目的:探讨鼻塞法持续呼吸道正压通气(NCPAP)在防治新生儿肺透明膜病的应用及护理。方法:60例胎龄29~35周的早产儿随机分为两组,观察组(30例)生后即给以NCPAP,对照组(30例)生后即给以鼻导管前庭吸氧,观察生命体征、血气指标变化。结果:观察组的各项指标比较稳定优于对照组,差异有显著性(P〈0.05)。观察组总有效率93.33%,对照组总有效率66.67%,差异有高度显著性(P〈0.01)。结论:NCPAP防治新生儿肺透明膜病效果显著且安全。 相似文献

目的评价肺表面活性物质(PS)联合鼻塞持续正压通气(CPAP)治疗新生儿Ⅲ度肺透明膜病(HMD)的疗效。方法经X射线胸片检查为Ⅲ度HMD的患儿40例,6小时内经气管内滴入PS后予鼻塞CPAP方式通气20例,行常规机械通气20例。结果PS联合鼻塞CPAP组在生后24小时胸片改善明显,且氧疗时间、肺部感染发生率明显低于机械通气组。结论PS联合鼻塞CPAP治疗Ⅲ度HMD效果明显,可缩短氧疗时间,减少肺部感染。 相似文献

目的检验经鼻持续正压通气对新生儿肺透明膜病(HMD)的治疗效果。方法选取2010年7月至2013年7月在湖北医药学院附属东风医院就诊的HMD患儿64例作为研究对象,男40例,女24例,所有患儿均在出生2h内转入湖北医药学院附属东风医院接受治疗。将64例患儿分为两组,每组32例。两组患儿均给予常规的基础治疗,通气组另采用小儿呼吸机进行经鼻的持续正压通气治疗,观察两组患儿治疗后住院时间、并发症发生率、肺部感染率、持续肺泡内正压参数变化、两组患儿吸入氧分数参数变化和血气分析参数。结果两组患儿的基本资料比较差异无统计学意义(P0.05)。两组患儿治愈例数、住院时间、持续肺泡内正压参数、吸入氧分数参数和血气分析参数比较差异有统计学意义(P0.05)。结论经鼻持续正压通气对HMD在常规治疗的基础上可以达到良好的治疗效果,但是容易出现肺部并发症。 相似文献

目的探讨肺表面活性物质(固尔苏)与鼻塞式持续气道正压呼吸(nCPAP)联合治疗早产儿肺透明膜病(HMD)的疗效和临床应用价值。方法设30例临床诊断HMD的早产儿为治疗组,给予固尔苏加nCPAP治疗;另外30例HMD患儿作为对照组,给予常规机械通气治疗。观察两组患儿的临床及胸片改善情况、吸入氧浓度(F iO2)、氧疗时间、并发症及住院时间。结果治疗组24 h内有明显的缺氧症状及胸片改善,氧疗时间短,肺部感染发生率低于对照组,住院时间短。结论固尔苏加nCPAP治疗HMD疗效肯定,并发症少,值得临床推广应用。 相似文献

After the introduction of Nasal Continuous Positive Airways Pressure (nCPAP) in a neonatal intensive care unit, the incidence of complications and outcomes of newborns was explored. The clinical and nursing records of 64 newborns treated with O2 and Hood's cuff (January 1996-June 1999) were compared to those of 68 patients treated with nCPAP (July 1999-July 2002). Newborns < 32 weeks, with congenital abnormalities and major myocardiopathies were excluded. The two groups were comparable for the main characteristics. Newborns treated with nCPAP needed less intubations (7 vs 14, p 0.004) and had less transferrals to more specialised structures (12 vs 26, p 0.001). The number of complications is comparable with the exception of a larger number of desaturations in the nCPAP group (20 vs 5, p 0.001) that anyhow, did not require resuscitation. The nCPAP allowed a reduction of the number of intubations and therefore newborns could be cared for in our centre, thus reducing the disadvantage of a transferral to a more specialised centre. This advantage, associated to the comparable number of complications, may be attributable to the short duration of the treatment but also to the attentive and appropriate nursing care. 相似文献

OBJECTIVE: The aim of the study was to assess the influence of nasal continuous positive airway pressure (NCPAP) on breathing pattern in preterm newborns. DESIGN: Prospective study. SETTING: Neonatal intensive care unit. PATIENTS: Ten premature newborn infants on NCPAP (gestational age range from 27 to 32 weeks, mean birth weight 1300+/-460 g) admitted in our neonatal intensive care unit (NICU) for respiratory distress syndrome. METHODS: Breathing patterns and changes in lung volumes level were obtained using respiratory inductive plethysmography (RIP), at random CPAP levels (0, 2, 4, 6 and 8 cmH2O). Raw data were analysed for end-expiratory lung volume level (EELV-level), tidal volume (Vt), respiratory rate, phase angle and labour breathing index (LBI). RESULTS: CPAP increased EELV-level by 2.1+/-0.3xVt from 0 to 8 cmH2O ( p<0.01). Vt increased by 43% from CPAP of 0 cmH2O to CPAP of 8 cmH2O ( p<0.01). We also found that CPAP lowered the phase angle (from 76+/-21 degrees at CPAP of 0 cmH2O to 30+/-15 degrees at CPAP of 8 cmH2O; p<0.01 ) and LBI (from 1.7+/-0.8 at CPAP of 0 cmH2O to 1.2+/-0.3 at CPAP of 8 cmH2O; p<0.05). CONCLUSION: NCPAP improves the breathing strategy of premature infants with respiratory failure, as reflected by improved thoraco-abdominal synchrony, increased Vt and reduction of the LBI. This effect is associated with an increase in EELV-level with CPAP level. However, further investigations are necessary to establish the best CPAP level that ensures both safety and efficiency. 相似文献

Nasal continuous positive airway pressure (n-CPAP) is an effective treatment for the obstructive sleep apnea syndrome (OSAS). It is currently regarded as the first line therapy for OSAS. The principal indication for n-CPAP treatment is daytime sleepiness. Nasal-CPAP improves daytime sleepiness dramatically in severe cases and the effect is objectively measurable with the multiple sleep latency test (MSLT). It is noteworthy that n-CPAP also improves symptoms, subjective daytime sleepiness, cognitive function, IQ, mood, quality of life and driving ability already in patients with mild sleep apnea with an apnea/hypopneaindex (AHI) between 5 and 15 per hour of sleep during overnight polysomnography. Although not yet 100% robust, there is clear evidence that patients with OSAS have an increased frequency of systemic hypertension. Some early and imperfect studies suggest that CPAP reduces cardiovascular and cerebrovascular outcomes; however unequivocal evidence that n-CPAP reduces mortality is still awaited. There is now good evidence that treatment with n-CPAP reduces the two- to sevenfold increased risk of road accidents of untreated patients with OSAS. In summary, there exists abundant evidence today that n-CPAP is an efficient therapy for symptomatic patients with the obstructive sleep apnea syndrome. A trial with n-CPAP is therefore justified in all symptomatic patients. Based on the large number of randomized controlled trials of n-CPAP a therapeutic trial is indicated even in only mildly symptomatic patients with OSAS. Nasal-CPAP use and outcomes of therapy can be improved by provision of an intensive CPAP-education and support program. 相似文献
