PURPOSE: To determine whether intraarterial digital subtraction angiography (DSA) can be replaced by contrast material-enhanced magnetic resonance (MR) angiography in the assessment of patency or thrombosis of the portal venous system in patients with portal hypertension. MATERIALS AND METHODS: Thirty-six patients with portal hypertension underwent contrast-enhanced MR angiography and intraarterial DSA for assessment of the portal venous system. The images were evaluated for vessel patency or thrombosis of the portal, splenic, or superior mesenteric vein. RESULTS: Of the 101 vessels evaluated, 42 were thrombosed. Overall sensitivity, specificity, and accuracy for the detection of thrombosis were 100%, 98%, and 99%, respectively, for MR angiography and 91%, 100%, and 96%, respectively, for DSA; differences between the imaging methods were not statistically significant. Only in four patients with six vessels (6%) were there discordant findings between MR angiography and DSA. CONCLUSION: Noninvasive contrast-enhanced MR angiography has the potential to replace intraarterial DSA as the standard method to assess the whole portal venous system.  相似文献

目的:评价磁共振血管成像(MRA)在随访43例病人的45个颅内动脉瘤GDC栓塞后的价值。材料和方法:以数字减影血管造影(DSA)作为诊断的金标准,回顾研究43例病人的45个颅内动脉瘤(前循环34个,后循环11个)MRA检查。所有的病人在GDC栓塞治疗后9个月之内的同一周行DSA和MRA检查,这期间不作处理。分析弹簧圈内残余血流、载瘤动脉和邻近动脉血流,MRA以最大强度投影(MIP)重建和源图像为基础。结果:所有病例中,MRA能提供良好的信息。对于动脉瘤的分析,MRA的敏感性、特征性、阳性预测价值、阴性预测价值,诊断弹簧圈内残余血流分别为87.5%、100%、100%、97%,诊断瘤颈的残余血流分别为90%、100%、100%、97%。对于动脉通畅性分析,MRA的敏感性和阴性预测价值,载瘤动脉分别是90.5%和100%,邻近动脉分别是85.7%和100%。结论:在随访GDC栓塞的动脉瘤中,MRA可以作为一个筛选手段,提高检查效率,减少病人的随访风险。  相似文献

PURPOSE: The purpose of our study was to prospectively evaluate 3D time-of-flight (TOF) MR angiography (MRA) in the follow-up of 27 intracranial aneurysms treated with Guglielmi detachable coils (GDCs). METHOD: From February 1997 to June 1998, 26 patients with 27 aneurysms were included in this prospective study. Aneurysms were located in the anterior circulation in 23 cases and in the posterior circulation in 4 cases. All patients underwent 3D TOF MRA and digital subtraction angiography (DSA) in the same week within 4 months after aneurysmal treatment with GDCs. No clinical events occurred during the follow-up. We analyzed residual flow within the coil mass and within the aneurysmal neck and the patency of the parent and adjacent arteries on MRA and DSA. MRA analysis was based upon MIPPED and source images. DSA was our gold standard. RESULTS: In all cases, the quality of MRA was good enough to be informative. In aneurysmal analysis, the sensitivity, specificity, positive predictive value, and negative predictive value of MRA were, respectively, 80, 100, 100, and 96% to diagnose residual flow within the coil mass (one false-negative case) and 83, 100, 100, and 95.5% to diagnose residual flow within the aneurysmal neck (one false-negative case). In arterial analysis, sensitivity and positive predictive value of MRA were 89 and 100% to diagnose patency of the parent artery (three false-negative cases) and 83 and 100% to diagnose patency of adjacent arteries (seven false-negative cases). CONCLUSION: In the follow-up of intracranial aneurysms treated with GDCs, 3D TOF MRA could be used as a screening test to select patients that should undergo DSA and thus could improve patient follow-up in terms of risk-benefit.  相似文献

OBJECTIVE: Our study was a prospective in vivo study to evaluate whether MR angiography is suitable for assessing stent patency and grading in-stent stenoses and to examine whether the accuracy of MR angiography changes with time after stent implantation. SUBJECTS AND METHODS: In a prospective study, 34 iliac stenoses in 27 patients were treated by implantation of 35 nitinol stents. MR angiography was performed immediately after stent placement for 32 stents, and both digital subtraction angiography (DSA) and MR angiography were repeated at the 6-month follow-up for 23 stents. Three blinded observers assessed stent patency and the degree of in-stent stenoses on MR angiography and DSA (the standard of reference) images. The difference between the observers' grading of stenoses on DSA and on MR angiography was determined. Statistical analysis was performed using the Student's t test for paired samples. RESULTS: Stent patency was assessed correctly for all stents and both sets of MR angiography images. Evaluation of DSA 1 images (obtained at end of implantation procedure) revealed that 96.9% of in-stent stenoses were less than 50%. On DSA 2 images (obtained at follow-up), 95.7% of in-stent stenoses were graded as less than 50%. The difference between grading of stenoses on DSA and MR angiography images was 15.0% +/- 16.0% (minimum, 0.0%; maximum, 63.3%) for DSA 1 versus MR angiography 1 (statistically significant, p = 0.037) and 9.8% +/- 13.5% (minimum, 0.0%; maximum, 63.3%) for MR angiography 2 versus DSA 2 (not statistically significant, p = 0.355). CONCLUSION: Patency was correctly assessed for all stents on MR angiography. The quality of MR angiography regarding characterization of in-stent stenoses improved with time after stent placement. However, discrepancies of more than 60% between grading of lumen narrowing on DSA and MR angiography images occurred even at the 6-month follow-up. Thus, MR angiography is not yet a reliable technique for characterization of in-stent stenoses.  相似文献

Accurate measurements of vessel dimensions are desirable in many clinical applications. This work uses the known relative motion between X-ray source and the patient in stepping-table digital subtraction angiography (DSA) to provide an accurate geometric calibration for quantitative measurements. The method results in a calibration factor that converts the size of the object measured in pixels on the image to its size in millimetres. The main sources of error relate to: (i) the assessment of relative displacement of a structure in a series of images; (ii) patient motion throughout data acquisition; and (iii) image distortion. Error was evaluated both with a test object consisting of a large grid of ball bearings (2x2 cm spaced) and, in vivo, in five renal DSA examinations performed with identical catheters of known diameter. The calibration factor was calculated with 0.1% accuracy for the test object and at least 2% accuracy in vivo, even with breath holding and pulsatile motion. This demonstrates that the calculation of the calibration factor can be very accurate, and that the method we propose is capable of the submillimetre accuracy required for clinical studies if used in conjunction with an accurate measurement of the vessel size in pixels. In conclusion, accurate geometric measurements can be performed in stepping-table DSA, without the need for external reference objects.  相似文献

Introduction  Extracranial–intracranial (EC/IC) bypass is a useful procedure for the treatment of cerebral vascular insufficiency or complex aneurysms. We explored the role of multidetector computed tomography angiography (MDCTA), instead of digital subtraction angiography (DSA), for the postoperative assessment of EC/IC bypass patency. Methods  We retrospectively analyzed a consecutive series of 21 MDCTAs from 17 patients that underwent 25 direct or indirect EC/IC bypass procedures between April 2003 and November 2007. Conventional DSA was available for comparison in 13 cases. MDCTA used a 64-slice MDCT scanner (Aquilion 64, Toshiba). The proximal and distal patencies were analyzed independently on MDCTA and DSA by a neuroradiologist and a neurosurgeon. The bypass was considered patent when the entire donor vessel was opacified without discontinuity from proximal to distal ends and was visibly in contact with the recipient vessel. Results  MDCTA depicted the patency status in every patient. Bypasses were patent in 22 cases, stenosed in one, and occluded in two. DSA always confirmed the results of the MDCTA (sensitivity = 100%, 95% CI = 0.655–1.0; specificity 100%, 95% CI = 0.05–1.0). Conclusions  MDCTA is a non-invasive and accurate exam to assess the postoperative EC/IC bypass patency and is a promising technique in routine follow-up.  相似文献

BACKGROUND AND PURPOSE: Intra-arterial contrast angiographies are generally used to confirm treatment results of endovascular neurointerventions such as aneurysm obliteration. We compared MR angiography with digital subtraction angiography (DSA) as a follow-up technique for the detection of aneurysmal remnant cavities and arterial patency in patients treated for intracranial aneurysms with Guglielmi detachable coils (GDCs). METHODS: In 20 consecutive patients, follow-up MR angiography and routine intra-arterial cerebral angiography were performed on the same day 1 to 7 months (mean, 4.5 months) after embolization with GDCs. MR angiographic data were postprocessed for subvolume maximum intensity projections centered on the region of the treated aneurysm. Hard copies of both imaging studies were interpreted independently in a blinded fashion to record and compare remnant cavities, location of residual flow, and adjacent arterial narrowing, using DSA as the standard of reference. The interpreters also established an occlusion grade for the treated aneurysms as evidenced on DSA images and evaluated MR angiograms for artifactual effects. RESULTS: Overall sensitivity and positive predictive value of MR angiography in revealing aneurysmal remnant cavities were both 90%. Specificity in ruling out a remnant cavity with MR angiography was 91%. One remnant cavity was missed by MR angiography, and in five patients, false adjacent arterial encroachments were reported. CONCLUSION: MR angiography may be useful in the long-term follow-up of successfully treated small and medium-sized aneurysms after concurrent primary verification of their occlusion with DSA.  相似文献

Venography to evaluate the patency of upper-extremity veins was performed with digital subtraction angiography (DSA) and conventional angiography. Venous thrombosis was easily diagnosed, and the innominate veins and superior vena cava were more easily visualized using DSA. Iodine concentration for DSA was one-third that of conventional venography, and the examination time was reduced by 50% using DSA. Patient comfort and acceptance were greater with DSA. DSA is a superior technique for upper-extremity vein evaluation in cooperative patients.  相似文献

Forty patients who had undergone coronary artery bypass surgery were studied with digital subtraction angiography (DSA) to develop an outpatient screening technique for coronary artery bypass graft visualization. Of 103 grafts in 40 patients, 101 were seen: 95 were clearly patent and in six the stump of an occluded graft was seen. Of 32 grafts seen in 14 patients using intraarterial DSA, only 13 (41%) were demonstrated using intravenous DSA. Intraarterial DSA is an effective screening procedure to determine bypass graft patency. However, volumes of 40-45 ml of 76% contrast medium injected at rates of at least 20 ml/sec must be used for each injection to obtain a diagnostic image, and since each projection is complementary and contributes some information, several injections must be used to complete an examination.  相似文献

BACKGROUND AND PURPOSE: Intravascular treatment of intracranial aneurysms is a relatively new therapeutic technique and long-term controlled angiographic trials are needed to assess persistence of aneurysm occlusion. Our purpose was to evaluate the effectiveness of 3D time-of-flight (3D-TOF) MR angiography as a noninvasive screening tool in the follow-up of cerebral aneurysms treated with Guglielmi detachable coils (GDCs). METHODS: Forty-nine patients with 50 intracranial aneurysms previously treated with GDCs were studied with both DSA and 3D-TOF MR angiography. In 14 cases, a second follow-up examination was performed, for a total of 64 aneurysms evaluated. In 25 aneurysms, both pre- and postcontrast MR angiographic studies were obtained. RESULTS: In seven of 64 aneurysms, the MR angiographic studies were considered to be unreliable owing to the presence of artifacts that obscured part of the parent artery and did not allow an accurate evaluation of the aneurysm neck. These seven aneurysms, however, all were shown to be completely occluded at digital subtraction angiography (DSA). In the remaining 57 aneurysms, DSA revealed complete occlusion in 39 and the presence of residual patency in 18, whereas MR angiography showed complete occlusion in 38 and residual patency in 19. Enhanced MR angiography proved to be useful in evaluating residual patency in large and giant aneurysms and in better depicting the distal branch arteries. CONCLUSION: Although artifacts related to the presence of coils are evident on a considerable number of imaging studies, our findings indicate that MR angiography is useful in the evaluation of residual patency of cerebral aneurysms treated with GDCs and may eventually prove valuable in the follow-up of those cases in which a good initial correlation with DSA was demonstrated.  相似文献

Motion artifacts create a severe problem in digital subtraction angiography (DSA) studies. Periodic motion can be eliminated by "gating," matching a precontrast mask with a postcontrast image at the same phase position in the cycle. Electrocardiogram (EKG) signals are used in cardiac DSA for this purpose. An alternate method relies on the generation of a density-time curve dependent on the attenuation changes of anatomical motion. A densitometric window placed at an appropriate location records the variation, from which individual images are tagged with phase position information encoded as delay time. Results are similar to the EKG gating method for cardiac DSA when using an appropriate window location. Periodic motion caused by superimposed respiration can be suppressed by tracking diaphragm or other object attenuation changes with the same algorithms. Using these techniques permits the nonarbitrary matching of mask and contrast images without physiologic monitoring. The techniques are straightforward and relatively easy to implement on systems designed for rapid digital imaging.  相似文献

Purpose: To compare patency rates of transjugular intrahepatic portosystemic shunts (TIPS) after placement of long-medium Palmaz stents or Wallstents. Methods: We performed a retrospective review of TIPS performed at our institution between December 1997 and December 1998. During this time period we placed long-medium Palmaz stents for TIPS procedures in 17 patients and Wallstents in 20 patients as the initial stent. Patency was determined on follow-up by ultrasound, angiography, or pathologic examination in the event of transplant. Results: Primary patency in the Palmaz stent group was 70.6% (12/17 patients) (follow-up 1–399 days, mean 127 days). Both primary assisted and secondary patency in the Palmaz group was 100% (17/17 patients) (follow up 1–399 days, mean 154 days). Primary patency in the Wallstent group was 50% (10/20 patients) (follow up 1–370 days, mean 65 days). Primary assisted patency in the Wallstent group was 80% (16/20 patients) (follow up 1–601 days, mean 141 days). Secondary patency in the Wallstent group was 100% (20/20 patients) (follow up 2–601 days, mean 142 days). Kaplan-Meier analysis of the two groups of patients yielded a primary patency of 266 days (standard error 45 days) for TIPS with the Palmaz stent and 139 days (standard error 45 days) for the Wallstent (p = .04). The 3, 6, and 12-month primary patency rates were .84, .63, and .42 respectively for the Palmaz stents and .36, .36, and .18 respectively for the Wallstent. There was no significant difference in primary assisted or secondary patency between the two stent groups. The mean tract curvature in the patients with Palmaz stents was 23.5° (SD 18.2°, range 0–69.0°) compared with 57° (SD 34.5°, range 7.0–144.0°) in patients with Wallstents (p = .01). Conclusions: Our nonprospective, nonrandomized study suggests that TIPS created with the long-medium Palmaz stent have a higher primary patency than those created with the Wallstent in tracts that are relatively straight.  相似文献

Multi-slice CT (MSCT) scanners provided significant improvement in vascular applications. In this study, our purpose was to determine the clinical utility of MSCTA in evaluation of extracranial-intracranial (EC-IC) bypass patency. Eleven (4 men and 7 women; mean age, 46 years; age range, 19-68 years) consecutive patients who underwent MSCTA and DSA after EC-IC bypass surgery were evaluated retrospectively. All patients underwent DSA within 3 weeks of MSCTA. The indications for EC-IC bypass were severe stenosis or occlusion of intracranial arteries in seven patients and therapeutic occlusion of intracranial artery for unclippable giant aneurysm in four patients. Ten patients underwent superficial temporal artery (STA)-middle cerebral artery (MCA) bypass and one patient underwent occipital artery-posterior cerebral artery (PCA) bypass. Eight STA-MCA bypasses in six patients were patent on MSCTA which were confirmed on DSA. Two STA-MCA bypasses and one occipital artery-posterior cerebral artery (PCA) bypass were occluded in three patients on MSCTA and DSA. In one patient, minimal stenosis of the STA-MCA bypass was identified on MSCTA and DSA. In 1 patient, STA-MCA bypass was not well seen on MSCTA and suspected for occlusion. DSA identified the occlusion of bypass on this patient. MSCTA seems to be a very promising noninvasive technique in evaluation of EC-IC bypass.  相似文献

目的探讨DSA测量技术误差的产生与控制。方法在GE公司生产的Advantx LCV Plus DSA机上测得数据。将显示屏分为中央区、中间区及边远区3个区带,对不同大小定标在不同检查床高度及不同点光源与增强器高度(SID)条件下,测量标的(人民币5角硬币)的放大、缩小情况。结果随着标的外移,由中央区到边远区标的逐渐放大,且纵向放大比横向放大显著。不同的定标对比,硬币(直径20.4mm)和钢球(直径7.7mm)测量相同标的结果相差较小,而导管(4F)定标有显著的低估实物倾向。同区带同轴向测量,测量误差控制在1.0%~-2.5%之间。结论将显示屏划分为中央区、中间区及边远区将有助于介入医师对测量误差的控制。以定标物的横向做定标来测量标的较为准确,同区带同轴向测量误差控制较好。  相似文献

Purpose: To compare three methods for standardizing volume rendering technique (VRT) protocols by studying aortic diameter measurements in magnetic resonance angiography (MRA) datasets.

Material and Methods: Datasets from 20 patients previously examined with gadolinium-enhanced MRA and with digital subtraction angiography (DSA) for abdominal aortic aneurysm were retrospectively evaluated by three independent readers. The MRA datasets were viewed using VRT with three different standardized transfer functions: the percentile method (Pc-VRT), the maximum-likelihood method (ML-VRT), and the partial range histogram method (PRH-VRT). The aortic diameters obtained with these three methods were compared with freely chosen VRT parameters (F-VRT) and with maximum intensity projection (MIP) concerning inter-reader variability and agreement with the reference method DSA.

Results: F-VRT parameters and PRH-VRT gave significantly higher diameter values than DSA, whereas Pc-VRT gave significantly lower values than DSA. The highest interobserver variability was found for F-VRT parameters and MIP, and the lowest for Pc-VRT and PRH-VRT. All standardized VRT methods were significantly superior to both MIP and F-VRT in this respect. The agreement with DSA was best for PRH-VRT, which was the only method with a mean error below 1 mm and which also had the narrowest limits of agreement (95% of cases between 2.1 mm below and 3.1 mm above DSA).

Conclusion: All the standardized VRT methods compare favorably with MIP and VRT with freely selected parameters as regards interobserver variability. The partial range histogram method, although systematically overestimating vessel diameters, gives results closest to those of DSA.  相似文献

The utility of a semi-automatic method of measuring left ventricular (LV) volume geometrically from gated blood-pool studies and digital subtraction angiography (DSA) was investigated using computerized edge detection and spatial calibration algorithms. LAO LV volumes determined from gated blood-pool studies were compared to volumes obtained from contrast left ventriculograms in 21 patients and the applicability of this method to DSA was evaluated in 25 additional patients who also had conventional left ventriculography. There was excellent correlation between the two, both for radionuclide studies and for DSA. Computer-based geometric determinations of LV volume appear to be rapid, accurate, and less dependent on subjective operator decisions than previously reported geometric approaches.  相似文献

Dynamic susceptibility contrast (DSC)-MRI is commonly used to measure cerebral perfusion in acute ischemic stroke. Quantification of perfusion parameters involves deconvolution of the tissue concentration-time curves with an arterial input function (AIF), typically with the use of singular value decomposition (SVD). To mitigate the effects of noise on the estimated cerebral blood flow (CBF), a regularization parameter or threshold is used. Often a single global threshold is applied to every voxel, and its value has a dramatic effect on the CBF values obtained. When a single global threshold was applied to simulated concentration-time curves produced using exponential, triangular, and boxcar residue functions, significant systematic errors were found in the measured perfusion parameters. We estimate the errors obtained for different sampling intervals and signal-to-noise ratios (SNRs), and discuss the source of the systematic error. We present a method that partially corrects for the systematic error in the presence of an exponential residue function by applying a linear fit, which removes underestimates of long mean transit time (MTT) and overestimates of short MTT. For example, the correction reduced the error at a temporal resolution of 2.5 s and an SNR of 30 from 29.1% to 11.7%. However, the error is largest in the presence of noise and at MTTs that are likely to be encountered in areas of hypoperfusion; furthermore, even though it is reduced, it cannot be corrected for exactly.  相似文献

Conventional arteriography, intraarterial digital subtraction angiography (DSA), and intravenous DSA were used in the evaluation of the patency of the pedal arterial arcades in 15 patients with advanced peripheral arteriosclerotic disease. Intraarterial DSA was found to be superior to conventional arteriography. In 12 of 15 patients, excellent or good results were achieved by intraarterial DSA compared with 5 or 15 patients examined by conventional arteriography. All intravenous DSA studies performed in 6 patients were classified as poor.  相似文献

Purpose: To retrospectively evaluate the results of renal artery stenting in patients with renovascular disease and a solitary functioning kidney. Methods: Palmaz stents were placed in 16 patients with a solitary functioning kidney, renal artery stenosis, hypertension and renal failure. Stenoses were evaluated with color Doppler ultrasound, MR angiography and digital subtraction angiography (DSA). Indications for stenting were: recoil after percutaneous transluminal renal angioplasty (PTRA) (63%), arterial dissection after PTRA (13%) and primary stenting (25%). Immediate results were evaluated by DSA. On follow-up (6-36 months), patients underwent periodical evaluation of clinical conditions (blood pressure and serum creatinine level) and stent patency, by means of color Doppler ultrasound. Results: Stent placement was successful in all patients (100%). Cumulative primary patency rate was: 100% at 1 day, 93.75% at 6 months, 81.25% at 12 months and 75% at 24 months. A significant reduction in diastolic blood pressure occurred (mean ± SD 104 ± 6 vs 92 ± 3; p < 0.05); renal function improved or stabilized in over 80% of patients. However, there was no significant difference in the creatinine values before and after treatment (mean ± SD 200 ± 142 mmol/l vs 197 ± 182 mmol/l; p > 0.05). Conclusion: Renal artery stenting, both after PTRA and as primary stenting, represents a safe procedure, able to preserve renal function in patients with a solitary functioning kidney.  相似文献

A method for non-invasive assessment of experimental bone healing in animals is described. Optimized radiographs were analysed by digital processing using a computer-based image analysis system. Values of the total mass were expressed as the equivalent mass of a simultaneously exposed aluminium roentgen-density reference. In order to assess the potential of the method, measurements were initially performed on phantoms and bone specimens. The systematic error was negligible. An excellent correlation (r = 0.999) was found between the densitometric weight (mu) and the corresponding dry weight (m) of the analysed bone samples and a calibration factor mu/m = 1.38 was established. The total random error was low, the coefficient of variation (CV) amounting to less than 5% or, if duplicate analyses were performed, less than 3%. The accuracy was not impaired when radiography was carried out with addition of water to mimic overlying soft tissue in the model. The method was applied to the assessment of callus mass longitudinally in vivo in the healing of a bone defect in the rabbit radius, yielding reproducible values (CV less than 5%) and a quantitative analysis of the healing sequence.  相似文献
