PURPOSE: Real-time 3-dimensional transthoracic echocardiography (RT-3D-TE) with real-time volume rendering (RTVR) offers multiple simultaneous views and spatial definition of intracardiac structures superior to that attainable by 2-dimensional transthoracic echocardiography (2D-TE). We hypothesized that RT-3D-TE would therefore improve identification of left ventricular apical thrombi (LVT). METHODS: Patients were referred to our echocardiography laboratory over an 8-month period. Those diagnosed with a "suspicious" or "definite" LVT on the basis of 2D-TE images underwent RT-3D-TE on the same day. All 2D-TE, RT-3D-TE, and RTVR images were reviewed by 2 independent observers. RT-3D-TE findings were considered positive for LVT if LVT was visualized in both B-scan (apical orthogonal) and C-scan (short axis, with and without tilting angle) planes and on RTVR images, nondiagnostic (or suspicious) for LVT if it was not visualized in all planes, and negative for LVT if it was not visualized in any plane. RESULTS: Thirty patients (19 men and 11 women) with a mean age (+/- standard deviation) of 52 +/- 13 years were enrolled. The interobserver agreement coefficient was 63% for 2D-TE interpretations of LVT and 93% for RT-3D-TE interpretations of LVT (p<0.05). The final interpretations by RT-3D-TE with RTVR were positive for LVT in 16 patients (53%), suspicious for LVT in 4 patients (13%), and negative for LVT in 10 patients (30%). CONCLUSIONS: RT-3D-TE with RTVR offers dramatically clearer echocardiographic findings than does 2D-TE. In particular, RT-3D-TE is able to provide a clear diagnosis of LVT/non-LVT when 2D-TE images are merely suggestive of the disorder. Therefore, RT-3D-TE with RTVR, which is a clinically feasible alternative to 2-dimensional echocardiography, has great potential to positively affect the diagnosis, follow-up, and care of patients with suspected LVT.  相似文献

Schizencephaly is a rare anomaly of neuronal migration characterized by the presence of brain clefts that communicate with the lateral ventricles. Type I is characterized by clefts with fused lips or margins, not communicating with the subarachnoid space. Type II is characterized by longer clefts that communicate with the subarachnoid space. Neonatal diagnosis of schizencephaly on transfontanellar two-dimensional (2D) sonography is rare, with only 1 report in the medical literature. The major limitation of 2D sonography is its inability to assess neonatal prognosis. There are no reports on MEDLINE about the use of transfontanellar three-dimensional (3D) sonography in the assessment of schizencephaly. We present a case of type II schizencephaly diagnosed on the 29th week of gestation with 2D sonography and describe the main findings with 3D sonography in surface and transparency modes performed in the neonatal period via the fontanel.  相似文献

In this report, we present the antenatal two‐ and three‐dimensional sonographic findings from a fetus with choledochal cyst as well as confirmatory postnatal MRI. A delayed diagnosis of choledochal cyst is common, leading to significant morbidity and mortality. Visualizing bile ducts entering a right upper quadrant cyst is pathognomonic, and early diagnosis can facilitate definitive treatment with Roux‐en‐Y hepaticojejunostomy. © 2014 Wiley Periodicals, Inc. PURPOSE: To compare three-dimensional sonography (3D US) with quantitative cholescintigraphy for assessing gallbladder contractility. METHODS: Gallbladder radioactivity was assessed in 35 patients with suspected gallbladder disease using a gamma camera 5, 30, 60, and 90 minutes after technetium 99m (Tc-99m) DISIDA injection and 30 and 60 minutes after ingestion of a high-fat meal. Immediate gallbladder images were obtained via 3D US. Gallbladder radioactivity at 120 minutes after injection of Tc-99m DISIDA was defined as 100%, and gallbladder contractility was calculated. Gallbladder volume on 3D US was calculated using a dedicated software. Pearson correlation analysis and simple linear regression analysis were used. RESULTS: The mean gallbladder volume on 3D US was 25.3 ml after fasting and 6.6 ml after a high-fat meal. The mean gallbladder contractility index was 77.7% on cholescintigraphy (range, 18-99) and 73.4 on 3D US (range, 16.7-97.3). A linear correlation between cholescintigraphy and 3D US contractility indices was observed. The r value on Pearson analysis was 0.92 and R(2) of the coefficient of determination was 0.85. The difference in measured contractility between the 2 methods ranged from +21.5% to -15.0% (mean +/- SD, 4.4 +/- 8.7%). CONCLUSIONS: 3D US is a reliable and easy method for clinical measurement of the volume of the gallbladder and its contractility.  相似文献

We describe the case of a 33-year-old pregnant woman, gravida 1 para 0, who was referred to us with a presumptive diagnosis of an interstitial pregnancy. The patient had had amenorrhea for 7 weeks and had previously undergone myomectomy. Two-dimensional sonographic examination revealed a gestational sac located outside the uterine cavity but could not define its exact anatomic position. Using transvaginal 3-dimensional sonography, we were able to correctly depict the sac, which was located where the fallopian tube crossed the uterine horn. We performed a cornual resection, during which the diagnosis of interstitial pregnancy was confirmed. The use of 3-dimensional sonography in this patient's case thus led to appropriate early treatment and avoidance of possible subsequent morbidity.  相似文献

In this report, we describe the application of 3-dimensional (3D) sonography for diagnosing choledochal cysts in 3 girls who were 18 months, 2 years, and 11 years old. The 3D images that were obtained during the sonographic examination revealed that all 3 patients had a type I choledochal cyst and also demonstrated dilatation of the biliary tracts. The 3 patients then underwent surgical excision of their cysts, cholecystectomy, and hepaticojejunostomy. Besides its other clinical applications, 3D sonography promises to be a valuable adjunct to conventional 2-dimensional imaging for the evaluation of choledochal cysts in pediatric patients.  相似文献

In this report, we describe the prenatal use of 3-dimensional 4-dimensional (3D/4D) sonography to visualize the facial dysmorphisms associated with holoprosencephaly in 6 fetuses (2 with cyclopia, 1 with cebocephaly, and 3 with holoprosencephaly-premaxillary agenesis). Ultrafast MRI was also used. The 3D/4D sonographic images allowed easy, rapid, and precise prenatal evaluation of the facial dysmorphisms associated with holoprosencephaly; the MRI scans provided little additional information. The 3D/4D sonograms helped the parents visualize the abnormalities and facilitated their acceptance of perinatal genetic counseling.  相似文献

PURPOSE: To use 3-dimensional sonography (3DUS) to measure contralateral lung volume and evaluate the potential of this measurement to predict neonatal outcome in isolated congenital diaphragmatic hernia (CDH). METHODS: Between January 2002 and December 2004, the contralateral lung volumes of 39 fetuses with isolated CDH were measured via 3DUS using rotational multiplanar imaging. The observed/expected contralateral fetal lung volume ratios (o/e-ContFLVR) were compared with the lung/head ratio (LHR), observed/expected total fetal lung volume ratio (o/e-TotFLVR), and postnatal outcome. RESULTS: Contralateral lung volumes are less reduced than total lung volumes in CDH. The bias and precision of 3DUS in estimating contralateral lung volumes were 0.99 cm(3) and 1.11 cm(3), respectively, with absolute limits of agreement ranging from -1.19 cm(3) to +3.17 cm(3). The o/e-ContFLVR was significantly lower in neonatal death cases (median, 0.49 cm(3); range, 0.22-0.99 cm(3)) than in survival cases (median, 0.58 cm(3); range, 0.42-0.92 cm(3) [p < 0.01]). Overall accuracy of the o/e-ContFLVR, o/e-TotFLVR, and LHR in predicting neonatal death were 67.7% (21/31), 80.7% (25/31), and 77.4% (24/31), respectively. CONCLUSION: Although o/e-ContFLVR can be precisely measured with 3DUS and can be used to predict neonatal death in CDH, it is less accurate than LHR and o/e-TotFLVR for that purpose.  相似文献

We report a case of a migrated Essure permanent birth control device. The correct diagnosis was made on conventional two‐dimensional and three‐dimensional pelvic sonography 7 years after placement of the device when the patient presented with persistent right‐sided pain. The 3‐month post placement hysterosalpingogram had shown an appropriately occluded right fallopian tube but had overlooked the abnormal position of the right Essure device, which was too proximal and extending slightly in the uterine cavity. © 2013 Wiley Periodicals, Inc. We report a case of a large fetal oropharyngeal teratoma in a 19-week fetus evaluated with 3-dimensional (3D) sonography. Ultrasound has been recognized as an important tool for pelvic floor evaluation. A main limitation of the two‐dimensional transvaginal examination is in delineation of the posterior vaginal compartment and its relation to the cervix. We describe the use of three‐dimensional saline infusion vaginography as a complementary technique for the assessment of the vaginal wall and pelvic floor descent. We present several cases that demonstrate the advantages of this technique in overcoming the limitations inherent in current approaches. The improved imaging obtained by this technique enabled us to measure pelvic floor parameters and assist in evaluating pelvic floor dysfunction. © 2013 Wiley Periodicals, Inc. OBJECTIVE: The purpose of this study was to evaluate the accuracy of 3-dimensional (3D) sonography in assessing fetal anatomy and to determine the intraobserver reproducibility and the effect of examiner experience. METHODS: Three-dimensional volumes of the head, face, thorax, and abdomen were obtained for 40 fetuses. The volume data sets obtained were explored offline with multiplanar navigation and tomographic ultrasound imaging on a personal computer. Each case was examined twice by the same observer at least 3 months apart. The percentage for identification of fetal anatomic structures, 2-dimensional (2D) and 3D measurements, and the time spent on 2D and 3D examinations were calculated and compared. RESULTS: Ninety-two percent of fetal anatomic structures were identified with multiplanar navigation and tomographic ultrasound imaging. The genitals, the entry of the vena cava, and the ears were visualized in less than 70% of cases. Tomographic ultrasound imaging allowed viewing of 14 structures not seen in the multiplanar study. Intraobserver agreement for anatomic examinations was good (kappa = 0.78). Intraobserver agreement for fetal measurements showed differences between both examinations that approached 0 and intraclass correlation indices close to 0.9. The mean 2D scanning time +/- SD was 10.11 +/- 2.5 minutes, and the acquisition time for the 3D volumes was 1.54 +/- 0.35 minutes. The total time for the second 3D study was 7.23 +/- 1.17 minutes, significantly shorter than the 9.96 +/- 1.53 minutes spent on the first study (P < .001). CONCLUSIONS: Three-dimensional volumes are highly effective for complete fetal anatomic surveys. They show excellent intraobserver reproducibility and take less time to study as the examiner's experience increases.  相似文献

OBJECTIVE: By evaluating a series of patients undergoing pelvic sonography with routine 2-dimensional (2D) as well as 3-dimensional (3D) reconstructed images in the coronal plane, we attempted to characterize the types of additional information that can be obtained. METHODS: Ninety randomly selected patients undergoing transvaginal pelvic sonography were imaged according to a standard 2D protocol. A 3D uterine volume was then acquired in the sagittal plane and reconstructed in the coronal plane. The endometrium and surrounding myometrium were evaluated for architecture, masses, the relationship of masses to the endometrial cavity, and the anatomic configuration of the cavity. RESULTS: Ninety-one studies were obtained. Additional findings were obtained on the coronal view in 28 studies (30.8%). No additional findings were obtained in 63 studies (69.2%). Normal endometrial and myometrial findings were obtained by conventional 2D imaging in 42 of 91 patients. Of this group, additional findings were shown in 2 (5%) patients. Forty-nine of the 91 patients had abnormal findings by 2D imaging. Additional information was obtained in 26 (53%) of these patients. Added information included uterine anomalies, better definition of the endometrium, more accurate delineation and location of endometrial polyps, location of leiomyomas, visualization of cystic areas within the myometrium, and confirmation of the location of intrauterine devices. CONCLUSIONS: The 3D reconstructed view of the endometrium and adjacent myometrium appears to be most helpful after a conventional transvaginal study, showing abnormalities within the endometrium and myometrium but being of little added benefit if the conventional findings are normal.  相似文献

In a case of histologically confirmed placenta increta, decidual protrusion into the myometrium was observed sonographically at 6 weeks' gestation, corresponding to placental protrusion from a disrupted placental-uterine wall interface seen in the later part of the first and second trimester. It is hypothesized that the histologic finding of decidual scarcity in placenta accreta is not a cause but rather an end result of the recruitment of trophoblasts across the decidual-placental interface in a maternal attempt at healing and/or repair in the presence of uterine injury, disease, or malformation that accounts for the associated factors and course of the condition.  相似文献

Pasteurella multocida is a common cause of infection in humans subsequent to bites or scratches by dogs and, particularly, cats. This infection usually results in superficial skin and soft tissue infections. Sonography can be used for diagnosing inflammatory conditions affecting tendons, including acute and chronic tenosynovitis. P. multocida tenosynovitis is rare, and the diagnosis can be missed if adequate tests are not performed. We report 2 cases of P. multocida tenosynovitis of the hand and wrist in which sonography played a valuable role in assessing the affected tissues and guiding fine-needle aspiration of fluid accumulations in the involved tendon sheaths. The diagnosis was confirmed microbiologically in each case.  相似文献
