Association between iris color and iris melanocytic lesions

In a case-control study of 212 white patients with primary melanocytic tumors of the iris and two control groups, contingency tables were used to evaluate the distribution of iris color. We found a strong association between light iris color (blue, gray, or green) and the presence of iris melanocytic lesions (P less than .001 by chi 2).

原发性闭角型青光眼(PACG)是亚洲,特别是中国青光眼的主要类型。PACG在激光周边虹膜切除术(LPI)后仍有很高比例发生房角关闭,高褶虹膜(plateau iris)是引起房角关闭的非瞳孔阻滞因素之一。随着超声生物显微镜(UBM)在眼科的广泛应用,对高褶虹膜的认识不断深入。本文将对高褶虹膜的概念、机制、患病率、与房角关闭的关系、诊断标准及治疗方法进行阐述,旨在更加深刻理解高褶虹膜与PACG的关系,为今后PACG的治疗和研究提供参考。     

Intraoperative management of iris prolapse using iris hooks

Iris prolapse has become an unusual and generally benign event in routine cataract surgery using endocapsular phacoemulsification techniques. We operated on the second eye of a patient with hyperopia whose first eye surgery in our care had been uneventful. In the second eye, as soon as the phaco incision was made and hydrodissection begun through the primary incision, the iris repeatedly prolapsed through the incision. This was managed definitively by applying iris hooks, 1 at each side of the primary incision, to the pupillary border so the prolapsing iris was held peripherally in the anterior chamber and under modest tension.     

Surgical management of iris defects with prosthetic iris devices

PURPOSE: To evaluate the safety and efficacy of surgical implantation of prosthetic iris devices in patients with iris deficiency. METHODS: Nine patients with traumatic iris defects, congenital aniridia or iris coloboma, and surgical or optical iridectomies were included in a noncomparative case series. Cataract surgery with intraocular lens and prosthetic iris implantation was performed in 10 eyes. The visual acuity, subjective degree of glare disability, postoperative anatomic results, and intraoperative and postoperative complications were evaluated. RESULTS: The mean follow-up was 17.75 months (range 4-48 months). Best-corrected visual acuity improved in nine of 10 eyes (90%) and remained unchanged in one eye. Glare subjectively improved in four of five eyes (80%) of patients complaining of glare preoperatively. All eyes achieved the desired anatomic result. Intraoperative complications included one anterior capsular tear. Postoperative complications included a short period of mild postoperative anterior uveitis in four eyes. Secondary glaucoma was absent. CONCLUSION: In patients with iris deficiency, implantation of prosthetic iris device, and intraocular lens implant following cataract surgery appears to be safe and effective in reducing glare disability and improving visual outcomes.     

人工虹膜隔的应用进展

无虹膜或虹膜大部分缺损眼硅油填充时,往往出现硅油和角膜内皮接触,导致角膜变性、混浊和眼压升高等并发症。为了避免这些并发症的发生,通常植入人工虹膜隔来代替虹膜。本文就人工虹膜隔的类型、形态、优缺点、植入方法、目前的治疗效果及原因等进行综述。     

Absorbable iris suture

Summary Suturing of traumatic or surgical iris wounds has proved to be a safe and feasible microsurgical procedure. Various suture materials have been tried experimentally and clinically. If the human iris forms scar tissue the ideal suture would be an absorbable, inert, proteinfree material causing no uveal reaction. It was the purpose of this study to evaluate the ocular tissue reaction to nylon, virgin-slik and polyglycolic acid (Dexon^®) after suturing of an experimental iridomy in 24 rabbit eyes. The tissue response was studied by biomicroscopy, iris fluorescein colour angiography, and histology. Nylon did not disintegrate and caused virtually no inflammatory reaction. Silk showed fragmentation and complete absorption after four to six months with moderate tissue response. Polyglycolic acid gave rise to a marked uveal reaction and depolymerised from the fifth to the sixth week. There was no wound dehiscence of the iris scar and the pupillary function remained intact. It is concluded that the non-absorbable materials are superior to the currently available absorbable sutures in iris surgery.     

Lack of association between iris color and primary iris cysts

A case-control study of 78 white patients with primary cysts of the iris was undertaken. Two control groups were established for comparison with the subjects. Contingency tables were used to evaluate the distribution of iris color among subjects and controls. Statistical testing (chi 2) indicated no significant association of iris color with the presence of a primary iris cyst (P greater than .05). These results indicated that primary iris cysts do not exhibit the association with light iris color previously documented for iris melanocytic lesions.
