The aim of this study was to evaluate the impact of national leprosy eradication programme (NLEP) in Satara District. A retrospective record based study was conducted in the district leprosy control unit with evaluation indicators like prevalence rate (PR), new case detection rate (NCDR), percentage of cases released from treatment, proportion of female cases among new cases, proportion of multi bacillary (MB) cases among new cases, proportion of grade-2 disability among new cases and proportion of child cases among new cases and trend was analyzed for statistical significance by using Chi-square test. During study period from year March 2003-04 to March 2007-08, there was decrease in PR of leprosy from 2.46/10000 population to 0.62/10000 population, NCDR decreased from 3.46/10000 population to 0.77/10000 population and trend was statistically significant (p<0.001*, P<0.001*) respectively. Declining trend of grade 2 disability among new cases was noted and this was statistically significant (p<0.01*). Proportion of female cases among new cases showed little fluctuation from 43.3% to 47.5% but trend was statistically significant (p<0.03*). Proportion of MB cases among new cases increased from 44.4% to 53.6% while proportion of child cases among new cases decreased from 16.5% to 12% and trend was statistically significant (p <0.001*, p<0.001*) respectively. The NLEP has a favorable impact towards elimination of leprosy in Satara District, Maharashtra.  相似文献

Multi-drug therapy (MDT) has been successfully implemented in all leprosy endemic countries. Prevalence of leprosy has declined remarkably after the introduction of MDT. Detection of new cases did not show expected decline in many endemic and low endemic situations. Bihar in India started implementing MDT in 1993. The Damien Foundation India Trust (DFIT) supported the leprosy control programme in Bihar by providing a district technical support team (DTST) for each district assigned to DFIT. Effective coverage was achieved in 1996-98. Data for the period 1996-2004 from 10 districts are presented in this paper. The total population in these districts was 29.4 million. Deformity among newly detected leprosy patients declined to 1% indicating effective early case-detection. Intensive new case-detection activities were in vogue contributing to high new case-detection rate (NCDR). The NCDR remained high during the 9-year period reported here and did not show any declining trend.  相似文献

The analysis of computerized data of patients in our Rural Field Operation Area (Kunrathur Taluk, Kancheepuram District, Tamil Nadu) from the start of MDT in 1986 has shown a decrease of leprosy prevalence from 275/10000 in 1986 to 0.7/10000 in 2005. Leprosy has been eliminated as a public health problem after 19 years of MDT implementation. Although the control programme was started in 1962, MDT implementation began only in 1986. The new case-detection rate has declined significantly from 27.3 in 1987 to 2.4/10000 in 2005 (y = -1.6x + 2325.1, p = < 0.05). The age-specific cumulative detection rates calculated showed highest case-detection at 10-14 years for total, 10-14 years for PB, 50-54 for MB, and 10-14 for both males and females. MB percentage was more among new cases in the last three years as compared to the initial three years, and this difference was found to be statistically significant, but there was no significant difference between the first three and the last three-year periods in child, male and disability rates (grade +/-2) among new cases. Thus, the declining trend in NCDR has not reflected any change in sex and age-groups of new cases. This analysis strengthens the hypothesis of sub-clinical cases possibly transmitting the disease and MB cases accruing after long incubation period.  相似文献

The annual reports of the national leprosy control programme in Nigeria were reviewed to study the trends of the indices of leprosy control from 1992 to 2003 and determine the influence of operational and policy factors. By 2003, both national prevalence and case detection rates had reached below 0.5 per 10,000. Sub-nationally, all except three contiguous States in the Southeast, had prevalence rates below one case per 10,000. Over the 12 years, the prevalence rate decreased by 94-1%, from 7.14 to 0.42 per 10,000, with two periods of rapid decline: 1992-1994 and 1998. Remarkable surges of discharges from multi-drug therapy (MDT) occurred in these same periods. The period 1992-1994 corresponds to the years of introduction of MDT, case reviews, and clean-up of leprosy registers nationwide, while 1998 corresponds to the year the programme adopted the shortened 12-month MDT regime for multibacillary (MB) leprosy. The overall trend of case detection since 1992 was relatively stable, but had three significant periods of initial increase (1992-1994), stability (1994-1999) and recent decline (1999-2003), apparently related to the changing levels of activeness of the national programme. The pattern of new cases detected revealed increasing MB classification and lower disability, but a relatively stable child rate since 1992. The trend of MB proportion was also related to the years of MDT introduction and the adoption of a new leprosy case definition and classification policies. Thus, Nigeria has attained a low leprosy endemic status-mainly through operational and policy influences. The challenges that remain include reducing the relatively high leprosy burden in the Southeastern States and evolving effective case detection interventions that will make an observable impact on the incidence of leprosy.  相似文献

The prevalence of leprosy fell below 1 per 100,000 in Shandong province in 1994. Since then, a few incident leprosy cases have been reported each year over the past 10 years. In order to explore whether or not the reduction in case detection in the province was due to the stopping of active case-finding activities, a rapid village survey was conducted in a formerly endemic county, using skin disease service. Ninety-one villages in 23 townships, covering a total population of 104,885, were surveyed. No leprosy case was detected. The results of the survey, along with other evidences, such as higher MB/PB ratio among newly detected cases and late onset of the disease, suggest that leprosy is dying out as a disease in the county as well as in Shandong. The efforts of the leprosy control programme should shift to other services such as prevention of disability and care for the disabled.  相似文献

目的:观察贵州省兴义市重复开展消除麻风运动对当地新发现麻风病人的影响.方法:收集1996~2004年新发现麻风病人的有关资料进行比较分析.结果:9年共发现麻风病人166例,平均每年发现18.4例,年均发现率为2.65/10万.1999年首次开展消除麻风运动后3年,新病人中的2级畸残率和延迟期呈明显下降趋势,低于开展前3年的水平.两次开展消除麻风运动时的畸残率和平均延迟期均达较高水平,提示该地区虽两次开展消除麻风运动但麻风流行程度仍处于较高水平.结论:重复实施消除麻风运动可促进某些流行病学指标的好转,应反复多次开展.  相似文献

目的:了解广东省韶关市1996~2015年麻风病例流行学特征,为低流行状态下麻风防治工作提供依据。方法:收集近20年新发现麻风病例资料,并对其流行病学特征进行分析。结果:20年来麻风病的发现率和患病率呈下降趋势,地区分布不均衡,南雄市新发病例数最多,占总数的64.46%。从发病到确诊的平均延迟期为(58.75±64.31)个月,以多菌型为主,占73.08%,2级畸残比为23.08%,出现2级畸残者的病期显著长于无畸残者(t=2.16,P<0.05)。病例发现以门诊发现等被动发现方式为主,约占67.31%。结论:近20年来韶关市麻风流行状况已经得到有效控制。  相似文献

目的:观察湖南省西部2个麻风高流行县重复开展消除麻风运动对当地麻风患者发现及临床指标的影响。方法:收集自1997—2002年麻风新发病例,其中包括开展消除麻风运动年份的病例资料,对各年发现病人数,平均确诊年龄,畸残率,延迟期等进行比较分析。结果:从1997年至2002年共发现133例患者。平均每年发现22.2例,年发现率2.4/10万。1998年开展LEC时发现的新病人数是接近开展IEC前一年的2倍,以后新发现病人数减少到一个稳定的低水平,在2002年重复开展LEC时,发现数又是接近前一年的2倍。但与1998年首次开展LEC时发现数相比,减少了50%。新发病人的疾病延迟期平均为34.8月,疾病延迟期在开展LEC前后未显示有缩短或延长的趋势。133例患者中,35例有可见畸残(26.3%)。但新发现病人中2级畸残率从1998年的32.0%下降到2002年重复实施LEC时的12%。结论:作者认为重复实施消除麻风运动促进了当地某些流行病学指标的好转。  相似文献

An impressive decline in leprosy prevalence rate (PR) in all endemic districts of India is seen in the post-MDT era. However, the new case-detection rate, an important statistical indicator in leprosy control programmes, has not shown significant decline in spite of all efforts. In Himachal Pradesh, a decline in PR from 7.8 to 0.56/10000 between 1991 to 2000 is seen, and recently the State has won national acclaim for having achieved the goal of elimination of leprosy in all the districts. The vertical leprosy programme has been integrated into general health services of the state. An analysis of data from 1991 to 2000 of two leprosy control units of Himachal Pradesh, the Urban Leprosy Clinic in Shimla (ULC-S) and the District Leprosy Control Unit in Mandi (DLCU-M), showed no significant decline in the new cases detected. 277 and 271 new cases were detected at these centres respectively; these included 2.2% and 1.5% children of less than 14 years of age. Almost 75% of these cases were males and of MB type. A steadily increasing trend in the annual detection of new cases was seen at both the centres during the decade. The cases registered at DLCU-M were mainly indigenous to the district. At ULC-S, 45 migrant cases from other endemic areas-mainly from Nepal, Bihar, and Uttar Pradesh-had also contributed to the increased number of new cases. Other possible causes discussed for this higher new case detection, e.g. overdiagnosis, detection of backlog "hidden cases" and voluntary reporting of patients, do not differ from those seen in other parts of the country or the world.  相似文献

目的:通过比较历年新发麻风病例2级畸残比,评价我省综合防治措施在麻风病例早期发现中的作用。方法:近10年来,我省相继开展了麻风可疑者筛查、密切接触者检查、大众宣传、人员培训、疑似病例转诊、省级诊断确认、报病奖励、强化转诊中心、麻风症状监测等病例发现措施。结果:我省2011-2020年共发现113例新发病例,其中男71例,女42例,平均年龄52.2岁,2例儿童病例。输入病例31例,主要来自云、贵、川等高发省份,以外来媳妇和务工人员为主。本土病例主要来自原高、中流行市县,以农民(工)为主。2011-2015年平均就诊延迟期为31.7个月,2016-2020年为13.0个月,差异有统计学意义(t=2.101,P=0.04)。2011年新发病例中G2D占比40%,2020年下降至20%。多因素分析发现,诊断延迟期超过24个月与2级畸残的发生相关。结论:我省采取的麻风病例综合早期发现措施有效降低了新发患者G2D占比。  相似文献

通过全国麻风防治管理信息系统(LEPMIS)收集陕西省2004-2020年麻风病例资料,对新发麻风患者的基本情况进行回顾性分析。研究纳入了305个麻风新发病例,其中男212例,女93例,MB型麻风272例(89.2%),PB型麻风33例(10.82%)。60岁以上患者49例,20~59岁247例,20以下患者9例。100例(32.8%)有家庭内接触史。42例(13.8%)出现麻风反应,247例(81%)出现神经损害,男性神经损伤率高于女性(86.3% vs 68.8%)。218例(71.5%)出现畸残,其中II级畸残122例,I级畸残96例。  相似文献

目的:分析2010-2020年广西新发现麻风病例的流行特征。方法:描述性分析2010-2020年广西全区在全国麻风防治管理信息系统中上报的新发现麻风病例的病历资料。结果:2010-2020年,广西麻风新发病例年发现率从0.13/10^(5)降至0.06/10^(5),年均下降7.1%;共发现新发麻风病例499例,其中男333例,女166例,平均年龄(44.16±16.87)岁,儿童病例12例(2.4%);2级畸残98例(19.6%),平均延迟期为27个月,早期发现比例为31.9%。发现方式以皮肤科就诊为主(347例,占69.5%),通过接触者检查发现占8.8%,传染源来自家内的病例占27.3%。14个市均有新发病例报道,百色、河池、崇左3个市报告的病例数占广西病例总数的51.9%。结论:广西新发现麻风病例数呈下降趋势,但新发病例的平均延迟期较长,需加强接触者检查、皮肤科医师培训等工作以便早期发现病例。  相似文献

The Bangladesh Acute Nerve Damage Study (BANDS) is a prospective cohort study designed to investigate epidemiological, diagnostic, therapeutic and operational aspects of acute nerve function impairment in leprosy. The study is based at a single centre in Bangladesh, in an area with a high prevalence of leprosy. The centre, Danish Bangladesh Leprosy Mission, has a well-established vertical leprosy control programme. In this paper, the study design and methodology are described, together with definitions of nerve function impairment (NFI) used in this and subsequent papers. The study recruited 2664 new leprosy cases in a 12-month period. The male:female ratio is 1.25:1, and 17.61% of the cohort are under 15 years of age. In all, 83.33% of the cohort are paucibacillary (PB), and 16.67% multibacillary (MB). However, the MB rate amongst males is 19.72%, and amongst females is 12.85%, despite an equal period of delay to diagnosis. 55% of patients presented for treatment within 12 months of developing symptoms 6.12% of the total number of cases were smear positive, and 36.71% of the MB cases were smear positive. 9.61% of the total number of cases were graded as having World Health Organisation (WHO) disability grade 1, and 5.97% had grade 2. Amongst MB cases, 27.48% had WHO grade 1 disability present, and 18.24% had grade 2 present, compared with 6.04% and 3.51%, respectively, amongst PB cases. A total of 11.90% of the cohort had sensory NFI of any kind, and 7.39% had motor NFI. Ninety patients presented with NFI needing treatment (3.38%), and of these, 61 (67.78%) had silent NFI. MB patients had a prevalence of reaction/NFI needing treatment nearly 7 times higher than PB cases (15.32% amongst MB; 2.30% amongst PB), and males nearly double that of females (5.67% amongst males, 2.96% amongst females). The most commonly affected nerve by function impairment was the posterior tibial (sensory) with 6.46% of nerves affected (9.38% of patients), followed by the ulnar nerve with 3.23% of nerves impaired (5.56% of patients). Future research and publications, building on this foundation, will focus on the following areas: the incidence of NFI and reactive events, the risk factors for developing NFI, and the response to treatment of patients developing acute NFI.  相似文献

Through the leprosy surveillance system established in 1990s the authors analysed the epidemiology of leprosy in Sichuan province from 1951 to 1996. By the end of 1996, the total number of registered cases was 32,772, the peak incidence rate (2.44/100,000) was in 1958. The annual average progressively decreasing speed (AAPDS) of the number of new cases, incidence rate, incidence rate among children, number of newly detected patients, detection rate, number of registered cases and registered prevalence rates were 4.7% (xg = 0.9534), 5.9% (xg = 0.9407), 7.4% (xg = 0.9263), 6.7% (xg = 0.9326), 8.1% (xg = 0.9195), 10.9% (xg = 0.8913) and 11.7% (xg = 0.8828) respectively. Among newly detected cases, the proportion of children gradually declined with an AAPDS of 2.9% (xg = 0.9712), but that of household contacts gradually increased, showing a gradual decline of source of infection. The multibacillary (MB) rate among new cases, newly registered cases and active cases also increased gradually. The MB rate among child cases was much lower than that among adults. The disability rates among newly registered cases tended to decline. The peak incidence was in the 20 to 35 years age group, the average age at onset being 31.92 years. Average age at onset gradually increased to 34.19 during 1991-1995. The average interval between onset of symptoms and diagnosis was 4.89 years and that had reduced gradually to 3.24 years during 1991-1995. The authors conclude that: (1) the epidemiological trend of leprosy in Sichuan province shows decline; (2) the case-finding activities have improved and intensified; but (3) the disability rate among newly detected cases is still high (> 20%) and the average delay in diagnosis is still too long, showing that early case finding is still not satisfactory.  相似文献

目的:分析云南省1990-2018年新发麻风患者2级畸残情况。方法:对1990-2018年11010例新发麻风患者资料进行分析。结果:新发病例中2级畸残2647例,2级畸残率24.04%。4006例少菌型患者中2级畸残者1294例(32.3%), 7004例多菌型患者中畸残者1353例(19.3%)。对其中的2202例畸残者进一步分析,单侧和双侧眼部畸残分别为138例和184例,手部单双侧畸残分别为742例和430例,足部单双侧畸残分别为384例和144例。结论:少菌型(PB)比多菌型(MB)麻风患者畸残比例更高,手部畸残明显高于眼部和足部,手部和足部畸残常呈不对称性。  相似文献

Background Leprosy is far from being eliminated with more than 200 000 new cases detected (NCD)/year. Objective Retrospective analysis between 2003 and 2009 to compare the New Case Detected Rate (NCDR) observed in Italy in the immigrant population with the NCDR of the same population in their country of origin to verify if the cases observed are those expected or not. Methods Leprosy statistics were retrieved from the Italian leprosy register and from official WHO data. Results The NCD in Italy were lower than expected, from 2003 when the expected number of NCD was 40.5 between the legally resident immigrants, but only one case was diagnosed (98% of lower from the expected), to 2009 when four NCD were diagnosed and 41 were expected (90% lower from expected). Conclusions This study points out a discrepancy between the observed and the expected cases of leprosy in Italy. Specifically, the number of NCD was less than expected for each studied year. Of course our data do not represent a validation, but only an indication of the leprosy diagnosis in Italy. Difficulty in accessing the health systems, fear of segregation, ignorance and illegal immigrant status with consequent fear of police arrest are possible explaining factors. The critical issue anyhow is the medical expertise. The role of the dermatologist is fundamental. For these reasons, there is still a need for wide spread leprosy teaching programmes. Although with few limitations, this study represents a first approach to validate the accuracy in leprosy diagnosis in Italy.  相似文献

From 1979 to 1999, the ALERT leprosy control programme has covered a well-defined area in central Ethiopia using standardized case finding strategies. During this period, the leprosy prevalence has decreased more than 30-fold, there has been a 3-fold decrease in case detection and a 6-fold decrease in the case detection rate. The proportion of MB patients among new cases increased by around 80% and the proportion of children among new cases decreased by around 60%. Several factors may have contributed to these trends. The impact of the introduction of MDT and the shortening of the duration of the MB regimen are shown, but other factors are also discussed at length: an increase in the population of the area, cleaning up of the registers, changing case definitions, changes in staff motivation and fluctuations, even small ones, in case finding intensity and coverage. Do the observed trends reflect a reduction in the transmission of the leprosy infection? Because of the many confounding factors, it would be difficult to answer that question positively at present. Additional rigorous data collection and analysis is required.  相似文献

Elimination of leprosy as a public health problem, defined by a registered prevalence of less than one case per 10,000 population, was achieved by Bangladesh in 1998, and steady reduction in prevalence is ongoing. It is less certain whether a sustained reduction in case detection is occurring, with little overall change in some longstanding programme areas, though the overall annual new case detection rate has fallen by over one-third between 1996 and 2004, from 9.8 to 6.1 per 100,000. Concerns about 'hidden' cases have been raised in Bangladesh as elsewhere, though the National Leprosy Elimination Campaign of 1999 detected relatively fewer new cases than in other countries, and mainly in low endemic areas. Investigation into the correct diagnosis of leprosy and recording and reporting practices has not suggested high levels of over-diagnosis or 'over-registration'. Both before and since achievement of the elimination target at national level, the collaboration of Non-Governmental Organizations with the national Leprosy Elimination Program has been considerable. NGOs now support ongoing leprosy control efforts in rural and urban populations, moderate to high endemic for leprosy, amounting to 50% of the entire population, and NGO staff look after 75% of all new cases in Bangladesh. This close collaboration has highlighted the potential for large-scale partnership in disease control, and has expanded to extensive partnership in tuberculosis control, which will hopefully enhance cost-effectiveness and quality of both programmes. Further challenges remain in the area of urban leprosy control, where leprosy case finding represents 30% of the whole country, but public health infrastructure and community organization is weakest. Sustaining of leprosy services in the long term is a significant concern, and new modes of collaboration, with a more technical, supportive role for NGOs in some areas is being piloted.  相似文献

An innovative method that combined awareness creation with screening of high school students by their peers was undertaken in 26 randomly selected schools in the project area of the Schieffelin Leprosy Research and Training Center, Karigiri, Vellore, India. This method entailed educating teachers and student leaders in grades 8-12 about leprosy and how to suspect leprosy among their peers. The student leaders in turn conducted a similar awareness programme for their peers and encouraged them to report if they suffered from any skin problem or skin lesion. Based on the reporting by their peers, the class leaders prepared a 'suspect list'. Within a fortnight of the awareness program, a trained leprosy worker visited the school and examined all the students on the 'suspect list'. Those diagnosed to have leprosy were referred to a medical officer, who then confirmed the diagnosis and initiated treatment. Among the 23,125 students enrolled in the 26 randomly selected schools, 234 student leaders were educated about leprosy and trained to detect suspect lesions among their peers. A total of 2200 (9.5%) children reported with skin lesions to their leaders and after screening by a leprosy supervisor and confirmation by a medical officer, 14 new cases (NCDR 6.05/10,000) were detected. This rate was found to be comparable with case detection rates of annual school surveys done during the National Leprosy Eradication Programme (NLEP), when all schoolchildren were examined. The paper suggests that schoolchildren can be used effectively in leprosy case detection and this method has the additional advantage of creating awareness among them, their teachers and communities.  相似文献
