three high-field-strength MR imaging systems to perform multistation peripheral contrast material-enhanced MR angiography in the lower extremity with the existing system phased-array coil. Each examination was performed in less than 45 minutes. Mounting of the stepping kinematic imaging platform was quick and simple and allowed rapid repositioning of a patient relative to the phased-array coil and acquisition of high-spatial-resolution MR angiograms of the peripheral vasculature with use of one injection of MR imaging contrast agent.  相似文献

Objective

To evaluate the influence of alternative erasing times of DenOptix® (Dentsply/Gendex, Chicargo, IL) digital plates on subjective image quality and the probability of double exposure image not occurring.

Methods

Human teeth were X-rayed with phosphor plates using ten different erasing times. Two observers evaluated the images for subjective image quality (sharpness, brightness, contrast, enamel definition, dentin definition and dentin-enamel junction definition) and for the presence or absence of double exposure image. Spearman''s correlation analysis and ANOVA was performed to verify the existence of a linear association between the subjective image quality parameters and the alternative erasing times. A contingency table was constructed to evaluate the agreement among the observers, and a binominal logistic regression was performed to verify the correlation between the erasing time and the probability of double exposure image not occurring.

Results

All 6 parameters of image quality were rated high by the examiners for the erasing times between 25 s and 130 s. The same erasing time range, from 25 to 130 s, was considered a safe erasing time interval, with no probability of a double exposure image occurring.

Conclusions

The alternative erasing times from 25 s to 130 s showed high image quality and no probability of double image occurrence. Thus, it is possible to reduce the operating time of the DenOptix® digital system without jeopardizing the diagnostic task.  相似文献

PURPOSE: To evaluate whether and to what extent greater number of projection images obtained at three-dimensional (3D) time-of-flight (TOF) magnetic resonance (MR) angiography versus conventional digital subtraction angiography (DSA) causes overestimation of internal carotid arterial (ICA) stenosis. MATERIALS AND METHODS: DSA (two or three projections), rotational angiography (16 or 32 projections), and 3D TOF MR angiography (12 projections) were performed in 47 stenotic ICAs of 38 symptomatic patients. Two observers independently measured maximum stenosis, and the mean differences among MR angiography, DSA, and rotational angiography were compared. RESULTS: Three rotational and five MR angiograms were nondiagnostic. Seven MR angiograms of ICA stenoses showed a signal void and were excluded from analysis. On the remaining 32 angiograms, mean differences in maximum stenosis for observers 1 and 2, respectively, were 7% (95% CI: 3%, 12%) and 8% (95% CI: 3%, 13%) at MR angiography versus DSA and 2% (95% CI: -2%, 7%) and -1% (95% CI: -5%, 3%) at MR angiography versus rotational angiography. ICA stenosis was graded significantly higher at MR angiography versus DSA, whereas, it was not overestimated at MR angiography versus rotational angiography. The difference in maximum stenosis at MR angiography versus DSA was significantly different from that of MR angiography versus rotational angiography. CONCLUSION: Apparent overestimation of ICA stenosis at 3D TOF MR angiography versus conventional DSA may be partly explained by the greater number of projection images available at 3D TOF MR angiography.  相似文献
