目的 :观察FEP驻极体薄膜是否具有促进烧 (烫 )伤创面愈合作用。方法 :3 8例烧 (烫 )伤患者 ,浅Ⅱ°烧 (烫 )伤创面 2 6处 ,薄层皮片供皮区 12处 ,治疗面积 1%~ 3 %。实验组用 -70 0V驻极体薄膜覆盖创面 ,对照组分别用 0 0 5 %洗必泰液纱布及用凡士林纱布覆盖 ,外用多层纱布敷料包扎。结果 :与对照组相比创面疼痛明显减轻 ,点状出血增多 ,愈合提前 ;渗液印片检查治疗组创面中性粒细胞、巨噬细胞、淋巴细胞均较对照组提早出现且数量多 ;计算机灰度积分法计算结果 ,治疗组创面面积缩小率大于对照组。结论 :驻极体薄膜具有促进创面愈合作用。  相似文献

Background. The purpose of the present study was to compare the performance of the Hydrogel dressing with that of traditional dressings in the treatment of wounds with skin defects.
Material and methods. 32 patients with traumatic skin defects were included in the study. 16 were treated with Hydrogel dressings, and 16 with traditional dressings. We compared the time required for complete epithelialization and the level of pain experienced on removal.
Results. The trial dressing demonstrated faster healing time (ave. 5-6 days faster). The patients from the experimental group experienced less during dressing changes.
Conclusions. We found that dressings demonstrated significantly faster epithelialization time in comparison with traditional wound dressings. The application of Hydrogel dressings enhanced patient comfort.  相似文献

目的探讨激光促进创面早期愈合,减少瘢痕生成作用机制。方法选择 9例中厚皮片移植术患者供皮区,低能量 He-Ne激光局部体外辐射,进行肉眼观察,胶原代谢定量测试, DNA含量测定,同等条件自体创面对照。结果受试区创面愈合时间较对照区提前 3 d,愈合质量佳,随访 3月未见明显瘢痕生成;受试区羟脯氨酸含量高于对照区, P< 0.01;成纤维细胞 DNA含量二者无显著差异, P >0.05。结论低能量 He-Ne激光体外局部辐射能显著地缩短愈合时间,预防供皮区瘢痕生成。  相似文献

目的:探讨促进中厚供皮区愈合,减少供皮区瘢痕的方法。方法:1999—2004年对45例中厚供皮区采用大张或邮票刃厚皮回植。结果:刃厚皮回植中厚供皮区创面愈合时间缩短,且愈后瘢痕增生不明显,色素改变较轻。结论:刃厚皮回植中厚供皮区手术方法,能促进供皮区创面愈合,减少瘢痕增生及色素改变。  相似文献

背景:亲水性纤维含银敷料在其他创面治疗中目前已取得较多临床证据,但其对肛瘘患者术后创面康复的作用研究较少。目的:观察亲水性纤维含银敷料覆盖对肛瘘患者术后创面康复的作用。方法:将 57 例肛瘘术后患者随机分为试验组 29 例和对照组 28 例,试验组在术后给予亲水性纤维含银敷料换药,3 d 1次,对照组术后给予无菌凡士林纱布换药,1 次/d,观察两组患者的首次换药创面疼痛程度、渗液明显减少时间、换药次数、创面康复时间及换药费用等指标。结果与结论:试验组首次换药时创面疼痛程度明显轻于对照组(P 〈 0.05),渗液明显减少时间、换药次数、创面康复时间明显少于对照组(P 〈 0.05);但试验组换药费用明显高于对照组(P 〈 0.05)。表明亲水性纤维含银敷料覆盖于肛瘘患者术后创面可减少刺激,明显减轻疼痛,提供湿性修复环境,加速创面愈合,减少换药次数,提高患者满意度。  相似文献

The wounds mean not only an important bio-psico-social problem for the patient who suffers them, but also an important economic burden for the sanitary system. One of the many wounds classifications allows us to differentiate between acute wounds (slight traumatism, surgical wounds...) and chronic wounds (pressure ulcers, vascular ulcers...), this differentiation allows us to distinguish between wound healing by primary intention (that occurs in acute wounds where filling of cavity is not needed). And wound repair by secondary intention (the way by which chronic wounds are normally solved), a long process in time that needs the filling of the cavity with a neoformation tissue, the granulation tissue. In the present article we described the use of a new dressing of exogenous collagen (Catrix) in the healing of wounds by secondary intention, as well as the actions of this dressing in the different phases of healing. For all it, we supported by a clinical case treated in our unit that corresponds to the healing by second intention of a post-surgical wound secondary to debride by a suppurative hydrosadenitis, that was admitted to our unit waiting for skin graft.  相似文献

Despite continuous exposure to environmental pathogens, injured mucosa within the oral cavity heals faster and almost scar free compared with skin. Saliva is thought to be one of the main contributing factors. Saliva may possibly also stimulate skin wound healing. If so, it would provide a novel therapy for treating skin wounds, for example, burns. This study aims to investigate the therapeutic wound healing potential of human saliva in vitro. Human saliva from healthy volunteers was filter sterilized before use. Two different in vitro wound models were investigated: (a) open wounds represented by 2D skin and gingiva cultures were used to assess fibroblast and keratinocyte migration and proliferation and (b) blister wounds represented by introducing freeze blisters into organotypic reconstructed human skin and gingiva. Re‐epithelialization and differentiation (keratin K10, K13, K17 expression) under the blister and inflammatory wound healing mediator secretion was assessed. Saliva‐stimulated migration of skin and oral mucosa fibroblasts and keratinocytes, but only fibroblast proliferation. Topical saliva application to the blister wound on reconstructed skin did not stimulate re‐epithelization because the blister wound contained a dense impenetrable dead epidermal layer. Saliva did promote an innate inflammatory response (increased CCL20, IL‐6, and CXCL‐8 secretion) when applied topically to the flanking viable areas of both wounded reconstructed human skin and oral mucosa without altering the skin specific keratin differentiation profile. Our results show that human saliva can stimulate oral and skin wound closure and an inflammatory response. Saliva is therefore a potential novel therapeutic for treating open skin wounds.  相似文献

目的比较密闭湿性敷料、凡士林纱布在皮片供皮区的应用效果。方法将手术治疗的60例供皮区创面(大腿外侧)患者随机分为湿性敷料组(30例)和凡士林纱布组(30例),比较2组方法对患者皮片供区的疼痛程度、渗出情况、愈合时间及切口的瘢痕程度的影响。结果湿性敷料组第4、7、10天患者疼痛程度较凡士林纱布组轻,差异有统计学意义(P0.05);但术后15 d后疼痛程度比较差异无统计学意义(P0.05)。湿性敷料组创面敷料渗出明显少于凡士林纱布组,创面愈合时间短于凡士林纱布组,色素沉着及瘢痕的比例低于凡士林纱布组,差异均有统计学意义(P0.05)。结论密闭湿性敷料运用于供皮区能够减轻患者早期术后疼痛,减少创面渗出,缩短愈合时间,减少色素沉着和瘢痕增生。  相似文献

背景:传统的供皮区创面修复以抗感染、支持及保护创面为主,最终为自然愈合,时间较长.研究表明,重组表皮生长因子促进皮肤切割、烧伤等动物模型创面的愈合,加速表皮再生的速度.目的:验证重组表皮生长因子对供皮区创面的修复效果.方法:选择泸州医学院附属医院整形烧伤科需要自体植皮修复创面的患者共32例,男18例,女14例.将32处新鲜供皮区创面随机均分为对照组和治疗组.治疗组将重组表皮生长因子药液(15 mL/支,2 000 IU/mL)直接喷洒于单层内敷纱布上直至浸透,然后覆盖到供皮区,2次/d;对照组将单层生理盐水纱布覆盖供皮区,外加敷料包扎,2次/d.48 h后,采用半暴露疗法.主要观察从治疗开始至供皮区完全愈合所需要的时间,不良事件及副反应,治疗前、后血常规及肝肾功能检测.结果与结论:在重组表皮生长因子喷洒组较生理盐水喷洒组明显缩短创面愈合时间,两组间比较具有显著差异(P<0.01).重组表皮生长因子喷洒组未见明显的不良事件及副反应发生.提示经重组人表皮细胞生长因子处理后,能明显缩短创面的愈合时间,减少了瘢痕的过度增生,在加速创面愈合及再上皮化中具有显著的促进作用.  相似文献

Injury potential may have a regulatory role in the wound healing process, and exogenous electrical stimulation (ES) may mimic natural endogenous bioelectric current that can improve wound healing. Until now, the influence of externally applied ES on injury potential has not been demonstrated during the healing of acute wounds. Thirty-nine male guinea pigs were randomly divided into a control group (sham treatment) and two experimental groups: anodal and cathodal direct current (DC). A 2.5 cm-long full-thickness skin incision was made on each animal's dorsal region. Differential skin surface potential was measured before and immediately after the injury and also through day 21 of the healing period; wound surface area (WSA) was also measured throughout the 21-day healing period. Immediately after injury, wound potential significantly increased in all three groups, reaching a maximum on day 1 for the control and cathodal groups and day 3 for the anodal group (p < 0.05), then decreasing through the healing period. Wound potential returned to preinjury levels by the end of the healing period in the anodal group only. By days 19 and 21, wound potential had decreased more for the anodal group than the control group (p < 0.05). By day 15 for the anodal group and day 17 for the cathodal group, WSA had decreased more compared with the control group (p < 0.05). Anodal microamperage DC ES is appropriate for improving the healing of acute skin wounds because it causes both the wound surface to close and the wound potential to return to preinjury levels faster.  相似文献

目的:评价不同类型创面应用脱细胞生物敷料的效果.方法:将脱细胞生物敷料用于106例烧伤患者150个创面.供皮区创面84个,浅Ⅱ度创面30个,深Ⅱ度创面16个,Ⅲ度创面10个,自体植皮间隙创面5个,后期残余小创面5个.分别在手术取皮后、清创后、植皮间隙后立即用脱细胞生物敷料覆盖干纱布包扎.结果:在150个不同类型创面中,未黏附4个(2.67%)均为Ⅲ度创面,感染2个(1.3%)均为后期残余小创面,渗出物显著积聚而需更换4个(2.67%)均为深Ⅱ度创面.其他刨面黏附好,无感染,渗血少,疼痛明显减轻.结论:脱细胞生物敷料用于供皮区创面、浅Ⅱ度及深Ⅱ度新鲜创面、自体植皮间隙创面效果满意.  相似文献

人工真皮移植修复儿童严重创伤创面   总被引:1,自引:0,他引:1  
背景:近年来组织工程学取得巨大进展,皮肤组织工程尤为引人注目。人工真皮(PELNAC)相对应用较广泛,但报道以成人为主,缺乏修复儿童创面的报道。目的:探讨人工真皮作为儿童严重创伤创面真皮覆盖物的临床效果。方法:回顾性总结分析22例创面负压引流、人工真皮复合自体刃厚皮片或薄中厚皮片移植覆盖儿童严重创伤创面(实验组)的治疗效果,并与19例创面负压引流、创面肉芽培养、自体刃厚皮片或薄中厚皮片移植于同等创面(对照组)的治疗效果进行对比。分析评价指标包括人工真皮的存活率、创面完全覆盖所需的手术次数、植皮后创面完全愈合时间、随访创面移植区域的色泽、质地、皮下丰满度、瘢痕增生情况以及关节功能影响等。结果与结论:人工真皮移植后10-14 d存活率达90%以上。实验组2例第二次刃厚皮片移植,对照组8例第二次刃厚皮片移植,实验组创面植皮存活率优于对照组(P〈0.05)。实验组植皮后创面完全愈合的平均时间为(13.86±3.09) d,对照组为(19.10±4.62) d,实验组平均植皮后创面完全愈合时间短于对照组(P〈0.05)。经超过10个月随访,实验组的植皮区色泽、弹性优于对照组;实验组创面移植区域较对照组真皮部分皮下丰满度良好、瘢痕增生明显较轻;实验组关节功能受到不同程度影响5例,对照组关节功能受到影响10例,两组差异有显著性意义(P 〈0.05)。提示人工真皮抗感染力强、存活率高。将人工真皮应用于儿童严重创伤创面,与创面负压引流、培养肉芽及自体刃厚皮片或薄中厚皮片移植相比,能缩短创面植皮后愈合时间,提高愈合创面的质量,减少对关节功能的影响。  相似文献

背景:Meek植皮法是近5年来引进并陆续在国内多家医院采用的一项创面修复的新技术。目的:观察Meek微型皮片移植修复在大面积深度烧伤患者皮肤缺损的效果。方法:对16例大面积深度烧伤创面患者,采用早期切(削)痂后及晚期肉芽创面Meek植皮法植皮,其中6例选取Ⅲ度烧伤面积30%左右患者同一个体行相同面积Meek微型皮片植皮法,和自体小邮票植皮作为对照。结果与结论:采用Meek微型皮片植皮法皮片成活率65%~95%,创面愈合时间21~65d。Meek微型皮片与自体邮票植皮相比缩短了手术时间,节省了皮源,创面愈合后瘢痕平整,挛缩率低,关节功能恢复良好。  相似文献

目的探讨早期应用辐照氟银猪皮对浅Ⅱ度烧伤创面的愈合作用。方法对20例大面积浅Ⅱ度烧伤患者早期应用辐照氟银猪皮治疗,观察其对创面愈合作用的影响。结果所有病例创面均顺利愈合,创面平均愈合时间为(9.2±1.2)d,且愈合后色素沉着、瘢痕少,无一例发生感染,平稳度过休克期,均无局部刺激症状和过敏反应,肝、肾功能,血常规检查指标均正常。结论早期应用辐照氟银猪皮治疗大面积浅Ⅱ度烧伤创面愈合快,最大限度地保留创面再生能力,给创面愈合提供了良好的内环境。同时患者病情平稳、疼痛轻、瘢痕少,提高了生活质量。  相似文献

[目的]比较生理盐水、4%硼酸溶液、0.5%碘伏+3%过氧化氢3种临床常用护理换药方式对糖尿病小鼠和正常小鼠创面愈合的影响,以期探索最适合糖尿病难愈性创面的护理换药方式。[方法]取雄性8周龄C57BL/6J小鼠30只,设糖尿病组与对照组小鼠各15只,糖尿病小鼠以链脲佐菌素(STZ诱导)在背部制作3个同等大小的全层皮肤创面后,每日1次分别以生理盐水(A处理)、4%硼酸溶液(B处理)和0.5%碘伏+3%过氧化氢溶液(C处理)对创面换药,记录创面结痂及感染情况并计算创面愈合率。[结果]在同一时相点、A处理换药方式下,糖尿病组小鼠的创面愈合率明显低于对照组小鼠;对照组小鼠在3种不同换药处理方式下,其创面愈合率差异无统计学意义(P〉0.05);糖尿病小鼠应用B处理的创面愈合率显著高于应用A和C处理法(P〈0.05)。[结论]正常小鼠创面应用3种换药方式对创面的愈合率无显著影响;糖尿病小鼠创面使用硼酸溶液换药较其他换药方式更有利于伤口愈合。  相似文献

目的 探讨同种异体真皮加自体刃厚皮片移植的临床应用,丰富手部严重烧伤的治疗。方法 2001-2005年采用同种异体真皮加自体刃厚皮片移植修复55个手部严重烧伤创面。结果 55个创面中,52个创面植皮完全成活,无瘢痕增生,没有刺痛,搔痒症状。只有3个创面出现散在点状坏死,经过2~3次换药愈合,有较明显的瘢痕。伤愈6-24个月获随诊11例13个创面:皮肤色素沉着基本如常,创缘无瘢痕增生,质地柔软,弹性良好,无挛缩。与自体全厚植皮无明显差异。结论 采用同种脱细胞真皮+自体刃厚皮片移植修复手部严重烧伤,手术方法简便,疗效满意,没有排斥反应.是手部烧伤治疗的重要补充。  相似文献

OBJECTIVE: To clinically compare the healing of abscess wounds dressed with either crude undiluted honey or Edinburgh University solution of lime (EUSOL). DESIGN: A prospective clinical randomized study. LOCATION: The Isolation Children's Ward of the Wesley Guild Hospital, Ilesa, an affiliate of the Obafemi Awolowo University, Ile-Ife, Nigeria. SUBJECTS: Thirty-two (32) Nigerian children with 43 pyomyositis abcesses. INTERVENTIONS: All subjects had fresh surgical incision and drainage of the abcesses and a 21-day course of ampicillin plus cloxacillin (Ampiclox) and gentamicin; the wounds were left to close spontaneously with twice-daily wound dressing with packing of the abscess cavity with either honey- or EUSOL-soaked gauze in two randomized treatment groups. OUTCOME MEASURES: The clinical conditions of the wound sites were documented on days 1, 3, 7, and 21 as either clean or dirty, dry or wet, granulation tissue present or absent, and epithelialization present or absent; the length of hospital stay was also measured. RESULTS: Honey-treated wounds demonstrated quicker healing and the length of hospital stay was significantly shorter in patients with honey-treated wounds than those treated with EUSOL (t = 2.45, p = 0.019). CONCLUSIONS: Honey is a superior wound dressing agent to EUSOL. Honey is recommended for the dressing of infected wounds, more so in tropical countries, where it is most readily available.  相似文献

[目的]探讨对腹部手术后发生高渗出性伤口的护理。[方法]评估伤口后应用一件式造口袋收集漏出液,或采用持续负压引流,合理换药及保护伤口周围皮肤,以促进伤口愈合。[结果]18例病人伤口周围皮肤完整无损,局部无感染,全部治愈出院。[结论]合理换药,选择正确的引流方式,加强伤口换药,采用皮肤保护膜避免渗出液刺激皮肤等相关护理措施,能有效保护伤口周围皮肤,减轻病人痛苦,减少医护人员工作量,促进伤口愈合。  相似文献

背景:富血小板血浆可用凝血酶凝固成胶冻状,不仅可以黏合组织缺损处,还可以防止血小板流失,使血小板在局部长时间分泌生长因子,避免了目前广泛应用于临床的液态重组生长因子试剂在伤口及移植区易流失易蒸发的缺点。目的:观察外用自体富血小板血浆在儿童面部Ⅱ度烧伤创面修复过程中的治疗作用。方法:选择2008-01/2010-01在广西昭平县人民医院外科收治的14岁以下面部浅Ⅱ度烧伤、深Ⅱ度烧伤儿童各30例。均以左侧面部为治疗组,右侧为对照组。伤后第3天,治疗组将自体富血小板血浆凝胶直接湿敷于创面上半暴露,换药1次/d;对照组创面单用等渗盐水纱布覆盖半暴露,换药1次/d。干预7,14d观察两组患者创面愈合率、愈合时间、疼痛与瘢痕情况及不良反应。采用目测类比评分法评估疼痛情况,改良温哥华瘢痕测量法测定深Ⅱ度创面瘢痕增生情况。结果与结论:在浅Ⅱ度烧伤创面患者中,治疗组7d愈合率明显高于对照组(P〈0.05),愈合时间明显短于对照组(P〈0.05);在深Ⅱ度烧伤创面患者中,治疗组14d愈合率明显高于对照组(P〈0.05),愈合时间显短于对照组(P〈0.05),瘢痕指数明显低于对照组(P〈0.05);两组患者创面疼痛评分无明显差别。说明外用自体富血小板血浆凝胶能显著加快儿童面部Ⅱ度烧伤创面愈合速度,缩短愈合时间,提高愈合质量,减少瘢痕形成,无明显不良反应。  相似文献

目的观察重组人表皮生长因子凝胶在浅Ⅱ度烧伤创面护理中的应用效果。方法选择50例浅Ⅱ度烧伤患者,采用同体对照法,将同一创面不同区域或同体对称部位的创面分为观察组和对照组。观察组采用重组人表皮生长因子凝胶加医用硅酮凝胶膏混合后湿敷创面;对照组采用医用硅酮凝胶膏湿敷创面。观察两组创面愈合时间、换药次数、疼痛状况及渗液情况的差异。结果观察组的创面平均愈合时间(8.6±1.6)d较对照组(11.8±1.7)d明显缩短,差异有统计学意义(t=-9.834,P〈0.05)。观察组换药次数(4.5±0.9)次较对照组(6.1-I-0.9)次明显减少,差异有统计学意义(t=-9.336,P〈0.05)。两组患者的换药疼痛程度相似,差异无统计学意义(x^2=0.272,P〉0.05)。两组在第2次换药时的渗液程度相似,差异无统计学意义(x^2=0.205,P〉0.05);而在第3和4次换药时,观察组渗液程度较对照组明显改善,差异有统计学意义(x2分别为8.988,5.316;P〈0.05)。结论重组人表皮生长因子凝胶能减少浅Ⅱ度烧伤创面的渗液量,减少换药次数,可明显促进表皮生长,加速创面愈合。  相似文献
