目的:报告45内蛛网膜囊肿(IAC),对其临床表现.诊断.手术适应证.手术方法选择和术中注意事项进行了讨论。方法:回顾性分析我院1995年5月~2003年12月收治的45例颅内蛛网膜囊肿。结果:手术45例在术后囊腔均缩小或消失,颅高压解除,症状消失。结论:ICA约占颅内占位性病变的⒈17%,最常见于外侧裂池。CT.MRI是最可靠的诊断方法。凡囊肿有颅内高压者,充分建立囊腔与周围脑池或腹腔之间的交通是手术成功的关键。  相似文献

目的探讨颅内蛛网膜囊肿(IAC)的临床特点及治疗方法。方法对我科手术治疗19例颅内蛛网膜囊肿进行回顾性分析总结。结果本手术组所有患者均恢复良好,无死亡病例,复查CT或MRI15例囊肿消失或缩小,4例变化不明显,术前颠痫患者,得到控制或发作次数减少。结论对确诊的颅内蛛网膜囊肿,积极的手术能有效地改善患者的临床症状体征,是较为安全和可行的方法。  相似文献

目的:为寻找一种治疗颅内蛛网膜囊肿(IcAC)的理想手术方法.方法:应用内窥镜技术治疗6例IcAC.年龄11~28岁.囊肿位于幕上4例,后颅窝2例.随访时间1~10个月.结果:1例死亡,5例近期效果满意.本方法术中术野清晰,可在镜下切开或部分切除囊肿脏层,连通囊肿与脑池或蛛网膜下腔的CSF循环,手术创伤小,较安全.结论;IcAC是内窥镜手术治疗的理想适应证.  相似文献

目的:研究颅内蛛网膜囊肿的临床分类及治疗方法的选择。方法:对36例颅内不同部位、经过手术治疗的蛛网膜囊肿进行分析,着重结合临床表现、影像学特点以及手术治疗进行临床分型,提出各型标准和手术治疗选择原则,并应用神经窥镜治疗蛛网膜囊肿,与常规手术比较。结果:本组34例(91.7%)治愈,临床症状消失。神经窥镜治疗者均获痊愈。结论:对颅内蛛网膜囊肿进行临床分型有助于科学选择治疗术式。神经窥镜治疗颅内蛛网膜囊肿是一种较好的方法。  相似文献

The magnetic resonance characteristics of 16 intracranial arachnoid cysts were evaluated and several features identified that allowed the differentiation from other cystic lesions. Uncomplicated arachnoid cyst contents respond like CSF to both T1 and T2 weighted sequences. Signal from the contiguous brain is of normal intensity. The absence of signal from cortical bone and the ease of obtaining multiplanar views enable the margins of the cyst, its contents, and the full extent of the lesion to be easily defined.  相似文献

Between July 1981 and August 1983 the authors studied 21 patients with intracranial arachnoid cysts using metrizamide-enhanced CT. With this method we are able to determine the degree of communication between the cysts and the surrounding subarachnoid space as well as to assess the evolving character of some of these lesions. BACKGROUND AND PURPOSE: The demonstration of communication between arachnoid cysts (ACs) and the adjacent subarachnoid space is a prerequisite for their proper management. CT cisternography (CTC) is the conventional method for functional evaluation of ACs. The sensitivity of MR imaging to CSF flow has been demonstrated, but reports of the clinical usefulness of MR CSF flow techniques in this application are limited. The purpose of our study was to prospectively evaluate the accuracy of MR CSF flow study as an alternative to CTC in this setting. METHODS: MR CSF flow study with retrospective ECG-gated 2D, fast low-angle shot, phase-contrast (PC), cine gradient-echo sequence was performed in 39 patients with an intracranial AC. Results were compared with intraoperative and CTC findings. RESULTS: PC cine MR imaging results were compatible with operative or CTC findings in 36 (92.3%) of 39 patients. Twenty-four cysts were noncommunicating, and 15 were communicating. Three cysts were evaluated as being noncommunicating on PC cine MR imaging (false-negative) but demonstrated contrast enhancement on CTC. No false-positive diagnoses occurred. All cysts regarded as being communicating on PC cine MR imaging were also found to be communicating on both confirmation methods. CONCLUSION: MR CSF flow imaging with a PC cine sequence can be incorporated in the imaging work-up of ACs. This is a reliable alternative to invasive CTC for the functional evaluation of ACs.  相似文献

目的:评价颅内先天性胆脂瘤的MRI影像学特征.方法:回顾性复习20例经手术与病理证实的颅内先天性胆脂瘤.术前均行MRI平扫与增强检查.结果:肿瘤发生于桥小脑角区11例,鞍上区3例,鞍旁区3例,四脑室1例,侧脑室1例,小脑蚓部1例.典型表现者16例,占80%,表现为T1WI低信号,T2WI高信号;非典型者4例,占20%,T1WI表现为欠均匀高信号,T2WI表现为低信号.增强扫描,16例肿瘤不强化,4例边缘强化.结论:MRI对诊断颅内先天性胆脂瘤有重要价值.  相似文献

Five cases of benign spinal arachnoid cysts in children are described with their clinical, neuroradiological, surgical and pathological findings. Intraspinal benign arachnoid cysts may be confidently diagnosed myelographically when there is complete or partial obstruction with multiple oily contrast/cerebrospinal fluid levels as in 3 cases in this series. The multiple fluid levels are probably produced by multiple cysts or a cyst loculated due to partial compression by arachnoid bands either related to the normal septum posticum, congenital or acquired adhesion rather than to trabeculations within the cyst. In this series, the origin of the cysts is considered to be congenital.  相似文献

A retrospective review of magnetic resonance imaging (MRI)—computed tomography (CT) correlation was performed in 29 patients with arachnoid cysts. Short TR, short TE spin echo (SE) pulse sequences provided the best anatomic definition whereas multiple echo long TR, TE sequences allowed comparison of the signal intensity of the cyst with that of cerebrospinal fluid (CSF). Simple arachnoid cysts were isointense while neoplastic, hemorrhagic or inflammatory cysts were hyperintense relative to CSF. The CT differential diagnosis of an arachnoid cyst (depending upon its location) may include other cystic collections such as craniopharyngioma, epidermoid, astrocytoma, and chronic subdural hematoma. However, on MRI the combination of extraaxial location, morphological features, and signal intensity matching that of CSF allows one to make the diagnosis of an uncomplicated arachnoid cyst with confidence.  相似文献
