The aim of this study was to compare the fracture resistance and failure pattern of endodontically treated maxillary incisors restored using composite resin with or without fiber-reinforced composite (FRC) posts under different types of full-coverage crowns. The null hypothesis tested was that fracture resistance and the failure pattern of these teeth were not affected by the use of FRC posts or by the type of full-coverage crown. One hundred twenty maxillary incisors were endodontically treated and divided into 4 groups of 30 each. Each group was divided into two subgroups: restoration with or without fiber post. PFM crowns were placed in group 1, Empress II crowns in group 2, SR Adoro crowns in group 3, and Cercon crowns in group 4. Fracture tests were performed by loading specimens to fracture. Data were analyzed with two-way analysis of variance (alpha = 0.05). The type of crown was not a significant factor affecting fracture resistance (p = 0.4), whereas the presence of a post was (p = 0.001). Both the presence of post and the type of crown had a significant influence on the proportion of restorable versus unrestorable fractures. Although prosthodontics textbooks do not generally advocate the placement of fiber posts in endodontically treated incisors, the results of this study indicate that the use of fiber posts in such teeth increases their resistance to fracture and improves the prognosis in case of fracture.  相似文献

目的比较不同桩修复离体牙对修复体整体抗折强度的影响。方法将32颗上颌前磨牙行根管治疗并从唇侧釉牙骨质界上2 mm处截断后随机分为4组,A组采用镍铬金属铸造桩核加铸造冠修复,B组采用Tenax FiberWhite纤维桩、树脂核加铸造冠修复,C组采用EverStick可塑纤维桩单根(直径1.5 mm)、树脂核加铸造冠修复,D组采用EverStick可塑纤维桩双根(直径1.5 mm和1.2 mm)、树脂核加铸造冠修复。所有离体牙均由自凝树脂包埋,电子万能试验机加载头与牙体长轴成45°,以1 mm/min的加载速度加载直至试件任何部分折裂,记录读数并观察修复体折裂模式。结果A、B、C、D组间修复体的整体抗折强度差异无统计学意义(P>0.05)。各纤维桩组牙体折裂位置较铸造桩更接近牙颈部。结论尽管可塑纤维桩自身的弯曲强度较低,但是修复离体牙后整体抗折强度较高,可以满足临床要求。各纤维桩组折裂模式较铸造桩组更有利于修复体失败后的再修复。  相似文献

This study determined the effect of different post designs and varying amounts of post-to-canal adaptation on the fracture resistance of endodontically treated teeth. Forty freshly extracted maxillary central incisors were endodontically treated. Groups of 10 teeth were prepared according to four experimental designs. Cast post and cores and crowns were waxed, cast, and luted with zinc phosphate cement on a static loading device. The teeth were embedded in acrylic resin and the crowns were loaded on a universal testing machine at 130 degrees to the long axis of the tooth until failure. Maximum adaptation of the residual root structure with a tapered post significantly increases the fracture resistance of endodontically treated teeth, but upon failure renders the tooth nonrestorable. Tapered posts resulted in fractures that were directed more apically and lingually. Parallel-sided posts had a lower frequency of fracture upon failure, involving less tooth structure. Parallel-sided posts surrounded by large amounts of cement had no significant effect on failure loads.  相似文献

STATEMENT OF PROBLEM: Little information exists regarding the outcome of crown build-ups on endodontically treated teeth restored with metal-ceramic crowns or with only a direct-placed composite. PURPOSE: The aim of this study was to evaluate the clinical success rate of endodontically treated premolars restored with fiber posts and direct composite restorations and compare that treatment with a similar treatment of full-coverage with metal-ceramic crowns. MATERIAL AND METHODS: Subjects included in this study had one maxillary or mandibular premolar for which endodontic treatment and crown build up was indicated and met specific inclusion/exclusion criteria. Only premolars with Class II carious lesions and preserved cusp structure were included. Subjects were randomly assigned to 1 of the following 2 experimental groups: (1) teeth endodontically treated and restored with adhesive techniques and composite or (2) teeth endodontically treated, restored with adhesive techniques and composite, and then restored with full-coverage metal-ceramic crowns. Sixty teeth were included in the first group and 57 in the second. All restorations were performed by one operator. Causes of failure were categorized as root fracture, post fracture, post decementation, clinical and/or radiographic evidence of marginal gap between tooth and restoration, and clinical and/or radiographic evidence of secondary caries contiguous with restoration margins. Subjects were examined for the listed clinical and radiographic causes of failure by 2 calibrated examiners at intervals of 1, 2, and 3 years. Exact 95% confidence intervals for the difference between the 2 experimental groups were calculated. RESULTS: At the 1-year recall, no failures were reported. The only failure modes observed at 2 and 3 years were decementations of posts and clinical and/or radiographic evidence of marginal gap between tooth and restoration. There was no difference in the failure frequencies of the 2 groups (95% confidence interval, -17.5 to 12.6). There was no difference between the number of failures caused by post decementations and the presence of marginal gaps observed in the 2 groups (95% confidence intervals, -9.7 to 16.2 and -17.8 to 9.27). CONCLUSION: Within the limitations of this study, the results upheld the research hypothesis that the clinical success rates of endodontically treated premolars restored with fiber posts and direct composite restorations after 3 years of service were equivalent to a similar treatment of full coverage with metal-ceramic crowns.  相似文献

OBJECTIVES: The present study was designed to test the null hypothesis that there is no difference in the fracture resistance of pulpless teeth restored with different types of post-core systems and full coverage crowns. METHODS: Extracted human upper premolars were restored with a fiber post, prefabricated metallic post or cast metallic post-core. Teeth with full crown preparations without post-core restorations served as a control. All teeth were restored with full coverage crowns. A 90-degree vertical or 45-degree oblique load was applied to the restored teeth with a crosshead speed of 0.5 mm/min, and the fracture loads and mode of fracture were recorded. RESULTS: Under the condition of vertical loading, the fracture load of teeth restored with the cast metallic post-cores was greatest among the groups (two-factor factorial ANOVA and Scheffe's F test, P<0.05). All fractures in teeth restored with all types of post-core systems propagated in the middle portions of roots, including the apices of the posts. Under the condition of oblique loading, the fracture load of teeth restored with pre-fabricated metallic posts was significantly smaller than that in other groups. Two-thirds of fractures in the fiber post group propagated within the cervical area, while most fractures in other groups extended beyond the middle of the roots. SIGNIFICANCE: From the results of the present investigations, it was concluded that under the conditions of vertical and oblique loadings, the combination of a fiber post and composite resin core with a full cast crown is most protective of the remaining tooth structure.  相似文献

PURPOSE: To compare the fracture resistance and failure patterns of endodontically-treated teeth with a progressively reduced number of residual walls restored using resin composites, with or without translucent glass fiber posts. METHODS: Ninety extracted human single-rooted maxillary premolars were used. After endodontic treatment, the following groups were created: Group 1 (control group): endodontically-treated single-rooted maxillary premolars with four residual walls; Group 2: three residual walls; Group 3: two residual walls; Group 4: one residual wall, and Group 5: no residual wall. Groups 2-5 were each divided into two subgroups: subgroups "a" were restored with resin composites, while subgroups "b" were restored with translucent glass fiber posts and resin composites. Static fracture resistance tests and statistical analyses (P= 0.05) were performed. RESULTS: The mean failure loads (N) were 502.4+/-152.5 (Group 1), 416.4+/-122.2 (Group 2a), 423.0+/-103.3 (Group 2b) 422.1+/-138.9 (Group 3a) 513.2+/-121.7 (Group 3b), 488.7+/-153.7 (Group 4a) 573.4+/-169.2 (Group 4b), 856.7+/-112.2 (Group 5a) and 649.5+/-163.5 (Group 5b), respectively. The samples restored with fiber posts exhibited predominantly restorable fractures. The number of residual cavity walls influenced the mechanical resistance of endodontically-treated teeth.  相似文献

目的比较纤维桩和金属铸造桩在前牙和前磨牙残冠残根修复中的临床疗效。方法选择2009年至2010年我科门诊治疗中因残冠残根而行桩核冠修复的196例(262颗患牙)病例,以纤维桩作冠修复,门诊随访24个月,并与我科2007年至2008年211例(278颗患牙)对金属铸造桩作冠修复的研究结果作比较,观察两种桩核材料的临床疗效。结果经24个月的随访,纤维桩的成功率为95.42%,金属铸造桩的成功率为88.13%,两组之间的差异有统计学意义(P<0.05)。纤维桩的失败病例中均为冠折或桩核脱落。结论在前牙和前磨牙桩核冠修复中使用纤维桩的临床疗效优于金属铸造桩核,且纤维桩失败后可以进行二次修复,以达到保留患牙的目的。  相似文献

The purpose of this study was to evaluate the combined effect of ferrule height and post length on fracture resistance and failure mode of endodontically treated teeth restored with glass fibre posts, composite resin cores and crowns. Ninety human maxillary central incisors were endodontically treated and divided into three groups (n = 30) according to the ferrule heights: 4, 2 and 0 mm, respectively. Post spaces in each group were prepared at 2/3, 1/2 and 1/3 of the root length (n = 10). The specimens were received fibre posts, composite resin core build up and cast metal crowns. After thermocycling, compressive static load was applied at an angle of 135° to the crowns. Two‐way analysis of variance showed significant differences in the failure load in the ferrule height groups, no significant differences in post length groups and no significant interaction between ferrule heights and post lengths. AIM: To investigate ex vivo the fracture resistance and failure mode of direct resin composite complete crowns with and without various root canal posts made on maxillary premolars. METHODOLOGY: The clinical crowns of 40 human extracted single-rooted maxillary premolars were sectioned at the cemento-enamel junction. The canals were prepared with Gates Glidden drills up to size 4. Thirty samples were provided with standardized post spaces in the palatal canal and all roots were embedded in acrylic. Minimal standardized preparations in the canal entrances were made. Groups of 10 samples were treated with (i) prefabricated metal posts, (ii) prefabricated glass fibre posts, (iii) custom-made glass fibre posts, and (iv) no posts (control). Posts were cemented with resin cement and resin composite complete crowns were made. All specimens were thermocycled (6000x, 5-55 degrees C). Static load until fracture was applied using a universal loading device (crosshead speed 5 mm min(-1)) at a loading angle of 30 degrees . Failure modes were categorized as favourable and unfavourable failures. RESULTS: No significant difference was observed between the mean failure loads (group 1: 1386 N, group 2: 1276 N, group 3: 1281 N, and group 4: 1717 N, P > 0.05), nor between frequencies of failure modes (P > 0.05). All failures were fractures of the resin composite crown in combination with tooth material (cohesive failures). CONCLUSIONS: Within the limits of this laboratory investigation it is concluded that severely damaged and root filled maxillary premolars, restored with direct resin composite complete crowns without posts have similar fracture resistances and failure modes compared to those with various posts, which suggest that posts are not necessarily required.  相似文献

目的:探讨如何选择合适直径的桩和根管重塑材料,使薄壁根管获得更好的抗折性能。方法:选择90颗完整的上颌中切牙,完成根管治疗后,截冠分为9组。其中,4组进行常规桩道预备后行1.3 mm、1.6 mm镍铬合金铸造桩(A1.3和A1.6)和玻璃纤维桩(B1.3和B1.6)黏结后全冠修复,其余5组桩道扩大至根管壁为1 mm后行铸造桩核(C)和玻璃纤维桩修复,其中1.3、1.6 mm纤维桩用Rely X Unicem(D1.3和D1.6)和ParaCore(E1.3和E1.6)进行根管重塑和黏结,再用复合树脂堆核型后全冠修复,比较各组的抗折力和折裂模式。采用SPSS16.0软件包对数据进行统计学分析。结果:常规根管中,A1.3组获得最大的抗折力(610.2±45.6)N,A1.3、A1.6和B1.3、B1.6之间无显著差异(P>0.05)。薄壁根管中,C组获得最大的抗折力(584.5±121.2)N,而纤维桩经ParaCore重塑根管后可以增加牙体的抗折力[E1.3为(420.6±95.7) N,E1.6为(517.9±67.2) N],分别较D1.3和D1.6有显著差异(P<0.05),而与B1.3、B1.6无显著差异(P>0.05)。纤维桩修复的折裂模式中大部分可以重新修复,而金属桩大部分为破坏性。结论:纤维桩修复后,折裂的牙体大部分可以再次修复,1.3 mm和1.6 mm直径的桩对于牙体的抗折性无显著影响,ParaCore重塑根管可有效增强牙体的抗折性能。  相似文献

PURPOSE: The present study aimed to compare the fracture resistance and failure patterns of endodontically treated premolars with MOD preparations restored using different material combinations. The null hypothesis postulated that there was no association between the fracture resistance of endodontically treated premolars and the resin composite materials or the post-and-core system used to build up the restorations. MATERIALS AND METHODS: Eighty single-rooted maxillary premolars were used. After endodontic treatment and preparation of MOD preparations, 8 groups of 10 samples each were created, using the following material combinations: group 1 (control), flowable and microhybrid resin composites; group 2, flowable A; group 3, flowable B; group 4, microhybrid resin A; group 5, microhybrid resin B; group 6, flowable B + microhybrid resin B; group 7, flowable A + microhybrid resin A + post A; group 8, flowable B + microhybrid resin B + post B. Mechanical static fracture tests were performed loading the specimens till fracture. RESULTS: The mean failure loads (N) were 502 (control), 470 (group 7), 445 (group 8), 441 (group 6), 405 (group 5), 364 (group 4), 317 (group 2), and 302 (group 3). Statistically significant differences were found between groups 1 vs 2, 1 vs 3, and 3 vs 7 (p < 0.05). CONCLUSION: The fracture resistance of endodontically treated premolars with MOD preparations was enhanced by the use of the sandwich technique. The samples restored with posts predominantly showed restorable fractures, while teeth restored without posts mostly displayed unrestorable failures.  相似文献

STATEMENT OF PROBLEM: Very little is known about the resistance to fracture of endodontically treated teeth restored with newly developed esthetic post systems. Purpose. This in vitro study compared the effect of 1 titanium and 3 esthetic post systems on the fracture resistance and fracture patterns of crowned, endodontically treated teeth. MATERIAL AND METHODS: A total of 40 recently extracted human maxillary canines with their crowns removed were endodontically treated. Four groups of 10 specimens were formed. Teeth were restored with titanium, quartz fiber, glass fiber, and zirconia posts and numbered as groups 1, 2, 3, and 4, respectively. All posts were cemented with Single Bond dental adhesive system and dual-polymerizing RelyX ARC adhesive resin cement. All teeth were restored with composite cores, and metal crowns were fabricated and cemented with glass ionomer cement. Each specimen was embedded in acrylic resin and then secured in a universal load-testing machine. A compressive load was applied at a 130-degree angle to the long axis of the tooth until fracture, at a crosshead speed of 1 mm/min. One-way analysis of variance and a Tukey test were used to determine the significance of the failure loads between groups (P<.001). A non-parametric chi(2) test was conducted for evaluation of the mode of failure (P<.001). RESULTS: The mean failure loads (kg) were 66.95, 91.20, 75.90, and 78.91 for groups 1 to 4, respectively. Teeth restored with quartz fiber posts (group 2) exhibited significantly higher resistance to fracture (P<.001) than the other 3 groups. Teeth restored with glass fiber and zirconia posts (groups 3 and 4) were statistically similar (P>.05). Fractures that would allow repair of the tooth were observed in groups 2 and 3, whereas unrestorable, catastrophic fractures were observed in groups 1 and 4 (P<.001). CONCLUSION: Within the limitations of this study, significantly higher failure loads were recorded for root canal treated teeth restored with quartz fiber posts. Fractures that would allow repeated repair were observed in teeth restored with quartz fiber and glass fiber posts.  相似文献

The elastic modulus of the restorative material is important in restoring endodontically treated teeth. This study aimed to compare the fracture resistance and failure patterns of 90 mandibular molars restored using resin composites with or without fiber posts, with respect to the number of residual cavity walls. Five restoration types were performed corresponding to different wall defects (groups 1-5). Groups were divided in two subgroups corresponding to the use or absence of fiber posts. Teeth were loaded and resistance of specimens was measured as the axial compressive load to cause fracture and macroscopic fracture patterns were observed. One way ANOVA revealed a significant difference in fracture resistance (p < 0.001). Tukey post hoc test also revealed significant differences between groups as samples restored with fiber posts exhibited mostly restorable fractures. It was concluded that the resistance of endodontically treated mandibular molars restored with composite resins is mainly affected by the number of residual walls. ObjectiveTo conduct a systematic review and meta-analysis to assess if the presence of an endodontic post may increase the fracture resistance of endodontically-treated maxillary premolars directly restored with composite.MethodsA comprehensive systematic literature search according to the PRISMA statement was conducted to assess in vitro studies including endodontically-treated maxillary premolars, restored with different type of posts supporting direct composite reconstructions. Two groups of meta-analyses were performed using fixed effects model and additional Trial Sequential Analysis (TSA) was carried out. Moreover, risk of bias was conducted and quality of evidence for any performed meta-analysis was assessed using Grading of Recommendations Assessment, Development and Evaluation (GRADE).ResultsTwenty-four articles met the inclusion criteria and 13 studies also underwent quantitative evaluation. Fracture resistance of endodontically-treated premolars restored with fiber posts was significantly less than sound teeth (p value: < 0.00001), however endodontically-treated premolars with a fiber post provided an increase in fracture resistance when compared to equivalent teeth restored without post (p value: 0.003). TSA revealed high power of both meta-analyses. Moderate risk of bias was reported in 23/24 studies; only 1 study showed high risk of bias. GRADE system showed moderate strength of evidence due to the presence of a study with high risk of bias and to wide confidence intervals.SignificanceWithin the limitation of the present systematic review and meta-analysis, it can be concluded that endodontically-treated maxillary premolars restored with a fiber post and direct composite restoration demonstrated increased fracture resistance when compared to equivalent teeth without post.  相似文献

The purpose of this study was to evaluate the influence of endodontic and restorative procedures on fracture resistance of teeth, and to compare the incidence of root fracture in teeth with clinical crowns removed that were restored with three different types of post and a composite core build-up. Seven groups of 10 extracted maxillary canines were used. A control group had only a crown preparation, but no endodontic treatment. Three groups had endodontic treatment, crown preparation, and the access restored. Three groups had endodontic treatment, the crown totally removed, a tapered, parallel, or carbon post placed, and a composite build-up. All specimens were subjected to a 45-degree load at 0.5 mm/min until failure occurred. The force at failure and the location of fracture were recorded. The groups with post and composite build-ups failed at significantly lower force than the teeth in which the crowns had not been removed. There were no significant differences in the amount of force required to produce failure among the three groups with different posts and a composite build-up. The group restored with the carbon post had no root fractures, whereas there were five fractures in each of the parallel and tapered post groups.  相似文献

Objectives

This study aims to evaluate the fracture resistance of endodontically treated anterior teeth restored with crowns made of composite or ceramic and retained without the use of a post (endocrowns) or with posts of 5 mm (short) and 10 mm in length (long).

Material and methods

Forty-eight intact maxillary incisors were selected for the study. After endodontic treatment, the crowns were sectioned 2 mm coronally to the cementoenamel junction provided with a ferrule of 2 mm. The roots were randomly divided into six groups (n?=?8) according to the post length and type of coronary restoration. The crowns were fabricated with the chairside economical restoration of esthetic ceramics system. Group 1 was restored with a 10-mm glass fiber post, composite core, and a full-coverage ceramic crown (LPCer); group 2, with a 5-mm glass fiber post, composite core, and a full-coverage ceramic crown (SPCer); group 3, with a 10-mm glass fiber post, composite core, and a full-coverage composite crown (LPCpr); group 4, with a 5-mm glass fiber post, composite core, and a full-coverage composite crown (SPCpr); and groups 5 (EndoCer) and 6 (EndoCpr) were restored with ceramic and composite endocrowns, respectively. The teeth were then thermomechanically loaded in a chewing machine. After fatigue, the specimens were loaded to fracture. Data were analyzed with ANOVA and chi-square test. Mode of failure was defined as repairable or non-repairable.

Results

Presence of post, post length, and crown material had no significant effect on the fracture resistance. Groups restored with endocrowns presented a higher number of repairable fractures in respect to the other groups.

Conclusions

Presence of a post had no effect on the restorations’ fracture strength.

Clinical relevance

Although this in vitro study has some limitations in respect to its clinical relevance, the restoration of largely destroyed anterior teeth with the use of an endocrown or a short glass fiber post might have advantages over a large glass fiber post.  相似文献

OBJECTIVES: Superior restorative methods for effectively strengthening pulpless teeth need to be identified, since vertical root fractures of pulpless teeth are still a major problem in everyday clinical practice. The present study tested the null hypothesis that there were no differences in static and fatigue fracture resistances of pulpless teeth restored with different types of post-core systems. METHODS: Extracted human premolars were restored with a combination of either a fiber post or metallic post and a composite resin core. Teeth with full crown preparations without post-core restorations served as a control. A 90 degrees vertical or 45 degrees oblique static compressive load was applied to restored teeth, and fracture loads and modes of fracture were recorded. Fatigue fracture tests were conducted by applying sinusoidal cyclic loads to restored teeth from vertical or oblique directions. Fatigue limits for each restoration were calculated using the staircase approach. RESULTS: In both static and fatigue fracture testing under vertical or oblique loadings, the fracture loads of teeth restored with fiber posts were significantly greater than those of teeth restored with metallic posts. The fatigue limits of teeth restored with fiber and metallic posts were 112 kgf and 82 kgf respectively under vertical loadings and 26 kgf and 20 kgf under oblique loadings. SIGNIFICANCE: The combination of a fiber post and a composite resin core showed superior fracture resistance against both static and fatigue loadings compared to restorations using a metallic post, and is therefore recommended in restoring pulpless teeth.  相似文献

AIM: The aim of the study was to evaluate the influence of different posts and types of cementation on the fracture load and fracture mode of crowned, endodontically treated premolars with class II cavities in an ex vivo setting. METHODS: Forty-eight single-rooted human premolars were endodontically treated and prepared with standardized MO (mesio-occlusal) cavities and a circular chamfer preparation. Eight teeth each received either no posts or were restored with screws (BKS), glass fiber posts (DentinPost), or quartz fiber posts (DT Light SL). Sixteen teeth were restored with zirconium dioxide posts (CeraPost). BKS-screws and eight zirconium dioxide posts were cemented conventionally with glass ionomer cement; Panavia F was used for all others. The specimens were restored with composite cores and crowns cast from a non-precious metal. Eight sound premolars served as the controls. After thermomechanical fatigue testing, the samples were loaded until fracture occurred at an angle of 45 degrees. All specimens were evaluated for fracture lines. RESULTS: The sound teeth showed the significant highest fracture load (792.50+/-210.01 N). Conventionally cemented zirconium dioxide posts showed the lowest fracture load (327.00+/-45.84 N); the highest fracture load occurred with quartz fiber posts (421.75+/-90.19 N). Only the difference between these two groups was statistically significant. With glass fiber posts and conventionally cemented zirconia posts, restored teeth failed mostly in an "unfavourable" mode. CONCLUSIONS: With respect to the fracture load, there was no statistical difference between the restoration of non-vital premolars with class II cavities with crowns and posts or crowns alone.  相似文献

OBJECTIVES: The objective of this study was to assess the resistance to fracture of crowned endodontically treated premolars with varying ceramic and metal post systems under simulated occlusal load. METHOD AND MATERIALS: The study consisted of 60 extracted intact premolars with similar root diameter and length. Teeth were divided equally into 4 groups (n = 15) and prepared for posts and cast crowns as follows: group 1 = preformed metal post, composite core, and cast crown; group 2 = cast post and core and cast crown; group 3 = zirconia post, composite core, and cast crown; and group 4 = zirconia post and heat-pressed ceramic core and cast crown. All prepared teeth had 2 mm of sound dentin on which the cast crowns were cemented. A continuous load (kg) was applied by an Instron testing machine to the buccal cusp at a 30-degree angle to the long axis of each tooth at a crosshead speed of 2 mm/min until failure. One-way analysis of variance with Scheffe test was used to statistically compare the differences between groups. RESULTS: Mean failure loads (in newtons) for the 4 test groups were as follows: 1,103.3 (group 1), 1,234.5 (group 2), 826.6 (group 3), and 870.6 (group 4). No significant difference in failure load values was found among all tested groups (F[3,59] = 2.05; P >.05). The primary mode of failure (85%) in all groups was an oblique radicular fracture, either apical to the post or at the post level. Horizontal fracture (15% to 20%) of the root and post was found in groups 3 and 4. CONCLUSION: Within the limitations of this study, varying ceramic and metal post systems did not affect the failure resistance of teeth restored with full-coverage cast metal crowns, under simulated occlusal load.  相似文献

IntroductionThe aim of this study was to evaluate the fracture resistance and fracture patterns of endodontically treated maxillary premolars with wedge-shaped cervical lesions restored with a resin composite core with or without a fiber post under nonaxial loading.MethodsSixty extracted human maxillary premolars with 2 root canals were randomly allocated into 4 groups (n = 15). Forty-five teeth were prepared with a wedge-shaped lesion at the buccal cervical area and root canal treatment. There were 3 restoration groups: no restoration, restoration with a resin composite core, and restoration with a resin composite core and a fiber post. Intact teeth served as the control group. All teeth were subjected to thermocycling and cyclic loading. A 45° compressive load was applied to the palatal plane of the buccal cusp until tooth fracture. The fracture resistance and facture patterns were analyzed using 1-way analysis of variance and the chi-square test (α = 0.05), respectively.ResultsThe no restoration group demonstrated the lowest fracture resistance and was significantly different from the other groups (P < .001). The intact teeth presented the highest fracture resistance, which was not significantly different from the restoration with a resin composite core and a fiber post group (P > .05). The failure patterns were significantly different between groups (P < .001). Most intact teeth fractured coronally to the cementoenamel junction level, whereas most teeth in the other groups fractured at the gingival margin of the cervical lesion.ConclusionsEndodontically treated maxillary premolars with wedge-shaped cervical lesions restored with fiber posts and a resin composite cores had a fracture resistance equivalent to intact teeth. However, their fracture patterns were not improved by the fiber post and a resin composite core.  相似文献
