Due to their biodegradability and biocompatibility, chitosan-based hydrogels have great potential in regenerative medicine, with applications such as bacteriostasis, hemostasis, and wound healing. However, toxicity and high cost are problems that must be solved for chitosan-based hydrogel crosslinking agents such as formaldehyde, glutaraldehyde, and genipin. Therefore, we developed a biocompatible yet cost-effective chitosan-based hydrogel system as a candidate biomaterial to prevent infection during wound healing. The hydrogel was fabricated by crosslinking chitosan with dialdehyde chitosan (CTS–CHO) via dynamic Schiff-base reactions, resulting in a self-healable and injectable system. The rheological properties, degradation profile, and self-healable properties of the chitosan-based hydrogel were evaluated. The excellent antibacterial activity of the hydrogel was validated by a spread plate experiment. The use of Live/Dead assay on HEK 293 cells showed that the hydrogel exhibited excellent biocompatibility. The results demonstrate that the newly designed chitosan-based hydrogel is an excellent antibacterial wound dressing candidate with good biocompatibility. 相似文献

The International Working Group on the Diabetic Foot (IWGDF) has published evidence‐based guidelines on the prevention and management of diabetic foot disease since 1999. In conjunction with advice from internal and external reviewers and expert consultants in the field, this update is based on a systematic review of the literature centred on the following: the Population (P), Intervention (I), Comparator (C) and Outcomes (O) framework; the use of the SIGN guideline/Cochrane review system; and the 21 point scoring system advocated by IWGDF/EWMA. This has resulted in 13 recommendations. The recommendation on sharp debridement and the selection of dressings remain unchanged from the last recommendations published in 2016. The recommendation to consider negative pressure wound therapy in post‐surgical wounds and the judicious use of hyperbaric oxygen therapy in certain non‐healing ischaemic ulcers also remains unchanged. Recommendations against the use of growth factors, autologous platelet gels, bioengineered skin products, ozone, topical carbon dioxide, nitric oxide or interventions reporting improvement of ulcer healing through an alteration of the physical environment or through other systemic medical or nutritional means also remain. New recommendations include consideration of the use of sucrose‐octasulfate impregnated dressings in difficult to heal neuro‐ischaemic ulcers and consideration of the use of autologous combined leucocyte, platelet and fibrin patch in ulcers that are difficult to heal, in both cases when used in addition to best standard of care. A novel and promising hydrogel drug delivery system (DDS) capable of releasing 5‑fluorouracil (5-FU) in a prolonged and controlled manner was obtained using ε‑caprolactone‑poly(ethylene glycol) (CL-PEG) or rac‑lactide-poly(ethylene glycol) (rac‑LA-PEG) copolymers. Copolymers were synthesized via the ring-opening polymerization (ROP) process of cyclic monomers, ε‑caprolactone (CL) or rac-lactide (rac-LA), in the presence of zirconium(IV) octoate (Zr(Oct)4) and poly(ethylene glycol) 200 (PEG 200) as catalyst and initiator, respectively. Obtained triblock copolymers were characterized by nuclear magnetic resonance (NMR) and gel permeation chromatography (GPC) techniques; the structure and tacticity of the macromolecules were determined. The relationship between the copolymer structure and the reaction conditions was evaluated. The optimal conditions were specified as 140 °C and 24 h. In the next step, CL-PEG and rac-LA-PEG copolymers were chemically crosslinked using hexamethylene diisocyanate (HDI). Selected hydrogels were subjected to in vitro antitumor drug release studies, and the release data were analyzed using zero-order, first-order, and Korsmeyer-Peppas mathematical models. Controlled and prolonged (up to 432 h) 5-FU release profiles were observed for all examined hydrogels with first-order or zero-order kinetics. Collagen possesses unique properties, e.g., biocompatibility, biodegradability, and non-toxicity. However, collagen material degrades too quickly and has low mechanical properties. One of the methods of polymers’ modification is mixing them to obtain blends. In this study, the influence of β-glucan for collagen material was analyzed. The interaction between the functional groups of the polymer was analyzed by ATR-FTIR (attenuated total reflection-fourier transform infrared) spectroscopy. The influence of β-glucan on mechanical properties was evaluated. The surface properties of materials were assessed using contact angle measurements and the topography of materials was evaluated by AFM (atomic force microscope). The structure of materials was analyzed according to SEM (scanning electron microscopy) pictures. Moreover, the DPPH-free radicals’ scavenging ability and biocompatibility against erythrocytes and HaCaT cells were evaluated. Collagen and β-glucan were bound together by a hydrogen bond. β-glucan addition increased the roughness of the surface of the film and resulted in a more rigid character of the materials. A small addition of β-glucan to collagen provided a more hydrophilic character. All the materials could swell in in vitro conditions and showed antioxidant activity. Materials do not cause erythrocyte hemolysis. Finely, our cytotoxicity studies indicated that β-glucan can be safely added at small (10% or less) quantity to collagen matrix, they sufficiently support cell growth, and the degradation products of such matrices may actually provide some beneficial effects to the surrounding cells/tissues. 相似文献

BACKGROUND AND OBJECTIVES: The growth factors derived from platelets and plasma proteins mediate the wound-healing process that is characterized by the sequential migration and differentiation of several cell populations that give rise to angiogenesis, collagen synthesis, wound contraction, and re-epithelialization. To evaluate the efficacy of the blood-derived factors in wound healing, we examined a novel wound dressing consisting of concentrated human plasma proteins and platelet releasate (CPPP). MATERIALS AND METHODS: To generate CPPP, plasma proteins and platelets in the peripheral blood (n = 5) were concentrated with the cold ethanol precipitation method. The thrombin obtained from the same blood unit and calcium chloride (CaCl(2)) were mixed to a concentrate. The CPPP has enough strength to dress cutaneous wounds and contains large amounts of cytokines and fibronectin. We applied the CPPP to excisional skin wounds in genetically healing-impaired model mice (n= 5) and the wounds were evaluated 10 days after the operation. RESULTS: The area of CPPP-treated wounds decreased significantly compared with that of the control wounds (65% vs. 94% of the original size, respectively, P= 0.032). The immunostained section revealed a striking effect of CPPP on vascularization compared with the control wounds (13.2 vs. 2.7 vessels per mm(2) as mean vascular density observed in the sections, respectively, P= 0.013). CONCLUSIONS: Our results suggest that CPPP is a promising biologically active dressing for full-thickness skin wounds. CPPP can be an entirely autologous biological dressing, suggesting that it is free from the risk of transmission of pathogens through blood products. 相似文献

Infection is one of several factors that can delay normal wound healing. Antibacterial wound dressings can therefore promote normal wound healing. In this study, we prepared an antibacterial wound dressing, consisting of visible light-cured methacrylated collagen (ColMA) hydrogel and a 2-hydroxypropyl-beta-cyclodextrin (HP-β-CD)/triclosan (TCS) complex (CD-ic-TCS), and evaluated its wound healing effects in vivo. The 1H NMR spectra of ColMA and CD-ic-TCS revealed characteristic peaks at 1.73, 5.55, 5.94, 6.43, 6.64, 6.84, 6.95, 7.31, and 7.55 ppm, indicating successful preparation of the two material types. In addition, ultraviolet–visible (UV–vis) spectroscopy proved an inclusion complex formation between HP-β-CD and TCS, judging by a unique peak observed at 280 cm−1. Furthermore, ColMA/CD-ic-TCS exhibited an interconnected porous structure, controlled release of TCS, good biocompatibility, and antibacterial activity. By in vivo animal testing, we found that ColMA/CD-ic-TCS had a superior wound healing capacity, compared to the other hydrocolloids evaluated, due to synergistic interaction between ColMA and CD-ic-TCS. Together, our findings indicate that ColMA/CD-ic-TCS has a clinical potential as an antibacterial wound dressing. 相似文献

This study aimed to investigate the validity of self‐reported hypertension and related factors in the Dehgolan Prospective Cohort Study (DehPCS). Data were obtained from 3996 participants aged 35–70 years in the enrolment phase of DehPCS. Self‐reported hypertension and sociodemographic factors were collected by well‐trained interviewers before hypertension diagnosis based on the reference criteria. The history of anti‐hypertensive medication use and/or systolic blood pressure ≥140 (mmHg), or diastolic blood pressure ≥90 (mmHg) were considered as hypertension. Disagreement between self‐reported and reference measures was assessed using sensitivity, specificity, positive, and negative predictive values (PPV and NPV), and kappa values. Binary and multinomial logistic regressions were used to investigate the correlates of validity of self‐reported hypertension. The hypertension prevalence based on self‐reports and the reference criteria was 19.49% and 21.60%, respectively. An acceptable percentage of kappa agreement value of 68.7% and relatively good overall agreement of 89.8% were found. Self‐reported hypertension was guaranteed moderate sensitivity of 72.0% and high specificity of 94.5%, as well as the NPV and PPV of 92/7% and 77/9%, respectively. The chances of false‐positive and false‐negative reporting increased with older age, higher BMI, and a family history of hypertension. Being female, older age, higher BMI, concurrent diabetes, and stronger family ties to hypertension patients significantly increased the chance of reporting true positives relative to true negatives. Although, self‐reported hypertension has an acceptable validity and can be used as a valid tool for screening epidemiological studies, it needs to be investigated because its validity is affected by age, gender, family history of hypertension, and other socio‐demographic characteristics. 相似文献

Objectives:Inflammation is critical in the early phases of wound healing. It has been reported previously that small intestinal and colonic wounds display a more rapid healing than those of other organs. However, the underlying mechanism has not yet been elucidated. Here we examined whether differences in the time course of specified cytokine expression, in colonic and small intestinal anastomotic lesions, might play a major role in this observation in comparison to lesions effecting skin and muscle tissue.Results:The characteristics of TNF-α, IL-6, and IFN-α expression during the healing process for intestinal and colonic lesions were comparable. However, data differed significantly with that observed during healing of skin and muscle lesions. Intestinal and colonic lesions exhibited a significant and sustained increase in specified cytokine levels on day 5 to day 14 as compared with day 1 and 3. Skin and muscle lesions had random or unaltered cytokine levels throughout the study period.Conclusion:Differences in expression of cytokines TNF-α, IL-6, and IFN-α indicate that these play an important role underlying the more rapid healing processes observed in small intestinal and colonic lesions. 相似文献

The present study aimed to compare the action of advanced platelet-rich fibrin (A-PRF+) alone with the action of A-PRF+ combined with autologous gingival fibroblasts. The components released from A-PRF+ conditioned with autogenous fibroblasts that were quantified in the study were fibroblast growth factor (FGF), vascular endothelial growth factor (VEGF), trans-forming growth factor-beta1 and 2 (TGFβ1 and TGFβ2), and soluble collagen. A-PRF+ combined with fibroblasts demonstrated significantly higher values of released VEGF at every time point and, after 7 days, significantly higher values of released TGFβ2. A viability test after 72 h showed a significant increase in proliferation fibroblasts after exposition to the factors released from A-PRF+ combined with fibroblasts. Similarly, the degree of wound closure after 48 h was significantly higher for the factors released from A-RRF+ alone and the factors released from A-RRF+ combined with fibroblasts. These results imply that platelet-rich fibrin (PRF) enhanced with fibroblasts can be an alternative method of connective tissue transplantation. 相似文献

BackgroundHealthcare personnel (HCP) are a priority group for annual influenza vaccination. Few studies have assessed the validity of recall of prior influenza vaccination status among HCP, especially for more than one preceding season.MethodsUsing data from a randomized controlled trial of influenza vaccination among 947 HCP from two US healthcare systems, we assessed agreement between participant self‐report and administrative record documentation of influenza vaccination status during the preceding five influenza seasons; kappa coefficients and sensitivity values were calculated. Administrative record documentation was considered the gold standard. Documented vaccination sources included electronic medical records, employee health records, outside immunization providers, and the state immunization information system.ResultsAmong 683 HCP with prior influenza immunization information, 89.7% (95% CI: 87.2%, 91.9%) of HCP were able to self‐report their vaccination status for the season preceding the survey. By the fifth preceding season, 82.6% (95% CI: 79.5%, 85.3%) of HCP were able to self‐report. Among HCP who self‐reported their vaccination status, agreement between self‐report and documented vaccination status ranged from 81.9% (95% CI: 77.2%, 86.7%) for the fifth season to 90.5% (95% CI: 87.2%, 93.9%) for the season preceding interview. HCP who received vaccine for only some of the preceding five seasons (18.3%) more commonly had ≥2 errors in their recall compared with those vaccinated all five preceding seasons (55.7% vs. 4.3%).ConclusionsSelf‐reported vaccination status is a reliable source for historical influenza vaccination information among HCP who are consistently vaccinated but less reliable for those with a history of inconsistent vaccination. 相似文献

Home blood pressure monitoring provides multiple measurements in the usual environment of each individual, allows the detection of intermediate hypertension phenotypes (white‐coat and masked hypertension), and appears to have superior prognostic value compared to the conventional office blood pressure measurements. Accumulating evidence suggests that home blood pressure monitoring improves long‐term hypertension control rates. Moreover, it is widely available, relatively inexpensive, and well accepted by patients. Thus, current guidelines recommend home blood pressure monitoring as an essential method for the evaluation of almost all untreated and treated patients with suspected or treated hypertension. Validated automated upper‐arm cuff devices with automated storage and averaging of readings should be used. The home blood pressure monitoring schedule for 4 to 7 days with exclusion of the first day (12–24 readings) should be averaged to provide values for decision making. 相似文献

OBJECTIVEChanna micropeltes extract contains albumin and omega-6 which possess antioxidant and anti-inflammatory agents that can promote macrophages in the wound healing process associated with diabetes mellitus (DM). In this study, we analyzed Nuclear Factor kappa B (NF-κB) and Vascular Endothelial Growth Factor (VEGF) expression as well as neovascular cells in the inflammatory stage of DM wound healing.METHODSThe 24 males Rattus novergicus were divided into 3 groups that were 20% Channa micropeltes ointment (Group I), 10% Channa striata extract ointment (Group II), and placebo ointment as a control (Group III). Ointments were applied 3 times daily.RESULTSThe highest expression of NF-κB was obser ved in Group III on Day 4 (15.50 ± 2.38), and the lowest was in treatment of Group I and Group II on Day 8 (4.75 ± 0.96). The highest expression of VEGF was obser ved in Group I on Day 8 (14.75 ± 0.96), and the lowest was Group III on Day 4 (7.00 ± 1.41). The highest count of neovascular cells was obser ved in Group I on Day 8 (11.00 ± 2.16), and the lowest was in Group III on Day 4 (5.50 ± 0.58).CONCLUSIONSChanna micropeltes has an anti-inflammator y effect by regulating NF-κB expression and elevating VEGF expression in the angiogenesis process of DM wound healing. 相似文献

BackgroundDuring the COVID‐19 pandemic, self‐reported COVID‐19 vaccination might facilitate rapid evaluations of vaccine effectiveness (VE) when source documentation (e.g., immunization information systems [IIS]) is not readily available. We evaluated the concordance of COVID‐19 vaccination status ascertained by self‐report versus source documentation and its impact on VE estimates.MethodsHospitalized adults (≥18 years) admitted to 18 U.S. medical centers March–June 2021 were enrolled, including COVID‐19 cases and SARS‐CoV‐2 negative controls. Patients were interviewed about COVID‐19 vaccination. Abstractors simultaneously searched IIS, medical records, and other sources for vaccination information. To compare vaccination status by self‐report and documentation, we estimated percent agreement and unweighted kappa with 95% confidence intervals (CIs). We then calculated VE in preventing COVID‐19 hospitalization of full vaccination (2 doses of mRNA product ≥14 days prior to illness onset) independently using data from self‐report or source documentation.ResultsOf 2520 patients, 594 (24%) did not have self‐reported vaccination information to assign vaccination group; these patients tended to be more severely ill. Among 1924 patients with both self‐report and source documentation information, 95.0% (95% CI: 93.9–95.9%) agreement was observed, with a kappa of 0.9127 (95% CI: 0.9109–0.9145). VE was 86% (95% CI: 81–90%) by self‐report data only and 85% (95% CI: 81‐89%) by source documentation data only.ConclusionsApproximately one‐quarter of hospitalized patients could not provide self‐report COVID‐19 vaccination status. Among patients with self‐report information, there was high concordance with source documented status. Self‐report may be a reasonable source of COVID‐19 vaccination information for timely VE assessment for public health action. 相似文献

ObjectiveTo explore the effect of WeChat‐based MUST nursing intervention on self‐care ability and quality of life in patients with chronic heart failure.MethodConvenient sampling was used to select CHF patients who received treatment in XX Hospital from January 1, 2020, to December 30, 2020, as the study subjects and was divided into the experimental group (n = 60) and the control group (n = 60) according to the random number table. The experimental group used the WeChat‐based MUST nursing intervention, and the control group used the routine education and follow‐up model. Cardiac function parameters, self‐care ability, and other indicators were compared between the two groups before and after nursing. The clinical effect of two groups was evaluated.ResultsAfter the nursing intervention, LVEF levels were increased to different extents and NT‐proBNP was decreased to different extents in both groups. LVEF level in the experimental group was higher than that in the control group, and the NT‐proBNP level in the experimental group was lower than that in the control group. After the nursing intervention, the self‐care ability and quality of life of the two groups were increased to varying degrees. Self‐care ability was higher in the experimental group than in the control group, and quality of life was higher in the experimental group than in the control group.ConclusionThe WeChat‐based MUST nursing intervention model in patients with chronic heart failure, compared with the conventional cardiology nursing model, can effectively improve the self‐care ability of patients, improve the knowledge level and quality of life of patients with heart failure, and reduce the readmission rate. 相似文献

Does the longevity phenotype offer an advantage in wound healing (WH)? In an attempt to answer this question, we explored skin wound healing in the long-lived transgenic αMUPA mice, a unique model of genetically extended life span. These mice spontaneously eat less, preserve their body mass, are more resistant to spontaneous and induced tumorigenesis and live longer, thus greatly mimicking the effects of caloric restriction (CR). We found that αMUPA mice showed a much slower age-related decline in the rate of WH than their wild-type counterparts (FVB/N). After full closure of the wound, gene expression in the skin of old αMUPA mice returned close to basal levels. In contrast, old FVB/N mice still exhibited significant upregulation of genes associated with growth-promoting pathways, apoptosis and cell-cell/cell-extra cellular matrix interaction, indicating an ongoing tissue remodeling or an inability to properly shut down the repair process. It appears that the CR-like longevity phenotype is associated with more balanced and efficient WH mechanisms in old age, which could ensure a long-term survival advantage. 相似文献

Self‐reported measures of health, in the context of developed countries, are well‐researched and commonly regarded as reliable predictors of the underlying health of the population. However, the validity of these measures is under‐researched and questionable in the context of low‐ and middle‐income countries. The authors used Longitudinal Ageing Study in India (LASI) survey data from India to compare self‐reported hypertension with biometrically‐measured hypertension. The results are reported in terms of sensitivity, specificity, and kappa as a measure of agreement. Logistic regression was undertaken to examine the characteristics of those who were unaware of their hypertensive status. Our analysis showed a low sensitivity of 56% and a high specificity of 90.5%. Agreement between self‐reported data and biometric measurement of hypertension was observed to be moderate (κ = 0.48). Large variations were observed among states and sub‐groups. The odds of false negative reporting of hypertension were lower in the individuals with higher age, high education, and greater wealth status. The authors conclude that self‐reported hypertension has important limitations and may be a source of systematic bias. It is recommended that planning and policy‐making in India be based more on an objective assessment of hypertension. 相似文献

Epi-Lock, a new semiocclusive, semipermeable dressing, was evaluated against standard therapy for three common emergency department wounds: partial thickness burns, deep abrasions, and selected lacerations. Comparison was made of quality of wound healing; rate of wound healing; level of pain experienced, postdressing, by the patient; and ease of use by the staff. One hundred patients in each group (standard treatment versus Epi-Lock) included approximately 40 burns, 35 abrasions, and 25 lacerations in each subgroup. The results of this study showed a statistically significant difference (P less than .01) in all categories for Epi-Lock versus standard therapy. Epi-Lock may be a useful new dressing for all types of wounds presenting to the emergency department. 相似文献
