目的探讨不同角度载荷对股骨头骨小梁形态学与力学性能的影响,为研究股骨头坏死、塌陷的生物力学机制提供理论依据。方法利用12月龄羊股骨头和人尸体股骨头分别制作羊股骨头骨小梁试件94个和人股骨头骨小梁试件43个。按照受力方向与骨小梁主压力方向之间的不同夹角,将骨小梁以10°间隔分为内翻10°、0°和外翻10°、20°、30°共计5组,模拟股骨颈骨折内固定术后不同戈登(Garden)对线指数下的复位情况。通过分别对羊股骨头骨小梁进行micro-CT扫描、计算与压缩破坏试验以及对人尸体股骨头骨小梁进行循环压缩试验,分析不同受力方向下股骨头骨小梁的骨体积分数(BV/TV)、骨表面积/骨体积(BS/BV)、骨小梁平均厚度(Tb.Th)、骨小梁数量(Tb.N)、骨小梁间距(Tb.Sp)等形态学指标以及弹性模量、极限强度、屈服强度、初始弹性模量、循环次数等力学指标。结果加载方向与骨小梁的主压力方向之间夹角为0°时,BV/TV、Tb.Th以及弹性模量、极限强度、屈服强度、初始弹性模量、循环次数均为最大,而BS/BV与Tb.N为最小,并随着夹角增大前者呈递减而后者呈递增趋势。结论 12月龄羊股骨头骨小梁BV/TV与极限强度随受力方向与骨小梁主压力方向之间夹角变化的趋势与人股骨头骨小梁一致;加载方向与主压力骨小梁之间夹角增大时,股骨头骨小梁形态学与力学性能均下降;Garden指数偏离160°越大时,股骨头内骨小梁越易发生损伤。  相似文献

不同结构骨块植入股骨头负重区缺损的力学性能比较   总被引:1,自引:0,他引:1  
目的股骨头缺血坏死早中期手术中,为防止股骨头的进一步塌陷,常需要植骨填充坏死区。本文研究不同结构植入物填充股骨头负重区缺损后对股骨头力学性能的影响。方法选取人正常股骨近段4例,在股骨头负重区沿压力骨小梁方向制造直径20mm,深30mm的骨缺损,并在缺损区内植入三种不同结构的植入物。Ⅰ组为取自异体股骨头负重区骨块;Ⅱ组为取自异体股骨髁负重区骨块;Ⅲ组为松质骨粒打压植骨。模拟单腿站立相加载,记录股骨头颈结合部的应变分布。结果直径20mm深30mm骨缺损的股骨近段与正常相比,头颈结合部上方拉应变平均增加93.0%,下方压应变平均增加203.4%。植入不同结构的植入物后,Ⅰ组头颈结合部上方应变较正常平均增加6.6%,下方应变平均增加67.6%;Ⅱ组上方应变较正常平均增加33.7%,下方应变平均增加136.9%;Ⅲ组上方应变平均增加150.4%,下方应变平均增加206.1%。结论股骨头负重区的缺损会改变头颈结合部应力分布,其中结合部下方和上方出现应力的集中。植入与缺损区的骨小梁结构相似的异体股骨头骨块后,股骨头下方和上方头颈结合部的应力集中得到缓解,其植入效果优于骨小梁结构不同的股骨髁骨块;松质骨粒打压植骨不能改变股骨头头颈结合部的应变分布。  相似文献

目的 探究股骨头坏死松质骨微观结构参数和力学性能的空间分布规律,为临床诊断中科学评估病灶对病情的影响提供理论依据。方法 结合影像学测量和数值模拟方法,定量分析不同区域松质骨的微观结构参数和力学性能,分别从冠状面、矢状面和水平面投影方向上探究松质骨生物力学特征的空间分布特点。结果 在股骨头冠状面和矢状面投影方向上,松质骨的微观结构特征与力学性能大致呈Y型分布,Y形区域内松质骨的力学性能高于其他区域,该分布特点与股骨头内的主压应力束的位置相吻合。结论 位于股骨头Y形区域内的坏死灶对股骨头内应力分布的影响程度会更大,更有可能导致病情恶化。临床诊断中应充分考虑坏死灶和Y形区域的位置关系。  相似文献

目的采用结合显微CT和显微有限元分析方法,即基于显微CT图像建立三维有限元模型并进行数值模拟仿真分析,无创研究不同分期的坏死股骨头松质骨的微结构和微观力学性能,以期了解在股骨头坏死的发展过程中,松质骨微结构和微观力学特性的变化规律,为临床预测股骨头坏死提供理论基础。方法采集10例股骨头坏死患者的股骨头标本的显微CT图像,按照国际骨循环研究学会分期标准分为Ⅱ期样本3例,Ⅲ期3例,Ⅳ期4例。将图像中骨组织进行阈值分割,分区域建立坏死区、侧向区、硬化区和远端区的松质骨块三维有限元模型(边长8 mm),并根据CT值赋予非均匀材料属性。利用ImageJ软件中的BoneJ插件通过识别显微CT组图像,计算测量各区域松质骨的微结构参数,包括骨体积分数、骨小梁厚度、骨小梁间隙、结构模型指数。对松质骨块施加表观应变为1%的压缩载荷,计算骨组织应力和松质骨表观刚度等参数,对比分析各分期、各分区结果。结果在松质骨微结构方面,Ⅳ期较Ⅱ期的股骨头内部硬化区和坏死区的变化最为明显,硬化区的骨体积分数不断上升,骨小梁间隙下降,结构模型指数减小,而坏死区域与之相反;在骨组织微观受力方面,Ⅱ期到Ⅲ期坏死区域的应力并没有明显变化,而硬化区域随着分期增加应力不断上升,侧向区的应力不断下降。表观刚度变化与应力变化一致。结论随着股骨头坏死程度的加剧,硬化区的松质骨微结构和力学性能变化最大,应作为临床早期诊断中重点关注的区域。此外,微结构参数并不能准确体现松质骨的力学行为,而股骨头塌陷最终取决于其力学特性,因此结合有限元分析方法可更加全面了解股骨头坏死的微观力学演变规律。  相似文献

背景:松质骨是一种多孔、非均质、各向异性和具有黏弹性的结构,是骨的重要组成部分。它在负载传输和能量吸收方面起着十分重要的作用。所以对松质骨的力学行为进行研究具有重要意义。目的:探讨单轴压缩实验中应力率对松质骨性能的影响以及不同应力值下松质骨的蠕变行为。方法:以新鲜的猪股骨松质骨为实验材料,施加不同的应力率,至试样的压缩应变达到5%卸载;在松质骨表面施加恒定的应力水平,并保持7 200 s,以便观察其蠕变应变变化。结果与结论:(1)单轴压缩实验中,在同一应变条件下,随着应力率的增加,应力值、杨氏模量也增加;(2)在不同的加载率作用下,松质骨的压缩应力-应变曲线不重合,说明松质骨的压缩力学性能具有率相关性;(3)在蠕变实验中,蠕变应变随着压缩的应力的增加而增加,蠕变柔量随着应力的增加反而减少;(4)轴压缩和蠕变实验结果说明,应力率和恒定应力水平对松质骨的力学性能影响很大,以上结果可以为如何避免松质骨损伤提供参考依据。  相似文献

目的运用有限元方法模拟腰椎爆裂骨折的过程,观察腰椎在轴向压缩载荷作用下松质骨内的应力分布情况。方法建立正常人体胸腰段(T12~L2)运动节段的三维有限元模型,在T12椎体上表面施加不同等级的压力(0.4、0.6、0.8、1.0、1.2 k N),模拟腰椎爆裂骨折发生时椎体承受的不同等级的轴向压缩载荷。将连接椎体上下终板凹面顶点的连线7等分,在此基础上将L1椎体中的松质骨划分为7个具有统计节点的层面,每个统计层面划分成6个统计区。分别测量L1椎体松质骨内3个层面(第1、4、7层面)18个统计区的平均应力。在同一等级载荷下对3个层面内的平均应力进行单因素方差分析,分析腰椎椎体松质骨内不同载荷作用下的应力分布情况。结果在5个不同等级载荷下,第1、7层松质骨平均应力分别与第4层比较有统计学意义(P0.05),而第1、7层平均应力比较无统计学意义(P0.05)。轴向加载时,相比第1、7层应力,椎体松质骨中间层面(第4层)应力最小。结论腰椎在轴向压缩载荷作用下,椎体松质骨内存在应力集中的现象,接近椎体上下软骨板的松质骨应力较大,而椎体松质骨中间层面应力较小,椎体内应力集中分布在上下软骨板的特点与腰椎爆裂骨折所致终板破裂的生物力学机制相一致,提示腰椎椎体骨结构损伤可能与椎体内应力集中有关。  相似文献

文题释义: 三维有限元模型:有限元模型主要是利用有限元分析方法建立的模型,而三维有限元模型主要是根据空间有限离散原则,将三维空间结构体分割不同区域,各区域为一单元且以节点相连接,因此节点间的变形运动是连续的,会引起周围节点及单元的相关变化。三维有限元模型精确逼真,且符合桡骨远端骨折的生物力学特性,模拟测试结果可为临床研究提供理论依据。 桡骨远端骨折:特指距桡骨远端关节面3 cm以内的骨折,常见于患者跌倒时损伤,是中老年人好发骨折之一,约占全部骨折患者的1/6。 背景:近年来关于桡骨远端骨折机制偏向定性的研究多数仅限于二维分析,易受多种因素影响,导致结果不甚满意,而利用CT数据建立三维有限元模型可更好评估人体骨骼变异情况。 目的:通过建立尺桡骨三维有限元模型,检测桡骨远端骨折生物力学,并进行桡骨远端骨折发生机制研究。 方法:获取1名50岁健康女性左侧上肢肱骨远端到腕关节中段CT扫描影像学资料,志愿者对试验方案知情同意,且经CT扫描显示正常,排除患有骨科相关疾病。利用三维有限元分析软件Ansys 16.0建立尺桡骨三维有限元模型,模拟手腕背伸、掌屈、尺偏、桡偏时力的加载,观察记录不同载荷下模型各部分应力;分析不同模型角度时桡骨远端骨折形成及裂纹走向。 结果与结论:①腕关节处于背伸位时,前臂旋前和前臂旋后桡骨背侧缘中点受压应力,随背伸角度增大而增大;前臂旋前和前臂旋后桡骨掌侧受张应力,随背伸角度增大而增大;②腕关节处于掌屈位时,前臂旋前和前臂旋后桡骨背侧缘中点受张应力,随掌屈角度增大而增大;前臂旋前和前臂旋后桡骨掌侧受压应力,随掌屈角度增大而增大;③前臂旋前背伸位及前臂旋后掌屈位时,桡骨裂纹首先出现于松质骨与密质骨交界处桡骨远端表面受最大张力一侧,骨折裂纹沿掌远端向近背端,与骨轴线45°角方向发展;④前臂旋前掌屈位及前臂旋后背伸位时,桡骨裂纹首先出现于松质骨与密质骨交界处桡骨远端表面受最大张力一侧,骨折裂纹沿远背端向近掌侧,与骨轴线45°角方向发展;⑤结果表明,手腕背伸、掌屈、尺偏、桡偏进行力的加载,骨折首先出现于松质骨与密质骨交界处桡骨远端表面受最大张力一侧,裂纹走向与所受应切力和张应力方向有关。 ORCID: 0000-0002-4577-2165(夏长江) 中国组织工程研究杂志出版内容重点:人工关节;骨植入物;脊柱;骨折;内固定;数字化骨科;组织工程  相似文献

目的探讨老年股骨颈骨折病人的股骨头样本各个区域的结构和生物力学性能差异,研究不同区域显微结构和生物力学特征及其对内固定物的影响。方法收集20个老年股骨颈骨折病人关节置换术后股骨头标本,以股骨头表面解剖标志点为参照,将股骨头按平分方法分为外侧、内侧、中间三部分。确定环钻的位置和钻取方向,用环钻于不同区域钻取直径10 mm、高10 mm圆柱形松质骨柱。通过Micro-CT系统扫描分析,分析不同区域内松质骨柱数据,包括骨体积分数(BVF)、骨小梁间隙(Tb.Sp)、骨小梁厚度(Tb.Th)、骨小梁数目(Tb.N)、骨表面积体积比(BS/BV)、结构模型指数(SMI)。应用微有限元计算不同区域骨组织的力学差异。结果老年股骨头颈内骨质含量下降,显微结构和生物力学性能区域变化明显,中间区域的骨结构和力学性能明显优于内侧和外侧。结论股骨头中间部位骨骼结构和力学强度明显优于内侧和外侧,在临床治疗骨质疏松股骨颈骨折的时候需要充分考虑内固定的部位。  相似文献

Over 200 schizophrenic patients belonging to three major and interrelated pedigree complexes have been investigated over the past 30 years in a North Swedish geographically isolated population, presently numbering about 6,000. An intensive investigation of a number of biochemical correlates and genetic markers in a few selected families belonging to one of the major pedigrees has indicated new strategies for the current research program.
Schizophrenia, as defined operationally, is significantly associated with decreased activities of two enzymes (1) blood platelet monoamine oxidase, (2) plasma dopamine-β-hydroxylase, and (3) with the genetic marker Gc2 (group specific antigen). Both enzymes are subject to genetic variation. A positive score for linkage between schizophrenia and low plasma DBH activity has been calculated, but, so far, available data are insufficient for discrimination between linkage and partial contribution of genetically controlled low plasma DBH to the pathogenesis of the disease. Alternatively, both mechanisms could be involved.
As a model for continued research, schizophrenia is explained as based on a double dominant-recessive genotype (Aabb), representing a vulnerability which in about 50 % of cases develops into clinical schizophrenia. It is suggested that the dominant mutation (A) operates on or affects MAO activity, and that the recessive genotype (bb) is instrumental in low variates of DBH activity and very likely such variates within the normal range of physiological variation. Moreover, it is suggested that the combined effects of MAO- and DBH-reduced efficiency on the metabolism of e.g. dopamine could be an essential pathogenic mechanism for the schizophrenic illness which is segregating in this population.  相似文献

A family is reported in which two sibs, one male and the other female, both died within 24 hours of birth with enlarged polycystic kidneys. Postmortem histology in the second child showed gross renal dysplasia. In both children the pancreas was enlarged, nodular and cystic but the liver appeared macroscopically normal. In the second child, histological examination confirmed pancreatic fibrosis with cystic dilation of ducts, but showed portal fibrosis with bile duct proliferation in the liver.
This combination of findings is very reminiscent of those in a girl and her brother reported by Ivemark et al. (1959). The children reported here also showed absence or hypoplasia of the spleen, cardiac anomalies and other features of the Ivemark syndrome (Ivemark 1955), a quite different, usually sporadic, congenital disorder. It is suggested that the children described here have a distinct lethal congenital disorder, probably inherited in an autosomal recessive manner.  相似文献

About 1900, modern food selection and processing caused widespread epidemics of the B vitamin deficiency diseases of beriberi and pellagra which, for genetic reasons, often expressed as different diseases ranging from bowel and heart disease to dermatoses and psychoses. But the B vitamins merely help convert essential fatty acids (EFA) into the prostaglandin (PG) tissue regulators and it now turns out that, through hydrogenation, milling and selection of w3-poor southern foods, we have also been systematically depleting, by as much as 90%, a newly discovered trace Nordic EFA (w3) of special importance to primates and sole precursor of the PG3(4) series, even as a concurrent fiber deficiency increases body demand for EFA. Since substrate EFA is processed by many B vitamin catalysts, an EFA deficiency will mimic a panhypovitaminosis B, i.e., a mixture of substrate beriberi and substrate pellagra resembling vitamin beriberi and pellagra but exhibiting as even more diverse endemic disease. This would consitute a second stage of the Modern Malnutrition and explain why some workers now hold the dominant diseases of modermized societies to be new, nutritionally based, pellagraform yet lipid-related and to range, once again, from heart disease to psychosis. It is an assumption that our dominant diseases are unrelated to each other or are merely revealed by our diagnostic acumen and therapeutic success; and that hydrogenating millions of tons of food oils annually, to destroy the rancidity producing w3-EFA, is safe for primates. Extensive beriberiform disease is reported here in 32 typical cases taken from medical practice which responds strikingly to linseed oil supplements (60% w3-EFA) in confirmation of identical results in Capuchins.  相似文献

Sick children were ubiquitous in early modern England, and yet they have received very little attention from historians. Taking the elusive perspective of the child, this article explores the physical, emotional, and spiritual experience of illness in England between approximately 1580 and 1720. What was it like being ill and suffering pain? How did the young respond emotionally to the anticipation of death? It is argued that children’s experiences were characterised by profound ambivalence: illness could be terrifying and distressing, but also a source of emotional and spiritual fulfilment and joy. This interpretation challenges the common assumption amongst medical historians that the experiences of early modern patients were utterly miserable. It also sheds light on children’s emotional feelings for their parents, a subject often overlooked in the historiography of childhood. The primary sources used in this article include diaries, autobiographies, letters, the biographies of pious children, printed possession cases, doctors’ casebooks, and theological treatises concerning the afterlife.  相似文献

Recent advancements in agricultural biotechnology have created a need for analytical techniques to determine introduced proteins in crops enhanced through modern biotechnology techniques. These proteins are expressed in plant tissues and may be present in food ingredients. Immunoassays are ideally suited for protein detection and may be used as both quantitative and threshold methods. Microplate ELISA and lateral flow devices are two of the most commonly used immunoassay formats for agricultural biotechnology applications. This paper provides general background information and a discussion of criteria for the validation and application of immunochemical methods to the analysis of proteins introduced into plants and food ingredients using biotechnology methods. It is the result of a collaborative effort of members of the Analytical Environmental Immunochemical Consortium. This collaborative effort represents the combined expertise of several organizations to reach consensus on establishing guidelines for the validation and use of immunoassays. Further, the paper offers developers and users a consistent approach to adopting the technology as well as aid in producing accurate and meaningful results.  相似文献

HLA-A,-B,-C,-DRB1 and -DQB1 alleles have been studied in Chimila Amerindians from Sabana de San Angel (North Colombian Coast) by using high resolution molecular typing. A frequent extended haplotype was found:HLA-A*24:02-B*51:10-C*15:02-BRB1*04:07-DQB1*03:02 (28.7%) which has also been described in Amerinndian Mayos Mexican population (Mexico, California Gulf, Pacific Ocean). Other haplotypes had already been found in Amerindians from Mexico (Pacific and Atlantic Coast), Peru (highlands and Amazon Basin), Bolivia and North USA. A geographic pattern according to HLA allele or haplotype frequencies is lacking in Amerindians, as already known. Also, five new extended haplotypes were found in Chimila Amerindians. Their HLA-A*24:02 high frequencies characteristic is shared with aboriginal populations of Taiwan; also, HLA-C*01:02 high frequencies are found in New Zealand Maoris, New Caledonians and Kimberly Aborigines from Australia. Finally, this study may show a model of evolutionary factors acting and rising one HLA allele frequency (-A*24:02), but not in others that belong to the same or different HLA loci.  相似文献

The preparation steps usually necessary for obtaining ultrathin frozen sections of biological material (chemical prefixation, enclosing, cryoprotective treatment, freezing, sectioning, and post-staining the sections for transmission electron microscopy) are submitted to a critical analysis. The application of cryo-ultramicrotomy, in particularly for cytochemical purposes, is reviewed. Fundamental considerations of chemical prefixation and poststaining are supported by examples from yeast cytology. Furthermore, the efficiency of the cryo-ultramicrotomy (electron optical resolution of ultrastructural details) is demonstrated on yeast cells and protoplasts.  相似文献

Starting with the integument, we see many organs are contractile sacs or multiples thereof, which tubes or bags constitute the major part of the entire body. Recognition of this basic unit and its characteristics sheds new light, individually and collectively, on many disorders previously considered unrelated. Muscular tears and perforations develop in the walls of these chambers, being no way peculiar to those organs, wherein, hydrochloric acid occurs. So, it is not necessary to explain the absence of excessive acid from patients who exhibit holes in the gastric, uterine, aortic, duodenal, rectal, pulmonary, retina, and other walls. Muscle, not acid is the great common factor relating idiopathic disorders in the gastrointestinal tract to each other and to similar diseases in other systems. When the units are linked together, the lesions tend to appear as arthropathies, i.e. at the joints. Rephrasing common-place observations, frees us from conventional, conceptual cul-de-sacs. An observation is only as good as its interpretation, so all possibilities must be considered, otherwise, we will remain blinded by our misconceptions.  相似文献

Karzinoid-Syndroms wurde untersucht. Die in die Untersuchung einbezogenen Nahrungsmittel waren: Tee, Kaffee, Mandeln, Ananas, Käse, Walnüsse, Vanillepudding, Bananen, Tomaten und Milchschokolade. Außerdem wurde der Einfluß des Zigarettenrauchens auf die Bestimmung von AD, NA, VMS und MN untersucht.Walnüsse führten zu einer starken Erhöhung der 5-HIE-Ausscheidung. Bananen erhöhten die Ausscheidung von AD, NA, VMS, MN und 5-HIE. Kaffee und Ananas bewirkten eine geringe Zunahme der MN-Werte. Rauchen von 20–30 Zigaretten/Tag beeinflußte keine der vier Variablen.Wenn die beschriebenen Methoden benutzt werden, sollte lediglich auf den Verzehr von Bananen und Walnüssen vor und während der Harnsammelperioden verzichtet werden, da die oberen Normgrenzen im Harn überschritten werden könnten. Ein Verzicht auf Kaffee und Ananas in normalen Mengen ist nicht erforderlich. Es besteht kein Anlaß, weiterhin die bisherigen umfangreichen Restriktionen der übrigen Nahrungsmittel beizubehalten.  相似文献
