目的探讨用体外亚甲蓝作为染色剂寻找前哨淋巴结(sentinel lymph node,SLN)的方法在结直肠癌SLN定位中的临床价值。方法将根治性切除大肠癌标本,在肿块四周浆膜下注射亚甲蓝后常规固定送病理检查。蓝染的淋巴结视为SLN。结果67例患者中找到SLN的有63例(91.3%),SLN镜下转移14例(22.22%)。结肠的SLN转移阳性的诊断价值与非SLN相比差异有统计学意义(P=0.0005),但在直肠癌中SLN转移阳性的诊断价值与非SLN相比差异无统计学意义(P=0.60),且SLN转移阳性在结肠癌病例中假阴性率为11.11%,而在直肠癌病例中则高达42.86%。结论体外亚甲蓝染色寻找SLN的方法在结肠癌中的应用是有效、可靠的,但在直肠癌中的应用其可靠性值得进一步的研究。  相似文献

目的探讨结直肠癌前哨淋巴结(SEN)体外亚甲蓝定位活检方法的可行性,研究前哨淋巴结组织学状况能否用于预测区域淋巴结转移情况。方法将32例手术切除的结直肠癌标本纵行剪开,在癌肿四周注射亚甲蓝,2—5min后沿着蓝染的淋巴管追踪寻找首先蓝染的前哨淋巴结。将其切下后单独进行病理切片,检测有无癌转移,并与系膜淋巴结病理结果予以比较。结果有30例标本成功显示57枚SLN,平均每例标本显示1.9枚SLN。在SLN阳性的13例患者中。5例非SLN呈阳性,8例非SLN呈阴性;在17例SLN为阴性的标本中,15例非SLN呈阴性,仅2例非SLN呈阳性。统计本组患者SLN标记成功率为93.8%(30/32),准确率为93.3%(28/30),假阴性率为11.8%(2/17),特异性为100%(13/13)。结论结直肠癌标本前哨淋巴结体外亚甲蓝标记法可行,其组织学状况可较准确反映区域淋巴结群的癌转移情况。  相似文献

目的:探讨腹腔镜下胃癌前哨淋巴结(SLN)检测的可行性及SLN活检预测胃周淋巴结转移状况的准确性,评价SLN活检在指导胃癌手术治疗中的应用价值。方法:2010年3—11月诊断明确的126例胃癌,在腹腔镜胃癌切除术中向肿瘤边缘正常胃壁浆膜下肌层、黏膜下层注射亚甲蓝标示SLN,腔镜下识别和切取蓝染淋巴结活检。以术后所有切除的胃周淋巴结常规病理检查结果为诊断金标准,观察SLN活检对预测胃周淋巴结有无肿瘤转移的准确性。结果:116例成功检测出SLN,检出率92.1%(116/126)。62例有淋巴结转移,其中前哨淋巴结与非前哨淋巴结(nSLN)均有转移47例,仅前哨淋巴结有转移10例,仅非前哨淋巴结有转移5例。前哨淋巴结预测胃周淋巴结转移的敏感性为91.9%(57/62),准确率为94.0%(109/116),假阴性率为8.1%(5/62)。结论:腹腔镜下胃癌SLN检测可行。通过SLN,术中能准确预测胃癌淋巴结转移状况,淋巴结转移阴性的胃癌患者,有望免除常规淋巴清扫。  相似文献

BACKGROUND: Sentinel lymph node biopsy (SLNB) has been shown to be relatively accurate in axillary nodal staging in breast cancer. In more than half of the patients with metastatic sentinel lymph node (SLN), the SLN was the only lymph node involved in the axilla. Methods: A retrospective analysis was performed for those female Chinese breast cancer patients who underwent SLNB. All patients had axillary dissection after SLNB. Those patients with metastatic SLN were selected for analysis. Various tumour factors and SLN factors were analysed to study the association with residual lymph node metastasis. Results: A total of 139 SLNB was performed. The success rate of SLN localization, false negative rate and accuracy were 92%, 9% and 95%, respectively. Fifty-five patients had metastases in the SLN. In 38 patients (69%), SLN was the only lymph node involved in the axilla. Tumours <3 cm, a single metastatic SLN, presence of micro metastases and the absence of extracapsular spread in the SLN were associated with the absence of metastasis in the non-sentinel lymph nodes. Conclusion: Sentinel lymph node biopsy is accurate in the nodal staging of Chinese breast cancer patients. Several factors such as tumour <3 cm, a single metastatic SLN, micro metastases and the absence of extracapsular spread in the sentinel node(s) are useful predictors for the absence of residual disease in the axilla. With further studies and verification, these factors may prove to be important in determining which patients with metastatic SLN will require further axillary treatment. Until such information is available, axillary dissection should be performed when positive sentinel nodes are found.  相似文献

目的探讨亚甲蓝存结直肠癌前哨淋巴结(sentinel lymph node,SLN)新鲜离体标本定位方法的可行性及临床意义。方法以1%业甲蓝对49例新鲜结直肠癌离体标本作SLN定位,标本染色后10分钟内先蓝染的第1—4个淋巴结视为SLN.所有标本均行常规石蜡切片.阴性的SI。N冉行多层切片,观察SLN对区域淋巴结转移的预测价值。结果49例病人中有47例检出SLN,检出率是95.92%,共获得SLN98枚,平均每例2.09枚(1~4枚)。SLN预测区域淋巴结转移的准确率为97.87%;特异性为95.83%;敏感性为94.45%;假阴性率为3.70%.结论以1%亚甲蓝作结直肠癌新鲜离体标本SLN定位简单、易行、可靠。SLN转移与否,可基本能反映区域淋巴结的转移状况。多层切片可降抵SLN的假阴性率。  相似文献

Sentinel lymph node mapping has already been accepted as part of the treatment for malignant melanomas of the skin and in breast carcinomas. The status of lymph nodes is an important prognostic marker in colorectal carcinoma as well. The authors tried the feasibility of this technique in colorectal carcinomas. The technique is analogous to the one used in breast cancer and melanoma: 2 ml of 2.5% Patentblau dye was given subserosally around the tumor. After resection the specimen was immediately sent to pathology where the lymph nodes were removed. This technique has been tried on 31 patients, 22 with colonic and 9 with rectal tumors. Of these patients, 15 were Dukes stage C, 14 were Dukes stage B and 2 were Dukes stage A. An average 4.3 blue lymph nodes were found in colon tumors and 5.4 in rectal tumors and an average 14 unstained lymph nodes were found in colon tumors, and 7 in rectal tumors. The blue nodes were predictive of the nodal status in 9 of the 15 Dukes stage C patients. In these cases the blue lymph nodes contained metastases and there were 2 cases where metastases were limited to the blue lymph nodes. SUMMARY: The authors found a high false negative rate for lymphatic mapping with the vital dye technique, therefore they try to change the method according to that used by Saha et al. The aim of sentinel node identification in colorectal carcinomas would be improved staging rather than reducing of the extent of lymphadenectomy. The role of lymphatic mapping in large bowel cancers needs further investigations. Until the results are reliable, as many lymph nodes as possible have to be excited and sent for histology.  相似文献

目的探讨甲状腺乳头状癌前哨淋巴结(SN)的检测方法及其对颈淋巴结转移的预测价值。方法术中使用亚甲蓝对24例甲状腺乳头状癌患者进行SN示踪,观察冷冻快速切片检查SN转移与常规病理淋巴结转移的符合率,及SN对颈淋巴结转移的预测价值。结果24例中成功检出21例SN,成功率为87.5%,SN平均每例3枚;1例假阴性,假阴性率为4.8%(1/21);无假阳性。SN对颈淋巴结转移阳性预测值为83.3%。结论亚甲蓝染色法检测SN可较准确地预测甲状腺乳头状癌的颈淋巴结转移状况。  相似文献

Abstract: The indocyanine green fluorescence (ICGf) navigation method provides real‐time lymphatic mapping and sentinel lymph node (SLN) visualization, which enables the removal of SLNs and their associated lymphatic networks. In this study, we investigated the features of the drainage pathways detected with the ICGf navigation system and the order of metastasis in axillary nodes. From April 2008 to February 2010, 145 patients with clinically node‐negative breast cancer underwent SLN surgery with ICGf navigation. The video‐recorded data from 79 patients were used for lymphatic mapping analysis. We analyzed 145 patients with clinically node‐negative breast cancer who underwent SLN surgery with the ICGf navigation system. Fluorescence‐positive SLNs were identified in 144 (99%) of 145 patients. Both single and multiple routes to the axilla were identified in 47% of cases using video‐recorded lymphatic mapping data. An internal mammary route was detected in 6% of the cases. Skip metastasis to the second or third SLNs was observed in 6 of the 28 node‐positive patients. We also examined the strategy of axillary surgery using the ICGf navigation system. We found that, based on the features of nodal involvement, 4‐node resection could provide precise information on the nodal status. The ICGf navigation system may provide a different lymphatic mapping result than computed tomography lymphography in clinically node‐negative breast cancer patients. Furthermore, it enables the identification of lymph nodes that do not accumulate indocyanine green or dye adjacent to the SLNs in the sequence of drainage. Knowledge of the order of nodal metastasis as revealed by the ICGf system may help to personalize the surgical treatment of axilla in SLN‐positive cases, although additional studies are required.  相似文献

BACKGROUND: Orderly progression of nodal metastases has been described for melanoma and breast cancer. The first draining lymph node, the sentinel node, is also the first to contain metastases and accurately predicts nodal status. The aim of this study was to assess the feasibility of lymphatic mapping and sentinel node biopsy in colorectal cancer. METHODS: In 50 patients with colorectal cancer patent blue dye was injected around the tumour. After resection of the tumour the specimen was examined to identify blue-stained lymph nodes. Routine histopathological examination was performed on all nodes and the blue, haematoxylin and eosin-stained tumour-negative nodes were tested immunohistochemically. RESULTS: Lymphatic mapping was possible in 35 of 50 patients (70 per cent). Pathological examination with haematoxylin and eosin staining showed lymph node metastases in 20 of 35 patients. In eight of these 20 patients the blue nodes showed tumour, while in 12 the blue nodes were not involved. This represents a false-negative rate of 60 per cent. CONCLUSION: Lymphatic mapping using patent blue dye is feasible in colorectal cancer. The blue-stained nodes do not predict nodal status of the remaining lymph nodes in the resected specimen. The concept of lymphatic mapping and sentinel node identification is not valid for colorectal cancer.  相似文献

目的探讨乳腺癌前哨淋巴结(sentinel lymph node,SLN)预警腋窝淋巴结转移的价值. 方法对56例乳腺癌行亚甲蓝前哨淋巴结定位、活检和腋窝淋巴结清扫术,标本常规行HE染色、免疫组化病理检查. 结果 SLN成功检出52例(52/56,92.8%),常规病理检查证实SLN转移22例;SLN无转移,但非SLN发现转移者1例,假阴性率为4.3%(1/23).常规病理检查无转移的29例患者,免疫组化检测发现1例CK-19( )、EMA( ),另1例CK-19( ),CEA( ),而所属非前哨淋巴结无肿瘤转移. 结论乳腺癌亚甲蓝前哨淋巴结定位、活检可以预示腋窝淋巴结转移.  相似文献

目的研究乳腺癌新辅助化疗后前哨淋巴结活检(SLNB)的可行性和效果。方法利用新型示踪剂——^99mTc-利妥昔配合专利蓝染料对60例原发性乳腺癌新辅助化疗后病例进行SLNB,并对SLN进行常规病理检查和免疫组织化学检查。SLNB后常规腋窝淋巴结清扫。结果SLN检测成功率95%(57/60)。SLN转移阳性23例(40%),其中18例为常规病理检查转移阳性(78%),5例为免疫组织化学检出的微转移(22%)。23例SLN有转移病例中,9例同时存在其他腋窝淋巴结转移,另外14例为惟一转移淋巴结。1例SLN转移假阴性。灵敏度96%(23/24),准确性98%(56/57),特异度100%(33/33),假阴性率4.3%(1/23),阴性预测值97%(36/37),阳性预测值100%(24/24)。内乳淋巴结显像11例,活检病理检查均为转移阴性。结论同位素示踪剂和蓝染料联合检测方法对原发性乳腺癌新辅助化疗后进行SLNB同样适用,内乳前哨淋巴结活检不应做常规推荐。  相似文献

目的探讨结直肠癌前哨淋巴结(SLN)体外定位技术方法及其可行性、准确性和临床价值。方法选择2003年3月至2003年10月间中山大学肿瘤防治中心腹科住院行根治手术的结直肠癌患者60例,62个肿瘤(2例患者为多原发),进行体外SLN定位。标本离体后尽早进行异硫蓝SLN定位,传统病理检查阴性的SLN进行细胞角蛋白免疫组化检查。结果62例肿瘤成功检出SLN的59例,检出率95.2%。59例患者总共获得并检测1114枚淋巴结,平均每人18.9(4~46)枚。其中SLN157枚(14.9%),平均每人2.7(1~9)枚。SLN敏感性39.1%(9/23),假阴性率23.7%(14/59),准确率76.3%(45/59)。50例SLN阴性的中有12例(24%)细胞角蛋白免疫组化检测阳性。36例HE和细胞角蛋白免疫组化检查全阴性者中4例(11.1%)SLN发现微转移灶。14例仅非SLN阳性中8例SLN发现微转移灶。结论结直肠癌异硫蓝SLN体外定位活检技术是可行的,结合免疫组化检测微转移可以提高术后分期,可以提高送检淋巴结个数,结合免疫组化技术,可以减少淋巴结转移漏诊发生率。但该方法假阴性率较高,不能完全取代常规淋巴结病理检查。  相似文献

Background: Sentinel lymph node (SLN) biopsy has become a standard method of staging patients with cutaneous melanoma. Sentinel lymph node biopsy usually is performed by intradermal injection of a vital blue dye (isosulfan blue) plus radioactive colloid (technetium sulfur colloid) around the site of the tumor. Intraoperative gamma probe detection has been shown to improve the rate of SLN identification compared to the use of blue dye alone. However, multiple sentinel nodes often are detected using the gamma probe. It is not clear whether these additional lymph nodes represent true sentinel nodes, or second-echelon lymph nodes that have received radiocolloid particles that have passed through the true sentinel node. This analysis was performed to determine the frequency with which these less radioactive lymph nodes contain metastatic disease when the most radioactive, or hottest, node does not.Materials and Methods: In the Sunbelt Melanoma Trial, 1184 patients with cutaneous melanoma of Breslow thickness 1.0 mm or more had sentinel lymph nodes identified. Sentinel lymph node biopsy was performed by injection of technetium sulfur colloid plus isosulfan blue dye in 99% of cases. Intraoperative determination of the degree of radioactivity of sentinel nodes (ex vivo) was measured, as well as the degree of blue dye staining.Results:Sentinel nodes were identified in 1373 nodal basins in 1184 patients. A total of 288 of 1184 patients (24.3%) were found to have sentinel node metastases detected by histology or immunohistochemistry. Nodal metastases were detected in 306 nodal basins in these 288 patients. There were 175 nodal basins from 170 patients in which at least one positive sentinel node was found and more than one sentinel node was harvested. Blue dye staining was found in 86.3% of the histologically positive sentinel nodes and 66.4% of the negative sentinel nodes. In 40 of 306 positive nodal basins (13.1%), the most radioactive sentinel node was negative for tumor when another, less radioactive, sentinel node was positive for tumor. In 20 of 40 cases inding a positive sentinel node other than the hottest node.Conclusions: If only the most radioactive sentinel node in each basin had been removed, 13.1% of the nodal basins with positive sentinel nodes would have been missed. It is recommended that all blue lymph nodes and all nodes that measure 10% or higher of the ex vivo radioactive count of the hottest sentinel node should be harvested for optimal detection of nodal metastases.Preliminary findings presented at the annual meeting of the Society of Surgical Oncology, Orlando, Florida, March 4–7, 1999.  相似文献

Sentinel lymph node biopsy (SLNB) is an important surgical advance in the management of early breast cancer. Localization of the sentinel lymph node (SLN) in breast cancer and other cancer patients is widely used now utilizing isosulfan blue dye. Few alternative types of dye have been used and investigated. In a prospective study of 54 patients, methylene blue dye was used as an alternative to isosulfan blue to localize the SLN in breast cancer patients. The methylene blue dye technique was successful in 91.1% of patients after excluding the initial learning cases; the results are similar for isosulfan blue dye. This study describes methylene blue dye localization as a successful alternative to isosulfan blue dye in identifying the sentinel node in breast cancer patients. Methylene blue dye is readily available in most Egyptian institutes, with a very low cost.  相似文献

目的评价前哨淋巴结活检术 (sentinellymphnodebiopsy,SLNB)预测腋淋巴结肿瘤转移的准确性及其临床意义。方法使用专利蓝染色法和 /或99mTc标记的硫胶体示踪法对我院收治的81例乳腺癌患者进行前哨淋巴结活检。两种方法联合检测 3例前哨淋巴结 (sentinellymphnode ,SLN)均阴性者未行腋淋巴结清扫术。结果 81例患者SLN总检出率为 96 3% (78/81) ,总准确率为97 5 % ,总假阴性率 9 7%。 5 3例单纯染色法检出率为 92 5 % ,准确率 94 2 % ,假阴性率 15 8% ;2 8例99mTc示踪法和 /或染色法联合检测结果分别为 10 0 % ,10 0 %和 0。结论SLNB能够准确预测腋窝淋巴结的转移状况。两种方法联合检测为最佳。术前化疗对假阴性率可能有影响。  相似文献

目的探讨前哨淋巴结活检(SLNB)在结直肠癌根治术中临床应用的可行性及其价值。方法应用美蓝对67例结直肠癌患者行前哨淋巴结(SLN)定位活检,分体内、体外组,采用HE染色病理检查法、CK-20免疫组化染色(SP法)检测SLN中转移癌。结果共检出淋巴结660枚,其中SLN130枚,检出率19.7%。腹腔镜结直肠癌根治术和开腹结直肠癌根治术对SLN的检出差异无统计学意义(P=0.742);体内、体外两种SLN的标记方法差异无统计学意义(P=0.564);SP法检测SLN癌转移的敏感性明显高于HE染色,而假阴性率明显低于后者;肿瘤细胞在SLN的转移率明显高于区域淋巴结的转移率(P〈0.01)。结论结直肠癌根治术中体内、体外SLN定位方法均可以获得成功,均具有切实的可行性,与手术方式无关,并能够预测区域淋巴结的转移状况;通过SP法检查有助于明确结直肠癌的病理分期,有利于判断预后和个体化治疗方案的制定。  相似文献

Background: The sentinel lymph node is the first draining node from a cancer‐bearing area and is therefore the first to manifest metastasis. In breast cancer it has been shown to predict the axillary status. Axillary dissection provides information determining prognosis and need for adjuvant therapy but carries a certain morbidity. Our aim was to determine the feasibility of detecting the sentinel node in a teaching hospital and whether the sentinel node accurately predicts the axillary status. Methods: All patients with stage I and II breast cancer and non‐palpable axillary nodes were eligible, including those with previous excision biopsy. We excluded pregnant women, those with previous axillary surgery and women with advanced breast cancer with enlarged axillary nodes. The sentinel node was detected with technetium‐99m‐labelled tin colloid and vital blue dye and removed, and axillary clearance was performed. Results: A total of 312 patients were examined from August 1996 to December 1998. The mean age was 53 years (range 28?83) and mean tumour size 2.6 cm (range 0.2?9.0). The detection rate of the sentinel node was 86%. The sentinel lymph node predicted the axillary status with a sensitivity of 83% and specificity of 100%. The false‐negative rate was 16.7%. Conclusions: Detection of the sentinel lymph node is feasible and it can accurately predict the nodal status of the axilla. ­However, the high false‐negative rate precludes as yet the use of sentinel lymph node biopsy in replacing axillary clearance as the standard of care for breast cancer.  相似文献

目的 观察胃癌前哨淋巴结的分布,探讨其临床意义。方法 回顾性分析288例胃癌前哨淋巴结术中染色后显影的范围及特征。术中向肿瘤边缘的正常胃壁浆膜下肌层、黏膜下层注射亚甲蓝,观察淋巴结显影的情况;切取各站淋巴结行病理检查。结果 288例胃癌术后病理诊断为T1期102例,T2期126例,T3期60例。术中成功显影270例,阳性率为93.8%。102例患者有淋巴结转移,其中前哨淋巴结(SNs)与非前哨淋巴结(non—SNs)均有转移者66例,仅前哨淋巴结有转移者18例,仅非前哨淋巴结有转移者18例。结论 通过前哨淋巴结,术中能准确预测胃癌淋巴结转移状况。在手术治疗淋巴结转移阴性的胃癌患者中,前哨淋巴结术中标识有望免除常规淋巴清扫。  相似文献

Aim Sentinel lymph node mapping has been used in colon cancer to improve prognosis. This study aimed to determine the accuracy of in vivo SLNM in patients with colon carcinoma undergoing surgery with curative intent. Method Thirty‐one patients operated for colon carcinoma underwent in vivo sentinel lymph node mapping using patent blue dye. Each sentinel lymph node (SLN) was marked intraoperatively, and histological examination was performed after en bloc resection. If no metastasis was found, step sectioning with immunohistochemistry was performed. Results The SLN was successfully identified in 28 (90%) of 31 patients. The false‐negative rate to identify stage III disease was 66% (eight of 12), the negative predictive value was 46% (19 of 27) and the accuracy was 14% (four of 28). One patient negative on routine histopathology had micrometastasis on step sectioning of the SLN. Conclusion Sentinel lymph node mapping in colon carcinoma cannot accurately predict nodal status.  相似文献

BACKGROUND: This analysis was performed in order to determine whether primary tumor location in breast cancer affects the axillary sentinel lymph node (SLN) identification (ID) rate, the false negative (FN) rate, incidence of axillary nodal metastases, or the number of SLN identified. METHODS: In this prospective multi-institutional study, SLN biopsy was performed on clinical stage T1-2, N0 breast cancer patients using blue dye alone or in combination with radioactive colloid, followed by completion axillary LN dissection. RESULTS: Central tumor location was associated with an improved FN rate, which may be related to reliable drainage from the subareolar lymphatic plexus. Tumor location did not significantly affect the SLN ID rate or the mean number of SLN identified. Medial tumor location was associated with a decreased rate of axillary nodal metastasis. CONCLUSIONS: Breast cancers drain reliably to the axillary lymph nodes regardless of tumor location within the breast.  相似文献
