How I do it…. How infection doctors approach the PJI MDT

The management of prosthetic joint infection (PJI) requires discussion between experts working towards the aims and expectations of the patient. Each discipline typically does not offer a single-minded approach, but instead collectively weighs up the strengths and weaknesses of different management options. At a basic level, the infection specialist provides antimicrobial solutions based on laboratory results. During multi-disciplinary team (MDT) discussions however, they can offer additional insights into how the microbiology could influence the surgical decisions, respond to the surgical debate by designing a rational approach to antimicrobials and contribute to an option-appraisal that can be presented to the patient. In this short opinion piece, we describe some of the thoughts we have during MDT discussions to ultimately recommend an optimised treatment plan for the individual patient: is there infection? what is the pathogen? where did it come from? (and where else has it gone), and what is the best treatment strategy?

‘I Don't Want to See that the People Are Suffering’: Nurses in an Impoverished Community Talk About Caring for Women Following Stillbirths

This article explores the way in which nurses working in a high-risk and dangerous environment understand and speak about their work caring for mothers following stillbirths. As far as we are aware, it is the first study of its kind from a low- or middle-income country, and the first on this topic to apply the theoretical insights of Menzies Lyth in such contexts. In order to obtain rich exploratory data, a qualitative research methodology was used. The primary data source was interviews with nurses about their practices with women who have stillbirths. Reflecting on the findings from these interviews, we believe that the nurses’ disclosures followed an overarching narrative that connected their cultural identity and personal suffering to the care that they administer. These connections between identity and profession perpetuate a healthcare system where the nurse often gives, not only out of duty and selflessness, but also out of her own sense of vulnerability. As hospitals in low-income countries seek to improve their capacity to heal and support those in need of medical attention, nurses should be a focus of research. While their role is generally seen as a support to the doctors, in the case of stillbirths, they are far more central and often ill-equipped for their role as grieving partner. Nurses need to be acknowledged and adequately supported.

收集广东省各类人群血清标本２２２４例，用间接免疫荧光法检测人类嗜Ｔ细胞病毒Ｉ型抗体，结果：ＨＴＬＶ－１抗体阳性３５份，总阳性率为１．５７％，其中健康人群血清１８１０份，抗体阳性２３份，阳性率为１．２７％，献血员血清２４８份，抗体阳性１份，阳性率０．４０％，白血病患者血清１０９份，阳性８份，阳性率７．３４％，与健康人群非常显著性差异（Ｐ〈０．０５）；神经系统疾病患者血清５７份，阳性３份，阳性率５．２

２５只白兔用卵清蛋白为抗原经全身和鼻粘膜局部免疫，４ｗ后其中１７天动物血清卵清蛋白抗特异性ＩｇＧ抗体上升到免疫前５倍以上，并经抗原刺激鼻粘膜诱发出喷样反应。流清涕等变应性鼻炎临床症状，这１７只动物用作Ｉ型变态反应动物模型，其中９只进一步用制备的异种基因型ＩｇＥＦｃ片段为抗原免疫，２ｗ后在血清中检测到较高浓度的抗ＩｇＥ抗体，同时伴有血清部ＩｇＥ抗体浓度下降，上述变化持续观察１０ｗ无明显改变，提示异常

目的；探讨ＩＤＤＭ患者Ｃ肽（ＣＰ）水平与糖尿病慢性并发症的相关性。方法：１９９３年至１９９８年在本院住院的ＩＤＤＭ患者共４６例，均伴有各种糖尿病并发症，用放免法测定空腹，餐后Ｃ肽水平及记录各种慢性并发症发生时间，用ＳＡＳ软件包分析两者的关系，结果：ＩＤＤＭ患者Ｃ肽水平与糖尿病慢性并发症的发病时间呈正相关，结论：ＩＤＤＭ患者体内残留Ｃ肽水平越高，糖尿病慢性并发症的出现越迟，体内Ｃ肽可能对ＩＤＤＭ患者     

胃癌细胞角蛋白mRNA和I型胶原mRNA的原位杂交及杂交电…

采用两种ｃＤＮＡ探针，建立了胃癌组织角蛋白ｍＲＮＡ和Ｉ型胶原ｍＲＮＡ的原位分子杂交 杂交电镜技术。结果显示：胃癌细胞具有旺盛合成角蛋白的能力，而胃癌间持间质细胞对Ｉ型胶原基因的转录合成与正常胃粘膜无异。并对原位杂交、杂交电镜的技术方法和两种ｍＲＮＡ表达的意义进行了探讨。
