尿视黄醇结合蛋白在糖尿病肾病早期的诊断意义   总被引:1,自引:1,他引:0  
目的探讨尿视黄醇结合蛋白(RBP)在糖尿病肾病早期的诊断意义.方法采用酶联免疫法(BLISA)测定78例非胰岛素依赖型糖尿病(NIDDM)患者和30例健康人的尿RBP,同时测定尿微量白蛋白(Alb)和尿β2-微球蛋白(β2-MG).结果NIDDM患者RBP较正常对照组增高,尿RBP与尿A1b、尿β2-MG呈正相关.结论尿RBP可作为诊断早期糖尿病肾病的敏感指标.  相似文献

目的探讨尿微量清蛋白(micro-albumin,mAlb)、β2微球蛋白(β2-microglobulin,β2-MG)、尿视黄醇结合蛋白(retinoi-binding protein,RBP)、尿N-乙酰-β-D-氨基葡萄糖苷酶(n-acety-β-D-glucosaminidase,NAG)对糖尿病早期肾损伤的诊断价值。方法采用免疫透射比浊法测定mAlb、RBP、β2-MG的含量,联合应用氯硝基苯-乙酰氨基葡萄糖速率法测定尿NAG酶的活性,分别检测46例糖尿病患者(实验组)、30例正常人(阴性对照组)和30例慢性肾炎患者(阳性对照组)的尿mAlb、RBP、NAG、β2-MG的含量,并将三者的检测结果进行比较。结果在对不同观察组尿微量蛋白及尿酶检测结果进行分析时发现:糖尿病组mAlb高于正常组(P<0.05),而与慢性肾炎组相比显著低于慢性肾炎组(P<0.01);糖尿病组RBP明显高于对照组RBP(P<0.01),但慢性肾炎组RBP与正常组RBP相比差别不大(P>0.05);糖尿病组β2-MG明显高于正常组和慢性肾炎组(P<0.01);糖尿病组NAG和慢性肾炎组NAG可明显高于对照组(P<0.01)。结论尿mAlb、RBP、NAG、β2-MG可反映糖尿病患者的早期肾损伤改变。尿mAlb、RBP、NAG、β2-MG是监测早期糖尿病肾病较敏感指标,其联合检测对早期肾损伤有重要意义。  相似文献

目的 为了探讨糖尿病肾病患者与慢性肾小球肾炎患者的尿微量蛋白的差异。方法 用酶联免疫法分别测定了40例糖尿病肾病患者和36例慢性肾小球肾炎患者的尿视黄醇结合蛋白(RBP)和白蛋白(Alb)。结果 糖尿病肾病患者尿RBP较慢性肾小球肾炎患者高,但尿Alb较慢性肾小球肾炎患者低。结论 糖尿病肾病对肾小管损害较慢性肾小球肾炎明显,但对肾小球损害不如慢性肾小球肾炎明显。尿RBP、Alb可分别作为早期诊断两种疾病的敏感指标。  相似文献

目的探讨糖尿病肾病早期联合检测Cys-C(血清胱抑素C)和β_2-MG(β_2微球蛋白)的临床价值。方法选择2016年1月~2017年5月在我院住院的80例2型糖尿病患者,将其分为A组:无糖尿病肾病组40例,B组:糖尿病肾病组40例;同期选取40例健康体检者为C组。分别对比分析三组受试者Cys C、β_2-MG、SCr(血肌酐)、BUN(血尿素氮)水平。结果 A、B组患者SCr、BUN、Cys C、β_2-MG水平较C组明显升高(P<0.05);B组患者SCr、BUN、Cys C、β_2-MG水平较A组明显升高(P<0.05)。Cys C诊断糖尿病肾病早期诊断的灵敏度、特异度及阴性预测值最低,β_2-MG诊断糖尿病肾病早期诊断的阳性预测值、有效性最低,联合检测Cys-C和β_2-MG对糖尿病肾病早期诊断的有效性、灵敏度、特异度、阳性预测值及阳性预测值最高。结论糖尿病患者采用Cys-C、β_2-MG联合检测,能早期诊断糖尿病肾病,值得临床推广。  相似文献

目的:探索尿白蛋白(Alb)、免疫球蛋白G(IgG)和β2-微球蛋白(β2-MG)联合测定在肾脏疾病(肾病)患儿早期诊断和病变部位的价值。方法127例肾脏疾病患儿,其中急性肾炎61例,为肾炎组,肾病综合征66例,为肾病组,另选取正常健康小儿50例,为对照组。采用RIA法对三组尿液中的Alb、IgG和β2-MG进行测定。结果肾炎组和肾病组尿中的Alb、IgG明显高于对照组(P<0.01);肾病组尿中的β2-MG明显高于对照组和肾炎组(P<0.01)。结论尿中的Alb、IgG和β2-MG联合测定能高灵敏早期反映肾脏的损伤,且能区分是肾小球还是肾小管的损害,并与病情呈正相关。  相似文献

目的探讨检测尿微量白蛋白(mALB)、β2微球蛋白(β2-MG)、免疫球蛋白(IgG)、视黄醇结合蛋白(RBP)水平与不同病程2型糖尿病患者肾脏损害的相关性及对早期糖尿病肾病的诊断价值。方法选取120例2型糖尿病患者,按病程分组,A组病程1~7年,40例;B组病程8~14年,42例;C组病程15~20年,38例。选取65例体检健康职工为对照组。比较各组对象尿mALB、β2-MG、IgG、RBP水平。结果糖尿病组与对照组比较,尿mALB、β2-MG、IgG、RBP水平均明显升高,且与病程正相关。结论联合检测尿mALB、β2-MG、IgG、RBP对早期糖尿病肾病的诊断及防治具有重要临床意义。  相似文献

目的:探讨尿视黄醇结合蛋(RBP)在海洛因依赖者早期肾损害中的诊断意义。方法:采用酶联免疫法(ELISA)测定78名海洛因依赖者(吸毒组)、30名健康人(正常对照组)的尿RBP,同时测定尿微量白蛋白(Alb),β2-微球蛋白(β2-MG)。结果:海洛因依赖者尿RBP均较正常对照组增高,尿RBP与尿β2-MG、尿Alb呈正相关。结论:尿RBP的变化可作为判断海洛因依赖者早期肾功能损害的敏感指标。  相似文献

目的探讨血清胱抑素C(CysC)、尿微量清蛋白(mAlb)、尿β2-微球蛋白(β2-MG)和尿视黄醇结合蛋白(RBP)的变化对2型糖尿病早期肾损伤的诊断价值。方法采用免疫比浊法检测70例2型糖尿病早期肾损伤和30例健康人血清CysC及尿mAlb、β2-MG和RBP的含量。结果 2型糖尿病早期肾损伤患者血清CysC,尿mAlb、β2-MG、RBP显著高于健康人(P〈0.01),且血清CysC与尿mAlb水平呈显著正相关(r=0.924,P〈0.01)。结论血清CysC可作为2型糖尿病早期肾损伤的敏感指标,联合检测尿mAlb、β2-MG和RBP有助于提高早期诊断的敏感性。  相似文献

目的探讨尿特种蛋白微量白蛋白(Malb)、α1-微球蛋白(α1-MG)、β2-微球蛋白(β2-MG)、转铁蛋白(TRF)、视黄醇结合蛋白(RBP)与糖尿病早期肾损害关系。方法随机选择126例作为正常对照组,我院糖尿病患者据尿微量白蛋白排泄率分为无肾病组、初期肾病组、临床肾病组三组各126例,进行尿特种蛋白及尿肌酐(Cr)测定。结果糖尿病三组均与正常对照组进行t检验有统计学意义(按α=0.05水准,P<0.05)。结论尿特种蛋白与早期肾损害呈正相关,尿特种蛋白可作为发现糖尿病早期肾损害的常规筛查项目。  相似文献

In the Netherlands, the drug problem among adolescents developed as a new phenomenon in the mid 1960s. Without remarkable success, methadone maintenance programs were introduced in 1968. Drug free treatment in therapeutic communities, developed since 1972, was successful after the first experimental years. This success provoked the illusion that drug related problems could be solved by treatment; low threshold programs, putting few demands on participants, were established to dispense methadone. The illusion was that addicts can be motivated into further treatment if they are brought into contact with professionals. As the addict population kept growing and their street crimes increased in number, Dutch drug policies changed in 1978, using methadone as a means to decrease criminal activities rather than to treat addicts. This also turned out to be an illusion. Currently, there is strong political pressure to distribute heroin to addicts to diminish the negative side effects of drug abuse. Dutch drug policies are characterized by the opinion that drug problems cannot be solved and have to be accepted.  相似文献

1. Brain sparing is a feature of intra-uterine growth retardation (IUGR). This implies that there is a redistribution of metabolic supply so that body growth slows to a greater extent than brain growth. 2. Intra-uterine growth retardation, as evidenced by a low birthweight for gestational age is a predisposing factor for hypertension, cardiovascular disease and diabetes mellitus in adult life. 3. In species like humans, nephrogenesis is complete before birth. In the rat, it is completed shortly after birth. In both species, it can be shown that either undernutrition or IUGR is associated with reduced nephron number. 4. It has been proposed that oligonephropathy results in hyperfiltration, which ultimately leads to glomerulosclerosis and hypertension. The renin-angiotensin system (RAS) is necessary for normal renal development and fetal renal function. In the rat, blockade of the RAS in the first weeks of life by pharmacological agents reduces glomerular number and has been shown to cause hypertension in adult life. Renal denervation reduces the activity of the fetal RAS and also causes abnormal development of the renin-secreting cells. 5. There is tonic renal sympathetic nerve activity in the late gestation fetal sheep. The level of renal sympathetic nerve activity (RSNA) is influenced by the fetal behavioural state. 6. However, interactions between the developing kidney and the developing sympathetic nervous system are poorly understood. On the one hand, renal innervation may be important in the provision of neurotrophic factors that stimulate the development of the RAS and kidney. On the other, high levels of RSNA associated with circulating catecholamines and vasopressin may cause vasoconstriction and limit nephrogenesis. This latter effect could be a predisposing factor to adult hypertension and cardiovascular disease.  相似文献
