新生儿高胰岛素血症性低血糖症一例

患儿男,14d ,因间歇抽搐12d入院。系第二胎第二产,足月剖宫产,其姐体健。于生后第二天开始抖动,第三天出现抽搐,表现双眼上翻、大汗、头后仰、四肢抖动,持续2～3min自行缓解,之后每日均有频繁抽搐,最多时1日抽搐10余次,不发热、吃奶可、无吐泻。查体:T 36 8℃,体重3 8kg,头围34cm ,发育营养可,精神反应差,面色苍灰,双眼凝视,前囟2cm×2cm ,平坦,心肺(- ) ,腹软,肝脾不大,四肢肌张力低。实验室检查:血糖0 7mmol/L、钙2 4 0mmol/L、镁0 9mmol/L、磷1 4 1mmol/L、二氧化碳结合力2 3 9mmol/L、钾4 2 7mmol/L、钠133 7mmol/L、氯化物10 …     

Hypoketosis as a cause of symptoms in childhood hypoglycemia

Children with symptomatic hypoglycemia and asymptomatic hypoglycemia-prone children were shown to differ in the degree of ketosis after a 20 h fast. In the latter children the close negative correlation between ketone body levels and glucose levels yielded a regression line against which the former children's data could be compared. Half of the patients were found to be hypoketotic during hypoglycemia. The significance of this hypoketosis in the symptomatology is discussed. The finding of other abnormal responses to fasting, particularly in the patients' nitrogen metabolism, suggests that all these aberrations have a common cause which may be of hepatic origin.     

Induced hypoglycemia. A unusual case of child battering

We describe a case of Munchausen's syndrome by proxy in a 12 years old child. The administration of glibenclamide by the mother led to severe hypoglycemias in the child, who underwent various instrumental researches and a subtotal pancreatectomy before the final diagnosis could be reached. The diagnosis also was possible with the substantial help of an accurate psychological survey. The case solution, with disappearance of hypoglycemias, was made possible by the removal of the maternal presence settled by the juvenile court.     

A case of spontaneous chylothorax in an adolescent

We discuss an male adolescent who presented to the emergency department with fever and respiratory distress. He was subsequently diagnosed with spontaneous chylothorax. We review his clinical presentation and diagnostic and therapeutic interventions and provide a discussion of the subject.
