This study aimed to (a) investigate the effect of using a hearing aid in conjunction with a cochlear implant in opposite ears on speech perception in quiet and in noise, (b) identify the speech information obtained from a hearing aid that is additive to the information obtained from a cochlear implant, and (c) explore the relationship between aided thresholds in the nonimplanted ear and speech perception benefit from wearing a hearing aid in conjunction with a cochlear implant in opposite ears.Fourteen adults who used the Nucleus 24 cochlear implant system in 1 ear participated in the study. All participants had either used a hearing aid in the nonimplanted ear for at least 75% of waking hours after cochlear implantation, and/or, hearing loss less than 90 dB HL in the low frequencies in the nonimplanted ear. Speech perception was evaluated in 3 conditions: cochlear implant alone (CI), hearing aid alone (HA), and cochlear implant in conjunction with hearing aid in opposite ears (CIHA). Three speech perception tests were used: consonant-vowel nucleus-consonant (CNC) words in quiet, City University of New York style (CUNY) sentences in coincident signal and noise, and spondees in coincidental and spatially separated signal and noise. Information transmission analyses were performed on the CNC responses.Of the 14 participants tested, 6 showed significant bimodal benefit on open-set speech perception measures and 5 showed benefit on close-set spondees. However, 2 participants showed poorer speech perception with CIHA than CI in at least 1 of the speech perception tests. Results of information transmission analyses showed that bimodal benefit (performance with CIHA minus that with CI) in quiet arises from improved perception of the low frequency components in speech. Results showed that participants with poorer aided thresholds in the mid-to-high frequencies demonstrated greater bimodal benefit. It is possible that the mid-to-high frequency information provided by the hearing aids may be conflicting with the cochlear implants.  相似文献

More and more patients with residual hearing on the contralateral side are becoming candidates for cochlear implants (CI) surgery due to increasing CI. The major benefits of regular binaural hearing are spatial hearing, localization, and signal source discrimination in both quiet and noisy surroundings. In most of the reports, hearing aid fitting was carried out without balancing both the devices. Twelve children and eight adults with residual hearing on the non-operated side were binaurally fitted. Our fitting procedure for the hearing aid was based on the desired sensation level [i/o] method. A loudness scaling was used to adjust the loudness perception monaurally and to balance the volume of both devices. Speech audiometry in quiet and noisy surroundings was conducted both monaurally and in the bimodal mode. The fitting was modified according to the respective test results. In all children and six adults, a measurable gain and/or a subjective improvement of speech perception was achieved. Two adult patients did not accept the new fitting. In seven younger children, loudness scaling was impossible to perform because of age. This was also the case with speech audiometry for two children. A structured bimodal fitting using loudness scaling for both the cochlear implant and the hearing aid results in a subjective and objective amelioration of the patient’s hearing and speech perception.  相似文献

OBJECTIVE: To assess the advantages of binaural hearing for cochlear implant (CI) users using a hearing aid (HA) for the contralateral ear. MATERIAL AND METHODS: The subjects comprised 3 males and 3 females (age range 48-84 years). All of them had been using a CI and HA for > 6 months. Their speech perception was examined in quiet using monosyllables and Japanese Hearing in Noise Test (J-HINT) sentences. Speech perception in noise was examined using J-HINT sentences. Late cortical waves were measured while subjects listened to 1 kHz frequent and 2 kHz target tone stimuli. The latency of the event-related potential (P300) wave was compared for monaural and binaural hearing conditions. RESULTS: Three subjects showed significantly better results for binaural than monaural (CI alone) hearing for monosyllables and HINT sentences (p < 0.05; paired t-test). Subjects with better speech perception had been using an HA for longer than those with poor performance (18.3 vs 4.0 years). The overall average score was better for binaural than monaural hearing in the speech perception test under quiet and noisy conditions. Comparison of the latency of the P300 wave under monaural and binaural hearing conditions showed a significantly shorter latency for the latter (p = 0.02; paired t-test). CONCLUSIONS: Although the use of an HA alone showed marginal benefit for CI users, binaural hearing (CI+HA) resulted in a significant improvement in speech perception under various circumstances.  相似文献

The aims of this study were to: (1) determine if spatial unmasking existed and differed for children with normal hearing, a hearing aid and a cochlear implant (CIHA), and bilateral implants (BICI); (2) determine if binaural advantage and headshadow effect differed between children with CIHA and BICI. Results indicated that most of the CIHA and BICI children demonstrated spatial unmasking, though to a lesser degree than children with normal hearing. Results also indicated that the children with BICI demonstrated greater headshadow effect than those with CIHA. The CIHA and BICI children also differed in binaural advantage, which could be due to the differences in headshadow effect and in detection abilities with the hearing aid versus the second implant.  相似文献

目的 研究调查听障儿童一侧耳植入人工耳蜗后对侧耳配戴助听器,这种电声双模式同时刺激的应用效果.方法 随机测试78名儿童人工耳蜗植入者在使用人工耳蜗和助听器(CI+HA)及单独使用人工耳蜗(CI)时听觉事件相关电位MMN和P300潜伏期及波幅的变化,比较同一患者在三种环境下(安静、S/N-0dB,S/N-15dB)的听觉言语识别率,并进行家长问卷调查.结果 患者使用CI+HA与CI的MMN 引出率分别是 80%和75%,P300引出率分别是93.1%和89%;CI+HA组与CI组MMN和P300潜伏期以及CI+HA组与正常组P300波幅的比较有显著差异(P<0,05);在S/N-0dB时CI+HA组单音节、双音节和声调的识别率均高于CI组(P<0.05),而安静环境下和S/N-15dB时组内比较无统计学意义;开放式问卷调查结果 显示60.26%的患者认为使用CI+HA模式能获得更多不同的听觉帮助.闭合式问卷结果 显示使用CI+HA和单用CI的聆听效果有差别.结论 本文大多数听障儿童使用电声同时刺激的助听模式.大脑中枢处理系统并不拮抗,能发挥协同作用,尤其在改善噪声环境下的言语识别和声调感知.以及声源定位和声信息的利用等方面具有优势.  相似文献

Abstract Conclusion: The use of a hearing aid (HA) in combination with a cochlear implant (CI) significantly improved performance for speech perception in quiet, in noise, and for localization compared with monaural conditions. No significant differences in functional performance were observed following optimization of HA fitting. Objectives: To evaluate the binaural benefits derived from using a contralateral HA in conjunction with a CI in subjects with significant functional hearing in the nonimplanted ear and the effects of HA fitting optimization. Methods: Fifteen adult CI users, intra-subject controls, were enrolled in a prospective repeated-measure multicenter study. Evaluation of performance for speech understanding, localization, and subjective impressions was conducted before and following HA fitting optimization for CI alone, HA alone, and CI + HA. Results: For speech testing in quiet, bimodal scores were significantly better than for HA alone and CI alone conditions (p < 0.01). For speech and noise (S0N0) at 0° azimuth the scores were significantly better in the bimodal condition than for CI alone (p = 0.01), indicating binaural summation. When noise was presented to the HA side (S0NHA) bimodal scores were significantly better than for CI alone (p < 0.01 and p < 0.05, respectively), suggesting a significant binaural squelch effect. Sound localization ability was significantly improved in the bimodal condition compared with the CI alone condition (p = 0.002).  相似文献

A male with unilateral deafness in the right ear since 8 years of age developed a sudden hearing loss in the left ear at age 63. A hearing aid was fitted in the left ear with limited benefit. The right ear received a cochlear implant (CI) 20 months later. Cortical auditory evoked potentials (CAEPs) and speech recognition scores (SRS) were measured in free-field three, six and nine months after implantation with the hearing aid alone, CI alone and bimodal condition (hearing aid and CI together). Three months after implantation the cortical responses for the two ears were similar, despite more than 50 years of unilateral auditory deprivation. CAEPs measured over time show evidence of binaural interaction and improvements in SRS.  相似文献

PURPOSE: We propose theoretical guidelines for cochlear implant selection criteria in adults and children that emphasize the contribution of each ear to the binaural advantage. The guidelines outline options for implanting one ear only, implanting one ear plus use of a contralateral hearing aid, and implanting both ears. METHOD: Our method for selecting the best cochlear implant or hearing aid outcome is derived from the amount of usable hearing of the patient. This approach requires the measurement of aided speech perception with each ear separately and then bilaterally. Guidelines are based on the following: (a) overall level of performance with hearing aids relative to what is expected from a cochlear implant, (b) the contributions from each ear, and (c) whether there is a binaural advantage. CONCLUSIONS: Our general recommendation is to provide the patient with the best opportunity for binaural hearing. Since the guidelines put forth in this article are theoretically based, future efforts should be aimed at examining the validity of these guidelines.  相似文献

OBJECTIVE: The purpose of this pilot study was to document speech perception and localization abilities in patients who use a cochlear implant in one ear and a hearing aid in the other ear. DESIGN: We surveyed a group of 111 cochlear implant patients and asked them whether they used a hearing aid on their unimplanted ear. The first three patients who were available were tested on word and sentence recognition and localization tasks. Speech stimuli were presented from the front in quiet and in noise. In the latter conditions, noise was either from the front, the right, or the left. Localization was tested with noise bursts presented at 45 degrees from the right or left. In addition we asked the patients about their abilities to integrate the information from both devices. RESULTS: Speech perception tests in quiet showed a binaural advantage for only one of the three patients for words and none for sentences. With speech and noise both in front of the patient, two patients performed better with both devices than with either device alone. With speech in front and noise on the hearing aid side, no binaural advantage was seen, but with noise on the cochlear implant side, one patient showed a binaural advantage. Localization ability improved with both devices for two patients. The third patient had above-chance localization ability with his implant alone. CONCLUSIONS: A cochlear implant in one ear and a hearing aid in the other ear can provide binaural advantages. The patient who did not show a clear binaural advantage had the poorest hearing aid alone performance. The absolute and relative levels of performance at each ear are likely to influence the potential for binaural integration.  相似文献

Two profoundly deaf patients were evaluated using the Minimal Auditory Capabilities (MAC) battery of speech discrimination tests. One patient was a multiple-channel cochlear implant patient (MC1) using a wearable speech-processor, the other a prospective cochlear implant patient using a hearing aid (HA). Results from the MAC battery showed that MC1 received significantly more auditory information via the speech-processor than was provided by the hearing aid to HA. These results indicated that the cochlear implant could be a suitable alternative for HA. However, the results indicated that HA gained some minimal benefit from the hearing aid and these results should be taken into consideration when deciding which ear should be implanted.  相似文献

The interaural level difference (ILD) is an important cue for the localization of sound sources. The sensitivity to ILD was measured in 10 users of a cochlear implant (CI) in one ear and a hearing aid (HA) in the other severely impaired ear. For simultaneous presentation of a pulse train on the CI side and a sinusoid on the HA side the just noticeable difference (JND) in ILD and loudness growth functions were measured. The mean JND for pitch-matched electric and acoustic stimulation was 1.7 dB. A linear fit of the loudness growth functions on a decibel-versus-microampere scale shows that the slope depends on the subject's dynamic ranges.  相似文献

OBJECTIVE: To quantify binaural advantage for auditory localization in the horizontal plane by bilateral cochlear implant (CI) recipients. Also, to determine whether the use of dual microphones with one implant improves localization. METHODS: Twenty subjects from the UK multicenter trial of bilateral cochlear implantation with Nucleus 24 K/M device were recruited. Sound localization was assessed in an anechoic room with an 11-loudspeaker array under four test conditions: right CI, left CI, binaural CI, and dual microphone. Two runs were undertaken for each of five stimuli (speech, tones, noise, transients, and reverberant speech). Order of conditions was counterbalanced across subjects. RESULTS: Mean localization error with bilateral implants was 24 degrees compared with 67 degrees for monaural implant and dual microphone conditions (chance performance is 65 degrees). Normal controls average 2 to 3 degrees in similar conditions. Binaural performance was significantly better than monaural performance for all subjects, for all stimulus types, and for different sound sources. Only small differences in performance with different stimuli were observed. CONCLUSIONS: Bilateral cochlear implantation with the Nucleus 24 device provides marked improvement in horizontal plane localization abilities compared with unilateral CI use for a range of stimuli having different spectral and temporal characteristics. Benefit was obtained by all subjects, for all stimulus types, and for all sound directions. However, binaural performance was still worse than that obtained by normal hearing listeners and hearing aid users with the same methodology. Monaural localization performance was at chance. There is no benefit for localization with dual microphones.  相似文献

目的介绍一种有低频残余听力感音神经聋的人工耳蜗植入技术,探讨人工耳蜗植入手术对有残余听力患者的治疗效果和价值。方法15例有残余听力的患者接受了保护残余听力的人工耳蜗植入手术。术中电极植入深度在19mm~24mm左右。术后分别检测单纯使用助听器、单纯使用人工耳蜗、人工耳蜗结合助听器三种不同状态下的听力。结果15例患者中,有13例术后残余听力保存良好,仅分别丢失5~20dB听力,但另2例术后残余听力全部丧失。术后在安静、信噪比15dB和10dB三种不同状态下的言语测试结果显示,人工耳蜗结合助听器使用者测试得分始终保持在很高水平;单纯使用人工耳蜗者也有较好的成绩,但在信噪比达10dB的条件下,测试成绩下降;而单纯使用助听器者,不仅在安静状态下听力成绩不甚理想,一旦加入竞争性噪声,听力测试成绩急剧下降。结论保护和利用残余听力的人工耳蜗植入技术,使人工耳蜗植入手术对象从重度或极重度聋扩大到高频为重度或极重度聋,低频(≤500Hz)为中、轻度聋的患者。接受这项技术患者的听力和言语识别能力均明显优于其单纯配戴助听器和单纯使用人工耳蜗时的听力和言语识别能力。  相似文献
