3D TRICKS MRA对颈动脉粥样硬化狭窄的诊断价值   总被引:1,自引:0,他引:1       下载免费PDF全文
目的:探讨3D TRICKS MRA对颈动脉粥样硬化狭窄的诊断价值。方法:对43例临床疑诊颈动脉粥样硬化狭窄的患者行2D TOF MRA和3D TRICKS MRA检查,其中31例同期行DSA检查,以颈总动脉法(CC法)计算狭窄率。结果:颈动脉闭塞2支,重度狭窄10支,中度狭窄31支,轻度狭窄25支,正常18支。2D TOF MRA、3D TRICKS MRA与DSA对照,显示狭窄部位均与DSA所示相符。2D TOF MRA高估狭窄程度6支,3D TRICKS MRA高估狭窄程度1支。以DSA为标准,2D TOF MRA诊断颈动脉狭窄的敏感度、特异度、诊断符合率分别为91.6%,85.7%,90.3%(K=0.737);3D TRICKS MRA诊断颈动脉狭窄的敏感度、特异度、诊断符合率分别为97.9%,100%,98.4%(K=0.955)。两种方法对诊断颈动脉狭窄的敏感度、特异度和诊断符合率差异均具有统计学意义(P〈0.05)。结论:3D TRICKS MRA与DSA对评估颈动脉粥样硬化狭窄具有极好的一致性(K=0.955),明显优于2D TOF MRA(K=0.737,P〈0.05),是对颈动脉粥样硬化狭窄筛查、术前评估、术后随访的最佳检查方法,能基本替代DSA检查。  相似文献

目的 探讨磁共振血管造影(MRA)、CT血管造影(CTA)在诊断颈动脉狭窄及内膜切除术后随访中的价值。方法 11例22支颈动脉行MRA、CTA、CT仿真内窥镜(CTVE)、多普勒超声(DUS)检查。12支结果与DSA对照。3支颈动脉重度狭窄者行内膜切除术,MRA采用二维、三维时间飞越法。CTA经最大强度投影(MIP)及遮盖表现显示法(SSD)三维重建。应用导航功能行CTVE成像。结果 颈动脉轻度狭窄8支,中度狭窄3支,重度狭窄5支,闭塞1支,5支正常。12支颈动脉与DSA对照:CTA评估血管狭窄与DSA相符者11支,MRA与DSA相符者9支,DUS与DSA相符者7支。CTA、CTVE显示斑块、壁血栓6支,3支手术证实。术后MRA示狭窄解除,CTA示斑块消失。结论 MRA、DUS可用于颈动脉狭窄的筛选及术后随访。CTA评估血管狭窄程度与DSA、手术比较有较好的一致性,并能直接显示钙化斑块。  相似文献

PURPOSE: To compare helical CT-angiography (CTA) and two techniques of MR angiography (MRA) to conventional angiography in aorto-iliac occlusive disease. MATERIALS AND METHODS: The abdominal aorta and iliac arteries in 22 patients (4 for preoperative assessment of abdominal aortic aneurysm and 18 for peripheral vascular disease) were imaged using four techniques: digital subtraction angiography ("gold standard"), 2D TOF MR angiography, 3D Gd-enhanced MR angiography and helical CT angiography. Source (CTA and 2D TOF MRA) and MIPed images (after subtraction measures before and after gadolinium injection for 3D Gd-MRA) were reviewed. RESULTS: Sensitivity, specificity and accuracy for the detection of significant (>50%) stenosis and occlusion of aorto-iliac arteries were respectively: 38%, 89%, 77% for 2D TOF MRA; 75%, 71%, 72% for 3D Gd-MRA and 95%, 90%, 92% for CTA. Excluding the internal iliac arteries, results were 54%, 96%, 88% for 2D TOF MRA; 96%, 80%, 83% for 3D Gd-MRA and 92%, 93%, 95% for CTA. CONCLUSION: 3D Gd-MRA, a technique with potential for further improvement, is superior to 2D TOF MRA for detecting significant stenosis and occlusion of aorto-iliac arteries. Results at Gd-MRA are nearly similar to those at CTA (after excluding internal iliac arteries). Results at Gd-MRA are not affected by calcified plaque.  相似文献

BACKGROUND AND PURPOSE: Time-of-flight (TOF) magnetic resonance angiography (MRA) is commonly used to visualize the carotid arteries; however, flow void artifacts can appear. Our purpose was to determine the frequency and diagnostic meaning of flow voids by using real patient data, as part of a larger study of MRA compared with the criterion standard, digital subtraction angiography (DSA). METHODS: In 1997-2000, 390 consecutive patients with sonographic findings suggestive of carotid artery stenosis were included in this study. All patients subsequently underwent three-dimensional (3D) TOF MRA and conventional DSA. The frequency of flow void artifacts on 3D TOF MRA images were compared with stenosis measurements on DSA images. RESULTS: We recorded 107 flow voids (16%) during 3D TOF MRA of 662 carotid arteries. DSA images were available for comparison in 102 cases. The median percentage of stenosis in this subgroup of flow voids on MRA images was 80%, compared with measurements on DSA images according to the North American Symptomatic Carotid Endarterectomy Trial (NASCET) criteria. Stenoses ranged from 36% to 100% (occlusion). Three flow voids (2.9%) were in the 0-49% range; 11 (10.8%), in the 50-69% range; and 86 (84.3%), in the 70-99% range. Two flow voids (2.0%) represented occlusions. The positive predictive value of a flow void artifact for the presence of severe (70-99%) stenosis was 84.3% (95% CI: 77.3%, 91.4%). CONCLUSION: Flow void artifacts represented severe stenosis in most of the arteries. According to our data, the assumption that flow voids on 3D TOF MRA images represent severe stenosis is justified.  相似文献

The aim of this study was to compare image quality, level of diagnostic confidence and interobserver agreement in assessment of carotid stenosis with contrast enhanced MR angiography (CE MRA) in comparison with 2D time of flight MR angiography (2D TOF MRA). 60 carotid arteries in 30 patients were examined by three observers. Image quality and diagnostic confidence were assessed on the basis of a visual analogue scale. Interobserver variability was assessed with the help of intraclass correlation coefficient. Median values on the visual analogue scale for image quality and diagnostic confidence were higher for CE MRA compared with 2D TOF MRA for all three observers. Higher intraclass correlation values were recorded for interobserver variability for CE MRA compared with 2D TOF MRA both for visual estimation of carotid stenosis as well as for measurement of carotid stenosis on the basis of North American Symptomatic Carotid Endarterectomy Trial (NASCET) and European Carotid Surgery Trial (ECST) criteria. CE MRA provides better image quality, higher level of diagnostic confidence and more interobserver agreement compared with 2D TOF MRA.  相似文献

目的 评价颈动脉狭窄无创性影像学检查方法的临床应用价值,探讨颈动脉狭窄与缺血性脑血管病之间的联系。 材料与方法 对15例30支颈动脉行多普勒超声(DUS)、磁共振血管造影(MRA)及头部CT&MRI检查。其中5例同时行颈动脉CT血管造影(CTA),4例与DSA对照,6例颈动脉重度狭窄者行颈动脉内膜切除术。 结果 15例30支颈动脉轻度狭窄(<30%)8支,中度狭窄(30%~69%)6支,重度狭窄(70%~99%)8支(均为一侧),闭塞2支,未见狭窄6支。8支颈动脉重度狭窄者狭窄侧腔隙性脑梗死5例,狭窄对侧皮层梗死1例,双侧脑梗死1例,未见异常1例。颈动脉闭塞侧大脑中动脉分布区脑梗死2例。CTA显示硬化斑块3例。 结论 颈动脉狭窄与脑梗死的发生、发展密切相关。DUS、MRA、CTA结合使用能够在颈动脉狭窄的筛选、诊断、监测中发挥重要作用。  相似文献

BACKGROUND AND PURPOSE:Dual-source CTA and black-blood MRA are recently developed techniques for evaluating carotid stenosis. The purpose of this study was to compare dual-source CTA with black-blood MRA and conventional TOF MRA in both detecting carotid stenosis by using DSA as a reference standard and demonstrating plaque morphology.MATERIALS AND METHODS:Thirty patients with suspected carotid artery stenosis underwent unenhanced MRA by using black-blood and TOF MRA and dual-source CTA. Source images from unenhanced MRAs and dual-source CTA were reconstructed with MIP or curved planar reconstruction. The degree of carotid artery stenosis was measured, and plaque surface morphology at the stenosis was analyzed and compared among different techniques.RESULTS:Good correlation was observed for measuring the degree of carotid stenosis among dual-source CTA, black-blood MRA, TOF MRA, and DSA. Sensitivity and specificity for detecting severe stenosis were 100% and 97% with dual-source CTA, 100% and 95% with black-blood MRA, and 79% and 95% with TOF MRA. None of the 3 technologies resulted in stenosis of <50% being overestimated. Plaque surface irregularity or ulceration was more frequently detected with dual-source CTA and black-blood MRA than with TOF MRA and DSA.CONCLUSIONS:This preliminary study shows that black-blood MRA is a promising technique, comparable with dual-source CTA and DSA, but better than TOF MRA, in the evaluation of carotid stenosis. Unlike dual-source CTA, black-blood MRA requires no intravenous contrast or radiation.

Carotid artery atherosclerosis is a major cause of ischemic cerebrovascular disease.1,2 Measurement of carotid stenosis and demonstration of plaque morphology are critical for the management of patients with carotid atherosclerosis. DSA is the current reference standard for evaluating carotid artery stenosis. The diagnostic role of DSA has largely been replaced, however, by noninvasive techniques such as sonography, CTA, and MRA.Sonography has been the most commonly performed technique but may be restricted by its operator dependence and limited coverage. CTA is another widely used technique for the evaluation of carotid artery stenosis with high accuracy.3 Dual-source CTA (DSCTA) uses 2 x-ray sources and 2 detectors at the same time. With this technique, 2 images can be simultaneously acquired with different tube voltages; this feature has been shown to be an advantage for the evaluation of densely calcified carotid stenosis.4,5 Contrast-enhanced MRA has been established as an alternative for carotid imaging with a diagnostic accuracy similar to that of CTA.6,7 Both CTA and contrast-enhanced MRA use contrast media and are restricted in patients with impaired renal function, and CTA also requires ionizing radiation. As a result, unenhanced MRA without gadolinium is a desirable alternative, especially in patients with renal failure. Conventional TOF MRA has been widely used in clinical practice for carotid visualization, but it is limited by local reduction of signal intensity related to slow and turbulent flow and also prolonged imaging time.8 T2-weighted black-blood MRA (BB MRA) is a newly developed technique showing potential in the evaluation of both the lumen and the wall of the carotid artery after optimal suppression of the signal from flowing blood.9,10 Few studies, to our knowledge however, have compared DSCTA with BB MRA and conventional TOF MRA in evaluating carotid stenosis.The aim of this study was to prospectively and intraindividually compare these 2 unenhanced MRA methods with DSCTA in detecting carotid artery stenosis by using DSA as the standard of reference and in demonstrating plaque morphology.  相似文献

目的探讨64层螺旋CT血管成像(CTA)在诊断颈内动脉狭窄和粥样硬化斑块中的价值,并与当前血管成像的金标准DSA进行比较。方法对40例病人(80个颈动脉)进行了CTA和DSA检查,两项检查时间间隔不超过1周,并将CTA与DSA结果进行对照,计算出两者之间的敏感度和特异度。结果CTA对轻度(狭窄程度:0~29%)颈动脉狭窄和颈动脉闭塞显示得非常好,其敏感度和特异度均接近100%。在明确经DSA测量狭窄程度>50%的狭窄时,CTA敏感度、特异度分别为89%、91%。CTA在鉴别狭窄程度为50%~69%或70%~99%时的特异度高,而敏感度较低,分别为65%和73%。CTA同时可以检出DSA不能发现的所有类型的斑块。结论64层CTA在显示颈内动脉狭窄方面与DSA有很好的一致性,同时可发现与颈动脉狭窄相关的溃疡,而DSA只能显示狭窄。  相似文献

BACKGROUND AND PURPOSE: Although digital subtraction angiography (DSA) provides excellent visualization of the intracranial vasculature, it has several limitations. Our purpose was to evaluate the ability of helical CT angiography (CTA) to help detect and quantify intracranial stenosis and occlusion compared with DSA and MR angiography (MRA). METHODS: Twenty-eight patients underwent CTA, DSA, and 3D time-of-flight (TOF) MRA for suspected cerebrovascular lesions. All three studies were performed within a 30-day period. Two readers blinded to prior estimated or calculated stenoses, patient history and clinical information examined 672 vessel segments. Lesions were categorized as normal (0-9%), mild (10-29%), moderate (30-69%), severe (70-99%), or occluded (no flow detected). DSA was the reference standard. Unblinded consensus readings were obtained for all discrepancies. RESULTS: A total of 115 diseased vessel segments were identified. After consensus interpretation, CTA revealed higher sensitivity than that of MRA for intracranial stenosis (98% versus 70%, P < .001) and occlusion (100% versus 87%, P = .02). CTA had a higher positive predictive value than that of MRA for both stenosis (93% versus 65%, P < .001) and occlusion (100% versus 59%, P < .001). CTA had a high interoperator reliability. In 6 of 28 patients (21%), all 6 with low-flow states in the posterior circulation, CTA was superior to DSA in detection of vessel patency. CONCLUSION: CTA has a higher sensitivity and positive predictive value than MRA and is recommended over TOF MRA for detection of intracranial stenosis and occlusion. CTA has a high interoperator reliability. CTA is superior to DSA in the evaluation of posterior circulation steno-occlusive disease when slow flow is present. CTA results had a significant effect on patient clinical management.  相似文献

PURPOSE: The purpose of this work was to compare Gd-enhanced breath-hold fast imaging with steady-state precession (Gd-FISP) with unenhanced time-of-flight (TOF) sequences in evaluating internal carotid arteries (ICAs). METHOD: Thirty patients underwent three unenhanced TOF sequences [2D traveling saturation (Travelsat); 3D tilted optimized nonsaturated excitation (TONE); TOF 3D Multislab] and two breath-hold 3D Gd-FISP sequences with automated intravenous contrast agent injection (axial and coronal). ICAs were classified as normal (no stenosis); with mild (<30%), moderate (30-70%), or severe stenosis; or occluded (100%). Digital subtraction angiography (DSA) with aortic arch injection was used as a reference technique. RESULTS: DSA revealed 20 normal ICAs; 11 mild, 9 moderate, and 14 severe stenoses; and 2 occlusions. DSA and all MR angiography (MRA) sequences diagnosed the occlusion of four common carotid arteries. The TOF 2D overestimated 10 stenoses, TOF 3D TONE 9, and TOF 3D Multislab 5; Gd-FISP 3D overestimated only 2 of them, reaching the highest sensitivity and specificity for severe stenoses. Significant differences were found between the overestimation of Gd-FISP and each of the three unenhanced sequences (0.0020 < p < 0.0313, Wilcoxon and McNemar tests). Severe artifacts were observed with TOF techniques only. CONCLUSION: Gd-FISP is an interesting, largely artifact-free improvement for MRA of ICAs.  相似文献

Conventional time-of-flight (TOF) MR angiography (MRA) in carotid artery stenosis relies on flow-related enhancement to produce signal from vascular structures. Intravoxel phase dispersion, due to vortices, causes loss of signal and is the reason for the tendency to overestimate the degree of stenosis. In contrast-enhanced MRA, intravascular signal is mainly dependent on T1 shortening of the blood. We compared first-pass contrast-enhanced MRA (contrast-enhanced 3D gradient echo, ce3D GRE) and contrast-enhanced 2D TOF (ce2D TOF) sequences with an unenhanced 2D TOF in 13 patients with carotid artery stenosis, assessing delineation of the carotid bifurcation, enhancement of veins and grade of stenosis. The contrast-enhanced techniques produced more morphological detail, the ce3D GRE being superior to the ce2D TOF. Four carotid arteries were reclassified into lesser stenosis categories using the ce3D GRE technique. However, seven carotid arteries (27 %) were rated as nondiagnostic on the ce3D GRE, mainly due to masking of the carotid bifurcation by veins. The latter can be avoided by decreasing the acquisition time; on our 1.5-T system we could achieve a minimum time of 23 s per 3D GRE. Further reduction of acquisition time would be necessary to incorporate this method into clinical routine, requiring higher-performance gradients, which are not available in many UK hospitals. PURPOSE: We sought to prospectively evaluate the image quality and visualization of the intracranial arteries using high spatial resolution contrast-enhanced magnetic resonance angiography (CE-MRA) at 3 T and to perform intraindividual comparison with time-of-flight (TOF) MRA and multislice CT angiography (CTA). MATERIALS AND METHODS: Twelve patients (5 men, 7 women, 37-71 years of age) with suspected cerebrovascular disease prospectively underwent MRA and CTA. MRA was performed on a 3 T MR system, including both 3-dimensional (3D) TOF (Voxel dimension: 0.6 x 0.5 x 0.9 mm in 5 minutes and 40 seconds) and 3D CE-MRA (voxel dimension: 0.7 x 0.7 x 0.8 mm in 20 seconds, using parallel acquisition with an acceleration factor of 4). CTA images were acquired on a 16-slice CT scanner (voxel dimension: 0.35 x 0.35 x 0.8 mm in 17 seconds). The image quality and visualization of up to 26 intracranial arterial segments in each study was evaluated by 2 experienced radiologists. The arterial diameter for selective intracranial arteries was measured independently on each of the 3 studies, and statistical analysis and comparative correlation was performed. RESULTS: A total of 312 arterial segments were examined by CE-MRA, TOF-MRA, and CTA. The majority of intracranial arteries (87%) were visualized with diagnostic image quality on CE-MRA with a significant correlation to TOF (R values = 0.84; 95% confidence interval 0.79-0.86, P < 0.0001), and to CTA (R values = 0.74; 95% confidence interavl 0.68-0.78, P < 0.001). The image quality for small intracranial arteries, including the anterior-inferior cerebellar artery, the posterior communicating artery, and the M3 branch of the middle cerebral artery, was significantly lower on CE-MRA compared with TOF and CTA (P < 0.03). There was a significant correlation for the dimensional measurements of arterial diameters at CE-MRA with TOF (r = 0.88, 95% confidence interval 0.81-0.93), and CTA (r = 0.83, 95% confidence interval 0.73-0.90). CONCLUSION: The described 3 T CE-MRA protocol, spanning from the cervical to the intracranial vessels, visualized and characterized the majority of intracranial arteries with image quality comparable with that obtained using TOF-MRA and CTA. Further clinical studies are required to establish the accuracy of the technique in a broader clinical setting.  相似文献

Carotid artery atherosclerotic plaques (APs) can lead to brain ischemia, an event shown to correlate with both the degree of stenosis and the composition of the AP. Currently, accurate estimates of stenosis can be obtained by either x-ray angiography or three-dimensional time of-flight (TOF) magnetic resonance angiography (MRA). Our purpose was to determine whether three-dimensional TOF MRA images could also provide information on plaque location, morphology, and composition. Seven pre-endarterectomy patients underwent three-dimensional TOF MRA. After endarterectomy, plaque histology was evaluated. Three-dimensional TOF MRA images contained sufficient soft tissue contrast to differentiate the plaques from the surrounding tissues in all cases. Estimation of plaque morphology had 80% correlation with histology. Finally, intraplaque hemorrhage and calcification were depicted as regions of moderately high and very low intensity, respectively. These preliminary results suggest that three-dimensional TOF MRA may be useful in studying the development and progression of carotid atherosclerosis.  相似文献

BACKGROUND AND PURPOSE: We prospectively evaluated the influence of different imaging techniques (time-of-flight MR angiography [TOF-MRA], contrast-enhanced MR angiography [CE-MRA], multisection CT angiography [CTA]) and postprocessing methods (maximum intensity projection [MIP], multiplanar reformation [MPR]) on carotid artery stenosis grading. MATERIALS AND METHODS: Fifty patients (34 men, 16 women) with symptomatic stenosis of the internal carotid artery were examined with a 16-section spiral CT and a 1.5T MR unit. Two MRA techniques were applied: 3D-TOF and CE-MRA. MPR was used for postprocessing with all modalities; MIP was used only with MRA. Four readers measured and calculated the percentage diameter stenosis independently according to NASCET criteria. The Wilcoxon test was used to measure interobserver variability, and the Friedman test was used to test the null-hypothesis of equality of the modalities. RESULTS: The hypothesis for global equality was rejected (P < .001). TOF-MRA and CTA assessed with MPR showed the highest concordance (difference, 0.6%; confidence interval [CI], -3.0, 4.3%), and CE-MRA with MIP and CTA showed the lowest concordance in stenosis grading (difference, 7.0%; CI, 3.4, 10.6%). MPR resulted in lower degrees of stenosis than MIP for both MRA sequences, although not statistically significant (CE, -3.0%; CI, -6.6, 0.6%; TOF, -2.2%; CI, -5.8, 1.4%). When only studies with good or excellent image quality were considered, the differences decreased, but the trends remained. CONCLUSION: Stenosis grading is dependent on the examination method and postprocessing technique. CTA and TOF-MRA evaluated with MPR revealed highest concordance.  相似文献

目的:评价高分辨力三维增强磁共振血管成像术(3DCEMRA)对肾动脉狭窄的显示准确性。方法:对30例临床诊断或怀疑肾动脉狭窄患者进行高分辨力肾动脉3DCEMRA检查,分析所有患者的肾动脉和副肾动脉显示质量和病变显示情况,并与DSA做对照。结果:3DCEMRA显示了30例患者总共59支肾动脉主干(1例为单支移植肾动脉),显示率为100%。3DCEMRA显示4支副肾动脉,显示率100%。肾动脉段级分支的显示率为49%。所有病例在动脉显示区域内静脉均未显影或显影淡,对诊断不构成影响。DSA共显示59支肾动脉主干和4支副肾动脉(以63支计算),59支主干中2支闭塞,8支重度狭窄,11支中度狭窄,10支轻度狭窄,28支肾动脉主干和4支副肾动脉都正常。3DCEMRA除把2支轻度狭窄估为中度狭窄,1支中度狭窄估为重度狭窄外,其它结果都同DSA一致,故3DCEMRA显示肾动脉主干和副肾动脉狭窄程度超过50%的有血液动力学意义的明显狭窄的敏感性和特异性分别为100%和95%,阳性预测值为91%,阴性预测值为100%。结论:高分辨力肾动脉3DCEMRA能准确检出肾动脉主干和副肾动脉狭窄,并能较好判断其狭窄程度。  相似文献

We performed a preliminary study comparing three-dimensional time-of-flight (3 D TOF) magnetic resonance angiography (MRA) and spiral CT angiography (SCTA) in the detection and assessment of internal carotid artery stenosis. Digital subtraction angiography (DSA) was the reference examination. We examined 20 patients with signs of cerebrovascular insufficiency, who underwent MRA, SCTA and DSA within a 3 day period. Both internal carotid arteries were assessed by three blinded readers for degree of stenosis at two different levels (bulb and remaining section) giving a total of 80 assessments. Interobserver variability, sensitivity, specificity, diagnostic accuracy, concordance, overestimation and underestimation were assessed. Interobserver variability was not statistically significant. MRA showed higher sensitivity, specificity, diagnostic accuracy and concordance than SCTA (92.0 % vs 80.8 %, 98.2 % vs 96.4 %, 96.3 % vs 91.3 % and 96.0 % vs 88.0 %, respectively). MRA gave rise to a 5.0 % overestimation rate, whereas SCTA occasioned a 7.5 % underestimation rate. These differences are not statistically significant. These results suggest that MRA is a more useful, noninvasive modality for assessment of the internal carotid artery with a more than 70 % stenosis. 目的比较16层螺旋CT(16 multi-detector CT,16-MDCT)冠状动脉血管造影(CTA)和三维屏气冠状动脉MR血管造影(MRA)的图像质量以及诊断冠状动脉显著性狭窄(>50%)的准确性.方法40例疑有冠心病患者在3 d内均行冠状动脉CTA和MRA检查,其中31例患者在2周内行冠状动脉造影检查.将冠状动脉分成9个节段(右冠状动脉近、中、远段,左冠状动脉主干,前降支近、中、远段和旋支近、远段),由2名影像科医生共同对各个节段的图像质量按0~4级评分,比较CTA和MRA上各个节段的图像质量.以冠状动脉造影为标准,计算并比较CTA和MRA诊断31例冠状动脉显著性狭窄(>50%)各项准确性指标.结果CTA在右冠状动脉中段的图像质量低于MRA,右冠状动脉近段二者无区别,其他节段均优于MRA.冠状动脉造影显示31例患者共有43个节段狭窄>50%,CTA和MRA分别正确诊断出36和27个,其敏感性、特异性、阳性预测值和阴性预测值分别为83%、84%、49%、97%和63%、90%、55%、93%.结论除右冠状动脉中段,CTA大部分节段的图像质量优于MRA.CTA诊断冠状动脉显著性狭窄的敏感性高于MRA,但特异性低于MRA.冠状动脉CTA和MRA均表现了较高的阴性预测值,对排除冠状动脉狭窄具有临床价值.  相似文献

目的 评价颈动脉狭窄无创性影像学检查方法的临床应用价值 ,探讨颈动脉狭窄与缺血性脑血管病之间的联系。材料与方法 对 15例 3 0支颈动脉行多普勒超声 (DUS)、磁共振血管造影 (MRA)及头部CT、MRI检查。其中 5例同时行颈动脉CT血管造影 (CTA) ,4例与DSA对照 ,6例颈动脉重度狭窄者行颈动脉内膜切除术。结果  15例 3 0支颈动脉 ,轻度狭窄 ( <3 0 % ) 8支 ,中度狭窄 ( 3 0 %~ 69% ) 6支 ,重度狭窄 ( 70 %~ 99% ) 8支 (均为一侧 ) ,闭塞 2支 ,未见狭窄 6支。 8支颈动脉重度狭窄者狭窄侧腔隙性脑梗死 5例 ,狭窄对侧皮层梗死 1例 ,双侧脑梗死 1例 ,未见异常 1例。颈动脉闭塞侧大脑中动脉分布区脑梗死 2例。CTA显示硬化斑块 3例。结论 颈动脉狭窄与脑梗死的发生、发展密切相关。DUS、MRA、CTA结合使用能够在颈动脉狭窄的筛选、诊断、监测中发挥重要作用。  相似文献
