Objectives

To evaluate accuracy of computed tomography angiography (CTA) in evaluation of post traumatic renal vascular injury.

Patients and methods

38 patients were presented with post traumatic intermittent or persistent hematuria. Renal CTA and digital subtraction angiography (DSA) were done for all patients.

Results

CTA demonstrated pseudoaneurysm (PA) in 30 patients (78.9%) and no vascular lesions in 8 patient (21.1%). CTA had 86.11% sensitivity and 50% specificity in detection of post traumatic renal pseudoaneurysms, CTA missed diagnosis of renal arteriovenous fistula (RAVF) in 10 patients which discovered later by DSA.

Conclusion

CTA with MIP as non invasive technique widely replaced renal DSA in detection of posttraumatic renal pseudoaneurysm. Renal DSA is still best modality in detection of RAVF and also has the upper hand in planning of selective renal artery embolization for the management of persistent or delayed hemorrhage from renal vessels.  相似文献

Introduction

Endometrial polyps are a common cause of abnormal uterine bleeding. Ultrasound is the most accepted investigation to evaluate them. However, the diagnosis can be difficult and even missed on transabdominal and grayscale imaging. Our study aims to assess the evaluation of endometrial polyps on transvaginal color Doppler.

Patients and methods

A prospective study with performance of transvaginal Doppler and saline infusion sonohysterography with histopathological correlation.

Results

The pedicle artery sign on transvaginal color Doppler sonography was found to have sensitivity of 86.67%, accuracy of 86.67% and positive predictive value of 100%.

Conclusion

Addition of Doppler to transvaginal sonography and Saline infusion Sonohysterography improves characterization and diagnosis of endometrial lesions.  相似文献

Purpose

to evaluate the role of three dimentional power doppler ultrasonography (3D PDUS) of the uteroplacental circulation in early pregnancy as a screening tool for prediction of preeclampsia.

Patients and methods

50 pregnant women who came to do routine ultrasound scan between 11 and 13.6?weeks of pregnancy during the period from February 2016 to April 2016. Placental volume and vascularization indices were obtained, and vascular index (VI), flow index (FI), blood vessels and blood flow index (VFI) were calculated by three-dimensional Doppler histogram.

Results

Of the included 50 pregnant women, 22% developed preeclampsia and 78% did not develop preeclampsia. The placental blood flow perfusion in patients with pre-eclampsia had a lower VI, FI and VFI compared to non preeclamptic women with highly significant statistical difference (P?<?0.001)

Conclusion

3D PDUS can be used as an effective screening tool to predict preeclampsia in early pregnancy.  相似文献

Aim

The aim of this study is to evaluate the role of high resolution ultrasonography (HRUS) of the hip in premature neonates admitted to the neonatal intensive care unit (NICU) in diagnosis of septic hip arthritis.

Patients and methods

This prospective study was done for twenty premature neonates having clinical and laboratory findings compatible with the diagnosis of acute septic arthritis. They were subjected to HRUS of the hip as well as US-guided aspiration and analysis of synovial fluid.

Results

Hip ultrasonography showed synovial fluid containing echoes in twelve patients and was clear in six patients. Joint capsule was thickened in fourteen patients. Seventeen patients had sonographic features of septic arthritis. The sensitivity of HRUS was 93.8%, specificity 50%, positive predictive value 88.2%, negative predictive value 66.7% and the accuracy was 85%.

Conclusion

HRUS is beneficial in early diagnosis of septic hip arthritis in premature neonates admitted to the NICU. It is an easy, available and rapid procedure.  相似文献

Purpose

To compare the role of ultrasonography and MRI in the diagnosis of synovitis involving wrist and hand joints in Rheumatoid arthritis patients.

Materials and methods

Fifty patients with rheumatoid arthritis involving the wrist were subjected to hand and wrist ultrasonography Doppler and MRI and the results are compared and correlated during the period from December 2015 to January 2017.

Results

Ultrasound detected synovial hypertrophy (pannus) in 42 wrist joints while MRI detected it in 46. Power Doppler detected increased vascularity within 30 wrist joints (60%), MRI detected synovial enhancement within 38 (76%). Power Doppler detected synovial activity (vascularity) in MCP joints of 13 patients and MRI detected synovial activity (enhancement) in MCP joints of 9 patients. Ultrasound detected erosions in 35 wrist and 27 MCP joints and MRI detected erosions in 37 wrist and 25 MCP joints. Ultrasound detected tendinitis in 9 extensor tendons and MRI detected tendinitis in 8 tendons.

Conclusion

We found a significant correlation between power Doppler ultrasonography and MRI in the detection of signs of inflammation and bone destruction in the wrist and hand joints in rheumatoid arthritis, which raise the potential importance of power Doppler study in the assessment and monitoring of disease activity as a reliable non-invasive method.  相似文献

Background

The systematic use of MDCT angiography in patients with haemoptysis is especially helpful to identify ectopic bronchial arteries, non-bronchial systemic arteries and pulmonary artery pseudoaneurysms.

Aim of this study

Aim of this study is to assess the role of MDCT angiography in the evaluation of abnormal bronchopulmonary vasculature in patients with haemoptysis.

Results

The study was performed on 36 patients presented with haemoptysis and showing vascular abnormalities on CT angiography. Tuberculosis was the most common underlying disease. Dilated bronchial arteries (BAs) were detected in 86% of patients; they originate between D4 and D6 vertebral levels. About 95% of right BAs have their ostia between 9 and 10o’clock positions, while 75% of left BAs originated between 11 and 1o’clock positions. This study included 8 non bronchial systemic arteries with the inferior phrenic arteries were the most common. Significant pleural thickening (>3?mm) may predict non-bronchial arterial supply. Pulmonary arteries had a role in haemoptysis in only 2 patients.

Conclusion

This study has shown that MDCT angiography is a valuable non invasive diagnostic tool in the evaluation of haemoptysis. It provides a roadmap of the vascular abnormalities and aids in the diagnosis of the underlying disease.  相似文献

Objectives

Our aim in this study was to evaluate the efficiency of combined positron emission tomography and computed tomography (PET/CT), in the detection of the primary tumor origin in patients with cancer of unknown primary (CUP).

Methods

PET/CT were performed for 52 patients with CUP. These patients presented with metastatic lesions that were proven pathologically, and no primary site was detected by previous investigations.

Results

The number of patients with true positive primary tumor sites was 29 (55.8%), 4 patients with false positive results (7.7%), 9 patients with true negative results (17.3%) and 10 patients with false negative results (19.2%). The sensitivity was 74.4%, specificity was 69.2% and accuracy was 73.1%. Our positive predictive value in this study was 87.9% and our negative predictive value for this study was 47.4%.

Conclusions

This study showed an advantage of PET/CT in the detection of the site of the primary tumor in patients diagnosed with CUP.  相似文献

Aim

To evaluate the role of MRI in detecting possible causes of incontinence in patients with anorectal malformations after the initial corrective surgery.

Materials and methods

The study included 17 patients; age ranged from 1 to 18?years. T1 and T2 weighted images were obtained in axial, coronal and sagittal planes, sequences were obtained without fat saturation to easily identify low signal muscles against high signal fat.

Results

MRI accurately assessed the grade of development of sphincter muscles; muscles graded as good in 4 patients, fair in 2 patients, poor in 5 patients and 3 cases had asymmetrical muscles. The position of pulled through bowel was laterally mislocated to the left side of the sphincter muscles in 2 cases and was anteriorly mislocated in 5 cases. Inadvertently placed mesenteric fat around the pulled through bowel was depicted in 10 out of 17 patients. Associated anomalies of the vertebral column, spinal cord and urogenital system was depicted in 11 patients. One case had classical caudal regression syndrome.

Conclusion

MRI is very useful in the postoperative assessment in patients with anorectal malformations. It plays an important role in diagnosis of possible causes of incontinence, postoperative complications and detecting associated anomalies in one study.  相似文献

Objective

This work aimed for detection of different congenital and acquired abnormalities affecting the pulmonary veins using multislice computed tomography (MSCT).

Patients and methods

This study was carried on 74 patients. Cases were referred to Radiology department of Kasr Al-Ainy with different chest complaints for MSCT and only cases showing pulmonary venous abnormalities were included in this study. All patients underwent MSCT of the chest using different MSCT protocols according to the initial presentation of the patient.

Results

This study included 74 patients with pulmonary veins abnormalities; congenital anomalies of the pulmonary veins were observed in 36.4% of cases and acquired abnormalities in 62.2% of cases, one case had both congenital and acquired pulmonary vein abnormalities (1.4%). The most common congenital anomaly was pulmonary vein varices while the most common acquired abnormality was pulmonary vein stenosis or obstruction.

Conclusion

The presence of different pulmonary venous abnormalities can be easily delineated using MSCT, with its MPR capability, as it provides excellent resolution and a large field of view.  相似文献

Purpose

To estimate the accuracy of ultrasonography in detection of tendinous and ligamentous injuries around the ankle in comparison to MRI.

Materials and methods

60 patients referred with unilateral painful ankles are subjected to ankle ultrasonography and the results are compared with ankle MRI between December 2015 and September 2016.

Results

132 pathologies including 62 tendon lesions, 46 ligamentous lesions, 10 bursitis and 14 joint effusion were diagnosed by MRI. Ultrasonography detected 59 tendinous lesions (missed 3 partial tears) with allover accuracy of 96%, 41 ligamentous lesions (missed 1 stretching lesion, 2 partial tears and 1complete tear) with allover accuracy of 94.3%, all bursal lesions detected and 2 cases with joint effusion were missed by ultrasonography.

Conclusion

Ultrasonography is an accurate and sensitive modality in detecting tendinous and ligamentous lesions around the ankle and represent with MRI complementary tool for diagnosis and can be used alone is some conditions  相似文献

Objective

The goal of this study was to highlight the role of follow up CT angiography examination in detection and classification of endoleaks and therefore deciding management plans after endovascular abdominal aortic aneurysm repair (EVAR).

Patients and Methods

During one year duration 37 patients who have been operated were examined 1 and 6 months after EVAR as routine follow up. The images obtained were interpreted and reconstructed using dedicated software and work stations.

Results

Out of 37 cases, 14 cases (37.8%) had positive endoleaks and 23 cases (62.2%) were free. Type I endoleak was diagnosed in 4 cases (10.8%) and type II endoleaks was diagnosed in 10 cases (27%) as 7 cases (18.9%) showed leak through lumbar arteries and 3 cases (8.1%) showed leak through the inferior mesenteric arteries.

Conclusion

CT angiography can accurately detect and classify endoleaks and thus determine line of treatment. Endoleaks are often asymptomatic and may become evident intra operatively or many years after the operation, therefore lifelong imaging supervision is necessary.  相似文献

Objectives

Multi-detector computed tomography (MDCT) and cone beam computed tomography (CBCT) were compared regarding their ability to detect vertical root fractures.

Methods

Sixty four extracted posterior teeth were included in this study. Using a diamond disc, thirty six teeth were cut vertically to simulate a VRF. Twenty eight teeth were used as control. Fractured and non-fractured teeth were randomly positioned in 4 skulls and mandibles. Scanning was performed first on a 16 slice Siemens MDCT, then by i-CAT Next Generation CBCT. Two observers assessed the multiplanar images for vertical root fractures using a 3-point scale. The first observer repeated the assessment after 1?week. Later, the 2 observers re-assessed the images together to reach a consensus score.

Results

CBCT showed higher sensitivity, accuracy as well as negative predictive value compared to MDCT. The mean area under the curve was 0.917 for MDCT and 0.972 for CBCT. The difference in diagnostic accuracy between the 2 modalities was statistically significant P?=?.036. Inter-observer agreement was 0.971 for MDCT and 0.994 for CBCT, whereas intra-observer agreement was 0.981 for MDCT and 0.985 for CBCT.

Conclusion

Using the specified scanners at the specified exposure parameters, the diagnostic accuracy of CBCT in detecting vertical root fractures was significantly higher than MDCT.  相似文献

Objective

To compare the accuracy of 2D and 3D CT measurements of femoral anteversion angle, in pediatric patients with developmental hip dysplasia.

Materials and methods

Twenty patients (20 hips) with unilateral non syndromic DDH were studies. CT scans were performed using a 16 slice CT scanner to measure the femoral anteversion angle (FAVA) using 2D & 3D techniques. Findings were correlated with the intra operative measurements.

Results

There was a significant difference between 2D & 3D methods. Results of clinical assessment were comparable to results of 3D CT assessment which range from 30 to 50° with a mean of 37.5°. Mean percent difference between 3D and intra operative measurement of FAVA was significantly lower than the corresponding value between 2D and intraoperative measurement of FAVA.

Conclusion

3D is more accurate than 2D in measuring the degree of FAVA in DDH patients. It is easily applied and rapid and doesn't require sophisticated software.  相似文献

Objective

The aim of this study was to assess the role of multislice computed tomography (MSCT) using 2D and 3D images in evaluation of different types of Le Fort fractures.

Subjects and methods

30 cases referred from the Emergency unit in Zagazig University Hospital were included in this study, based on clinical evidence of facial trauma, they underwent multislice computed tomography (MSCT) examination and were shown to be positive for pterygoid fractures.All patients subjected to non contrast MSCT in axial cuts. Multiplanar reformatted (MPR) images were acquired using the machine software in sagittal and coronal planes. Finally images were transferred to a workstation for reconstruction of 3D images.

Results

Sensitivity of multislice computed tomography (MSCT) in detecting compound Le Fort fractures for radiologists A, B and C was 94.4%, 66.7% and 100% respectively. Sensitivity of MSCT in detecting isolated Le Fort fractures for radiologists A, B and C was 100%. The difference between radiologists A, B and C in detecting Le Fort fractures types was insignificant. Good agreement between radiologists A, B, C and intraoperative findings.

Conclusion

Both 2D-CT and 3D-CT are the best methods in imaging of Le Fort fractures.  相似文献

Objective

Vaginal or uterine outlet obstruction leads to hematocolpos or hematometra. Detection of the etiology of this entity is important to guide adequate surgical management and thereby avoid complications and to preserve fertility. The aim of this study was to evaluate obstructing vaginal malformations in adolescent girls presenting with hematocolpos or hematometra with MR imaging.

Materials and methods

A hospital based prospective study was conducted in a tertiary care centre from September 2015 to October 2016. The study included 17 adolescent females who were evaluated with MRI.

Result

Of 17 adolescent female with vaginal or uterine outflow obstructive anomalies with hematocolpos or hematometra, where 6 patients (35.3%) had HWWS, 6 patients (35.3%) had imperforate hymen, 2 patients (11.8%) had transverse vaginal septum, 1 patient each (5.9%) had cervico-vaginal atresia, unicornuate uterus and communicating rudimentary Uterine horn. MRI revealed hematocolpos in 15 patients (88.2%), hematometra in 13 patients (76.5%), endometriotic ovarian cysts in 6 patients (35.3%) and hematosalpnix in 3 patients (17.6%).

Conclusion

Early radiological diagnosis of the cause of vaginal or uterine outflow obstruction is important to guide adequate surgical management which if undertaken promptly helps to avoid complications due to reflux from vaginal or uterine outflow obstruction.  相似文献

Objective

to establish the role of transthoracic ultrasound as a bed-side, available, and affordable technique for imaging chest trauma patients and compared its sensitivity, specificity and accuracy for detecting chest trauma sequelae and complications to those of CT.

Patients and methods

This study included 107 cases. All patients had chest trauma or polytrauma with chest involvement. Transthoracic ultrasound and MSCT of the chest were evaluated. The results were assessed and compared by statistical analysis.

Results

Of the injuries, 13.1% were penetrating, and 86.9% were blunt trauma. With CT as the standard, the most common injury US detected injury was pleural in 60.7% of patients, with diagnostic accuracy of 93.4%. Parenchymal lesions were found in 39.3% of patients with a 64.4% US diagnostic accuracy. Chest wall lesions were found in 15.9% of patients with an 89.7% accuracy, and mediastinal lesions were detected in 9.3% with a 94.3% accuracy.

Conclusion

Chest ultrasonography has significant value for diagnosing complications of blunt and penetrating chest trauma with acceptable sensitivity and high specificity, particularly for pleural lesions and rib fractures. Ultrasound overcomes the difficulties involved in radiological examinations of small children and uncooperative patients.  相似文献

Objective

A comparative study of manual and automated post processing software tools to measure CT parameters before Transcatheter Aortic Valve Implantation (TAVI).

Patients and methods

51 patients were included; all admitted to the Cardiology Department with clinically established severe aortic stenosis, in whom TAVI was the treatment of choice. All patients underwent CT angiography (CTA) pre-TAVI. The CT images were post processed and the needed CT parameters were calculated using both manual and automated methods. The results were compared for statistical analysis.

Results

Only 43 patients completed the study, with mean age 65?years old. The automated software provided accurate almost similar results as the manual method, especially for aortic annulus area and aortic circumference measurements (p-value?=?1 and 0.548) and was accepted for left and right coronary artery ostium height (p-value?=?0.191 and 0.635).However, there was a statistically significant difference for the minor diameter axis and mean annular diameter (p-value?=?0.001 and 0.021).

Conclusion

The automated software introduced rapid, accurate and reliable measurements for the aortic annulus in cases prepared for TAVI and can be used as a standard method. Also, it can be performed by junior staff with avoidable human errors.  相似文献

Aim of work

To evaluate the role of ultrasound and color Doppler in diagnoses and predicting the outcome of necrotising enterocolitis patients at neonatal ICU.

Subjects and methods

Thirty neonates clinically diagnosed with NEC were examined, 51 ultrasound examinations were done including color Doppler sonography and findings were compared with laboratory data, clinical picture, staging, abdominal radiography and patient's outcome.

Results

Bloody stools and abdominal discoloration correlated with highest risk, low PH and neutropenia and increased DAAS score matched poor outcome. echoegnic free fluid, thin bowel wall, absent wall perfusion, aprestalsis, dilated anechoic bowel, increased bowel perfusion and pneumatoses intestinalis matched adverse outcome. sonographic signs of pneumatoses intestinalis interrelated with radiographic signs.

Conclusion

US was found to be highly sensitive and specific in diagnosing, staging and predicting the outcome of NEC patients, based on group of findings that were categorized per their risk ratio and correlated with outcome.  相似文献

Background and objective

Laparoscopic sleeve gastrectomy become widely employed and accepted operation in between Arab population as a permanent solution for morbid obesity. There are many acute complications associated with LSG such postoperative leaks or bleeding, abscess formation, splenic infarction and portal vein thrombosis. The purpose of the study is to present how multidetector CT is beneficial as a screening investigation to detect these complications.

Methods

We retrospectively review CT images done in the first week post operative for280 patients subjected to LSG the center in the period between July 2017-December 2017 (average age 39.5?years old).

Results

the leak was suspected in 3 patients documented by a direct leak of contrast in one of them, 2 patients active bleeding was clinically suspected, but the exact site of bleeding could not be identified by CT, 15 patients had serous fluid collection, splenic infarction was noticed in 18 patients, 3 patients developed postoperative hematoma one of them become infected, anterior abdominal wall hematoma seen in 5 patients, one patient developed portal vein thrombosis.

Conclusion

CT is an important imaging tool to diagnose post LSG complications.  相似文献
